ABSTRACT BACKGROUND: Liver transplantation represents the best therapeutic modality in end-stage chronic liver disease, severe acute hepatitis, and selected cases of liver tumors. AIMS: To describe a double retransplant in a male patient diagnosed with Crohn's disease and complicated with primary sclerosing cholangitis, severe portal hypertension, and cholangiocarcinoma diagnosed in the transplanted liver. METHODS: A 48-year-old male patient diagnosed with Crohn's disease 25 years ago, complicated with primary sclerosing cholangitis and severe portal hypertension. He underwent a liver transplantation in 2018 due to secondary biliary cirrhosis. In 2021, a primary sclerosing cholangitis recurrence was diagnosed and a liver retransplantation was indicated. Recipient's hepatectomy was very difficult by reason of complex portal vein thrombosis requiring extensive thromboendovenectomy. Intraoperative ultrasound with liver doppler evaluation was performed. Two suspicious nodules were incidentally diagnosed in the donor's liver and immediately removed for anatomopathological evaluation. RESULTS: After pathological confirmation of carcinoma, probable cholangiocarcinoma, at frozen section, the patient was re-listed as national priority and a new liver transplantation was performed within 24 hours. The patient was discharged after 2 weeks. CONCLUSIONS: The screening for neoplasms in donated organs should be part of our strict daily diagnostic arsenal. Moreover, we argue that, for the benefit of an adequate diagnosis and the feasibility of a safer procedure, the adoption of imaging tests routine for the liver donor is essential, allowing a reduction of the costs and some potential risks of liver transplant procedure.

RESUMO RACIONAL: O transplante de fígado representa a melhor modalidade terapêutica na doença hepática crônica terminal, hepatite aguda grave e casos selecionados de tumores hepáticos. OBJETIVOS: Descrever um retransplante duplo em paciente do sexo masculino, diagnosticado com doença de Crohn e complicado com colangite esclerosante primária, hipertensão portal grave e colangiocarcinoma diagnosticado no fígado transplantado. MÉTODOS: Paciente do sexo masculino, 48 anos, diagnosticado com doença de Crohn há 25 anos e complicado com colangite esclerosante primária e hipertensão portal grave. Foi submetido a um transplante de fígado em 2018 devido a cirrose biliar secundária. Em 2021, foi diagnosticada recidiva de colangite esclerosante primária e indicado retransplante hepático. A hepatectomia do receptor foi de alta complexidade devido à trombose complexa da veia porta, exigindo extensa tromboendovenectomia. Foi realizada ultrassonografia intraoperatória com doppler hepático. Dois nódulos suspeitos foram diagnosticados incidentalmente no fígado do doador e imediatamente removidos para avaliação anatomopatológica. RESULTADOS: Após confirmação patológica de carcinoma, provável colangiocarcinoma, pela congelação, o paciente foi relistado como prioridade nacional, e novo transplante hepático foi realizado em 24 horas. O paciente teve alta após 2 semanas. CONCLUSÕES: O rastreamento de neoplasias em órgãos doados deve fazer parte de nosso estrito arsenal diagnóstico diário. Além disso, defendemos que, em benefício de um diagnóstico correto e da viabilidade de um procedimento mais seguro, a adoção de uma rotina de exames de imagem é essencial em doadores hepáticos, permitindo a redução dos custos e alguns riscos potenciais do procedimento de transplante hepático.

Abstract The objective of this study was to obtain and characterize a freeze-dried powder of taioba leaves (Xanthosoma taioba) as a source of bioactive substances and potential food applications. The fresh leaves were cooked and lyophilized for further analysis. Its bioactive contents expressed in flavonoids, ascorbic acid and total polyphenols were quantified. Total antioxidant activity (TAA) was determined by ABTS assays. The chemical composition via infrared spectroscopy (FTIR) and its microstructure were visualized by scanning electron microscopy (SEM). The bioactive contents of flavonoids, ascorbic acid and polyphenols were 17.15 mg/100 g, 58.3 mg/100 g and 24.15 mg Eq. of gallic acid/100 g, respectively. A high content of TAA was found. ABTS 37.35 (µg TE/g). The FTIR spectrum revealed high-intensity bands at 3350 cm-1, 2928 cm-1, 1637 cm-1, and 1055 cm-1 related to vibrations associated with typical bands of −OH groups present in cellulose membranes, hemicellulose, carbohydrates, lignin and water. The micrographs showed irregular structures of the ground leaves with a fibrous structure. These results indicate a high potential of this raw material in food formulations as a source of bioactives suitable for applications in various industrial segments.

Abstract Background Resistant hypertension (RH) is manifested by the presence of blood pressure values resistant to antihypertensive therapy. RH is highly prevalent among black individuals, increasing cardiovascular risk in this population and requiring effective control of this comorbidity. Objectives To investigate the medication profile and therapeutic adherence in black people with apparent RH. Methods This is a cross-sectional study, with a convenience sample of individuals with apparent RH. Data were obtained from medical records. Therapeutic adherence was assessed using the Morisky Therapeutic Adherence Scale of 8 items (MMAS-8) and statistical analysis was performed using the SPSS, version 23. Significance was set at p <0.05. Results Of the 120 individuals, 90 (75%) were women and 72 (60%) were black. Mean SBP was 153.09 (SD 25.59) mm Hg and mean DBP, 90.82 (SD 16.91) mm Hg, with a statistical difference in relation to the target pressure for SBP. Regarding the medication profile, 79.2% of the individuals used the recommended regimen for RH (ACEI / ARB + Diuretic + CCB), with the fourth most used drug being beta-blockers. The average score in MMAS-8 was 6.62 (SD 1.38) points, with 19.2%, 50.0%, and 30.8% showing low, medium, and high adherence, respectively. Conclusions It was evidenced that two-thirds of the individuals did not have high therapeutic adherence and not all used the ideal regimen for the management of RH, nor full doses. Thus, most individuals were probably affected by pseudoresistance, which was initially diagnosed as apparent RH. (Int J Cardiovasc Sci. 2021; [online].ahead print, PP.0-0)

RESUMO Objetivo A presença de fontes alimentares ricas em compostos bioativos na dieta humana torna-se fator preponderante no combate à etiologia de diversas patologias. Assim, o objetivo desta pesquisa foi avaliar o perfil de ácidos graxos, índices de funcionalidade cardiovasculares, compostos bioativos e padrão espectroscópico do óleo da pupunha vermelha e suas implicações para a saúde humana. Métodos O óleo foi obtido por extração via soxhlet; seu rendimento e qualidade foram determinados segundo as normas da Association of Official Analytical Chemists. Para o perfil de ácidos graxos, seguiram-se as recomendações práticas da American Oil Chemists’ Society e da International Organization for Standardization. Os carotenoides e polifenóis totais foram estipulados por espectrofotometria; a constituição de grupos químicos, por espectroscopia de infravermelho. Os índices antiaterogênico, antitrombogênico e hipocolesterolêmico foram obtidos por modelos matemáticos. Resultados Os resultados evidenciaram um óleo com boa qualidade, com base nos índices de acidez e peróxido (2,45±0,33mg KOH g-1 e 5,47±1,05mEq kg-1). O principal composto bioativo desse fruto foi o β-caroteno (832,4±0,64µg/100g). O perfil cromatográfico revelou um elevado teor de ácidos graxos saturados (53,74%); os insaturados (46,25%) se mostraram distribuídos em monoinsaturados (39,66%) e poli-insaturados (6,59%). Os índices de antiaterogênico, antitrombogênico e hipocolesterolêmico foram, em média, respectivamente 1,10; 2,04 e 0,84. O perfil espectroscópico apresentou destaques com variação de 2918,8cm-1 a 714cm-1. Conclusão Os resultados indicam que o consumo do conteúdo lipídico isolado da pupunha atua na proteção da saúde, em especial para a prevenção de agravos cardiovasculares.

ABSTRACT Objective The presence of dietary bioactive compounds in the human diet becomes a major factor in combating the etiology of different pathologies. Thus, the aim of this investigation was to evaluate the fatty acids profile, cardiovascular functionality indices, bioactive compounds and spectroscopic pattern of peach palm oil (pupunha oil) and their impact on human health. Methods The oil was obtained by soxhlet extraction; the oil yield and qualities were determined according to the standards of the Association of Official Analytical Chemists. For the fatty acids profile, the practical recommendations of the American Oil Chemists’ Society and of the International Organization for Standardization were followed. Total carotenoids and polyphenols were determined by spectrophotometry; the composition of the chemical groups was determined by infrared spectroscopy. The anti-atherogenic, antithrombogenic and hypocholesterolemic indices were obtained using mathematical models. Results The results showed good quality oil based on acid and peroxyde indices (2.45±0.33mg KOH g-1 and 5.47±1.05mEq kg-1). The main fruit bioactive compound was β-carotene (832.4±0.64µg/100g). The chromatographic profile showed a high saturated fatty acid content (53.74%); unsaturated (46.25%); fats were monounsaturated (39.66%) and polyunsaturated (6.59%). The antiatherogenic, antithrombogenic and hypocholesterolemic indices were, on average, 1.10, 2.04 and 0.84, respectively. The spectroscopic profile exhibited bands with variation from 2918.8cm-1 to 714cm-1. Conclusion The results indicate that the consumption of isolated lipid content of the pupunha palm oil provides health protection with emphasis on the prevention of cardiovascular diseases.

Resumo Objetivos: Descrever os aspectos da epidemia de microcefalia no Estado do Piauí. Métodos: Foram incluídos todos os casos de microcefalia congênita confirmados no estado entre 2015-2016 (n = 100). Os formulários de investigação do Centro Regional de Referência em Microcefalia foram analisados. Os casos descartados (n = 63) foram usados como grupo de comparação. Resultados: Em outubro, novembro e dezembro de 2015, as taxas de incidência atingiram 4,46, 6,33 e 3,86/1.000 nascidos vivos, respectivamente; 44 casos foram relatados na capital do estado. Entre as mães de casos confirmados e descartados, a frequência de erupção cutânea durante a gravidez foi 50/97 (51,5%) e 8/51 (15,7%), respectivamente (p < 0,001); 33 casos confirmados (35,9%) apresentaram um escore z de perímetro cefálico entre -2 e -3, 23 (25%) entre -3 e -4 e 8 (8,7%) apresentaram escore z inferior a -4. As tomografias computadorizadas cerebrais revelaram calcificações em 78/95 (82,1%) dos casos. Lisencefalia, hidrocefalia e agenesia do corpo caloso também foram observadas com mais frequência. Os achados oftalmológicos incluíram rarefação e atrofia do epitélio pigmentar da retina. Foram observadas ausência de emissões otoacústicas em 21/70 casos. Um recém-nascido também apresentou atrofia muscular dos membros inferiores. Não houve diferenças significativas nas taxas de vacinação para gripe, vacina difteria tétano e coqueluche acelular e hepatite B em qualquer grupo. Conclusões: O Estado do Piauí, como outros na região Nordeste, enfrentou, entre 2015 e 2016, uma epidemia de microcefalia congênita, supostamente relacionada à infecção congênita pelo vírus Zika, mais intensa na capital. Os desafios atuais incluem melhora do controle de vetores, pesquisa básica, ampliação de ferramentas de diagnóstico para exame pré-natal do vírus Zika, vacinas e cuidados de saúde para crianças afetadas.

Abstract Objectives: To describe aspects of the microcephaly epidemic in the state of Piauí. Methods: All cases of congenital microcephaly confirmed in the state between 2015 and 2016 were included (n = 100). Investigation forms of the Regional Reference Center for Microcephaly were reviewed. Discarded cases (n = 63) were used as a comparison group. Results: In October, November, and December 2015 incidence rates reached 4.46, 6.33 and 3.86/1000 live births, respectively; 44 cases were reported in the state capital. Among the mothers of confirmed and discarded cases, the frequency of skin rash during pregnancy was 50/97 (51.5%) and 8/51 (15.7%), respectively (p < 0.001); 33 confirmed cases (35.9%) had a head circumference z-score between −2 and −3, 23 (25%) between −3 and −4, and 8 (8.7%) had a z-score of less than −4. Head computer tomography scans revealed calcifications in 78/95 (82.1%) cases. Lissencephaly, hydrocephalus and agenesis of the corpus callosum were also frequently observed. Ophthalmic findings included retinal pigment epithelium rarefaction and atrophy. Absence of otoacoustic emissions was observed in 21/70 cases. One newborn also presented lower limb muscle atrophy. There were no significant differences in vaccination rates for influenza, diphtheria-tetanus-acellular pertussis, and hepatitis B in either group. Conclusions: The state of Piauí, like others in the northeastern region, faced an epidemic of congenital microcephaly between 2015 and 2016, presumably related to congenital Zika virus infection, more intense in the capital. Current challenges include the improvement of vector control, basic research, scaling-up of diagnostic tools for pre-natal screening of Zika virus, vaccines, and health care for affected children.

Resumo Objetivo: Compreender a percepção de usuários da atenção básica diante do sofrimento mental e da participação em um grupo terapêutico de convivência. Métodos: Pesquisa Convergente Assistencial de abordagem qualitativa realizada com quatro participantes de um grupo terapêutico em uma Estratégia Saúde da Família. Foi utilizada a entrevista semiestruturada, realizadas em fevereiro de 2018, e o software Interface de R pour les Analyses Multidimensionnelles de Textes et de Questionnaires para codificação dos dados. Resultados: O grupo terapêutico em saúde mental pode contribuir para a redução do estigma relacionado ao adoecimento mental e para promoção e prevenção em saúde mental utilizando como principais ferramentas tecnologias relacionais de cuidado, de baixo custo e acessíveis aos profissionais. Conclusão: O estudo contribuiu para inspirar novas ações e revitalizar práticas de saúde mental por meio de uma intervenção passível de ser implementada e que pode trazer benefícios aos envolvidos no contexto da Atenção Básica.

Methods: Convergent Care Research of qualitative approach performed with four participants of a therapeutic group in a Family Health Strategy. Was used the semi-structured interview, carried out in February 2018, and the software Interface de R pour les Analyses Multidimensionnelles de Textes et de Questionnaires for coding the data. Results: The therapeutic group in mental health can contribute to the reduction of stigma related to mental illness and to promotion and prevention in mental health, using relational technologies, low-cost and accessible for professionals. Conclusion: The study contributed to inspire new actions and revitalize mental health practices through an intervention that can be implemented and that can bring benefits to those involved in the context of Primary Care.

RESUMO: Avaliou-se a qualidade sensorial de patês de carne de ovelhas e cabras com diferentes fontes e percentagem de gordura (10% ou 30%, e barriga de porco ou azeite). Um painel de provadores treinado realizou análise sensorial e foram consultados consumidores para perceber as suas preferências. A análise generalizada de Procrustes foi usada para testar as diferenças encontradas pelos provadores. Os painelistas conseguiram encontrar diferenças sensoriais entre os patês de ovelhas e cabras e também percentagens de gordura. Os patês de cabra apresentaram valores mais elevados de suculência e os patês de ovelhas apresentaram maiores valores de intensidade de sabor. Patês com 10% de gordura de barriga de porco tiveram os maiores valores para os atributos de cor e coesividade e também intensidade de aroma, enquanto patês com 30% de azeite apresentaram os maiores valores para os atributos de textura, exceto coesividade. Embora as diferenças entre os patês tenham sido encontradas pelos membros do painel, os consumidores não mostraram uma preferência particular por um deles.

ABSTRACT: Sensory quality of sheep and goat pâtés with different sources and percentage of fat (10% or 30%, and pork belly or olive oil) were evaluated. A trained taste panel performed sensory analysis and consumers were consulted to perceive their preferences. Generalised Procrustes Analysis was used to test the differences found by panellists. Panellists were able to find sensory differences between sheep and goat pâtés, and also fat percentages. Goat pâtés had higher values of juiciness and sheep pâtés had higher values of taste intensity. Ten percent pork belly fat pâtés had the highest values of colour and cohesiveness attributes and also aroma intensity, while 30% olive oil pâtés had the highest values of texture attributes, except cohesiveness. Although, differences between pâtés were found by panellists, consumers had no particular preference for one of them.

Abstract This study aimed to prepare hydrogel containing Cymbopogon citratus (DC.) Stapf, Poaceae, volatile oil encapsulated in poly (d,l-lactide-co-glycolide) nanoparticles and to evaluate its in vitro anti-herpetic activity. Polymeric nanoparticles were prepared by solvent emulsification-diffusion method and incorporated in carbomer hydrogels. In vitro release profiles for the nanogel, loaded nanoparticles and hydrogel containing free oil were evaluated by dialysis. Inhibitory activities against Herpes simplex for the formulations were investigated in Vero cells. Hydrogel was developed using nanoparticles with mean diameter of 217.1 nm and negative Zeta potential (−20.5 mV). Volatile oil release profile showed a biphasic pattern with an initial faster release and subsequent sustained phase in all formulations. Nanogel strongly inhibited virus in a non-cytotoxic concentration, 42.16 times lower than free oil, 8.76 and 2.23 times than loaded nanoparticles and hydrogel containing free oil, respectively. These results highlight the potential of nanogel to protect oil against volatilization, control release and improve its anti-herpetic activity.

RESUMO Introdução: Em todo o mundo existem relatos de aumento das infecções causadas por Streptococcus pneumoniae resistentes aos antimicrobianos. Objetivo: Avaliar o perfil de suscetibilidade dos sorotipos de Streptococcus pneumoniae, associando-os com as doenças invasivas pneumocócicas (DIP), bem como com as terapias antimicrobianas. Método: Trata-se de um seguimento retrospectivo com enfoque em dados secundários de 1998 a 2013, na macrorregião nordeste do estado de São Paulo, Brasil, composta pelas regiões de Araraquara, Barretos, Franca e Ribeirão Preto, com 90 municípios. No Instituto Adolfo Lutz (IAL), as cepas isoladas de pacientes com DIP foram submetidas a identificação, sorotipagem e teste de suscetibilidade aos antimicrobianos. Resultados: Das 796 cepas analisadas, 14,8% (n = 118) apresentaram resistência a penicilina, sendo 3% (n = 24) com resistência intermediária e 11,8% (n = 94) com resistência plena, principalmente em pacientes com meningite. Para ceftriaxona, a resistência foi de 5,3%: 34 (4,3%) com resistência intermediária e 8 (1%) com resistência plena. Há de salientar que o maior nível de resistência das cepas foi observado para sulfametoxazol/trimetoprima (SMT): 350 (49,4%). Por outro lado, a suscetibilidade foi descrita acima de 90% para cloranfenicol: 99,6% (n = 696); eritromicina: 94,7% (n = 664); ceftriaxona: 94,7% (n = 754) e total para vancomicina. Entre os 18 sorotipos mais frequentes, 9V e 14 apresentaram menor suscetibilidade a SMT, penicilina e ceftriaxona; 19A a SMT e penicilina; 1 a SMT; 12F e 3 a cloranfenicol; 6A a SMT; 6B a eritromicina e 23F a penicilina. Conclusão: O monitoramento da resistência antimicrobiana do Streptococcus pneumoniae é fundamental para direcionar o tratamento empírico das doenças pneumocócicas.

ABSTRACT Introdution: There are reports worldwide about the increase in infections caused by Streptococcus pneumoniae resistant to antimicrobials. Objective: Evaluate the susceptibility profile of serotypes of Streptococcus pneumoniae associating them with pneumococcal invasive diseases (PID), as well as antimicrobial therapies. Method: This is a retrospective cross-sectional research involving secondary data from 1998 to 2013, in the northeastern macroregion of São Paulo state, Brazil, composed of Araraquara, Barretos, Franca and Ribeirão Preto regions, with 90 municipalities. At Instituto Adolfo Lutz (IAL), isolated strains from patients with PID were subjected to identification, serotyping and antimicrobial susceptibility testing. Results: From 796 strains analyzed, 14.8% (n = 118) were resistant to penicillin, being 3% (n = 24) with intermediate resistance and 11.8% (n = 94) with full resistance, especially in patients with meningitis. Moreover, resistance to ceftriaxone was 5.3%: 34 (4.3%) with intermediate resistance and 8 (1%) with full resistance. We point out that the greatest level of resistance profiles was observed against sulfamethoxazole/trimethoprim (SMT): 350 (49.4%). On the other hand, antimicrobial susceptibility was described above 90% to chloramphenicol: 99.6% (n = 696), erythromycin: 94.7% (n = 664), ceftriaxone: 94.7% (n = 754) and fully susceptible to vancomycin. Among the 18 most common serotypes, 9V and 14 showed less susceptibility to SMT, to penicillin and ceftriaxone; 19A to SMT and penicillin; 1 to SMT; 12F and 3 to chloramphenicol; 6A to SMT; 6B 23F to erythromycin and penicillin. Conclusion: Monitoring of Streptococcus pneumoniae antimicrobial resistance is essential to guide the appropriate empirical treatment of pneumococcal disease.

RESUMO Objetivo Avaliar atributos essenciais (acesso de primeiro contato, longitudinalidade, integralidade e coordenação da atenção) e derivados (orientação familiar e comunitária) da Atenção Primária em Saúde (APS) na percepção de usuários com e sem deficiência. Método Estudo transversal observacional utilizando o questionário Primary Care Assesment Tool em usuários autodeclarados com e sem deficiência de cinco unidades básicas de saúde de um município no qual 55% da população depende do SUS. Foram atribuídos escores para as respostas dadas a cada atributo de atenção primária à saúde avaliado. Resultados Participaram 93 usuários (67 com e 26 sem deficiência). Não houve diferença estatisticamente significante para nenhum dos atributos na comparação entre pessoas com e sem deficiência. Para o grupo como um todo, obtiveram-se escores acima do critério de corte: Grau de afiliação, Acesso de primeiro contato (utilização), Longitudinalidade e Coordenação da atenção (sistema de informação). Receberam pior avaliação: Acesso de primeiro contato (acessibilidade), Coordenação da atenção (integralidade do cuidado), Integralidade (serviços disponíveis e prestados) e Orientação familiar e comunitária. Os usuários reconhecem que as equipes identificam problemas de locomoção e movimentação, mas que existem falhas no reconhecimento de problemas para ouvir, falar e enxergar; e na orientação das pessoas que necessitam de auxílio e das que prestam cuidados. Conclusão Os usuários com e sem deficiência avaliaram de forma semelhante a atenção que recebem e indicaram fragilidades no reconhecimento de demandas específicas. Há necessidade de mudanças (estruturais e do processo de trabalho) para assegurar acessibilidade, integralidade do cuidado e orientação familiar e comunitária e, assim, melhor qualificar a APS.

ABSTRACT Purpose To evaluate the core (First Contact, Longitudinality, Comprehensiveness, and Coordination of Services) and derivative (Family Orientation, Community Orientation) attributes of primary health care (PHC) from the perspective of users with and without disabilities. Methods Observational, cross-sectional study using the Primary Care Assessment Tool (PCAT) with users with and without disabilities of five basic health units (BHU) in a municipality where 55% of the population depends on the Brazilian Unified Health System (SUS). Scores were assigned to the responses given to each of the PHC attributes evaluated. Results Study participants were 93 (67 physically disabled and 26 without disabilities) PHC users. No statistically significant differences were observed for any attribute on the comparison between the populations. For both groups, the attributes Degree of Affiliation, First Contact - Utilization, Longitudinality, and Coordination of Services - Information System received satisfactory (above cutoff) scores, whereas the attributes First Contact - Accessibility, Coordination of Services - Care Integration, Comprehensiveness, Family Orientation, and Community Orientation received unsatisfactory (below cutoff) scores. Users reported that the health teams are able to satisfactorily identify mobility issues, but there are failures in the recognition of problems of hearing, voice/speech and vision, and in the orientation of services available and services provided. Conclusion Users with and without disabilities evaluated the health care received similarly, indicating fragilities on the recognition of specific demands. Structural and work process changes should be conducted to ensure Accessibility, Comprehensiveness, and Family and Community Orientation, and thus increase the quality of PHC.

Abstract Introduction Infections caused by Streptococcus pneumoniae (pneumococcus) still represent a challenge for health systems around the world. Objective The objective of this study was to assess microbiological and clinical aspects in hospitalized patients with invasive pneumococcus disease between 1998 and 2013. Materials and methods This was a retrospective study that analyzed the results of pneumococcus identification, serotyping, and susceptibility testing found in the Adolfo Lutz Institute databank. Personal variables, medical history and clinical outcome of patients admitted with invasive pneumococcal disease were analyzed. These were obtained from records of a public teaching hospital – Hospital das Clínicas Faculdade de Medicina Ribeirão Preto. Results The sample comprised 332 patients. Patient age ranged from less than one month to 89 years old (mean 20.3 years) and the sample was predominately male. Pneumonia (67.8%) was the most common disease, accounting for 18.2% of deaths. Serotypes 14, 1, 3, 9V, 6B, 6A, 23F, 19A, 18C, 19F, 12F, and 4 were the most common (75.3%). Most patients, or 67.5%, were cured without any complication (success), 6.9% had some type of sequela (failure), and 25.6% died (failure). In the case of deaths due to meningitis, strains of fully penicillin resistant pneumococcus were isolated. Furthermore, 68.2% of patients who died presented some type of comorbidity. The 60 and older age group presented the most significant association (Odds Ratio = 4.2), with outcome failure regardless of the presence of comorbidity. Serotype 18C was the most significant risk factor both in raw analysis (Odds Ratio = 3.8) and when adjusted for comorbidity (Odds Ratio = 5.0) or age (Odds Ratio = 5.4). The same occurred with serotype 12F (respectively, Odds Ratio = 5.1, Odds Ratio = 5.0, and Odds Ratio = 4.7) Conclusion The present findings highlight the importance of IPD among young adults and older adults. In the era of conjugate vaccines, monitoring serotypes in different age groups is essential to assess the impact and adequacy of immunization.

A hidrocefalia de pressão normal (HPN) em geral ocorre entre 7ª e 8ª décadas de vida. Composta por uma tríade de sintomas (marcha, urinária e cognição), é uma das causas de demência potencialmente reversíveis, através do tratamento cirúrgico (implante de válvula cerebral). Exames complementares como o tap test em série (exame da punção lombar) podem melhorar o diagnóstico. O objetivo foi estudar o impacto do tap test sobre a cognição de portadores de HPN. 17 participante (sete homens e 10 mulheres), idade entre 76 e 41 anos e escolaridade de 9,1 anos, selecionados de um grupo de 53 sujeitos segundo critérios de Relkin foram avaliados em três etapas envolvendo domínio cognitivo e marcha: antes da punção (pré-tap); Após a primeira punção lombar (pós-tap 1); e no dia seguinte, após a nova punção (pós tap 2). Neste subgrupo análise estatística apontou diferença significativa entre os resultados pré-tap e póstap na prova de dígitos ordem direta (p<0,013), dígitos ordem inversa (p<0,013); fluência de animais (p<0,004) e na primeira etapa do teste de Stroop (p<0,033). Neste grupo os pacientes obtiveram melhora do desempenho em alguns testes de funções executivas, o que parece estar relacionado a melhora na velocidade de processamento mental.

Idiopathic normal pressure hydrocephalus (INPH) is a syndrome characterized by the classic triad of gait disturbance, potentially reversible dementia (progressive mental deterioration) and urinary incontinence. The dementia can involve executive dysfunction, memory failures and bradyphrenia. The criteria for referral for ventriculoperitoneal shunting are still controversial but include the results of a spinal tap, which is performed to simulate the effect of shunting and to analyze the impact of cerebrospinal fluid (CSF) drainage on the patient’s gait. Objective: To evaluate the immediate effect of a spinal tap on the executive functions of patients with a diagnosis of probable INPH. Methods: Seventeen participants (seven men and ten women) aged 76.41+6.24 years (mean+SD) with 9.01+4.40 years (mean+SD) of schooling were selected from a group of 130 individuals tested. The neuropsychological tests of executive functions used were the Stroop test, digit span task, verbal fluency (animals and letters) (FAR) and MMSE. Patients were evaluated in three stages: before the first CSF drainage and after the first and second CSF drainages. Results: Statistical analysis showed significant differences between the pre-spinal tap and post-spinal tap results in the forward (p<0.013) and backward (p<0.037) digit span task, the animal fluency (p<0.004) and the first stage of the Stroop test (p<0.033). Significant differences were not observed in the orientation test (WMS), mental control (WMS) or MMSE. Conclusion: Of the various tests performed, executive function tests most clearly showed changes resulting from CSF drainage in patients with normal pressure hydrocephalus (NPH). The improvement appears to be related to an improvement in mental processing speed.

The aim of the present study was to assess the prevalence of Haemophilus influenzaetype b (Hib) nasopharyngeal (NP) colonisation among healthy children where Hib vaccination using a 3p+0 dosing schedule has been routinely administered for 10 years with sustained coverage (> 90%). NP swabs were collected from 2,558 children who had received the Hib vaccine, of whom 1,379 were 12-< 24 months (m) old and 1,179 were 48-< 60 m old. Hi strains were identified by molecular methods. Hi carriage prevalence was 45.1% (1,153/2,558) and the prevalence in the 12-< 24 m and 48-< 60 m age groups were 37.5% (517/1,379) and 53.9% (636/1,179), respectively. Hib was identified in 0.6% (16/2,558) of all children in the study, being 0.8% (11/1,379) and 0.4% (5/1,179) among the 12-< 24 m and 48-< 60 m age groups, respectively. The nonencapsulate Hi colonisation was 43% (n = 1,099) and was significantly more frequent at 48-< 60 m of age (51.6%, n = 608) compared with that at 12-< 24 m of age (35.6%, n = 491). The overall resistance rates to ampicillin and chloramphenicol were 16.5% and 3.7%, respectively; the co-resistance was detected in 2.6%. Our findings showed that the Hib carrier rate in healthy children under five years was very low after 10 years of the introduction of the Hib vaccine.

OBJECTIVES: To determine the prevalence of pneumococcal serotypes and antimicrobial susceptibility in patients with meningitis, and to evaluate the implications for vaccine coverage. METHODS: Pneumococcal strains obtained from normally sterile fluids from patients admitted with meningitis were isolated at the Hospital de Clínicas of the Universidade Federal de Uberlândia, Minas Gerais State, and sent to the Instituto Adolfo Lutz, city of São Paulo, São Paulo State, for further identification, serotyping, and antimicrobial susceptibility determination. RESULTS: From April 1999 to April 2009, 338 pneumococcal strains were isolated, and 72 obtained from patients with meningitis, were analyzed. Patients' ages varied from one month to 82.2 years (mean of 18.4 ± 22.9 years; median of 5.2 years) and 46 (63.9%) patients were male. Strains were isolated from cerebrospinal fluid [66 occasions (91.7%)] and blood [6 occasions (8.3%)]. The most commonly identified serotypes were 14, 19F, 3, 7F, 6A, 6B, 10A, 18C, 23F, 5, and 34. Of the 20 [27.8%] oxacillin-resistant strains, 17 [23.6%] were resistant to penicillin and nine [12.5%] to ceftriaxone, both resistance patterns being more common in children aged two years or less and during the 2005-2009 period. CONCLUSIONS: Resistance to penicillin and ceftriaxone was detected in 23.6% and 12.5% of the strains, respectively, and predominated in children aged two years or less and during the 2005-2009 period. There were 24 different serotypes of pneumococcus and 79.8% of the serotypes were represented in the 7-valent conjugated vaccine [PVC7].
