ABSTRACT BACKGROUND: Residents play the role of teachers in almost one-quarter of their activities in residency programs. OBJECTIVE: To evaluate whether a 45-minute class using summarize, narrow, analyze, probe, plan, and select (SNAPPS) could improve psychiatry residents’ case discussion skills in diverse practical learning settings. DESIGN AND SETTING: This case-control, randomized, blinded study was conducted in a psychiatry hospital at Fortaleza-Ceará. METHODS: Using “resident as teacher” (RaT), objective structured teaching encounters (OSTEs), and SNAPPS, we conducted a study with 26 psychiatry residents. We analyzed video footage of psychiatric cases in three settings: outpatient, nursing, and emergency. An intervention was held two months later with the residents, who were then assigned to two groups: group A (lecture on SNAPPS) and group B (lecture on a topics in psychiatry). Shortly after the lectures, they were video recorded while discussing the same cases. Three blinded examiners analyzed the videos using an instrument based on the Stanford Faculty Development Program (SFDP-26). RESULTS: We found high internal consistency among external examiners and an interaction effect, group effect, and moment effect (P < 0.05). The residents who received the SNAPPS lecture scored significantly higher than their counterparts who received a traditional case presentation. CONCLUSION: This study indicates the efficacy of SNAPPS over traditional case presentation in all three settings as assessed by OSTEs and supports its implementation to improve the teaching of clinical reasoning.

Abstract: Introduction: Podcasts are audio broadcasts distributed over the internet that can be consumed through various platforms. From students to independent clinicians, the profile of medical podcast users is varied and a richer understanding of this user is essential for effective podcast development. Objective: To review the current literature on the use of podcasts as a health education tool and to understand the stages involved in preparing a medical podcast. Methods: The study is a narrative review developed through research in databases carried out at Unichristus University, in the city of Fortaleza-CE. An analysis of articles on podcasts or data on the prevalence of use, production or results of the use of podcasts for health education purposes was performed. A screening was performed by reading the abstracts and titles of 165 articles found in the Medline database, using the keywords ‘Podcasts’ and ‘medical education’, which resulted in 23 articles that met the inclusion criteria. Results: Based on the content found in the literature, the review was divided into seven categories: formats, episode duration, activities carried out while listening to a podcast, learning assessment, suggestions, developing a medical podcast and the podcast as a complementary teaching tool. Conclusion: the podcast is considered a promising medium of communication as a complementary tool to the traditional teaching methods, and educators should focus on developing evaluation processes for this technology, refining evidence-based guidelines for creating new podcasts.

Resumo: Introdução: Podcasts são transmissões de áudio distribuídas através de internet e podem ser consumidas por meio de plataformas variadas. De estudantes a clínicos independentes, o perfil de quem consome podcast médico é variado e uma compreensão mais rica deste usuário é essencial para desenvolvimento eficaz do podcast. Objetivo: Revisar a literatura atual sobre uso do podcast como ferramenta de educação médica e entender sobre as etapas de elaboração de um podcast médico. Métodos: O estudo trata-se de uma revisão narrativa desenvolvida através de pesquisa em bases de dados realizada na Universidade Unichristus, na cidade de Fortaleza-CE. Foi realizada análise de artigos sobre podcast ou com dados sobre prevalência de uso, produção ou resultados do uso de podcasts para fins de ensino médico. Foi feita triagem por resumo e título de 165 artigos encontrados na base de dados no Medline com as palavras-chave Podcasts e educação médica, que resultou em 23 artigos que preencheram os critérios de inclusão. Resultados: A partir do conteúdo encontrado na literatura, a revisão foi dividida em sete categorias: formatos, duração do episódio, atividades realizadas enquanto se escuta um podcast, avaliação do aprendizado, sugestões, desenvolvendo um podcast médico e o podcast como ferramenta de ensino complementar. Conclusão: O meio é considerado promissor como ferramenta complementar aos métodos tradicionais de ensino, devendo os educadores se concentrarem no desenvolvimento de processos de avaliação desta tecnologia, refinando diretrizes baseadas em evidências para criação de novos podcasts.

Abstract: Introduction: In Brazil, the lack of studies evaluating self-esteem in medical students is evident. In the academic field, it is known that individuals with high self-esteem and resilience are more likely to be successful. In previous studies, resilience was negatively associated with anxiety and Burnout Syndrome (BS). The BS has become a public health problem due to its increased incidence, especially among medical professionals during the coronavirus disease (COVID-19) pandemic, making it essential to expand and develop this issue. Objectives: To evaluate the level of self-esteem in medical students and its association with the degree of resilience and burnout during the pandemic. Method: A cross-sectional cohort study was conducted with medical students from the first to the eighth semesters at Centro Universitário Christus in Fortaleza - CE, Brazil. The students answered three surveys: in the beginning of the semester, at the midterms and during finals. We applied the Maslach Burnout Inventory, Wagnild and Young’s resilience scale, the Rosenberg Self-Esteem scale and a sociodemographic questionnaire. Results: Students with higher levels of emotional exhaustion showed less professional efficiency (P < 0.001). Disbelief and professional efficiency are inversely related (P < 0.001). There was no significant variation in the prevalence of burnout throughout the semester (P = 0.593). However, this prevalence has been high since the first period of the study. Additionally, the higher the resilience, the higher the self-esteem (P < 0.001). Conclusion: Medical students show elevated levels of BS. There was no statistically significant variation in burnout, resilience, and self-esteem throughout the semester. Therefore, further studies are necessary to analyze these variables.

Resumo: Introdução: No Brasil, é perceptível a escassez de estudos avaliando a autoestima dos alunos de Medicina durante a pandemia da Covid-19. No campo acadêmico, sabe-se que indivíduos com alta autoestima e resiliência são mais propensos ao sucesso. De acordo com a literatura atual, a resiliência foi negativamente associada à ansiedade e à síndrome de burnout (SB). A SB tornou-se um problema de saúde pública por causa do aumento de sua incidência, principalmente entre os profissionais de saúde durante a pandemia da doença por coronavírus 2019 (Covid-19), tornando imprescindível o aprofundamento desse tema. Objetivo: Este estudo teve como objetivo avaliar o nível de autoestima de estudantes de Medicina e sua associação com o grau de resiliência e de burnout durante a pandemia. Método: Trata-se de um estudo de coorte transversal com estudantes de Medicina do primeiro ao oitavo semestre realizado no Centro Universitário Christus, em Fortaleza, Ceará, Brasil. Foram realizados três pontos de corte: no início do semestre, no meio do semestre e no final. Aplicamos as escalas de Maslach, Wagnild e Young e Rosenberg, e um questionário sociodemográfico. Resultado: Estudantes com maior desgaste emocional apresentaram menor eficácia profissional (p < 0,001). A descrença e a eficácia profissional estão inversamente relacionadas (p < 0,001). Não houve variação significativa na prevalência de burnout ao longo do semestre (p = 0,593), mas essa prevalência foi alta desde o primeiro período do estudo. Além disso, quanto maior a resiliência, maior a autoestima (p < 0,001). Conclusão: Estudantes de Medicina apresentam altos níveis de burnout. Variações de burnout, resiliência e autoestima não se mostraram estatisticamente relevantes ao longo do semestre. Mais estudos são necessários para analisar essas variáveis.

Abstract: Introduction: Mobile applications are considered relevant in the health area. As rheumatology is a complex and prevalent specialty in clinical practice, the development of tools that favor learning becomes necessary. Objective: To evaluate the impact of a mobile platform on learning, obtaining the degree of satisfaction with the teaching tool and the effects on different scenarios of educational practice, including performance evaluation regarding questions about the theoretical contents of the platform. Methods: Quantitative, descriptive study, carried out in Christus University Center, located in Fortaleza, Brazil. Questionnaires prepared by the authors were used to assess the impact on the different active methodologies used in the institution and the degree of student satisfaction regarding their use. The estimate of the students’ cognitive gain was measured through pre-test and post-test, using the multiple-choice format. Results: The questionnaires were applied to 71 students to assess the effects and satisfaction. A total of 90 students participated in the pre-test and 32 in the post-test phase. The platform showed a positive perception of learning for 83.1% of the students, with greater impact on lectures, although there was a greater impact on the simulation scenarios in the eighth semester. A good degree of satisfaction was observed in 94.4% of the answers, with improved performance in the tests, increasing from 43.7% to 63.3% (P < 0.001). Conclusions: The development and application of the mobile platform in rheumatology showed excellent results, with favorable effects on the teaching of the specialty, disclosing a good degree of satisfaction regarding its use and better performance on the questioning about the theoretical contents of the platform.

Resumo: Introdução: Aplicativos móveis são considerados relevantes na área da saúde. Sendo a reumatologia uma especialidade complexa e prevalente na prática clínica, torna-se necessário o desenvolvimento de ferramentas que favoreçam a aprendizagem. Objetivo: Este estudo teve como objetivo avaliar o impacto no aprendizado que uma plataforma móvel proporcionou, obtendo o grau de satisfação com a ferramenta de ensino e a repercussão nos diferentes cenários de prática educacional, incluindo avaliação de performance quanto a questionamentos do conteúdo teórico da plataforma. Método: Trata-se de estudo transversal com delineamento descritivo e abordagem quantitativa do tipo analítico, realizado no Centro Universitário Christus, localizado em Fortaleza, Brasil. Pesquisaram-se, por meio de questionários elaborados pelos autores, a repercussão nas diferentes metodologias ativas presentes na instituição e o grau de satisfação dos alunos quanto ao uso. A estimativa do ganho cognitivo dos alunos foi mensurada por meio de pré-teste e pós-teste, no formato de múltiplas escolhas. Resultado: Participaram 71 alunos durante do questionário sobre a repercussão e a satisfação. Nas questões de múltiplas escolhas, 90 alunos participaram do pré-teste e 32 do pós-teste. A plataforma apresentou percepção positiva na aprendizagem para 83,1% dos alunos, havendo maior impacto nas aulas expositivas, embora no oitavo semestre tenha havido maior repercussão nos cenários de simulação. Houve boa satisfação em 94,4% das respostas, com melhora no aproveitamento quanto às questões de múltiplas escolhas, evoluindo de 43,7% para 63,3% (p < 0,001). Conclusão: A elaboração e a aplicação da plataforma móvel em reumatologia obtiveram excelentes resultados, com repercussão favorável no ensino da especialidade e boa satisfação quanto ao uso e melhor performance no que concerne ao questionamento sobre os conteúdos teóricos da plataforma.

Abstract Introduction: Psychiatric diseases are increasing in prevalence in recent decades, being also the pathologies in which stigmatizing attitudes are most often observed. Objective: To translate the MICA-4 scale into Portuguese with cross-cultural adaptation for use in Brazil and to verify the possibility for the contribution of the tool to assess improvements in medical training through its application to medical students. Method: Seven steps were required for its translation and validation. Moreover, the work consisted of applying the test in two moments to a group of 60 medical students who participated in an internship rotation in a psychiatric hospital in a Brazilian city. Result: The validation of the MICA-4 scale consisted of the steps of translation, synthesis, back-translation, expert committee, pre-test, textual verification and application, which were successfully performed. The scale achieved an agreement among the experts and there was no difficulty among the students during the test application. In the data analysis after application of the questionnaire during the two moments, of the 16 items of the scale, item 9 obtained statistical relevance (p<0.05). Conclusion: This study analyzed the students’ perception of stigmatizing situations, and most of the answers were consistent with less prejudiced actions even before the internship experience, being corroborated after this period. The issues that still showed stigmatizing attitudes demonstrate the need to improve teaching tools that can reduce these negative attitudes and contribute to the training of good professionals and, consequently, better quality of care.

Resumo Introdução: As doenças psiquiátricas estão em crescente prevalência nas últimas décadas, sendo também as patologias nas quais mais se observam atitudes estigmatizantes. Objetivo: Este estudo teve como objetivos traduzir a escala MICA-4 em língua portuguesa com adaptação de forma transcultural para uso no Brasil e verificar a possibilidade de a ferramenta contribuir para avaliações de melhorias na formação médica por meio da aplicação em estudantes de Medicina. Método: Foram sete etapas para sua tradução e validação. Além disso, o trabalho consistiu na aplicação do teste em dois momentos em um grupo de 60 estudantes de Medicina que participaram de um rodízio de estágio em um hospital psiquiátrico do Brasil. Resultado: A validação da escala MICA-4 consistiu nas etapas de tradução, síntese, back-translation, comitê de experts, pré-teste, averiguação textual e aplicação, que foram executadas com sucesso. A escala obteve concordância entre os experts, e não houve dificuldade entre os estudantes durante a aplicação do teste. Na análise dos dados após aplicação do questionário durante os dois momentos, dentre os 16 itens da escala, o item 9 obteve relevância estatística (p < 0,05). Conclusão: Este estudo analisou a percepção dos estudantes em relação a situações estigmatizantes, e a maioria das respostas foi condizente com ações menos preconceituosas antes mesmo da experiência do estágio, sendo corroboradas após esse período. As questões que ainda expressaram atitudes estigmatizantes demonstram a necessidade de aprimoramento de ferramentas de ensino que possam diminuir essas atitudes negativas e contribuir para a formação de bons profissionais e, consequentemente, melhor qualidade de atendimento.

Resumo: Introdução: A residência médica representa um momento desafiador, com elevado nível de exigência e necessidade de desenvolvimento de inúmeras competências pessoais e profissionais. Com base nisso, implantou-se, em abril de 2019, um programa de mentoria inédito para 30 residentes de clínica médica, buscando oferecer suporte pessoal e estimular desenvolvimento profissional. Relato de experiência: O programa de mentoria foi estruturado em abril de 2019, com 30 residentes de clínica médica e seis mentores com experiência em preceptoria clínica. Com duração de 11 meses, o programa abordou temas não usualmente discutidos na formação desses especialistas, como profissionalismo, empatia, burnout e qualidade de vida, gestão de tempo, gestão de conflitos e role-model. Os encontros ocorriam mensalmente, com local definido pelo grupo. Após o fim da atividade, foi realizada pesquisa de opinião utilizando questionário padronizado com respostas em escala Likert para avaliação de relevância dos temas discutidos e impacto na prática dos médicos residentes. Obtiveram-se 90% de respostas, nas quais se destacou a relevância dos temas burnout e qualidade de vida e empatia. Dos residentes, 96,3% afirmaram que as discussões mudaram a forma deles de pensar e agir profissionalmente. Destacaram que o mentoring “abria a mente acerca de diversidade de opiniões, permitindo crescimento pessoal e profissional” e que “Gerir o tempo, conflitos, burnout estão presentes no nosso dia a dia e também fazem parte da medicina que praticamos”, pontuando dificuldade de consenso entre os membros do grupo para definição do horário e dia a ser realizada a reunião. A atividade demonstrou avaliação positiva dos mentorados. Discussão: Na formação de um médico residente, o programa de mentoria melhora a autoconfiança e promove um impacto positivo na carreira profissional apesar das dificuldades enfrentadas. A mentoria funciona ainda como suporte psicossocial, pois cria-se um livre acesso entre residente e preceptor, o que culmina, na nossa avaliação, em uma excelente relação entre mentores e mentorados. Conclusão: No Ceará, o programa de mentoring para os residentes de clínica médica foi inédito. Houve efeito positivo, em que temas importantes foram discutidos com mais profundidade, com maior crescimento pessoal e profissional. Os autores sugerem que o mentoring seja mantido e incluído como atividade essencial no currículo dos programas de residência médica.

Abstract: Introduction: Medical residency represents a challenging moment, with a high level of demand and the need to develop several personal and professional skills. Therefore, in April 2019, the unprecedented mentoring program for thirty internal medicine residents was implemented, seeking to offer personal support and encourage professional development. Experience Report: A mentoring program was structured in April 2019, lasting 11 months, consisting of 30 internal medicine residents and 6 mentors with experience in clinical preceptorship, covering topics that are not usually discussed in the training of these specialists, such as professionalism, empathy, burnout and quality of life, time management, conflict management and role-model. The meetings took place monthly, in a location defined by the group. After the end of the activity, an opinion poll was conducted using a standardized questionnaire with answers arranged on a Likert scale to assess the relevance of the discussed topics and the impact on the medical residents’ practice. A 90% response rate was obtained, highlighting the relevance of the topic Burnout and quality of life and empathy. A total of 96.3% of residents stated that the program changed their mind about their professional behavior. They declared that mentoring “opened their mind about the diversity of opinions, allowing personal and professional growth” and that “to manage time, conflicts, burnout are present in our daily lives and are also part of the medicine we practice”. However, there were complaints about some difficulties among the group members to define the time and day when the meetings should be held. In summary, the activity had a positive evaluation from the mentees. Discussion: There is a positive impact of the mentoring program on the training of a resident doctor, improving self-confidence and having a positive impact on the professional career despite the faced difficulties. Mentoring also serves as a psychosocial support, creating free access between the resident and the preceptor, culminating, based on our evaluation, in an excellent relationship between mentors and mentees. Conclusion: In Ceará, the mentoring program for internal medicine residents was unprecedented. There is a positive effect, in which important topics were discussed in more depth, with greater personal and professional growth. We suggest that mentoring be maintained and included as an essential activity in the curriculum of Medical Residency Programs.

Resumo: Introdução: A pandemia ocasionada pelo vírus Sars-CoV-2 acelerou uma revolução educacional que repercutiu na assistência à saúde e no ensino médico, e gerou algumas inseguranças e incertezas. O artigo relata a experiência do Centro Universitário Christus (Unichristus) quanto às modificações ocorridas nos cenários práticos de vivências clínicas durante o período do primeiro semestre de 2020, marcado pelo distanciamento social. Relato de Experiência: Os atendimentos na Clínica Escola de Saúde (CES) foram suspensos, e os hospitais conveniados passaram a não receber alunos, inviabilizando os cenários de vivência, resultando no início de um projeto caracterizado por atendimento a pacientes com infecção por coronavírus por meio da telemedicina. Havia a participação presencial de equipe de enfermagem, enquanto os alunos do oitavo semestre do curso de Medicina presenciavam a consulta por meio de compartilhamento da tela pelo programa Google Meet®. Após concluída a consulta, havia uma discussão sobre o caso e outros aspectos relevantes, de maneira similar ao que ocorreria em ambiente de vivência. Em paralelo ao projeto, os estudantes também assistiam a aulas expositivas que abordavam aspectos da doença desde o nível primário até o terciário de saúde. Discussão: De acordo com a tendência mundial, houve atendimento e ensino por um meio flexível, inovador, acessível e seguro, o gerou oportunidade de desenvolvimento profissional e inovações no ensino médico. A experiência com a telemedicina pode ser complementada por um e-learning, possibilitando o desenvolvimento de um novo modelo de ensino híbrido. Conclusão: A circunstância atual talvez promova alguma perda educacional, como a impossibilidade de se treinar exame físico e de interagir melhor com a equipe de saúde e com os pacientes, entretanto os recursos tecnológicos podem gerar oportunidades para mudanças, aprimoramento e desenvolvimento de metodologias de ensino, em concordância com a geração atual de nativos digitais.

Abstract: Introduction: The pandemic caused by the SARS-CoV-2 virus has accelerated an educational revolution, with implications for health care and medical education, generating some insecurities and uncertainties. The article reports the experience of Centro Universitário Christus (Unichristus) about the changes that occurred in the practical scenarios of clinical experiences during the first semester of 2020, marked by social distancing. Experience Report: The services at Clínica Escola de Saúde (CES) were suspended and the associated hospitals stopped receiving students, making the experience of real-life scenarios unfeasible, resulting in the beginning of a project characterized by care of patients with coronavirus infection through telemedicine. The face-to-face nursing team’s participation occurred concomitantly, while students attending the eighth semester of the medical course were present at the consultation through screen sharing using the Google Meet ® program. After the consultation was concluded, there was a discussion about the case and other relevant aspects, similarly to what would happen with a face-to-face experience. In parallel with the project, students also attended lectures that addressed aspects of the disease from primary to tertiary level of health care. Discussion: Patients received care and students were taught through a flexible, innovative, accessible and safe media, following a worldwide trend, generating opportunities for professional development and innovations in medical education. The experience with telemedicine can be complemented by e-learning, allowing the development of a new hybrid teaching model. Conclusion: The current circumstances may result in some educational loss, such as the impossibility to perform physical examinations and to interact better with the health care team and patients; however the technological resources can result in opportunities for changes, improvement and development of teaching methodologies, in line with the current generation of digital natives.

Resumo: Introdução: O Mini-CEX é um método de avaliação que abrange os seguintes domínios: anamnese, exame físico, aconselhamento, julgamento clínico, organização e profissionalismo. Foi testado e validado para utilização em qualquer cenário de prática. Com sua característica de fornecer feedback após uma avaliação clínica, o Mini-CEX também serve como um método de formação para guiar o desenvolvimento profissional de formandos e formadores, promovendo maior retenção de conhecimento no corpo discente e fornecendo, continuamente, informações para que o estudante perceba o quão distante está dos objetivos almejados. O objetivo deste estudo foi verificar a percepção de internos, residentes e preceptores da clínica médica (CM) sobre instrumento Mini-CEX. Método: Trata-se de um estudo qualitativo realizado pela técnica de grupo focal no período de fevereiro a julho de 2017. Participaram 20 internos, 13 residentes e cinco preceptores de CM. Constituíram-se seis grupos focais, dois com os internos, dois com residentes e dois com docentes, com perguntas semiestruturadas que identificaram as percepções, por meio da metodologia empregada, na qualidade da avaliação e eventuais repercussões para o processo de ensino-aprendizagem. Resultados: No grupo focal dos internos, o momento de feedback da avaliação foi considerado essencial para o processo de aprendizado embora a avaliação a beira-leito tenha se mostrado tensa pela presença do tutor. Os residentes relataram que a avaliação foi válida, pois os levou a revisar alguns pontos da literatura médica, além de estimular o raciocínio clinico diante de uma situação real. Os preceptores validaram a importância do feedback para os avaliados e identificaram a avaliação a beira-leito como momento de melhor análise das individualidades. Conclusão: Por meio das percepções dos grupos em foco, identificamos o Mini-CEX como ferramenta fundamental para o processo de ensino-aprendizagem de todos os envolvidos e a necessidade de estruturação do momento de feedback para um resultado mais eficaz. Durante a avaliação a beira-leito, foi identificado o estímulo ao raciocínio clínico como ponto positivo e o estranhamento, a ansiedade e a tensão como pontos negativos.

Abstract: Introduction: Mini-CEX is an evaluation method that covers the domains: anamnesis, physical examination, counseling, clinical judgment, organization, and professionalism. It has been tested and validated for use in any practice scenario. With its characteristic of providing feedback after a clinical assessment, the Mini-CEX can also be used as a training method to guide the professional development of students and teachers, promoting greater knowledge retention in undergraduate students and continuously providing information for students to realize how far they are from the desired objectives. The aim of this study was to assess the perception of interns, residents, and preceptors of Internal Medicine (IM) regarding the Mini-CEX instrument. Methods: Qualitative study, using the focus group technique, carried out from February to July 2017. Twenty interns, thirteen residents, and five IM preceptors participated. It consisted of six focus groups, two with interns, two with residents, and two with preceptors, using semi-structured questions that identified perceptions, through the methodology used, on the quality of the evaluation and possible repercussions for the teaching-learning process. Results: In the focus group of interns, the feedback moment of the assessment was considered essential for the learning process, although the bedside assessment was tense due to the preceptor’s presence. The residents reported that the evaluation was a valid one, as it led them to review some points in the medical literature, in addition to stimulating clinical reasoning in the face of a real situation. The preceptors validated the importance of the feedback for those who were evaluated and identified the bedside assessment as a moment for the best analysis of the individualities. Conclusion: Through the perceptions of the groups in focus, the mini-CEX was identified as a fundamental instrument for the teaching and learning process of all those involved and the need to structure the moment of feedback aiming to attain a more effective result. During the bedside assessment, the stimulus to clinical reasoning was identified as a positive point and the strangeness, anxiety, and tension as negative points.

RESUMO Historicamente, várias doenças são causadoras de estigma. O estigma e o preconceito em torno da doença mental existem amplamente em todo o mundo e pesquisas têm mostrado que a população em geral tem conhecimentos limitados sobre doenças mentais. Entre os transtornos mentais, a esquizofrenia é uma das mais estigmatizadas. Os estudantes de Medicina, por também fazerem parte da sociedade, não ficam imunes ao estigma em relação a pessoas com transtornos mentais. Vários estudos sugeriram que a educação psiquiátrica do estudante de Medicina, principalmente as experiências que envolvem contato direto com o paciente, como o internato, pode ter um impacto positivo, como no engajamento direto ao atendimento do paciente, interesse em participar de outras atividades como terapia grupal, gerenciamento de casos, além do entendimento de que pacientes com condições psiquiátricas podem ser tratados com sucesso. A maioria dos médicos, no entanto, recebe pouco treinamento ao interagir com pacientes com doenças mentais. Geralmente sentem-se desconfortáveis ou ineficazes ao se comunicarem com eles, mesmo sobre queixas físicas. O objetivo deste estudo foi avaliar se o estágio do internato em um hospital psiquiátrico de Fortaleza diminui o estigma dos alunos de Medicina em relação à doença mental. Foi aplicado o questionário validado AQ-9 aos estudantes de Medicina no período inicial do estágio do internato em Psiquiatria e repetida a mesma avaliação no final do período. Do total de 88 estudantes, observou-se que 37 (42%) eram do sexo masculino e 51 (58%) eram do sexo feminino. A média de idade foi de 24,68 anos. Pôde-se observar que houve diferença entre os três primeiros domínios do AQ-9, que evidenciaram, respectivamente, uma diminuição significativa em piedade (p = 0,029), periculosidade (p = 0,004) e medo (p < 0,001). Admite-se que existe estigma na população de estudantes analisada e que o estágio em Psiquiatria do internato de duas faculdades de Medicina estudadas reduziu significantemente três dos nove domínios avaliados. Apenas o gênero como dado sociodemográfico influenciou o resultado. Alunos do sexo feminino apresentaram maior média do que os alunos do sexo masculino em relação ao domínio medo, enquanto os alunos do sexo masculino apresentaram maior média do que os alunos do sexo feminino em relação ao domínio segregação. Fortalece-se a importância do estágio em Psiquiatria durante o internato para além do aprendizado técnico, já que o mesmo tem a capacidade de diminuir o estigma em relação aos pacientes psiquiátricos, principalmente os pacientes esquizofrênicos.

ABSTRACT Historically, there is stigma associated to several diseases. There are widespread stigma and prejudice regarding mental illness throughout the world and research has shown that the general population has limited knowledge about mental illness. One mental disorder in particular, schizophrenia, is highly stigmatized. As members of society, medical students are not immune to stigma assigned to people with mental disorders. Several studies have suggested that the psychiatric training, especially when involving direct contact with the patient, such as medical internship, can have a positive impact on the medical student through their direct engagement in patient care, interest in participating in other activities such as therapy group management, case management, and gaining an understanding that patients with psychiatric conditions can be treated successfully. Most doctors, however, receive little training when interacting with mental illness patients. They usually feel uncomfortable or ineffective in communicating with them, even regarding physical complaints. The objective of this study was to evaluate whether the internship in a psychiatric hospital in Fortaleza reduces the stigma of medical students in relation to mental illness. AQ-9 validated questionnaire was applied to medical students in the initial period of the internship in psychiatry and repeated at the end of the period. From a total sample of 88 students, 37 (42%) were male and 51 (58%) were female, and the mean age was 24.68 years. There was a noticeable difference between the first three domains of AQ-9, which showed a significant decrease in pity (p = 0.029), danger (p = 0.004) and fear (p < 0.001). Stigma does seem to reside in the student population analyzed and the psychiatry internship of the two medical schools studied significantly reduced the results in three of the 9 domains evaluated. Of the sociodemographic variables, only gender influenced the result. Female students presented higher scores than male students in relation to the fear domain while male students presented a higher average than their female counterparts in relation to segregation. The importance of the psychiatry internship which goes beyond technical learning is reinforced, since it has the capacity to reduce stigma in relation to psychiatric, and especially schizophrenic, patients.

RESUMO A residência médica, instituída legalmente no Brasil em 1977 pelo Decreto nº 80.281, é considerada a melhor estratégia de ensino em cenário de prática, sendo o padrão ouro da especialização médica. A característica mais marcante da residência é o treinamento em serviço, que articula ensino no cenário de prática, além de construir um perfil profissional. Os residentes desenvolvem também a função docente. Pesquisas americanas estimam que exerçam tal função em boa parte de suas atividades, chegando a um quarto do período total de tempo de seus programas de residência. Esse processo de ensino-aprendizagem durante a residência ainda é pouco estudado, principalmente no Brasil. O objetivo deste estudo foi realizar uma ampla revisão narrativa sobre RaT (Resident as Teacher), tema pouco explorado na literatura brasileira, avaliando historicamente o processo de ensino-aprendizagem dos programas de residência médica. Foi realizada uma revisão na literatura acerca do processo de ensino-aprendizagem da residência médica no Brasil e no mundo. Diversos países estão implementando treinamentos formais de ensino denominados programas de Resident as Teacher (RaT). Somente nos EUA, mais de 50% dos programas de residência têm alguma forma de treinamento RaT. Vários programas foram desenvolvidos e se diferenciam no conteúdo, na duração e no formato, porém são baseados em atributos de ensino considerados essenciais ao ensino. No que diz respeito ao conteúdo, os programas RaT enfatizam predominantemente o modelo preceptor minuto (One Minute Preceptor – OMP), a estrutura de ensino clínico do Programa de Desenvolvimento da Faculdade de Stanford ou os domínios mostrados por Irby como essenciais à excelência em ensino clínico. Em conclusão, sugere-se que os programas brasileiros de residência médica invistam em estudos e, consequentemente, em estratégias efetivas para aprimorar as técnicas de ensino para médicos residentes.

ABSTRACT The medical residency program, legally established in Brazil in 1977, is considered the best practical teaching strategy and it’s a gold standard to physician specialization. Resident, house officer and registrar are synonymous to refer to post medical training after internship, who attending a program or programme, depends on the region of the globe you are. The teaching of the residency training program is conducted during patient care in all settings (i.e., bedside teaching), which brings together teaching in a practical setting and the improvement of a professional profile. Residents also develop their teaching skills. American researchers estimate that residents acts as teachers in almost one quarter of their residency programs. This teaching-learning process hasn’t been thoroughly studied, especially in Brazil. The aim of this study was to do a narrative review about Rat (Resident as Teacher), an issue not explored in the Brazilian literature, evaluating the teaching-learning process in the medical residency programs. A review in the Brazilian and worldwide literature was conducted. In many countries, several formal training courses are being implemented worldwide under the “Resident as Teacher – RaT” denomination. In the United States, more than half of the residency programs have RaT training. The developed programs are different in their approach, duration and format. However, they are all based on attributes considered fundamental for teachers. Regarding content, RaT programs emphasize the One Minute Preceptor (OMP) model, the clinical teaching structure of the Stanford Faculty Development Program, or the domains shown by Irby to be essential for the excellence of clinical teaching. In conclusion it would be important for Brazilian programs to develop studies and, consequently, effective strategies to improve RaT.

RESUMO Introdução Na residência médica, os residentes exercem o duplo papel de professor e aprendiz. Treinamentos de como ensinar residentes a exercer a função de docência com qualidade estão sendo implementados em todo o mundo e são denominados programas de Residente como Professor (Resident as Teacher – RaT). De acordo com as Diretrizes Curriculares Nacionais do Curso de Graduação em Medicina, pode-se aprender a estabelecer objetivos educacionais e matriz de competência, além de habilidades como comunicação, empatia, mediação de conflitos, entre outras. A implementação desses programas deve ser baseada na compreensão do contexto de cada residência médica na qual se deseja inseri-los, e os residentes também devem conhecer as potencialidades de cada cenário de atuação da sua especialidade para promover uma docência eficaz. Objetivo O objetivo do estudo foi o desenvolvimento de uma estrutura de programa de treinamento de docência (RaT) dividido em módulos e detalhado com suas respectivas referências bibliográficas. Metodologia Realizou-se uma revisão de literatura sobre ensino médico e testaram-se alguns métodos em cenário de prática durante uma dissertação de mestrado, utilizando a técnica SNAPPS, e durante uma tese de doutorado, utilizando a técnica OMP. Resultado Foi construído um programa de treinamento de docência nas residências médicas baseado nos resultados positivos encontrados na literatura sobre ensino médico. Propõe-se que cada módulo tenha a duração de um mês aproximadamente, com atividades de um turno por semana. O formato da discussão pode variar de acordo com cada especialidade e com o número de residentes de cada programa. Sugere-se a realização de flipped classroom (sala de aula invertida) com o envio do referencial bibliográfico de suporte sobre os temas aos residentes por e-mail uma semana antes da realização de cada módulo. Diversas atividades podem ser postas em prática em cada módulo para sedimentação do material estudado. Conclusão Após a estruturação de um roteiro a ser implementado pelos programas de residência médica no Brasil, espera-se que os residentes desempenhem com maior eficácia o papel que muitos já exercem na prática e que consequentemente o nível de aprendizagem dos alunos de graduação e internos melhore.

ABSTRACT Introduction In medical residency, residents play the dual role of teachers and learners. Training programs aiming at teaching residents how to become teachers are being implemented around the world and are referred to as ‘Resident as teacher’ (RaT) programs. According to the National Curricular Guidelines of the Medical Undergraduate Course, one can learn to establish educational goals and competence matrix, as well as skills such as communication, empathy, mediation of conflicts, among others. The implementation of these programs should be based on the understanding of the context of each medical residency where they are to be included and the residents should also know the potential of each scenario of their specialty to promote an effective teaching. Objective The objective of the study was the development of the structure of a Teaching Training Program (Resident as Teacher) divided into modules and detailed with their respective bibliographical references. Method a literature review was performed on medical education and some methods were tested in a practice scenario during a master’s degree dissertation, using the SNAPPS technique, and during a doctoral thesis presentation using the OMP technique. Outcome A Teaching Training Program was built in medical residencies based on the positive results found in the medical education literature. It is proposed that each module has a duration of approximately one month, with activities lasting one shift per week. The format of the discussion may vary with each specialty and with the number of residents in each program. It is suggested that a flipped classroom situation be carried out, sending the bibliographic reference of support on the subjects to the residents by email one week before the completion of each module. Several activities can be implemented in each module to consolidate the studied material. Conclusion After structuring a script to be implemented by the residency programs in Brazil, residents are expected to more effectively play the role that many already exercise in practice and that the level of learning of undergraduate and internship students consequently improves.

RESUMO INTRODUÇÃO Feedback é uma informação provida pelo professor para melhorar o desempenho do estudante. Embora seja uma importante ferramenta de ensino e seja desejada pelos discentes, ainda é utilizada de maneira insuficiente. OBJETIVO Este estudo criou um manual para uso de feedback estruturado com base nos resultados de grupos focais que verificaram a percepção dos alunos do internato de Medicina sobre o tema. MÉTODOS Trinta e um alunos do internato de Medicina de uma instituição de ensino superior participaram de entrevistas de grupo focal sobre suas impressões relativas ao feedback. Os dados foram analisados pela Análise de Conteúdo de Bardin. RESULTADOS Os alunos compreendem o que é feedback e o vivenciam por meio das metodologias ativas de ensino. Reconhecem sua importância para o aprendizado, mas se queixam da baixa frequência desse instrumento. Sobre como gostariam de receber o feedback e as virtudes inerentes aos docentes e discentes para o sucesso do mesmo, notou-se o alinhamento de suas ideias com o que diz a literatura. CONCLUSÃO Os alunos conhecem feedback e o vivenciam na prática, reconhecendo sua importância para o aprendizado e sendo receptivos a ele. Desenvolveu-se um manual com estratégia de feedback estruturado voltado ao uso em ambulatórios didáticos na graduação em Medicina.

ABSTRACT INTRODUCTION Feedback is information provided by the teacher to improve student performance. Despite being an important teaching tool, and desired by the students, it is still not sufficiently used. OBJECTIVE This study created a manual for the use of feedback, structured based on the results of focus groups that explored the views of medical school interns on the subject. METHODS 31 medical interns of a Higher Education Institution took part in focus group interviews to gather their impressions on feedback. The data were analyzed using Bardin Content Analysis. RESULTS The students understand what feedback is, and experience it through active teaching methodologies. They recognize its importance for learning; however they complain that they do not receive feedback often enough. As to how they would like to receive feedback, and the virtues inherent to teachers and students for it success, it was found that the students’ ideas are aligned with the literature. CONCLUSION Students know what feedback is, and experience it in practice; they recognize its importance for learning and are receptive to it. A manual was developed with a structured feedback strategy for use in university undergraduated students

ABSTRACT BACKGROUND: There is extensive evidence, mainly from the United States and Canada, that points towards the need to train medical residents in teaching skills. Much of the “informal curriculum”, including professional values, is taught by residents when consultants are not around. Furthermore, data from the 1960s show the importance of acquiring these skills, not only for residents but also for all doctors. Teaching moments can be identified in simple daily situations, like discussing a clinical situation with patients and their families, planning patients’ care with the healthcare team or teaching peers and medical students. The aim here was to examine the significance of resident teaching courses and estimate the effectiveness of these courses and the state of the art in Brazil. METHODS: We conducted a review of the literature, using the MEDLINE, PubMed, SciELO and LILACS databases to extract relevant articles describing residents-as-teachers (RaT) programs and the importance of teaching skills for medical residents. This review formed part of the development of a doctoral project on medical education. RESULTS: Original articles, reviews and systematic reviews were used to produce this paper as part of a doctoral project. CONCLUSIONS: RaT programs are important in clinical practice and as role models for junior learners. Moreover, these educational programs improve residents’ self-assessed teaching behaviors and teaching confidence. On the other hand, RaT program curricula are limited by both the number of studies and their methodologies. In Brazil, there is no such experience, according to the data gathered here, except for one master’s thesis.

O diabetes mellitus (DM), uma doença endócrino-metabólica de alta e crescente prevalência, é citada como responsável pela ocorrência das candidíases orais. A candidíase constitui um espectro de infecções causadas por fungos do gênero Candida, sendo o seu agente mais comum a Candida albicans, embora outras espécies tenham sido identificadas (Candida tropicalis, Candida guillermondii, Candida glabrata, Candida krusei). O objetivo deste trabalho foi avaliar a freqüência e a atividade enzimática de Candida spp. na cavidade oral de pacientes diabéticos atendidos no Serviço de Endocrinologia do Hospital Universitário Walter Cantídio da Universidade Federal do Ceará (HUWC/UFC). Foram coletadas amostras de 48 pacientes diabéticos, de ambos os sexos, com situações variáveis de controle glicêmico. Os materiais clínicos foram colhidos com ajuda de swabs e semeados em placas de Petri contendo ágar-Sabouraud dextrose com cloranfenicol e incubado a 37°C. Os crescimentos foram identificados pelas provas clássicas usadas em micologia. Depois, essas cepas de Candida foram submetidas a provas de detecção de enzimas fosfolipase e proteinase. Destas, 15 amostras (31,25%) apresentaram cultura positiva para o gênero Candida. A espécie mais freqüente foi a C. albicans, com 80%, seguida de C. tropicalis (13,3%) e C. guillermondii (6,7%). Quanto à pesquisa da atividade enzimática de Candida spp., foi observado que 86,6% delas apresentaram atividade de proteinase e 80%, de fosfolipase. Conclui-se com tais resultados que a C. albicans é a mais freqüente e que as espécies de Candida isoladas possuem fortes atividades enzimáticas.

Diabetes mellitus, a endocrine-metabolic disease, of high and increasing prevalence, is cited as responsible by the occurrence of oral candidiasis. Candidiasis constitutes a specter of infections caused by fungi of genera Candida; the most common agent is Candida albicans, but other species have also been identified (Candida tropicalis, Candida guillermondii, Candida glabrata, Candida krusei). The objective of this work was to evaluate frequency and enzymatic activity of Candida spp. in the oral cavity of diabetic patients taken care in the service of endocrinology of the University Hospital Walter Cantídio of the Federal University of the Ceará. Samples had been collected of 48 diabetic patients, men and women, with various situations of glicemic control. Clinical materials had been collected with aid of swab and harvested in plates of Petri contend Sabouraud agar dextrose with cloranfenicol and incubated to 37°C. The grown were identified by the used classic tests in mycology. In the following, these Candida strains were submitted to tests to detect phospholipase and proteinase enzymes. Of these, 15 samples (31,35%) presented positive culture for the genera Candida. The species more frequent was C. albicans with 80%, followed by C. tropicalis (13.3%) and C. guilliermondii (6.7%). Asfor the research on the enzymatic activity of Candida sp. it was observed that 86.6% presented activity of proteinase and 80% of phospholipase. It was concluded with these results that C. albicans is more frequent and that Candida spp. isolated species have strong enzymatic activity.
