Type 1 diabetes mellitus (T1DM) is associated with chronic complications that lead to high morbidity and mortality rates in young adults of productive age. Intensive insulin therapy has been able to reduce the likelihood of the development of chronic diabetes complications. However, this treatment is still associated with an increased incidence of hypoglycemia. In patients with “brittle T1DM”, who have severe hypoglycemia without adrenergic symptoms (hypoglycemia unawareness), islet transplantation may be a therapeutic option to restore both insulin secretion and hypoglycemic perception. The Edmonton group demonstrated that most patients who received islet infusions from more than one donor and were treated with steroid-free immunosuppressive drugs displayed a considerable decline in the initial insulin independence rates at eight years following the transplantation, but showed permanent C-peptide secretion, which facilitated glycemic control and protected patients against hypoglycemic episodes. Recently, data published by the Collaborative Islet Transplant Registry (CITR) has revealed that approximately 50% of the patients who undergo islet transplantation are insulin independent after a 3-year follow-up. Therefore, islet transplantation is able to successfully decrease plasma glucose and HbA1c levels, the occurrence of severe hypoglycemia, and improve patient quality of life. The goal of this paper was to review the human islet isolation and transplantation processes, and to describe the establishment of a human islet isolation laboratory at the Endocrine Division of the Hospital de Clínicas de Porto Alegre – Rio Grande do Sul, Brazil.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Metaloproteinases 2 e 9 são preditoras de remodelação ventricular esquerda após o infarto do miocárdio

FUNDAMENTO: O papel de metaloproteinases (MMP) séricas após o infarto do miocárdio (IM) é desconhecido. OBJETIVO: O objetivo deste estudo foi o de avaliar o papel das MMP-2 e -9 séricas como marcadores prognósticos da remodelação ventricular seis meses após o IM anterior. MÉTODOS: Fizemos um registro prospectivo dos pacientes após o seu primeiro IM anterior. A atividade de MMP foi analisada entre 12 a 72 horas após o IM. Foi feito um ecocardiograma durante a internação e seis meses depois. RESULTADOS: Incluímos 29 pacientes; 62% mostraram remodelação ventricular. Os pacientes que mostraram remodelação tinham maior tamanho de infarto baseado nos valores pico da creatinofosfoquinase (CPK) (p = 0,037), alta prevalência de insuficiência cardíaca congestiva em hospitais (p = 0,004), e redução da fração de ejeção (FE) (p = 0,007). Os pacientes com remodelação ventricular tiveram menores níveis séricos de MMP-9 inativa (p = 0,007) e maiores níveis da forma ativa da MMP-2 (p = 0,011). Em um modelo de regressão logística multivariada, ajustado pela idade, pico de CPK, FE e prevalência de insuficiência cardíaca, os níveis séricos da MMP-2 e -9 estavam associados à remodelação (p = 0,033 e 0,044, respectivamente). CONCLUSÃO: Níveis séricos mais elevados da MMP-9 inativa foram associados com a preservação dos volumes ventriculares esquerdos, e níveis séricos mais elevados da forma ativa da MMP-2 foram um preditor da remodelação seis meses após o IM.
BACKGROUND: The role of serum metalloproteinases (MMP) after myocardial infarction (MI) is unknown. OBJECTIVE: The aim of this study was to evaluate the role of serum MMP-2 and -9 as predictors of ventricular remodeling six months after anterior MI. METHODS: We prospectively enrolled patients after their first anterior MI. MMP activity was assayed 12 to 72 hours after the MI. An echocardiogram was performed during the hospitalization and six months later. RESULTS: We included 29 patients; 62% exhibited ventricular remodeling. The patients who exhibited remodeling had higher infarct size based on creatine phosphokinase (CPK) peak values (p = 0.037), higher prevalence of in-hospital congestive heart failure (p = 0.004), and decreased ejection fraction (EF) (p = 0.007). The patients with ventricular remodeling had significantly lower serum levels of inactive MMP-9 (p = 0.007) and significantly higher levels of the active form of MMP-2 (p = 0.011). In a multivariate logistic regression model, adjusted by age, CPK peak, EF and prevalence of heart failure, MMP-2 and -9 serum levels remained associated with remodeling (p = 0.033 and 0.044, respectively). CONCLUSIONS: Higher serum levels of inactive MMP-9 were associated with the preservation of left ventricular volumes, and higher serum levels of the active form of MMP-2 were a predictor of remodeling 6 months after MI.

Rural communities have accumulated knowledge about survival techniques in the midst of difficulties, such as ecological, economic or social, and one of the transformation spaces and extraction of resources to these communities is the natural vegetation and homegarden. The aim of this study was to list medicinal plants and characterize the use of these plants at the Sítio do Gois, Apodi-RN, Brazil. Open-interview, semi-structured and guided tours by local experts (field guide person, foresters, herb doctors, healers, farmers, housewives) were realized. Medicinal plants are distributed in 34 families, 46 genera and 52 species. The family with the greatest number of individuals was Lamiaceae (seven species). The species most mentioned were Amburana cearensis (77%) followed by 69% de citation of Myracrodruon urundeuva, Sideroxylon obtusifolium, Ximenia americana. Informants are aware of specific uses of native medicinal plants of the caatinga and homegardens. The most common preparations are teas and syrups, used part is the leaf and stem bark and great number of citations for disorders of the digestive and respiratory systems. The most important species with greater use agreement were A. cearensis M. urundeuva, S. obtusifolium and X. americana they deserve pharmacological studies and validation.

Um novo crocodilomorfo da Formação Presidente Prudente, Cretáceo Superior (Campaniano-Maastrichtiano) do Grupo Bauru é descrito com base em dois exemplares, consistindo de crânio e mandíbulas quase completos. O material é procedente da localidade conhecida como "Tartaruguito", situada em um antigo ramal ferroviário que ligava as cidades de Pirapozinho e Presidente Prudente, Estado de São Paulo, Brasil. A nova espécie, denominada de Pepesuchus deiseae gen. et sp. nov., é classificada no clado Peirosauridae, com base em três sinapomorfias: presença de cinco dentes pré-maxilares, os dois primeiros alvéolos da pré-maxila quase confluentes e a secção transversal do jugal ao longo da barra temporal inferior oval. O novo táxon aumenta a marcante diversidade de crocodiliformes do Grupo Bauru e dos Peirosauridae, que poderá vir a ser um dos clados mais representativos de mesoeucrocodilianos gondwânicos.
A new crocodyliform from the Upper Cretaceous (Campanian-Maastrichtian) Presidente Prudente Formation of the Bauru Group is described based on two almost complete skulls and mandibles. The material comes from the "Tartaruguito" site, situated at an old railroad between the cities of Pirapozinho and Presidente Prudente, state of São Paulo, Brazil. The new species, Pepesuchus deiseae gen. et sp. nov., is classified in the clade Peirosauridae on the basis of three synapomorphies: the presence of five premaxillary teeth, the anterior two premaxillary alveoli nearly confluent, and the oval cross-section of the jugal along the lower temporal bar. The new taxon increases the outstanding crocodyliform diversity of the Bauru Group, particularly of the Peirosauridae, which might turn out to be one of the most representative clades of gondwanan mesoeucrocodylians.
