Bioavailability of polycyclic aromatic hydrocarbons in Santos Bay (Brazil) and its adjacent continental shelf

Abstract This study evaluated the bioavailability of polycyclic aromatic hydrocarbons (PAHs) in Santos Bay (SB) and the adjacent Santos Continental Shelf (SCS) in Brazil. Biliary metabolites were measured in several fish species to establish a baseline for future monitoring programs. Bile samples from different species of fish were collected monthly from July to December 2005 in SB, and in August 2005 and February 2006 on SCS. Metabolite concentrations were determined using high-performance liquid chromatography with fluorescence detectors. Naphthalene, phenanthrene, and benzo[a]pyrene metabolite concentrations ranged from 24 to 810 µg g -1 of bile, 1.8 to 68 µg g -1 of bile, and below the limit of quantitation to 1.3 µg g -1 of bile, respectively. Despite its high concentrations, the levels of naphthalene metabolites were in regions of low-contamination, while benzo[a]pyrene metabolite were in the same range as those reported in moderately contaminated areas, which may indicate pyrolytic contamination by PAHs. No significant differences in the metabolite concentrations were found between the SB and the SCS samples or during the periods of collection. Future studies with a single biomonitoring species should be conducted, considering age, sex, and feeding condition of the individuals. The metabolite data presented in this study is an important baseline information for this urbanized region, which hosts several sources of contaminants.

Abstract Acute lung injury is an inflammation that triggers acute respiratory distress syndrome with perialveolar neutrophil infiltration, alveolar-capillary barrier damage, and lung edema. Activation of the toll-like receptor 4 complex (TLR4/MD2) and its downstream signaling pathways are responsible for the cytokine storm and cause alveolar damage. Due to the complexity of this pulmonary inflammation, a defined pharmacotherapy has not been established. Thus, this study evaluated the anti-inflammatory potential of milonine, an alkaloid of Cissampelos sympodialis Eichl, in an experimental model of lung inflammation. BALB/c mice were lipopolysaccharide-challenged and treated with milonine at 2.0 mg/kg. Twenty-four hours later, the bronchoalveolar fluid, peripheral blood, and lungs were collected for cellular and molecular analysis. The milonine treatment decreased the cell migration (mainly neutrophils) to the alveoli, the pulmonary edema, and the cytokine levels (IL-1β, IL-6, TNF-α). The systemic IL-6 level was also reduced. The milonine docking analysis demonstrated hydrophobic interaction at TLR4/MD2 groove with Ile124 and Phe126 amino acids. Indeed, the alkaloid downregulated the kinase-Akt and NF-κB through TLR4/MD2. Therefore, milonine is an effective inflammatory modulator being a potential molecule for the treatment of lung inflammation.

Abstract The Program for Biodiversity Research (PPBio) is an innovative program designed to integrate all biodiversity research stakeholders. Operating since 2004, it has installed long-term ecological research sites throughout Brazil and its logic has been applied in some other southern-hemisphere countries. The program supports all aspects of research necessary to understand biodiversity and the processes that affect it. There are presently 161 sampling sites (see some of them at Supplementary Appendix), most of which use a standardized methodology that allows comparisons across biomes and through time. To date, there are about 1200 publications associated with PPBio that cover topics ranging from natural history to genetics and species distributions. Most of the field data and metadata are available through PPBio web sites or DataONE. Metadata is available for researchers that intend to explore the different faces of Brazilian biodiversity spatio-temporal variation, as well as for managers intending to improve conservation strategies. The Program also fostered, directly and indirectly, local technical capacity building, and supported the training of hundreds of undergraduate and graduate students. The main challenge is maintaining the long-term funding necessary to understand biodiversity patterns and processes under pressure from global environmental changes.

Problems related to the use of fossil fuels and greenhouse gas emissions have become global concerns. To address this issue, the strategy adopted in the present study was to use Ananas comosus residues as a source of fermentable carbohydrates to produce second generation ethanol and briquettes. The demand for biofuel is addressed in the present study by the integrated processing of several stages of treatment of the lignocellulosic substrate: chemical characterization, pretreatment, enzymatic hydrolysis and fermentation. It was verified under the conditions used that the enzymatic hydrolysis liberated after 72 hours 58.44 and 21.91 g L-1 of glucose and xylose, respectively. Conversion of the sugars in the fermentation step resulted in 18.80 g L-1 ethanol in 24 hours. The briquettes produced from the remaining solid fraction of the enzymatic hydrolysis presented a calorific value of 18.41 kJ kg -1.

Warifteine, an alkaloid of Cissampelos sympodialis, modulates allergic profile in a chronic allergic rhinitis model

ABSTRACT Cissampelos sympodialis Eichler, Menispermaceae, a Brazilian medicinal plant and its alkaloid warifteine present immunomodulatory activity on asthma experimental model by reducing antigen-specific IgE levels, eosinophil infiltration and lung hyperactivity. Allergic rhinitis is a chronic inflammatory disorder of the nasal tissue that affect the quality of life and it is a risk factor for asthma exacerbation. This study evaluated the effect of inhaled warifteine in an allergic ovalbumin rhinitis model. Inhaled warifteine (2 mg/ml) treatment of ovalbumin-sensitized BALB/c mice significant decreased total and differential number of cells on the nasal cavity and decreased ovalbumin-specific IgE serum levels. Hematoxylin & eosin staining of histological preparations of ovalbumin nasal tissues showed changes such as congestion and a massive cell infiltration in the perivascular and subepithelial regions characterizing the nasal inflammatory process. However, inhaled warifteine or dexamethasone treatment decreased cell infiltration into the perivascular regions and it was observed an intact nasal tissue. Periodic acidic staining of nasal epithelium of ovalbumin animals demonstrated high amount of mucus production by goblet cells and inhaled warifteine or dexamethasone treatment modulated the mucus production. In addition, toluidine blue staining of the nasal epithelium of ovalbumin animals demonstrated an increase of mast cells on the tissue and inhaled warifteine or dexamethasone treatment decreased in average of 1.4 times the number of these cells on the nasal epithelium. Taken these data together we postulate that warifteine, an immunomodulatory alkaloid, can be a medicinal molecule prototype to ameliorate the allergic rhinitis conditions.

RESUMO: Poincianella pyramidalis (catingueira) é uma planta tóxica associada a abortos, perdas embrionárias e malformações em caprinos e animais de laboratório. Surtos e casos esporádicos de intoxicação espontânea em ovinos vem sendo relatados nos últimos cinco anos no agreste de Pernambuco. Devido à ocorrência destes casos espontâneos e a escassez de pesquisas na espécie ovina, o objetivo desse trabalho foi realizar um estudo experimental para avaliação dos efeitos teratogênicos de P. pyramidalis em ovelhas. No experimento foram utilizadas 16 ovelhas e três carneiros, sem raça definida (SRD). As fêmeas foram divididas em quatro grupos, com quatro animais por grupo e mantidas em baias individuais, foram submetidas a sincronização do estro e acasalamento por monta natural, após 30 dias foi realizado o diagnóstico para confirmação da gestação. Os grupos foram denominados como: grupo 1 (G1), grupo 2 (G2), grupo 3 (G3) e grupo 4 (G4). No G1 (controle) as ovelhas foram alimentadas com 1% de suplementação concentrada (SC) baseado no peso vivo (PV) e 2% de volumoso a base de silagem de milho (SM), com uma ingestão diária de 3% de matéria seca (MS) com base no PV. No G2 a alimentação era constituída de 1% de SC com base no PV e 2% de volumoso. Sendo que destes 2% de volumoso, 50% era feno de P. pyramidalis e os outros 50% de SM. No G3 a alimentação era constituída de 1% de SC com base no PV e 2% de volumoso. Sendo que destes 2% de volumoso, 80% foi de feno de P. pyramidalis e os outros 20% de SM. No G4 as ovelhas foram alimentadas com 1% de SC com base no PV e o volumoso constituído de 100% de feno de P. pyramidalis. Todos os grupos receberam SC à base de farelo de milho, farelo de algodão, sal mineral especifico para ovinos e água ad libitum. Mensalmente os animais foram submetidos à ultrassonografia para detecção de perdas embrionárias ou fetais. Foram realizados exames sorológicos para Neospora caninum e Toxoplasma gondii antes da introdução dos animais no experimento. Os fetos com malformações foram avaliados no setor de Patologia da UFRPE / Unidade Acadêmica de Garanhuns (UAG) para descrição das lesões. No G2, ocorreram cinco casos de malformações de oito cordeiros nascidos. No G3, ocorreram quatro casos de malformação, sendo um decorrente de aborto. No G4, uma ovelha abortou e as outras três pariram quatro fetos malformados. As principais malformações encontradas no trabalho foram artrogripose, palatosquise, queilosquise, hipoplasia da mandíbula, aplasia do osso incisivo, malformações de olho e língua, polidactilia e monodactilia. Nos grupos de maior percentual de ingestão da planta foram observadas maiores quantidades de malformações e chamou a atenção a ocorrência de retenção de placenta e a alta frequência de mortalidade perinatal nos cordeiros que apresentaram algum tipo de malformação. Demonstrou-se com esse trabalho que a P. pyramidalis é uma importante causa de malformação congênita, aborto e mortalidade perinatal em ovinos, sendo de extrema importância que se mantenha as ovelhas prenhes fora do alcance desta planta durante todo o período gestacional.
ABSTRACT: Poincianella pyramidalis (catingueira) is a toxic plant associated with abortions, embryonic losses and malformations in goats and laboratory animals. Outbreaks and sporadic cases of spontaneous intoxication in sheep have been reported in the last five years in the agreste of Pernambuco. Due to the occurrence of these spontaneous cases and the lack of researches in sheep, an experimental study was carried out to evaluate the teratogenic effects of P. pyramidalis on sheep. In the experiment, 16 female and three male sheeps, mixed breed, were used. Females were divided in four groups, with four animals per group and maintained in individual stalls, underwent estrus synchronization and mating by natural mating and after 30 days the diagnosis was made to confirm gestation. Groups were named group 1 (G1), group 2 (G2), group 3 (G3), and group 4 (G4). In G1 (control) the ewes were fed with 1% of concentrate supplementation (CS) based on live weight (LW) and 2% of maize silage (MS) with a daily intake of 3% dry matter (DM) based on LW. In G2 feeding was constituted of 1% of CS based on LW and 2% of forage, including 2% of roughage, 50% was hay of P. pyramidalis and the other 50% of DM. In G3 feed was constituted of 1% of CS based on LW and 2% of roughage. Among these 2% of roughage, 80% was hay of P. pyramidalis and the other 20% of DM. In G4 the sheep were fed with 1% CS based on the LW and the roughage consisting of 100% P. pyramidalis hay. All groups received CS with corn meal, cottonseed meal, mineral salt specific for sheep and water ad libitum. Animals were monthly submitted to ultrasonography to detect embryonic or fetal losses. Serological tests were performed for Neospora caninum and Toxoplasma gondii before introduction of the animals in the experiment. Fetuses with malformations were evaluated in the Laboratory of Animal Anatomy and Pathology of the Academic Unit of Garanhuns (UAG), UFRPE, to describe the lesions. In G2, there were five cases of malformations of eight born lambs. In G3, there were four cases of malformation, one due to abortion. In G4, one sheep aborted and the other three gave birth to four malformed fetuses. Main malformations found in the study were arthrogryposis, palatoschisis, chylosquisis, hypoplasia of the mandible, aplasia of the incisive bone, malformations of the eye and tongue, polidactyly and monodactyly. In the groups with higher doses, larger numbers of malformations were observed, and the occurrence of placental retention and the high frequency of perinatal mortality in lambs presenting some type of malformation were noted. It has been demonstrated that P. pyramidalis is an important cause of congenital malformation, abortion and perinatal mortality in sheep, therefore keeping pregnant sheep away from this plant throughout the gestation period is utmost important.

Evaluation of methods for the quantification of ether extract contents in forage and cattle feces

ABSTRACT The objective of this study was to compare the estimates of ether extract (EE) contents obtained by the Randall method and by the high-temperature method of the American Oil Chemist's Society (AOCS; Am 5-04) in forages (n = 20) and cattle feces (n = 15). The EE contents were quantified by using the Randall extraction or AOCS method and XT4 filter bags or cartridges made of qualitative filter paper (80 g/m²) as containers for the samples. It was also evaluated the loss of particles, and concentration of residual chlorophyll after extraction and the recovery of protein and minerals in the material subjected to extraction. Significant interaction was observed between extraction method and material for EE contents. The EE estimates using the AOCS method were higher, mainly in forages. No loss of particles was observed with different containers. The chlorophyll contents in the residues of cattle feces were not affected by the extraction method; however, residual chlorophyll was lower using the AOCS method in forages. There was complete recovery of the protein and ash after extraction. The results suggest that AOCS method produces higher estimates of EE contents in forages and cattle feces, possibly by providing greater extraction of non-fatty EE.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Parasitic fauna in hybrid tambacu from fish farms

O objetivo deste trabalho foi avaliar a parasitofauna de tambacu (Colossoma macropomum x Piaractus mesopotamicus) de pisciculturas e a relação parasito-hospedeiro. Cento e catorze peixes foram coletados de quatro pisciculturas de Macapá, AP, 80,7% dos quais estavam infectados por: Ichthyophthirius multifiliis (Ciliophora); Piscinoodinium pillulare (Dinoflagellida); Anacanthorus spatulatus, Notozothecium janauachensis e Mymarothecium viatorum (Monogenoidea); Neoechinorhynchus buttnerae (Acanthocephala), Cucullanus colossomi (Nematoda); Perulernaea gamitanae (Lernaeidae); e larvas de Proteocephalidae (Cestoda). Um total de 8.136.252 parasitos foram coletados dos peixes examinados. Esse é o primeiro registro de N. buttnerae, C. colossomi, N. janauachensis, M. viatorum e Proteocephalidae para híbridos tambacu no Brasil. Ichtyophthirius multifiliis foi o parasito mais prevalente, enquanto os endohelmintos, os menos. Observou-se correlação positiva entre número de I. multifiliis e comprimento total e peso corporal dos peixes, bem como entre número de P. gamitanae e comprimento total. A infeção por I. multifiliis mostrou associação com o parasitismo por Monogenoidea. A baixa qualidade da água contribui para o alto parasitismo do hibrido tambacu por ectoparasitas, que, no entanto, não influi no fator de condição relativo dos peixes.
The objective of this work was to evaluate the parasitic fauna of hybrid tambacu (Colossoma macropomum x Piaractus mesopotamicus) from fish farms and the host-parasite relationship. A hundred and fourteen fish were collected from four fish farms in Macapá, in the state of Amapá, Brazil, 80.7% of which were infected by: Ichthyophthirius multifiliis (Ciliophora); Piscinoodinium pillulare (Dinoflagellida); Anacanthorus spatulatus, Notozothecium janauachensis, and Mymarothecium viatorum (Monogenoidea); Neoechinorhynchus buttnerae (Acanthocephala); Cucullanus colossomi (Nematoda); Perulernaea gamitanae (Lernaeidae); and Proteocephalidae larvae (Cestoda). A total of 8,136,252 parasites were collected from the examined fish. This is the first record of N. buttnerae, C. colossomi, N. janauachensis, M. viatorum, and Proteocephalidae for hybrid tambacu in Brazil. Ichthyophthirius multifiliis was the most prevalent parasite, whereas endohelminths were the less. A positive correlation was observed between number of I. multifiliis and total length and weight of fish, as well as between number of P. gamitanae and total length. The infection by I. multifiliis had association with the parasitism by Monogenoidea. Low water quality contributes to high parasitism of hybrid tambacu by ectoparasites, which, however, does not influence the relative condition factor of fish.

This work shows the results of a Proficiency Testing performed by a partnership between INMETRO and ANP. The performance of 49 Brazilian laboratories (using the z-score statistical test) in determining 10 quality parameters of ethanol fuel and biodiesel was evaluated. The certified reference values were provided by INMETRO, allowing a more rigorous assessment of the laboratories. For hydrous ethanol, the acidity parameter showed the lowest number of laboratories with satisfactory results (48%), while 85% of the laboratories presented satisfactory results for ethanol content. For biodiesel, the percentage of laboratories with satisfactory results ranged from 46% (kinematic viscosity) to 92% (acid number).

FUNDAMENTO: A síndrome coronariana aguda (SCA) sem supradesnivelamento do segmento ST (SCASEST) está associada ao supradesnivelamento do peptídeo natriurético tipo B (BNP) e aos marcadores de necrose miocárdica, embora se desconheça a correlação dessa síndrome ao escore de trombólise no infarto do miocárdio (TIMI) e à função ventricular esquerda. OBJETIVO: Avaliar a correlação entre os níveis do fragmento N-terminal do peptídeo natriurético tipo B (NT-proBNP) e os marcadores de necrose miocárdica (creatinofosfoquinase fração músculo-cérebro CK-MB e troponina I), bem como entre o escore de risco TIMI e a fração de ejeção do ventrículo esquerdo (FEVE) nos pacientes com síndrome coronariana aguda sem supradesnivelamento do segmento ST. MÉTODOS: Oitenta e sete pacientes com síndrome coronariana aguda sem supradesnivelamento do segmento ST foram divididos em dois grupos: 37 (42,5%) com angina instável e 50 (57,5%) com infarto agudo do miocárdio sem supradesnivelamento do segmento ST (IAMSSST). RESULTADOS: A fração de ejeção do ventrículo esquerdo superior a 40% foi encontrada em 86,2% do total da amostra. Os níveis séricos de NT-proBNP foram maiores em pacientes com infarto do miocárdio sem supradesnivelamento do segmento ST, em comparação àqueles com angina instável (p<0,001). O aumento dos níveis de NT-proBNP foi associado ao aumento de troponina I (rs=0,425, p<0,001), pico de CK-MB (rs=0,458, p<0,001) e FEVE (rs=-0,345, p=0,002), não havendo correlação com o escore de risco TIMI (rs=0,082, p=0,44). Na análise multivariada, a fração de ejeção do ventrículo esquerdo e a troponina I correlacionaram-se de forma independente aos níveis de NT-proBNP (p=0,017 e p=0,002, respectivamente). CONCLUSÃO: O aumento dos níveis de NT-proBNP em pacientes com síndrome coronariana aguda sem supradesnivelamento do segmento ST não está relacionado exclusivamente à redução da fração de ejeção do ventrículo esquerdo, mas também pode ser provocado pela presença de isquemia miocárdica e necrose.
BACKGROUND: Non-ST-segment elevation acute coronary syndrome is associated with elevation of brain natriuretic peptide and markers of myocardial necrosis, although its relationship with the TIMI score and left ventricular function are largely unknown. OBJECTIVE: To evaluate the correlation between plasma N-terminal pro-brain natriuretic peptide (NT-proBNP) and markers of myocardial necrosis [creatine phosphokinase muscle-brain fraction (CK-MB) and troponin I], TIMI risk score and left ventricular ejection fraction in patients with non-ST-segment elevation acute coronary syndrome. METHODS: Eighty-seven patients with non-ST-segment elevation acute coronary syndrome were divided into two groups: 37 (42.5%) with unstable angina and 50 (57.5%) with non-ST-segment elevation myocardial infarction. RESULTS: Left ventricular ejection fraction more than 40% was found in 86.2% of the total sample. Serum levels of NT-proBNP was higher in patients with non-ST elevation myocardial infarction than in those with unstable angina (p<0.001). Increased levels of NT-proBNP were associated with increases in troponin I (rs=0.425, p<0.001), peak CK-MB (rs=0.458, p<0.001) and low left ventricular ejection fraction (rs=-0.345, p=0.002); no correlation was found with the TIMI risk score (rs=0.082, p=0.44). Multivariate analysis revealed that left ventricular ejection fraction and troponin I levels were independently correlated with NT-proBNP levels (p=0.017 and p=0.002, respectively). CONCLUSION: Increased levels of NT-proBNP in patients with non-ST-segment elevation acute coronary syndrome are not related exclusively to low left ventricular ejection fraction, but can also be caused by the presence of myocardial ischemia and necrosis.

Resumo A síndrome de tremor-ataxia associada ao X-frágil (FXTAS) é uma causa de distúrbios do movimento e de declínio cognitivo que provavelmente tem sido subdiagnosticada, especialmente se a sua prevalência for realmente similar às da paralisia supranuclear progressiva e esclerose lateral amiotrófica. Relatamos um caso de um homem de 74 anos que se apresentou com tremor de ação, ataxia de marcha e esquecimento. Havia história familiar de tremor e de demência e um de seus netos era mentalmente deficiente. A avaliação neuropsicológica demonstrou uma síndrome frontal. A ressonância magnética (RM) revelou hiperintensidade de ambos os pedúnculos cerebelares médios, um critério maior para o diagnóstico de FXTAS. Os testes genéticos confirmaram a presença da pré-mutação do gene FMR1, com uma repetição (CGG)90. O diagnóstico de FXTAS é importante para o aconselhamento genético porque as filhas dos indivíduos afetados tem alto risco de ter uma criança com síndrome do X-frágil. A presença de tremores e declínio cognitivo deve levantar a hipótese diagnóstica de FXTAS, que poderá ser reforçada pela RM e confirmada pela presença da pré-mutação do gene FMR1.
Abstract Fragile X-associated tremor/ataxia syndrome (FXTAS) is a cause of movement disorders and cognitive decline which has probably been underdiagnosed, especially if its prevalence proves similar to those of progressive supranuclear palsy and amyotrophic lateral sclerosis. We report a case of a 74-year-old man who presented with action tremor, gait ataxia and forgetfulness. There was a family history of tremor and dementia, and one of the patient's grandsons was mentally deficient. Neuropsychological evaluation disclosed a frontal network syndrome. MRI showed hyperintensity of both middle cerebellar peduncles, a major diagnostic hallmark of FXTAS. Genetic testing revealed premutation of the FMR1 gene with an expanded (CGG)90 repeat. The diagnosis of FXTAS is important for genetic counseling because the daughters of the affected individuals are at high risk of having offspring with fragile X syndrome. Tremors and cognitive decline should raise the diagnostic hypothesis of FXTAS, which MRI may subsequently reinforce, while the detection of the FMR1 premutation can confirm the condition.

FUNDAMENTO: A reserva contrátil diminuída pode já estar presente em pacientes portadores de regurgitação aórtica, assintomáticos com fração de ejeção (FE) normal, indicando a necessidade de avaliações frequentes e acuradas da função ventricular esquerda para detectar disfunção sistólica incipiente. OBJETIVO: Analisar se incrementos na FE em doses baixas de dobutamina podem predizer cirugia e/ou morte em pacientes com regurgitação aórtica. MÉTODOS: Eco de estresse com dobutamina foi realizado em 24 pacientes portadores de regurgitação aórtica para verificar se incrementos da FE em doses baixas de dobutamina seriam capazes de predizer a necessidade de cirurgia e/ou morte nesse grupo de pacientes. RESULTADOS: A idade média foi de 37,8±16,8 anos, e 16 (66%) eram homens. A FE aumentou de um valor basal médio de 62,3±7,9% para 71,5±10,5%, na dose de 20 µg/kg/min de dobutamina (p < 0,001). Os pacientes foram acompanhados por 36,6±20,1 meses: dois pacientes morreram (um de morte cardiovascular) e cinco foram submetidos à cirurgia cardíaca. A FE basal se correlacionou com cirurgia e morte no seguimento de pacientes. CONCLUSÃO: A FE basal se correlacionou com cirurgia ou morte no seguimento de pacientes jovens com regurgitação aórtica. Porém, o incremento percentual na FE com dose baixa de dobutamina não foi capaz de predizer eventos nesses pacientes.
BACKGROUND: Decreased contractile reserve may already be present in asymptomatic patients with aortic regurgitation and normal ejection fraction (EF), thus indicating the need for frequent and accurate assessments of the left ventricular function for the early detection of systolic dysfunction. OBJECTIVE: To analyze if increments in EF with low dose dobutamine could predict surgery and/or death in patients with aortic regurgitation. METHDOS: Dobutamine-stress echocardiography was performed in 24 patients with aortic regurgitation in order to analyze whether EF increments at low dobutamine doses could predict the need for surgery and/or death in this group of patients. RESULTS: Mean age was 37.8±16.8 years and 16 patients (66%) were male. EF increased from a mean baseline value of 62.3±7.9% to 71.5±10.5% at a dobutamine dose of 20 µg/kg/min (p<0.001). The patients were followed-up for 36.6±20.1 months; two patients died (one of cardiovascular death) and five underwent cardiac surgery. Baseline EF was correlated with surgery and death in the follow-up of patients. CONCLUSION: Baseline EF was correlated with surgery or death in the follow-up of young patients with aortic regurgitation. However, the percentage increase in EF at low dobutamine doses did not allow us to predict events in these patients.

OBJETIVO: Analizar se incrementos en la FE en dosis bajas de dobutamina pueden predecir cirugía y/o muerte en pacientes con regurgitación aórtica. MÉTODOS: Eco de estrés con dobutamina se realizó en 24 pacientes portadores de regurgitación aórtica para verificar se incrementos de la FE en dosis bajas de dobutamina serían capaces de predecir la necesidad de cirugía y/o muerte en ese grupo de pacientes. RESULTADOS: La edad promedio fue de 37,8±16,8, y 16 (66%) eran varones. La FE aumentó de un valor basal promedio de 62,3±7,9% para 71,5±10,5%, en la dosis de 20 µg/kg/min de dobutamina (p < 0,001). Los pacientes se siguieron por 36,6±20,1 meses: dos pacientes murieron (uno de muerte cardiovascular) y cinco se sometieron a cirugía cardiaca. La FE basal se correlacionó con cirugía y muerte en el seguimiento de pacientes. CONCLUSIÓN: La fe basal se correlacionó con cirugía o muerte en el seguimiento de pacientes jóvenes con regurgitación aórtica. Sin embargo, el incremento porcentual en la FE con dosis baja de dobutamina no fue capaz de predecir eventos en esos pacientes.
