Objective: To systematically review the literature focusing on obstetric and perinatal outcomes in women with previous or current eating disorders (EDs) and on the consequences of maternal EDs for the offspring. Methods: The study was performed following the systematic review and meta-analysis (PRISMA) statement. PubMed, SciELO, and Cochrane databases were searched for non-interventional studies published in English or Portuguese from January 1980 to December 2020. Risk of bias was assessed using the Methods guide for effectiveness and comparative effectiveness reviews (American Agency for Healthcare Research and Quality). Results: The search yielded 441 records, and 30 articles were included. The psychiatric outcome associated with EDs in women was mainly perinatal depression. The most prevalent obstetric outcomes observed in women with EDs were vomiting, hyperemesis, bleeding, and anemia. Most studies found maternal anorexia nervosa and bulimia nervosa to be associated with low birth weight and slow fetal growth. Women with binge EDs delivered children with increased birth weight. Of the 30 studies included, methodological quality was good in seven, fair in eight, and poor in 15 studies. Conclusion: A considerable body of evidence was reviewed to assess obstetric and perinatal outcomes in EDs. Acute and lifetime EDs, especially if severe, correlated with poor perinatal, obstetric, and neonatal outcomes. Obstetricians and general practitioners should be vigilant and screen for EDs during pregnancy.

Abstract Introduction: We investigated the clinical course and outcomes of patients submitted to cardiovascular surgery in Brazil and who had developed symptoms/signs of coronavirus disease 2019 (COVID-19) in the perioperative period. Methods: A retrospective multicenter study including 104 patients who were allocated in three groups according to time of positive real time reverse transcriptase-polymerase chain reaction (RT-PCR) for the SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2): group 1, patients who underwent cardiac surgery > 10 days after positive RT-PCR; group 2, patients with a positive RT-PCR within 10 days before or after surgery; group 3, patients who presented positive RT-PCR > 10 days after surgery. The primary outcome was mortality and secondary outcomes were postoperative complications, intensive care unit (ICU) length of stay, and postoperative days of hospitalization. Results: The three groups were similar with respect to age, the European System of Cardiac Operative Risk Evaluation score, and comorbidities, except hypertension. Postoperative complications and death were significantly higher in groups 2 and 3 than in group 1, and no significant difference between groups 2 and 3 was seen. Group 2 showed a high prevalence of surgery performed as an urgent procedure. Although no significant differences were observed in ICU length of stay, total postoperative hospitalization time was significantly higher in group 3 than in groups 1 and 2. Conclusion: COVID-19 affecting the postoperative period of patients who underwent cardiovascular surgery is associated with a higher rate of morbidity and mortality. Delaying procedures in RT-PCR-positive patients may help reduce risks of perioperative complications and death.

RESUMO Objetivo: Descrever as práticas de ressuscitação volêmica em unidades de terapia intensiva brasileiras e compará-las com as de outros países participantes do estudo Fluid-TRIPS. Métodos: Este foi um estudo observacional transversal, prospectivo e internacional, de uma amostra de conveniência de unidades de terapia intensiva de 27 países (inclusive o Brasil), com utilização da base de dados Fluid-TRIPS compilada em 2014. Descrevemos os padrões de ressuscitação volêmica utilizados no Brasil em comparação com os de outros países e identificamos os fatores associados com a escolha dos fluidos. Resultados: No dia do estudo, foram incluídos 3.214 pacientes do Brasil e 3.493 pacientes de outros países, dos quais, respectivamente, 16,1% e 26,8% (p < 0,001) receberam fluidos. A principal indicação para ressuscitação volêmica foi comprometimento da perfusão e/ou baixo débito cardíaco (Brasil 71,7% versus outros países 56,4%; p < 0,001). No Brasil, a percentagem de pacientes que receberam soluções cristaloides foi mais elevada (97,7% versus 76,8%; p < 0,001), e solução de cloreto de sódio a 0,9% foi o cristaloide mais comumente utilizado (62,5% versus 27,1%; p < 0,001). A análise multivariada sugeriu que os níveis de albumina se associaram com o uso tanto de cristaloides quanto de coloides, enquanto o tipo de prescritor dos fluidos se associou apenas com o uso de cristaloides. Conclusão: Nossos resultados sugerem que cristaloides são usados mais frequentemente do que coloides para ressuscitação no Brasil, e essa discrepância, em termos de frequências, é mais elevada do que em outros países. A solução de cloreto de sódio 0,9% foi o cristaloide mais frequentemente prescrito. Os níveis de albumina sérica e o tipo de prescritor de fluidos foram os fatores associados com a escolha de cristaloides ou coloides para a prescrição de fluidos.
Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.

Abstract The Program for Biodiversity Research (PPBio) is an innovative program designed to integrate all biodiversity research stakeholders. Operating since 2004, it has installed long-term ecological research sites throughout Brazil and its logic has been applied in some other southern-hemisphere countries. The program supports all aspects of research necessary to understand biodiversity and the processes that affect it. There are presently 161 sampling sites (see some of them at Supplementary Appendix), most of which use a standardized methodology that allows comparisons across biomes and through time. To date, there are about 1200 publications associated with PPBio that cover topics ranging from natural history to genetics and species distributions. Most of the field data and metadata are available through PPBio web sites or DataONE. Metadata is available for researchers that intend to explore the different faces of Brazilian biodiversity spatio-temporal variation, as well as for managers intending to improve conservation strategies. The Program also fostered, directly and indirectly, local technical capacity building, and supported the training of hundreds of undergraduate and graduate students. The main challenge is maintaining the long-term funding necessary to understand biodiversity patterns and processes under pressure from global environmental changes.

Resumo A persistência de altos níveis de letalidade em ações da polícia constitui um dos grandes desafios da democracia no Brasil contemporâneo. Desde o processo de redemocratização, nos anos 1980 a 1990, a letalidade e a violência policiais têm se colocado no cerne do debate em torno da segurança pública. Iniciativas de controle surgiram nas duas últimas décadas. Estudos mostraram as múltiplas faces e dimensões do problema, mas políticas efetivas de controle da violência policial não foram implantadas até o presente momento. O presente artigo propõe uma análise comparada da letalidade policial a partir da experiência das polícias dos Estados do Rio de Janeiro e de São Paulo e explorando a influência desta letalidade sobre a taxa de homicídios. Por fim, propomos um debate por meio de uma revisão dos estudos sobre o tema, sinalizamos possibilidades de compreensão da persistência da violência policial, bem como perspectivas em torno do seu controle.
Abstract The persistence of high levels of lethality in police actions is one of the great challenges of democracy in contemporary Brazil. Since the process of redemocratization in the 1980s and 1990s, police lethality and violence have been at the heart of the debate around public security. Control initiatives have emerged in the past two decades. Studies have shown the multiple features and dimensions of the problem, but effective policies to control police violence are yet to be implemented. This article presents a comparative analysis of police lethality based on polices’ experiences in the states of Rio de Janeiro and São Paulo. It further explores the influence of this lethality on homicide rates. Finally, we discuss our data taking into account previous studies on this topic, point out possibilities for understanding the persistence of police violence, as well as perspectives around its control.

ABSTRACT Traditionally, the treatment of overactive bladder syndrome has been based on the use of oral medications with the purpose of reestablishing the detrusor stability. The recent better understanding of the urothelial physiology fostered conceptual changes, and the oral anticholinergics – pillars of the overactive bladder pharmacotherapy – started to be not only recognized for their properties of inhibiting the detrusor contractile activity, but also their action on the bladder afference, and therefore, on the reduction of the symptoms that constitute the syndrome. Beta-adrenergic agonists, which were recently added to the list of drugs for the treatment of overactive bladder, still wait for a definitive positioning – as either a second-line therapy or an adjuvant to oral anticholinergics. Conservative treatment failure, whether due to unsatisfactory results or the presence of adverse side effects, define it as refractory overactive bladder. In this context, the intravesical injection of botulinum toxin type A emerged as an effective option for the existing gap between the primary measures and more complex procedures such as bladder augmentation. Sacral neuromodulation, described three decades ago, had its indication reinforced in this overactive bladder era. Likewise, the electric stimulation of the tibial nerve is now a minimally invasive alternative to treat those with refractory overactive bladder. The results of the systematic literature review on the oral pharmacological treatment and the treatment of refractory overactive bladder gave rise to this second part of the review article Overactive Bladder – 18 years, prepared during the 1st Latin-American Consultation on Overactive Bladder.

ABSTRACT Abstract: Overactive bladder syndrome is one of the lower urinary tract dysfunctions with the highest number of scientific publications over the past two decades. This shows the growing interest in better understanding this syndrome, which gathers symptoms of urinary urgency and increased daytime and nighttime voiding frequency, with or without urinary incontinence and results in a negative impact on the quality of life of approximately one out of six individuals – including both genders and almost all age groups. The possibility of establishing the diagnosis just from clinical data made patients' access to specialized care easier. Physiotherapy resources have been incorporated into the urological daily practice. A number of more selective antimuscarinic drugs with consequent lower adverse event rates were released. Recently, a new class of oral drugs, beta-adrenergic agonists has become part of the armamentarium for Overactive Bladder. Botulinum toxin injections in the bladder and sacral neuromodulation are routine modalities of treatment for refractory cases. During the 1st Latin-American Consultation on Overactive Bladder, a comprehensive review of the literature related to the evolution of the concept, epidemiology, diagnosis, and management was conducted. This text corresponds to the first part of the review Overactive Bladder 18-years.

Some Properties of Astronium graveolens Wood Along the Stem

RESUMO Determinamos as variações axiais da densidade aparente, resistência ao cisalhamento paralelo à grã e anatomia da madeira de Astronium graveolens, relacionando estas propriedades ao longo do fuste. Cortamos cinco árvores com 20 anos de idade e estudamos amostras próximas da casca na base do tronco, a um, dois e três metros de altura. As variações axiais encontradas no lenho de A. graveolens parecem propiciar um equilíbrio entre a resistência mecânica da madeira na base do fuste pela maior densidade e também pela maior resistência ao cisalhamento devido a maior frequência de raios que contribuem no travamento das células orientadas no sentido vertical evitando assim que o tronco quebre facilmente ao ser flexionado. Pensando na condutividade hidráulica, vasos com menor diâmetro e frequência na base do fuste podem garantir maior segurança ao diminuir a ocorrência de embolismos que poderiam prejudicar o fluxo de água ao longo do fuste.
ABSTRACT We investigated the axial variation of specific gravity, shear parallel to the grain and anatomical features of Astronium graveolens wood and related these properties to the anatomy along the stem. We felled five 20-year-old trees and cut discs from four different stem heights, including stem base, 1 meter, 2 meters and 3 meters, for a total of 20 discs, and studied wood samples near the bark and at the base of trunk. Axial variations found appear to provide a balance between mechanical strength of the wood at stem base by the higher density and higher shear by the increase in ray frequency that contributes to locking the vertical cells and growth rings, thus preventing the stem from easily breaking when bent. For hydraulic conductivity, vessels with smaller diameter and frequency at the stem base help prevent the occurrence of embolisms that would reduce water flow along the main stem.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Deforming mandibular osteomyelitis in a cow caused by Trueperella pyogenes

Relata-se caso incomum de osteomielite mandibular deformante em vaca, causada por Trueperella (Arcanobacterium) pyogenes, na face ventro-lateral da porção caudal do ramo direito da mandíbula. A punção aspirativa de fragmento da lesão permitiu a caracterização citológica, o isolamento microbiano e identificação de T. pyogenes. Exame radiográfico mostrou acentuada reação periodontal na mandíbula direita, com predomínio de áreas líticas e destruição da cortical óssea. Apesar da instituição do tratamento baseado no teste de sensibilidade microbiana in vitro, foi recomendada a eutanásia, em virtude da piora progressiva do estado geral do animal. No exame post-mortem foram observados múltiplos abscessos na lesão que, histologicamente, foi caracterizada como piogranuloma. A colheita de material da região mandibular afetada, após a necropsia, resultou no reisolamento microbiológico de T. pyogenes
This study reports an unusual case of deforming mandibular osteomyelitis in a cow caused by Trueperella (Arcanobacterium) pyogenes, on the face of the ventrolateral caudal portion of the right branch of the mandible. Fragment aspired of lesion by fine needle allowed cytological characterization, isolation and identification of T. pyogenes. Radiographic examination showed marked periosteal reaction in the right mandible, numerous lytic areas and cortical bone destruction. Despite of treatment based on in vitro antimicrobial sensitivity test, it was recommended the euthanasia due to progressive worsening of the cow's condition. Multiple abscesses were observed in the mandibular region at necropsy. Pyogranuloma was characterized in histological exam. Sampled material collected from the lesion after necropsy resulted in microbiological reisolation of T. pyogenes

INTRODUÇÃO: A segurança e a eficácia do uso de stent farmacológico para o tratamento de lesões em ponte de veia safena (PVS) ainda é motivo de controvérsia. Este estudo avaliou a evolução tardia de pacientes com lesões em PVS tratados com stent farmacológico. MÉTODOS: Registro unicêntrico que incluiu todos os pacientes submetidos a intervenção em PVS com stent farmacológico (n = 82), sem restrições clínicas ou angiográficas, no período de 2003 a 2009. Foram avaliadas as taxas de eventos cardíacos adversos maiores (ECAM), óbito, infarto agudo do miocárdio (IAM), revascularização do vaso-alvo (RVA) e trombose de stent. RESULTADOS: A média de idade foi de 67,8 ± 10,2 anos, a maioria era do sexo masculino (85,4%), 40,2% eram diabéticos e 52,4% eram portadores de angina estável. Foi utilizado 1,45 ± 0,5 stent por paciente, empregando-se o stent CypherTM na maioria (61%) dos casos. O diâmetro dos stents foi de 3,22 ± 0,39 mm e o comprimento, de 20,1 ± 7,3 mm. A taxa de sucesso angiográfico foi de 96,3%. No seguimento de 4,1 anos, a taxa de ECAM foi de 28%, com 6% de óbito, 19,5% de IAM e 18,2% de RVA. Nesse período ocorreram dois casos de trombose de stent definitiva ou provável (2,4%). CONCLUSÕES: Os resultados demonstraram, em seguimento muito tardio, altas taxas de ECAM em pacientes com lesões de PVS tratados com stent farmacológico, provavelmente pelo aspecto mais agressivo da doença vascular em enxertos venosos.
BACKGROUND: The safety and efficacy of drug-eluting stents in the treatment of saphenous vein graft (SVG) lesions remains controversial. This study assessed the late follow-up of patients with SVG lesions treated with drug-eluting stents. METHODS: Single center registry including patients undergoing SVG interventions using drug-eluting stents (n = 82), without clinical or angiographic exclusion criteria, from 2003 to 2009. The rates of major adverse cardiac events (MACE), death, acute myocardial infarction (AMI), target vessel revascularization (TVR) and stent thrombosis were evaluated. RESULTS: Mean age was 67.8 ± 10.2 years, most of them were male (85.4%), 40.2% were diabetic and 52.4% had stable angina. An average of 1.45 ± 0.5 stents per patient were implanted and CypherTM was the stent used in most of the cases (61%). Stent diameter was 3.22 ± 0.39 mm and stent length was 20.1 ± 7.3 mm. Angiographic success rate was 96.3%. In the 4.1-year follow-up, the rate of MACE was 28%, death 6%, AMI 19.5% and TVR 18.2%. There were two cases of definitive or probable stent thrombosis (2.4%) within the follow-up period. CONCLUSIONS: Long-term follow-up showed high MACE rates in patients with SVG lesions treated with drug-eluting stents, probably due to the accelerated atherosclerosis that develops within the grafted vein conduits.

Do extrato diclorometano do caule de Almeidea coerulea (Nees & Mart.) A. St.-Hil. foi isolado um triterpeno o 20-hidroxi-diidrolupeol (1), e do extrato metanólico foram isolados os alcalóides dictamina (2) e eskimianina (3), além de uma mistura dos esteróides sitosterol (4) e estigmasterol (5). A identificção substâncias foi realizada pela análise dos espectros de Massas, RMN ¹H, RMN 13C, HSQC, HMBC, e por comparação com dados da literatura.
From the dichloromethane extract of Almeidea coerulea stem one triterpene, 20-hydroxy-dihydrolupeol was isolated (1) and from the methanol extract the known alkaloids dictamnine (2) and skimmianine (3), as well the steroids sitosterol (4) and stigmasterol (5). The identification of these compounds was performed on the basis of spectroscopic analyses (MS, ¹H NMR, 13C NMR, HSQC and HMBC) as well as comparison with literature data.

Pressurização da calda de pulverização na viabilidade de microrganismos entomopatogênicos

Avaliou-se a influência da pressurização da calda de pulverização na viabilidade dos seguintes microrganismos entomopatogênicos: o fungo Metarhizium anisopliae, a bactéria Bacillus thuringiensis var. kurstaki, o nucleopoliedrovirus de Anticarsia gemmatalis (AgMNPV) e o nematóide Steinernema glaseri. Utilizou-se do sistema hidráulico de um pulverizador, sem filtros, equipado com ponta de pulverização de jato cônico JA-2. O delineamento experimental adotado foi o inteiramente casualizado, com sete tratamentos: pressurização da calda em 172; 345; 517; 689; 1.034 e 1.379 MPa, e a testemunha (calda não submetida à pressurização). A avaliação da viabilidade de M. anisopliae foi realizada por meio da porcentagem de germinação dos conídios. Para B. thuringiensis, foi considerado o número de colônias. A eficácia do AgMNPV foi estimada indiretamente, pela mortalidade causada às lagartas de A. gemmatalis no início do terceiro instar, alimentadas com dieta artificial impregnada com suspensão viral. No caso do S. glaseri, foi calculada a porcentagem de indivíduos vivos em lâmina de Peters. Conclui-se que se podem pulverizar os entomopatógenos M. anisopliae, B. thuringiensis, AgMNPV e S. glaseri, até a pressão de 1.379 MPa, sem que haja perda na viabilidade desses organismos utilizados como agentes de controle biológico.
The objective of this research was to evaluate the influence of spraying pressurization upon the viability of the following entomopathogens microorganisms: the fungus Metarhizium anisopliae, the bacterium Bacillus thuringiensis var. kurstaki, the nucleopolyhedrovirus of Anticarsia gemmatalis (AgMNPV), and the nematode Steinernema glaseri. The sprayer hydraulical system was used without filters and equipped with JA-2 hollow-cone nozzle type. The experimental design was completely randomized, with seven treatments: pressurization of spraying suspension at 172; 345; 517; 689; 1,034 and 1,379 MPa, and check (suspension not submitted to the pressurization). Evaluation of the viability of M. anisopliae was estimated by the percentage of germinated conidia. For B. thuringiensis, by the number of colonies formed. Efficiency on AgMNPV was estimated indirectly, by means of mortality caused to A. gemmatalis infected as early third-instars larvae reared in an artificial diet impregnated with the viral suspension. For the experiment with S. glaseri it was calculated the percentage of live specimens on Peters slide-blade. It concluded that the entomopathogens M. anisopliae, B. thuringiensis, AgMNPV and S. glaseri can be sprayed up to the pressure of 1,379 MPa without losing the viability of those organisms as biological control agents.
