DYSPHAGIA OCCURRENCE IN COVID-19-POSITIVE PATIENTS IN TWO HOSPITALS IN BRAZIL

RESUMO Contexto A COVID-19 compreende uma infecção respiratória decorrente da contaminação pelo vírus SARS-CoV-2, sendo a insuficiência respiratória aguda uma de suas principais características, levando a uma alta frequência de intubação orotraqueal (IOT), que por sua vez aumenta o risco para a disfagia. Uma vez que esta pode levar ao comprometimento pulmonar, conhecer a real ocorrência de disfagia em parte da população brasileira e suas associações permite o manejo clínico precoce e eficaz da equipe multidisciplinar em relação aos pacientes. Objetivo: Verificar a ocorrência de disfagia em pacientes adultos positivos para COVID-19 em dois hospitais brasileiros, referências no atendimento à pandemia. Métodos: Trata-se de um estudo prospectivo, observacional longitudinal, realizado em dois hospitais privados no Brasil, ambos referências no atendimento de pacientes com isolamento por coronavírus. Inicialmente os dados foram levantados por meio de consulta aos prontuários de cada paciente. Foram também coletadas informações sobre sexo, idade, doenças anteriores, teste de COVID-19 e período de IOT. Após a coleta de dados, foi realizada a avaliação fonoaudiológica clínica da deglutição de cada paciente por meio do Gugging Swallowing Screen (GUSS) adaptado, do ASHA NOMS e da Functional Oral Intake Scale (FOIS). Resultados Foram avaliados 129 participantes, com média de idade de 72 anos. De acordo com a escala GUSS, 9,3% dos pacientes apresentaram deglutição normal/funcional, enquanto 90,7% apresentaram disfagia, sendo esta de grau leve em 17,05%, moderado em 33,33% e grave em 37,98%. Quanto aos resultados do ASHA NOMS, a maioria (36,5%) dos pacientes encontrava-se no nível 1, que representa o paciente que não consegue receber alimentação por via oral, tendo a necessidade do uso de alimentação por sonda. Esse dado está de acordo com os resultados observados com a escala FOIS, em que a maioria dos pacientes (42,1%) foi classificada como nível I, quando a ingestão de alimentos ocorre exclusivamente por sondas, sem oferta por via oral. Dos 129 participantes, 59% deles necessitaram de IOT. Ao comparar o tempo de IOT e a gravidade da disfagia, encontrou-se diferença estatisticamente significante, sendo que quanto mais grave a disfagia, maior o tempo que o paciente permaneceu intubado. Conclusão Existe alta incidência de disfagia orofaríngea em pacientes com COVID-19, com maior gravidade durante períodos mais longos de IOT.
ABSTRACT Background COVID-19 comprises a respiratory infection resulting from contamination by SARS-CoV-2, with acute respiratory failure being one of its main characteristics, leading to a high frequency of orotracheal intubation (OTI), which in turn increases the risk for dysphagia. Since this can lead to pulmonary impairment, knowing the real occurrence of dysphagia in part of the Brazilian population and its associations allows early and effective clinical management of the multidisciplinary team in relation to patients. Objective To verify the occurrence of dysphagia in COVID-19-positive adult patients in two Brazilian reference hospitals in the care of the pandemic. Methods This was a prospective, longitudinal observational study carried out in two private hospitals in Brazil, both references in the care of patients with coronavirus isolation. Data were initially collected by consulting the medical records of each patient. Information was collected regarding sex, age, previous diseases, COVID-19 testing, and the OTI period. After data collection, the clinical speech-language assessment of swallowing for each patient was carried out using the adapted Gugging Swallowing Screen (GUSS), the ASHA NOMS and the Functional Oral Intake Scale (FOIS). Results A total of 129 participants were evaluated, with a mean age of 72 years. According to the GUSS scale, 9.3% of the patients presented normal/functional swallowing, while 90.7% presented dysphagia, with mild dysphagia in 17.05%, moderate dysphagia in 33.33%, and severe dysphagia in 37.98%. As for the results of the ASHA NOMS, the majority (36.5%) of the patients were at level 1, which represents the patient who is not able to receive his or her food supply orally, having the need to use tube feedings. This is in line with the results observed with the FOIS scale, whereby most patients (42.1%) were classified as Level I, when food intake occurs exclusively through feeding tubes, with no oral supply. Of the 129 participants, 59% of them required OTI. When comparing the time of OTI and the severity of dysphagia, there was a statistically significant difference, with more severe dysphagia, the longer the patient remained intubated. Conclusion: There is a high incidence of oropharyngeal dysphagia in patients with COVID-19, with increased severity during longer periods of OTI.

ABSTRACT Objective: This study aimed to explore the patterns of radioactive iodine (RAI) use for differentiated thyroid cancer (DTC) in Brazil over the past 20 years. Materials and methods: A retrospective analysis of the DTC-related RAI prescriptions, from 2000 to 2018, retrieved from the Department of Informatics of the Unified Health System (Datasus) and National Supplementary Health Agency (ANS) database was performed. RAI activities prescriptions were re-classified as low (30-50 mCi), intermediate (100 mCi), or high activities (>100 mCi). Results: The number of DTC-related RAI prescriptions increased from 0.45 to 2.28/100,000 inhabitants from 2000 to 2015, declining onwards, closing 2018 at 1.87/100,000. In 2018, population-adjusted RAI prescriptions by state ranged from 0.07 to 4.74/100,000 inhabitants. Regarding RAI activities, in the 2000 to 2008 period, the proportion of high-activities among all RAI prescriptions increased from 51.2% to 74.1%. From 2009 onwards, there was a progressive reduction in high-activity prescriptions in the country, closing 2018 at 50.1%. In 2018, the practice of requesting high-activities varied from 16% to 82% between Brazilian states. Interestingly, variability of RAI use do not seem to be related to RAI referral center volume nor state socio-economic indicators. Conclusion: In recent years, there has been a trend towards the lower prescription of RAI, and a reduction of high-activity RAI prescriptions for DTC in Brazil. Also, significative inter-state and inter-institutional variability on RAI use was documented. These results suggest that actions to advance DTC healthcare quality surveillance should be prioritized.

PEG-PCL Nanocapsules Containing Curcumin: Validation of HPLC Method for Analyzing Drug-loading Efficiency, Stability Testing and Cytotoxicity on NIH-3T3 Cell Line

Abstract Curcumin (CUR) shows potential use for treating cancer. However, CUR has low solubility and reduced bioavailability, which limit its clinical effect. Therefore, the development of nanocarriers can overcome these problems and can ensure the desired pharmacological effect. In addition, it is mandatory to prove the quality, the efficacy, and the safety for a novel nanomedicine to be approved. In that sense, this paper aimed (a) to prepare CUR-loaded polyethylene glycol-poly(ε-caprolactone) nanocapsules; (b) to validate an analytical method by high performance liquid chromatography (HPLC) for quantifying CUR in these nanoformulations; (c) to evaluate the physicochemical stability of these formulations; and to investigate their cytotoxicity on NIH-3T3 mouse fibroblast cells. The HPLC method was specific to CUR in the loaded nanocapsules, linear (r = 0.9994) in a range of 10.0 to 90.0 µg.mL-1 with limits of detection and quantification of 0.160 and 0.480 µg.mL-1, respectively. Precision was demonstrated by a relative standard deviation lower than 5%. Suitable accuracy (102.37 ± 0.92%) was obtained. Values of pH, particle size, polydispersity index, and zeta potential presented no statistical difference (p > 0.05) for CUR-loaded nanoparticles. No cytotoxicity was observed against NIH-3T3 mouse embryo fibroblast cell line using both the tetrazolium salt and sulforhodamine B assays. In conclusion, a simple and inexpensive HPLC method was validated for the CUR quantification in the suspensions of nanocapsules. The obtained polymeric nanocapsules containing CUR showed suitable results for all the performed assays and can be further investigated as a feasible novel approach for cancer treatment.

Abstract Objective: To analyze whether demographic and socioeconomic factors can influence the OHRQoL in schoolchildren. Material and Methods: A cross-sectional study using a representative sample of 1137 children aged 8-10 years, enrolled in public schools in Florianópolis, Brazil, was conducted. Children answered the Brazilian version of the Child Perceptions Questionnaire (CPQ8-10) quality of life questionnaire and their parents responded to socioeconomic and demographic questions. Logistic regression models were used to analyze the data. Results: Higher probability of presenting a negative impact on the OHRQoL was observed in children whose skin color was non-white (OR 1.46, 95% CI 1.04-1.69) and female (OR 1.33, 95% CI 1.04-1.69). Children in whom the head of the family had years of schooling 9-11 (p=0.001), 5-8 (p=0.005) and ≤4 (p=0.000) were more likely to have a negative impact on the OHRQoL. CPQ8-10 domains evaluation showed that variables education, gender, skin color, and age were associated with a negative impact on OHRQoL (p≤0.05). Conclusion: The educational level of the head of the family and the demographic characteristics of the children are associated with a negative impact on the OHRQoL.

RESUMO: Bivalves permianos da Bacia do Paraná evoluíram em um imenso mar interior, sob condições de extremo isolamento e estresse ambiental. Embora conhecidos desde 1918, a história evolutiva desses bivalves é ainda obscurecida pelo conhecimento incompleto e enviesado sobre a composição faunística e a distribuição estratigráfica de várias assembleias. Portanto, a descrição da assembleia de Tiaraju, a única conhecida no Grupo Passa Dois do sul do Brasil, contribui com novas informações-chave sobre a composição, biocorrelação e idade dessa fauna de moluscos únicos. Terraia falconeri, Cowperesia emerita, Holdhausiella elongata e Terraia altissima foram registradas e descritas. T. falconeri é a espécie mais comum, seguida por C. emerita, H. elongata e T. altissima. A fauna é maiormente composta por Terrainae, faltando os Pinzonellinae. A assembleia é, desse modo, pobremente diversificada, no que tange à composição faunística e guildas (somente bivalves de infauna, facultativamente móveis, suspensívoros estão presentes). Cowperesia emerita e T. altissima sugerem, fortemente, biocorrelação com assembleias das formações Rio do Rasto e Gai-As, Brasil e Namíbia, indicando idade não mais jovem que o Permiano médio (Wordiano-Capitaniano). À luz dessas informações, o mapa geológico da região de Tiaraju, São Gabriel, e o contato local entre as formações Teresina e Rio do Rasto, requerem, ambos, reavaliação.
ABSTRACT: Permian bivalves of the Paraná Basin evolved in a large inland sea, under conditions of extreme isolation and environmental stress. Although known since 1918, its evolutionary history is still obscure due to the incomplete and biased information on faunal composition and stratigraphic distribution of various assemblages. Hence, the description of the Tiaraju assemblage, the only known bivalve occurrence in the Passa Dois Group from the southernmost Brazil, adds new key information on the composition, biocorrelation and age of this unique molluscan fauna. Terraia falconeri, Cowperesia emerita, Holdhausiella elongata, and Terraia altissima were recorded and described. T. falconeri is the commonest species, followed by C. emerita, H. elongata and T. altissima. The faunule is mainly composed by Terrainae, lacking Pinzonellinae. Assemblage is, thus, poorly diversified, both regarding the faunal composition and guild structure (only infaunal, facultatively motile, unattached, suspension feeders present). Cowperesia emerita and T. altissima strongly indicate biocorrelation with assemblages of the Rio do Rasto and Gai-As formations, Brazil and Namibia, providing an age no younger than the mid-Permian (Wordian-Captianian). On the light of this information, the current geological map of the Tiaraju region, São Gabriel, and the local boundary between the Teresina and Rio do Rasto formations requires reevaluation.

RESUMO: O objetivo do trabalho foi avaliar se o uso do óleo essencial de Aloysia triphylla (OEAT) na água de transporte de peixes, in vivo como sedativo, poderia aumentar a estabilidade oxidativa de filés de jundiá (Rhamdia quelen) durante o armazenamento congelado. Avaliou-se a composição química do OEAT e dos filés dos peixes expostos ao OEAT (0, 30 ou 40µL L-1), bem como o pH e indicadores de oxidação lipídica (dienos conjugados, DC; substâncias reativas ao ácido tiobarbitúrico, TBARS) dos filés ao longo do armazenamento (-18±2oC/17 meses). O α- e o β-citral foram os compostos majoritários do OEAT. O tratamento com OEAT não modificou a composição centesimal dos filés de jundiá, mas 40µL L-1 de OEAT reduziu o pH dos filés, comparado ao controle (P<0,05). Foi observado maior teor inicial de DC nos filés dos tratamentos 30 e 40µL L-1 de OEAT e menor valor de TBARS nos filés do tratamento 40µL L-1 de OEAT após 6, 9 e 17 meses de congelamento, em comparação com os filés controle (P<0,05). Os resultados indicam que o uso do OEAT como sedativo na água de transporte de jundiás retarda a degradação de produtos primários da oxidação lipídica (DC) em produtos secundários (TBARS) nos filés congelados. Este retardo na velocidade de oxidação lipídica pode ampliar a vida útil dos filés congelados.
ABSTRACT: This research aimed to evaluate whether the essential oil of Aloysia triphylla (EOAT) used in vivo as a sedative in the water for transporting fish could increase the oxidative stability of silver catfish (Rhamdia quelen) fillets during frozen storage. The chemical composition of EOAT and of fillets from fish exposed to EOAT (0, 30 or 40µL L-1) were assessed. The pH and lipid oxidation parameters (conjugated dienes, CD; thiobarbituric acid-reactive-substances, TBARS) were evaluated in the fillets throughout the storage period (-18±2oC/17 months). The main compounds found in EOAT were α- and β-citral. Treatment with EOAT did not modify the proximate composition of the fillets, but 40µL L-1 EOAT reduced pH levels when compared to the control fillets (P<0.05). Compared to the control fillets, the fillets from fish treated with 30 and 40µL L-1 EOAT had higher initial CD values (P<0.05), whereas fillets from fish treated with 40µL L-1 EOAT had lower TBARS levels after 6, 9 and 17 months of storage (P<0.05). Results indicated that use of EOAT as a sedative in silver catfish transport water delays the degradation of primary oxidation products (CD) into secondary products (TBARS) in the frozen fillets. This delay in the lipid oxidation rate may increase the shelf life of frozen fillets.

Thin films containing silver nanoparticles homogeneously dispersed, with narrow size distribution below 10 nm, were synthesized on flat glass surface, by using an ionic silsesquioxane as stabilizer and crosslinking agent. The films can be prepared without previous functionalization of substrate surfaces and without addition of other components. The films were heat treated up to 200 ºC and characterized by ultraviolet-visible, transmission electron microscopy, atomic force microscopy, thermogravimetric analysis and ellipsometry. The films were thermally stable when heated up to 200 ºC, presenting the same thickness, and maintaining both optical and morphological properties of silver nanoparticles. The antibacterial activity of the films, containing the silver nanoparticles, was evaluated against Staphylococcus aureus by using the film applicator coating method, showing an excellent performance even after the third cycle of sterilization.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Intervenção psicológica a gestantes: contribuições do grupo de suporte para a promoção da saúde

O presente artigo visa a apresentar um modo de o psicólogo atuar com gestantes, utilizando princípios do grupo de suporte. A intervenção grupal procurou promover a saúde da mulher que passa por essa fase vital, criando um espaço para compartilharem reflexões e informações acerca das mudanças que atravessam. A metodologia consistiu no relato da experiência ocorrida num hospital privado, no Município de Resende. Foram realizados 22 encontros. Para orientar o trabalho, cada encontro previa três momentos: apresentações, discussão do tema através de palestra interativa e momento de avaliação. Como resultados, constatamos que a identificação entre as gestantes, o compartilhar sentimentos e o apoio mútuo funcionaram como suporte social. Concluímos que o grupo constituiu uma intervenção primária, à medida que tomou uma postura de promoção da saúde, atingindo os níveis de atuação psicoterapêutico, psicopedagógico e psicoprofilático.
The present article aims at showing a psychologist treatment with pregnant women using the support group principles. This treatment attempted to promote the health of the women who are in this vital phase, creating a space to share their reflections and informations concerning the changes in this period. The methodology consisted in one experienced description that happened in a private hospital in Resende-RJ. There were 22 meetings and in each of them there were specific moments: presentations, discussion of the topic by an interactive talk and an evaluation moment. As results, we verified that the identification between the pregnant women, the feelings shared and the mutual support functioned as social supports. We concluded that the group constituted a primary intervention and reached the levels of psychotherapeutic, psycopedagogic and psycoprophylactic actuation.
El presente artículo pretende presentar un modo para que el psicólogo actué con gestantes, utilizando principios del grupo de soporte. La intervención de grupo buscó promover la salud de la mujer que pasa por esa fase vital, creando un espacio para que compartan ponderaciones e informaciones acerca de los cambios que atraviesan. La metodología consistió en el relato de la experiencia ocurrida en un hospital privado, en el Municipio de Resende. Fueron realizados 22 encuentros. Para orientar el trabajo, cada encuentro preveía tres momentos: presentaciones, discusión de la tema a través de exposición interactiva y momento de evaluación. Como resultados, constatamos que la identificación entre las gestantes, compartirlo sentimientos y el apoyo mutuo funcionaron como soporte social. Concluimos que el grupo constituyó una intervención primaria, a la medida que tomó una postura de promoción de la salud, alcanzando los niveles de actuación psicoterapéutico, psicopedagógico y psicoprofiláctico.
