ABSTRACT. Unpaid caregivers of people living with dementia tend to suffer mental health problems as a result of the negative effects associated with the care tasks. Thus, psychosocial interventions for this population group are necessary. iSupport is an online support program for caregivers that was created by the World Health Organization. Objective: To describe the design of a randomized clinical trial to measure the efficacy of the iSupport-Brasil version on caregivers’ mental health and well-being. Methods: The participants will be randomized into Intervention Group (IG) (n=195) and Control Group (CG) (n=195). For three months, the IG will access the iSupport-Brasil platform, the CG will enter the electronic page of the Brazilian Alzheimer’s Association, and both groups will be emailed the preliminary version of the “Guia de cuidados para a pessoa idosa” e-book (a guide to providing care to the elderly) from the Ministry of Health. The data will be collected at three moments: baseline, and three and six months after the beginning of the intervention. Results: It is expected that it will be possible to provide diverse validity evidence about iSupport-Brasil as an online and free intervention alternative, as a preventive means and as a way to promote mental health among caregivers of people living with dementia. Conclusion: Through the evaluation protocol of this randomized clinical trial on the effects of the iSupport-Brasil program, it may become a reference for countries that plan to adapt and improve the iSupport program using digital health solutions.

RESUMO. Cuidadores informais de pessoas que vivem com demência tendem a sofrer problemas de saúde mental como resultado dos efeitos negativos associados às tarefas de cuidado. Assim, intervenções psicossociais para essa população são necessárias. O iSupport é um programa de apoio online para cuidadores criado pela Organização Mundial da Saúde. Objetivo: Descrever o desenho de um ensaio clínico randomizado para medir a eficácia da versão iSupport-Brasil na saúde mental e no bem-estar de cuidadores. Métodos: Os participantes serão randomizados em Grupo Intervenção (GI) (n=195) e Grupo Controle (GC) (n=195). Durante três meses, o GI acessará a plataforma iSupport-Brasil, o CG entrará na página eletrônica da Associação Brasileira de Alzheimer e ambos os grupos receberão por e-mail a versão preliminar do e-book Guia de cuidados para a pessoa idosa, do Ministério da Saúde. Os dados serão coletados em três momentos: linha de base, três e seis meses após o início da intervenção. Resultados: Espera-se que seja possível fornecer diversas evidências sobre os efeitos do iSupport-Brasil como alternativa de intervenção online e gratuita, de forma a promover a saúde mental entre os cuidadores de pessoas que vivem com demência. Conclusão: O protocolo de avaliação deste ensaio clínico randomizado sobre os efeitos do programa iSupport-Brasil pode se tornar uma referência para os países que planejam adaptar e melhorar esta intervenção, usando soluções digitais de saúde.

Background: A population study is an important tool that can be used to understand the actual epidemiological scenario of the Covid-19 in different territories, identify its magnitude, understand its transmission dynamics, and its demographic, geographical, and social distribution. Objective: The aim of this study was to determine the prevalence and dynamics of SARS-CoV-2 infection in the population of two Brazilian cities during the pandemic first wave and subsequent socioeconomic and health effects. Materials & methods: This paper described the methodological procedures adopted and the prevalence of the SARS-CoV-2 infection in the population. A household survey was conducted between October and December 2020, in two historic cities of Brazil's mining region. Anti-SARS-CoV-2 antibody was detected using the Wondfo® rapid test. The face-to-face interview consisted of administration of a questionnaire containing registration data, sociodemographic and economic variables, living habits, general health condition, mental health, sleep habits, and eating and nutrition. Results: We evaluated 1,762 residents, of which 764 (43.4%) were in Mariana and 998 (56.6%) in Ouro Preto. For both cities, 51.9% of the interviewees were female, with a predominance of the age range 35 to 59 years old (47.2%). The prevalence of the SARS-CoV-2 infection was 5.5% in all cities, 6.2% in Ouro Preto, and 4.7% in Mariana. The prevalence was similar between cities (P>0.05). Conclusion: The study was effective in verifying the seroprevalence of infection by the virus and its findings will enable further analyses of the health conditions of the population related to social isolation and the risk of infection with SARS-CoV-2.

To evaluate the clinical outcomes of daptomycin therapy and adherence to treatment recommendations, a retrospective cohort study was conducted with patients that received daptomycin during the period of the study. The adherence and nonadherence to clinical guidelines were assessed through organism identification, dose and time of treatment, management of bacteremia, and vancomycin treatment failure. A multiple logistic regression model analyzed the association between independent variables and clinical success (dependent variable), considering 5% of statistical significance. The study presented 52 patients who received daptomycin for the treatment of bacteremia (21.1%) or infections (osteomyelitis [63.5%], synovial fluid [15.4%]). Most patients (86.5%) received daptomycin as the second line of treatment, and 51.9% achieved clinical success. The patients had a better chance of clinical success when they followed the guideline indications (OR = 16.86; 95% CI = 1.45-195.88) and the medication was prescribed by a specialist in infectious diseases (OR = 4.84; 95% CI = 1.11-21.09). The study demonstrated lower clinical success than that described in the literature because of patients who were not eligible according to the clinical guidelines. Adherence to recommendations and appropriate prescription of reserve antibiotics is important in limiting early resistance, and avoiding clinical failure and unnecessary expenditure.

Resumo Objetivo: analisar os fatores sociodemográficos relacionados ao não uso do preservativo nas relações sexuais e a prevalência de infecções sexualmente transmissíveis (IST) em comunidades rurais de Ouro Preto, Minas Gerais, Brasil, entre 2014 e 2016. Métodos: foram coletados dados de entrevista individual e realizados testes rápidos; as associações foram testadas pelo modelo de regressão de Poisson, com intervalo de confiança de 95% (IC95%). Resultados: foram detectados 3,8 casos/10 mil habitantes de hepatite B e sífilis, e 1,3/10 mil hab. de hepatite C; não foram detectados casos de HIV; na análise multivariada, foram encontradas maiores prevalências de não uso de preservativos entre indivíduos casados/em união estável/viúvos (RP=1,20 - IC95% 1,06;1,36). Conclusão: o grupo com maior prevalência de não uso de preservativo é o de pessoas com relacionamento fixo; novos casos de sífilis e de hepatites virais foram detectados pelo teste rápido, aplicado no inquérito.

Abstract Objective: to investigate socio-demographic factors associated with non-use of condoms, and to describe the prevalence of sexually transmitted infections (STI) in rural communities of Ouro Preto, Minas Gerais, Brazil, 2014 to 2016. Methods: data were gathered from individual interviews and rapid tests were performed; associations were tested using Poisson regression, with a 95% confidence interval (95%CI). RESUMO As membranas poliméricas vêm sendo bastante utilizadas no processo de tratamento de efluentes em todo mundo, solucionando problemas médicos, sociais e ambientais. Sendo assim, membranas de fibra oca foram produzidas a partir da polietersulfona (PES), adicionando a argila Cloisite Na – CLNa e também inserindo o solvente N,N dimetilformamida (DMF), com o intuito de modificar a morfologia da membrana. A argila foi caracterizada por fluorescência de raios-X (FRX) e difração de raios-X (DRX). Por FRX, ficou visto que a argila apresentou característica naturalmente sódica e com pouco materiais acessórios. O DRX mostrou que a Cloisite Na possui característica de uma argila montmorilonita contendo íons Na+. As membranas foram caracterizadas por ângulo de contato (membrana plana), microscopia eletrônica de varredura (MEV) e medidas de fluxo. Por meio das medidas do ângulo de contato foi visto que a presença de argila favoreceu a um aumento da hidrofilicidade das membranas. Nas membranas de fibra oca, foi observado por MEV que a presença da argila modificou a morfologia da membrana, alterando a uniformidade dos “fingers” e dos macroporos e modificando a camada densa no centro do suporte poroso, A adição do ácido no líquido interno modificou ainda mais a morfologia, diminuindo a camada densa no centro do suporte poroso da membrana e também alterando os “fingers” e os macroporos, tendo uma maior uniformidade dos mesmos em relação a membrana sem a presença do DMF no líquido interno. A partir das medidas de fluxo foi possível observar que a membrana contendo 5% de CLNa com a presença do solvente no líquido interno obteve um maior fluxo com o tempo, isto ocorreu devido à morfologia mais uniforme apresentada da membrana.

ABSTRACT Polymer membranes have been widely used in the treatment of effluents worldwide, solving medical, social and environmental problems. Thus, hollow fiber membranes were produced from polyethersulfone (PES) with adding Cloisite Na -CLNa clay and also by inserting the N, N -dimethylformamide (DMF) solvent, in order to modify the membrane morphology. Clay was characterized by X-ray fluorescence (FRX) and X-ray diffraction (XRD). By FRX, it was seen that the clay presented naturally sodium and with little accessory materials. The XRD showed that Cloisite Na has a characteristic of a montmorillonite clay containing Na+. The membranes were characterized by contact angle (flat membrane), scanning electron microscopy (SEM) and flow measurements. By means of the contact angle measurements it was seen that the presence of clay favored an increase the hydrophilicity of the membranes. At the hollow fiber membranes, it was observed by SEM that the presence of clay modified the membrane morphology, changing the uniformity of the “fingers” and the macropores and modifying the dense layer in the center of the porous support. The addition of the acid in the internal liquid modified even more the morphology, decreasing the dense layer at the center of the membrane porous support and also changing the “fingers” and the macropores, having a greater uniformity of the same with respect to the membrane without the presence of DMF in the internal liquid. From the flow measurements it was possible to observe that the membrane containing 5% CLNa with the presence of the solvent in the internal liquid obtained a greater flow over time, this occurred due to the presented more uniform morphology of the membrane.

ABSTRACT Intestinal parasitic infections are a common health problem among Amerindian populations and schistosomiasis represents one of the most prevalent diseases in Maxakali people. The Kato-Katz is the diagnostic method recommended by WHO for epidemiological studies; however, one of the technique’s limitations is the failure to detect parasites in individuals with low parasite load. The aim of this study was to establish the prevalence of Schistosoma mansoni in indigenous Maxakali villages, evaluating the TF-Test® performance for diagnosis compared to the Kato-Katz technique. Stool samples from 545 individuals were processed by the TF-Test® (1 sample) and Kato-Katz (1 slide). The positivity rate for S. mansoni by Kato-Katz was 45.7%. The rate by the TF-Test® was 33.2%, and 51.9% by the combined parasitological techniques. The amplitude of parasite load was 24 to 4,056 eggs per gram of feces (epg), with a geometric mean of 139 epg. The co-positivity, co-negativity, and accuracy values by TF-Test® in relation to Kato-Katz were 59.0%, 88.5%, and 75.0%, respectively. The agreement between these techniques was moderate (k=0.486) as determined by the kappa index. Thus, the results of this study demonstrated that the performance of Kato-Katz was superior (p <0.05) to that of TF-Test® in the detection of S. mansoni. The combination of TF-Test® and Kato-Katz resulted in an increased positivity rate of S. mansoni, demonstrating the high risk of infection to which indigenous populations are exposed and the importance of the implementation of control strategies in Maxakali villages.

Na região Noroeste do Estado de São Paulo, foi estabelecida, em 1965, a Usina Hidrelétrica de Ilha Solteira (CESP), que produziu extensas áreas degradadas, de onde foram retiradas as camadas superficiais do solo. Para o restabelecimento da vegetação, em áreas como estas, é necessária a adição de insumos químicos e matéria orgânica. Associando-se a presença na região de macrófitas à necessidade de recuperação de áreas degradadas e a disponibilidade de sementes de espécies nativas de cerrado, propôs-se o desenvolvimento deste trabalho, com o objetivo de avaliar os efeitos da adição de resíduo orgânico e fósforo sobre material de solo degradado e sobre o crescimento inicial de Dipteryx alata, espécie arbórea de cerrado. O material de solo foi coletado em área degradada pela construção da Usina Hidrelétrica de Ilha Solteira (SP). O experimento foi conduzido em dois ambientes, casa de vegetação e a pleno sol. Os tratamentos consistiram de quatro doses de fósforo (0, 100, 200 e 300 mg dm-3de P2O5), com ou sem adição de macrófitas, produzindo 16 tratamentos com 10 repetições cada um. As macrófitas utilizadas, coletadas na Usina Hidrelétrica de Jupiá (MS), foram secas e passadas em picadeira, incorporadas ao material de solo e incubadas por 60 dias. As sementes de Dipteryx alata, coletadas nas proximidades da área, foram semeadas em areia lavada e após 60 dias introduzidas nos tratamentos. As plantas foram avaliadas mensalmente quanto à altura, ao diâmetro do caule e à clorofila foliar. Após 210 dias, as plantas foram coletadas e avaliadas para peso fresco e seco da parte área e do sistema radicular e o material de solo foi avaliado quanto à fertilidade e atividade microbiana pelo carbono do CO2 liberado. A incorporação de resíduo orgânico ao material de solo degradado afetou positivamente a fertilidade deste e a atividade microbiana. As doses de fósforo aplicadas influenciaram o comportamento de Ca, SB, H+Al e CTC e, quando aplicadas na ausência de resíduo orgânico, promoveu incrementos significativos e lineares nos valores da acidez potencial, mostrando a importância da presença do resíduo orgânico. O crescimento inicial do Dipteryx alata foi favorecido pelo cultivo protegido, pela incorporação de resíduo orgânico e pela adição de fósforo ao solo. Pode-se concluir ainda que há boa perspectiva para deposição deste resíduo sobre o material de solo degradado, embora estudos em condições de campo sejam necessários.

In northwestern São Paulo State, it was established, in 1965, the hydroelectric station of Ilha Solteira (CESP) that produced extensive degraded areas, from where it was removed the soil superficial layers. For the re-establishment of the vegetation, in areas like these, it is necessary the addition of chemical inputs and organic material. Associating the presence in the area of macrophytes, to the need of recovery of degraded areas and the availability of seeds from native species of Cerrado, it was intended the development of this work, with the objective of evaluating the addition effects of organic residue and phosphorus on the material of degraded soil and on the initial growth of Dipteryx alata, arboreal species of 'cerrado'. The soil material was collected in area of degraded soil by the construction of Ilha Solteira hydroelectric station. The experiment was conducted in two environmental conditions, greenhouse and plenty of sun. The treatments consisted of four phosphorus doses (0, 100, 200 and 300 mg dm-3 of P2O5), with or without macrophytes addition, with 16 treatments and 10 repetitions each one. The used macrophytes, collected at Jupiá hydroelectric station, in Mato Grosso do Sul state, were dried, sliced, incorporated to the soil material and incubated during 60 days. The seeds of Dipteryx alata, collected around of the area, were sowed in washed sand and after 60 days introduced in the treatments. The plants were appraised monthly for height, stem diameter and leaves chlorophyll. After 210 days, the plants were collected and evaluated for fresh and dry weight of aerial part and radicular system and the soil material was evaluated for fertility and microbial activity by the carbon of liberated CO2. The organic residue, incorporated to the material of soil degraded, positively affects the soil fertility and the soil microbial activity. The applied phosphorus doses influenced the behavior of Ca, SB, H+Al and CTC, when applied in the absence of organic residue, it promoted significant and linear increments for values of potential acidity, and indicated the organic residue importance. The initial growth of the Dipteryx alata was favored by the protected cultivation, for the incorporation of organic residue and for the phosphorus addition to the soil. At the end although the material of degraded soil comes as good perspective for disposal of this residue, studies about the field conditions are necessary.

The domestic processing of the common bean resulted in reduction in the antinutritional factors phytates and tannins, in the starch content and in the flatulence factors raffinose, stachiose and verbascose. The objetive of this study was to evaluate the effect of the soaking step and the domestic processing of the common bean, on the chemical composition, the levels of phytate, tannin, starch and flatulence factors by utilizing the follows treatments: raw bean (FC), freeze-dried cooked unsoaked bean (FCSM), freeze-dried cooked bean without the non-absorved soaking water (FCSAM), freeze-dried cooked bean with the non-absorved soaking water (FCCAM) and the soaking water (AM). The beans were soaking for a period for 16 hours in the proportion 3:1 (water:beans) at room temperature. The effect of the phytates and tannins on the net protein efficiency ratio (NPR) and protein digestibility using male Wistar rats were studied. A decrease in the phytate content of the beans (85%) with use of soaking was observed. In the case of the tannin content, only the cooking of the beans promoved high decomposition (84%). In the (FCSAM) treatment a decrease in the raffinose (25.0%), stachiose (24.8%), verbascose (41.7%) and starch (26.8%) contents was observed. Diets containing casein (control), casein plus the soluble solids obtain from the soaking water showed no significant difference (p>0.05) for the NPR, as well as for the different bean treatments, although these showing lower values. The treatment (FCSM) showed the higher digestibility (74.3± 5.8%) of the bean treatments, the casein diets showing 94.6± 0.9%. The reduction of the phytates, tannin, starch contents and flatulence factors in the common bean was most effective when the soaking water not absorved was descarted (FCSAM).

Avaliou-se o efeito da maceração e do processamento doméstico do feijão-comum na composição química, nos teores de fitatos, de taninos, de amido e de fatores de flatulência, utilizando os seguintes tratamentos: feijão-comum cru (FC), cozido sem maceração liofilizado (FCSM), cozido sem água de maceração não absorvida liofilizado (FCSAM), cozido com água de maceração não absorvida liofilizado (FCCAM) e na água de maceração (AM). Os feijões foram macerados por um período de 16 horas, na proporção 3:1 (água:feijão), em temperatura ambiente. Através de ensaio biológico com ratos machos Wistar, estudou-se o efeito de fitatos e taninos no quociente de eficiência protéica líquida (NPR) e digestibilidade protéica. Observou-se redução nos teores de fitatos (85%) pela utilização da maceração. No caso do conteúdo de taninos, somente o processo de cozimento do feijão promoveu alta decomposição dos mesmos (84%). No tratamento (FCSAM) observou-se redução nos teores de rafinose (25,0%), estaquiose (24,8%) e verbascose (41,7%) e no teor de amido (26,8%). No ensaio biológico observou-se que o NPR da dieta controle (caseína) e da dieta caseína mais sólidos solúveis da água de maceração, não apresentaram diferença significativa (p> 0,05); entre os tratamentos de feijão também não houve diferença significativa, sendo os valores dos NPR menores do que aqueles da caseína. A digestibilidade do tratamento (FCSM) foi a maior dentre os tratamentos de feijão (74,3± 5,8%); a caseína apresentou valor de 94,6± 0,9%. A remoção dos fatores antinutricionais fitatos e taninos, teor de amido e dos fatores de flatulência do feijão-comum foi mais efetiva no tratamento onde a água de maceração não absorvida era descartada (FCSAM).
