ABSTRACT There is insufficient evidence about the acute effects of short-term static stretching on body balance and training volume in strength training. Thus, the objective of the present study was to investigate the acute effect of different static stretching times on the performance and balance of physically active young people during strength training. Ten physically active male subjects, aged 21.5± 2.12 years old, participated in the experiment, which took place over seven sessions. In the first three sessions, familiarization, test and retest of 10 maximum repetitions and balance tests were performed, through the Star Excursion Balance Test. From the 4th to the 7th session, stretching of 0, 5, 10 or 15 seconds was used, randomly. The results showed that there were no statistically significant differences between the stretching volumes. It is concluded that there was no reduction in the number of repetitions with the load of 10RM and the distances of the steps in the balance test when preceded by static stretching. Static stretching at moderate intensity and duration of up to 15 seconds did not affect the performance or the balance of physically active young people. Furthermore, there is no difference between stretching 5, 10 or 15 seconds for the mentioned acute strength and balance responses.

RESUMO Há insuficiência de evidências acerca dos efeitos agudos do alongamento estático de curta duração sobre o equilíbrio corporal e volume de treino no treinamento de força. Assim, objetivou-se investigar o efeito agudo de diferentes tempos de alongamento estático no desempenho e no equilíbrio de jovens fisicamente ativos, durante o treinamento de força. Participaram 10 indivíduos do sexo masculino, fisicamente ativos, com 21,5± 2,12 anos. O experimento ocorreu ao longo de sete sessões. Nas três primeiras foi realizada a familiarização e o teste e reteste de 10 repetições máximas e de equilíbrio, através do Star Excursion Balance Test. Da 4ᵃ a 7ᵃ sessão utilizou-se alongamentos de 0, 5, 10 ou 15 segundos, de forma randomizada. Os resultados mostraram que não houve diferenças estatisticamente significativas entre os volumes de alongamento. Conclui-se que não houve redução no número de repetições com a carga de 10RM e nas distâncias alcançadas no teste de equilíbrio, quando precedidos por alongamento estático. O alongamento estático em intensidade moderada e duração de até 15 segundos, não afetou o desempenho tampouco o equilíbrio de jovens fisicamente ativos. Ademais, não há diferença entre alongar 5, 10 ou 15 segundos para as respostas agudas de força e equilíbrio mencionadas.

ABSTRACT Background: Recent studies show an increase in nonalcoholic fatty liver disease (NAFLD) in populations with higher consumption of red meat, processed and cooked at high temperatures. On the other hand, the single nucleotide polymorphism rs738409 in the Patatin-like phospholipase domain containing 3 (PNPLA3) gene has been implicated in susceptibility to NAFLD and liver fibrosis. However, the synergistic effect between red meat consumption and the PNPLA3 gene polymorphism in NAFLD has not yet been evaluated. Objective: To evaluate the association between the presence of the polymorphism in the PNPLA3 gene and the consumption of macronutrients, including meat consumption and its cooking method among NAFLD patients. Methods: This was a cross-sectional study with 91 patients diagnosed with NAFLD by liver biopsy with genotyping for the polymorphism in the PNPLA3 gene were included. The consumption of calories and macronutrients was verified using the semi-quantitative food frequency questionnaire and the specific questionnaire on meat consumption. PNPLA3 gene polymorphism was analyzed by real-time polymerase chain reaction (RT-PCR) and anthropometric evaluation was realized. Results: The mean BMI was 32.38±4.58 kg/m² and the waist circumference was 107±10 cm. On liver biopsy, 42% of patients had significant fibrosis (F≥2). The odds ratio of F≥2 was 2.12 for the GG group and 1.54 for the CG group, compared to the CC group. The mean caloric intake was 1170±463.20 kcal/d. The odds ratio in the CC group concerning high red meat consumption in comparison to low consumption was 1.33. For white meat, the odds ratio was 0.8 when comparing high and low intake, also in the CC group. Conclusion: High red meat intake and PNPLA3 gene polymorphism seem to synergistically affect NAFLD and liver fibrosis, requiring confirmation in a larger number of patients and in different populations.

RESUMO Contexto: Estudos recentes mostram um aumento da doença hepática gordurosa não alcoólica (DHGNA) em populações com maior consumo de carne vermelha, processada e cozida em altas temperaturas. Por outro lado, o polimorfismo rs738409 no gene Patatin-like fosfolipase contendo 3 (PNPLA3) tem sido implicado na suscetibilidade à DHGNA e fibrose hepática. No entanto, o efeito sinérgico entre o consumo de carne vermelha e o polimorfismo no gene PNPLA3 na DHGNA ainda não foi avaliado. Objetivo: Avaliar a associação entre a presença do polimorfismo no gene PNPLA3 e o consumo de macronutrientes, incluindo o consumo de carne e seu modo de cozimento em pacientes com DHGNA. Métodos: Realizamos um estudo transversal com 91 pacientes diagnosticados com DHGNA por biópsia hepática e genotipados para o polimorfismo no gene PNPLA3. O consumo de calorias e macronutrientes foi verificado por meio do questionário de frequência alimentar semi-quantitativo (QFA) e do questionário específico sobre consumo de carnes. O polimorfismo no gene PNPLA3 foi analisado por reação em cadeia da polimerase em tempo real (RT-PCR) e a avaliação antropométrica foi realizada. Resultados: O índice de massa corporal médio foi de 32,38±4,58 kg/m² e a circunferência da cintura foi de 107±10 cm. Na biópsia hepática, 42% dos pacientes apresentavam fibrose significativa (F≥2). O odds ratio de F≥2 foi de 2,12 para o grupo GG e 1,54 para o grupo GC, comparado ao grupo CC. A ingestão calórica média foi de 1.170±463,20 kcal/d. O odds ratio para alto consumo de carne vermelha no grupo CC em comparação ao baixo consumo foi de 1,33. Para a carne branca, este valor foi de 0,8 ao comparar o alto e o baixo consumo, também no grupo CC. Conclusão: A alta ingestão de carne vermelha e o polimorfismo no gene PNPLA3 parecem afetar sinergicamente a DHGNA e a fibrose hepática, necessitando de confirmação em maior número de pacientes e em diferentes populações.

Abstract Background Community-Acquired Pneumonia (CAP) is the primary cause of hospitalization in the United States and the third leading cause of death in Brazil. The gold standard for diagnosing the etiology of CAP includes blood culture, Gram-stained sputum, and sputum culture. However, these methods have low sensitivity. No studies investigating the etiology of CAP have been conducted in Brazil in the last 20-years, and the empirical choice of antimicrobials is mainly based on the IDSA guidelines. This is the first national study with this aim, and as a result, there's potential for the Brazilian consensus to be impacted and possibly modify its guidelines rather than adhering strictly to the IDSA's recommendations. Methods The aim of this study is to identify the main microorganisms implicated in CAP by employing a multiplex Polymerase Chain Reaction (mPCR) at the foremost public hospital in Brazil. All patients who were admitted to the emergency department and diagnosed with severe CAP underwent an mPCR panel using nasopharyngeal and oropharyngeal swabs, with the aim of detecting 13 bacterial and 21 viral pathogens. Results A total of 169 patients were enrolled in the study. The mPCR panel identified an etiological agent in 61.5% of patients, with viruses being the most common (42.01%), led by Rhinovirus, followed by Influenza and Coronavirus (non-SARS-CoV-2). Bacterial agents were identified in 34.91% of patients, with S. pneumoniae being the most common, followed by H. influenzae, M. catarrhalis, and S. aureus. Additionally, we found that the prescription for 92.3% of patients could be modified, with most changes involving de-escalation of antibiotics and antiviral therapy. Conclusion Our study revealed different etiological causes of CAP than those suggested by the Brazilian guidelines. Using molecular diagnostic tests, we were able to optimize treatment by using fewer antibiotics.

Abstract Objectives The present study aimed to evaluate the measurement invariance of a general measure of the perception of governmental responses to COVID-|19 (COVID-SCORE-10) in the general population of 13 Latin American countries. Methods A total of 5780 individuals from 13 Latin American and Caribbean countries selected by non-probabilistic snowball sampling participated. A confirmatory factor analysis was performed and the alignment method was used to evaluate invariance. Additionally, a graded response model was used for the assessment of item characteristics. Results The results indicate that there is approximate measurement invariance of the COVID-SCORE-10 among the participating countries. Furthermore, IRT results suggest that the COVID-SCORE-10 measures with good psychometric ability a broad spectrum of the construct assessed, especially around average levels. Comparison of COVID-SCORE-10 scores indicated that participants from Cuba, Uruguay and El Salvador had the most positive perceptions of government actions to address the pandemic. Thus, the underlying construct of perception of government actions was equivalent in all countries. Conclusion The results show the importance of initially establishing the fundamental measurement properties and MI before inferring the cross-cultural universality of the construct to be measured.

This paper evaluates the effect of adding glycerol to the electrodeposition bath on the deposition efficiency and characteristics of Ni-Mo coating in concentrations ranging from 0.07 to 0.82 mol.L-1. Evaluation of the corrosion resistance was performed by means of weight loss tests in four different periods of immersion. Electrochemical techniques, such as obtaining polarization curves, linear polarization resistance, corrosion current density, corrosion potential, and electrochemical impedance spectroscopy were applied. Corrosion measurements were obtained in 0.5 mol.L-1 NaCl acid solution and 0.5 mol.L-1 NaOH alkaline solution. The morphology and microstructures of electrodeposited were analyzed using Scanning Electron Microscopy and Spectrometry X-Ray Diffraction. The effect of glycerol on Ni-Mo hardness was evaluated by Vickers microhardness measurements. The presence of glycerol in the electrodeposition bath increased the grain size and decreased the hardness of the coating. However, it promoted the formation of a more compact and less porous coating, increasing the corrosion resistance of the coating. Moreover, the addition of 0.82 mol.L-1 glycerol increases current efficiency, thereby reducing the amount of energy consumed during electroplating.

RESUMO Objetivo: fazer uma análise pelas resistências à colaboração interprofissional nas práticas profissionais de residentes na atenção primária à saúde. Método: Pesquisa qualitativa Sócio-clínica com 32 residentes de uma Residência Multiprofissional, realizada de 2017 a 2018. A produção de dados incluiu Análise Institucional das Práticas Profissionais, análise documental; diário do pesquisador; e observação. Os dados foram analisados a partir de conceitos da Análise Institucional. Resultados: Revelaram-se contradições entre a reprodução da educação uniprofissional com foco na especialidade e práticas colaborativas interprofissionais. A análise resistencial apontou dois eixos: não-saber como analisador de resistências à colaboração; interferências interprofissionais e relações de saber-poder. As práticas dos residentes foram caracterizadas pela resistência à colaboração interprofissional. Conclusão: A análise resistencial na Residência Multiprofissional evidenciou movimentos integrativos de assimilação e disputas com o poder médico-centrado, com prejuízos ao compartilhamento do cuidado e à comunicação interprofissional. A análise coletiva questionou a formação de profissionais de saúde, revisitando a perspectiva do cuidado integral orientado pelas necessidades dos usuários.

ABSTRACT Objective: to analyze the resistance to interprofessional collaboration in the professional practices of residents in primary health care. Method: Social and clinical qualitative research with 32 residents of a Multiprofessional Residency, carried out from 2017 to 2018. Data production included Institutional Analysis of Professional Practices, document analysis; investigator’s diary; and observation. Data were analyzed based on Institutional Analysis concepts. Results: There were contradictions between the reproduction of uniprofessional education with a focus on the specialty and interprofessional collaborative practices. The resistance analysis pointed to two axes: not-knowing as an analyzer of resistance to collaboration; interprofessional interference and knowledge-power relations. Residents’ practices were characterized as resistant to interprofessional collaboration. Conclusion: The resistance analysis in the Multiprofessional Residency showed integrative movements of assimilation and disputes with physician-centered power, with damage to the sharing of care and interprofessional communication. The collective analysis questioned health professionals education, revisiting the perspective of comprehensive care guided by the users’ needs.

RESUMEN Objetivo: Hacer un análisis por medio de las resistencias a la colaboración interprofesional en las prácticas profesionales de residentes médicos en la atención primaria a la salud. Método: Investigación cualitativa socio clínica con 32 residentes de una Residencia Multiprofesional, realizada entre 2017 y 2018. La producción de datos incluyó Análisis Institucional de las Prácticas Profesionales, análisis documental; apuntes diarios del investigador; y observación. Los datos fueron analizados a partir de conceptos del Análisis Institucional. Resultados: Se revelaron contradicciones entre la reproducción de la educación uniprofesional con énfasis en la especialidad y prácticas colaborativas interprofesionales. El análisis de la resistencia destacó dos ejes: el no saber cómo método de análisis de resistencias a la colaboración; interferencias interprofesionales y relaciones de saber y de poder. Las prácticas de los residentes fueron caracterizadas por la resistencia a la colaboración interprofesional. Conclusión: El análisis de resistencia en la Residencia Multiprofesional evidenció movimientos integrativos de asimilación y disputas con el poder médico centrado, con daños a la división del cuidado y a la comunicación interprofesional. El análisis colectivo cuestionó la formación de profesionales de salud, revisitando la perspectiva del cuidado integral orientado por las necesidades de los pacientes.

RESUMO Objetivo: Determinar a epidemiologia e a prevalência da maculopatia miópica e da miopia patológica e os fatores de risco associados. Métodos: Trata-se de estudo observacional transversal retrospectivo realizado em um serviço de oftalmologia, com 59 pacientes com idade entre 7 e 70 anos e equivalente esférico maior que -6 dioptrias. Suas retinografias foram laudadas segundo a classificação META-PM, por dois oftalmologistas e um retinólogo experiente. A análise estatística foi realizada conforme o Matlab R2010, com o Excel 2010 e o Statistical Package for the Social Sciences , versão 20.0, sendo utilizado o resultado da análise de regressão logística binária múltipla. Resultados: De acordo com a META-PM, a prevalência da maculopatia miópica nos cem olhos analisados foi de 19% para C0, 53% para C1,18% para C2,2% para C3 e 8% para C4. A prevalência da miopia patológica foi de 39%, sendo que 37% desses olhos possuíam maculopatia miópica C1 com lesões plus , C2 ou pior ou estafiloma posterior, e 2% apresentavam categoria menor que C2, sem lesões plus , porém com estafiloma posterior. A análise de regressão logística binária múltipla revelou associação entre idade e equivalente esférico com a presença da miopia patológica (p<0,05), evidenciando que o aumento de 1 ano na idade implicou em 1,05 vez (razão de chance de 1,05) mais chance de apresentar miopia patológica (p<0,001; intervalo de confiança de 95% de 1,02-1,08). O aumento de 1 dioptria no equivalente esférico maior que -6 dioptrias acarretou 1,19 vez (razão de chance de 1,19) maior risco de apresentar miopia patológica (p=0,001; intervalo de confiança de 95% de1,08-1,32). Por fim, não houve associação entre sexo e presença da miopia patológica (p=0,784). Conclusão: A classificação META-PM é uma ferramenta importante na padronização do estadiamento da lesão miópica, permitindo comparação entre estudos e normatização de condutas. O avançar da idade e o equivalente esféricomiópico estão relacionados à severidade da maculopatia miópica e à presença da miopia patológica.

ABSTRACT Objective: To determine the epidemiology and prevalence of myopic maculopathy and pathologic myopia and associated risk factors. Methods: This is a retrospective cross-sectional observational study performed at an ophthalmology center, including 59 patients aged 7 to 70 years, and spherical equivalent higher than -6 diopters. Their retinographies were assessed by two ophthalmologists and an experienced retina specialist, using the META-PM study classification. Statistical analysis was performed using Matlab R2010, Excel 2010 and Statistical Package for the Social Sciences version 20.0, based on the result of multiple binary logistic regression analysis. Results: According to META-PM, the prevalence of myopic maculopathy in 100 eyes analyzed was 19% C0; 53% C1; 18% C2; 2% C3; 8% C4. The prevalence of pathologic myopia was 39%, and 37% of these eyes having myopic maculopathy category C1 with lesions plus, C2 or worse, or posterior staphyloma, and 2% in category smaller than C2, without lesions plus, but with posterior staphyloma. Multiple binary logistic regression analysis revealed an association between age and spherical equivalent inpathologic myopia (p<0.05), demonstrating the increase by 1 year in age implied in 1.05-fold (odds ratio=1.05) more likelyto present pathologic myopia (p<0.001; 95%CI 1.02-1.08). The increase by 1 diopter in the spherical equivalent higherthan -6 diopters, led to 1.19-fold (odds ratio=1.19) greater risk of presenting pathologic myopia (p=0.001; 95%CI 1.08-1.32). Finally, there was no association between sex and pathologic myopia (p=0.784). Conclusion: The META-PM study classification is an important tool to standardize myopic lesion staging, allowing comparison between studies and establishing management. Advanced age and myopic spherical equivalent are related to severity of myopic maculopathy and pathologic myopia.

Abstract Transgender is a broad-spectrum term referring to people who do not match their assigned sex at birth. Several issues, including social detachment, poor access to healthcare services, and the lack of social/economic opportunities, have historically affected this population. In this scenario, pharmacists - not mentioned in national and international clinical guidelines/protocols regarding transgender care - might be key professionals to serve and care for this population. The main goal of this literature review was to identify the pharmacists’ role in the care of transgender people. The articles were retrieved from scientific databases. After applying filters (e.g., language, full-text availability, and coverage of the research question), seven articles were included in this review. The articles did not present pharmaceutical experiences or practices regarding transgender healthcare. Instead, some articles stressed the inadequate or inappropriate use of hormones and their side effects, also covering the role of this issue in professional development, gaps, and potential opportunities for research and pharmaceutical services. Pharmacists, included in a multi-professional team, could be a key element to promote access to healthcare and the well-being of the transgender population.

RESUMO O presente estudo objetivou investigar os motivos de ingresso, interrupção e permanência em cursos de graduação em Educação Física (EDF) de Porto Velho/RO, comparando a Licenciatura (LIC) ao Bacharelado (BAC). Participaram 238 estudantes, que responderam ao questionário. Os motivos de ingresso mais citados foram identificação pessoal e profissional. A necessidade de prover o sustento próprio ou familiar foi o motivo de interrupção mais frequente. O principal motivo de permanência foi o gosto ou a identificação com a EDF. Estudantes de LIC apresentaram menor renda e maior frequência de interrupção do curso. Conclui-se que, embora haja significativas similaridades entre os cursos quanto às variáveis estudadas, existem diferenças entre eles que merecem ser consideradas.

ABSTRACT The present study aimed to investigate the reasons for entering, interrupting and staying in undergraduate courses in physical education (EDF) in Porto Velho, comparing licentiate (LIC) and bachelor (BAC) students. 238 students participated, answering the questionnaire. The main reasons were personal identification and professional identification. The need to support themselves or the family was the most frequent reason for interruption. The main reason for staying was liking/identification with EDF. LIC students had lower income and higher frequency of course interruptions. It is concluded that although there are significant similarities between the courses regarding the variables studied, there are differences between them that deserve to be considered.

RESUMEN Este estudio tuvo como objetivo investigar los motivos de ingreso, interrupción y permanencia en los cursos de graduación en Educación Física (EDF) en Porto Velho, comparando licenciatura (LIC) y bachillerato (BAC). Participaron 238 alumnos, respondiendo el cuestionario. Las razones más citadas para ingreso fueron identificación personal y profesional. La necesidad de proveer el sustento fue el motivo más frecuente de interrupción. La principal razón para permanencia fue el gusto/identificación con la EDF. Estudiantes de LIC presentaron menor renta familiar y mayor frecuencia de interrupción del curso. Se concluye que si bien existen similitudes significativas entre los cursos en cuanto a las variables estudiadas, existen diferencias entre ellos que merecen ser consideradas.

RESUMO: Discutiremos os desdobramentos psicopatológicos na zona arcaica da subjetividade quando a identificação projetiva perde sua dimensão de comunicação primitiva, assumindo um caráter radical de evacuação e prejudicando a rêverie materna. Estudaremos as experiências precoces do psiquismo englobadas na parte bebê do self, fundamentais aos processos de simbolização primária. Suas falhas se manifestam na clínica através de intensos sofrimentos capitaneados por angústias primitivas, como as de abandono e de separação, que acarretam respostas radicais, tais como a fragmentação e a despersonalização. Apresentaremos um fragmento clínico para ilustrar a forma de desorganização psíquica própria às falhas na parte bebê do self.

Abstract: We will discuss the psychopathological evolution in the archaic zone of subjectivity when the projective identification loses its primitive communication dimension, assuming a radical role of evacuation and damaging the maternal rêverie. We will study the early experiences of the psyche embedded in the baby part of the self, fundamental to the processes of primary symbolization. Their failures are manifested in the clinic through intense sufferings captained by primitive anxieties, such as abandonment and separation, which lead to severe responses such as fragmentation and depersonalization. We will present a clinical fragment to illustrate the archaic forms of psychic disorganization proper to failures in the baby part of the self.

In Brazil, the graduation in Physical Education (EDF) occurs in two types of course, Licentiate (LIC) and Bachelor (BAC). The present study aimed to investigate the reasons for entering, interrupting and staying in undergraduate courses in EDF in Porto Velho town, comparing LIC and BAC students. A total of 238 EDF students from two higher education institutions participated in this descriptive, quantitative and qualitative research, one public, with a LIC course and the other private, with a BAC course. Questionnaire with closed and open questions was used for data collection. Content analysis, with categorization of responses, was used for analysis and interpretation of responses, followed by presentation in frequency and percentages. The main reasons for admission were: personal (48.31%) and professional (16.39%) identification. Most undergraduate students didn't interrupt (63.44%) and never thought about giving up (69.48%). About interruption, extrinsic factors, related to family or self support (47,82%) or health problems (23,95%) were the most frequent reason. The main reasons for staying were liking/identifying with the EDF (28.14%); and opportunity/need to complete higher education (25.92%). The results show that the main reason for joining undergraduate courses in EDF differs from the main reason for permanence, finding that the experiences and contents of the courses bring students closer to a more adequate knowledge about the concrete reality of professional practice in such field, as as well as the skills required to do so. These results viabilize better compreehension about Brazilian reality about professional’s formation process.

La formación en educacion física (EDF) en Brasil puede realizarse en cursos superiores de dós tipos: licenciatura (LIC) y bachirellato (BAC). Nuestro objetivo es investigar las razones para ingressar, interrumpir y permanecer em cursos de grado presencial em EDF em Porto Velho, comparando estudiantes de LIC e BAC. Em esta investigación descriptiva y cuanticalitativa. Participaron 238 estudiantes de EDF de dos instituiciones de educación superior, uma publica con curso LIC, y la otra privada, con curso BAC. Para la recolección de datos se utilizo um cuestionario con preguntas abiertas y cerradas. Para el análisis e interpretación de las respuestas se aplico el analisis de contenido. Los principales motivos de comienzo fueron: identificación personal (48,31%) y profesional (16,39%). La mayoria de los estudiantes nunca interrumpierón la graduación (63,44%) y nunca pensaron em hacerlo (69,48%). Sobre la interupción, factores extrinsecos, relacionados al sustento familiar o proprio (47,82%) o problemas de salud (23,95%) fueron las razones más frecuentes. Los principales motivos de permanencia fueron el agrado/identificación con EDF (28,14%); y oportunidade/necesidad de completar la educación superior (25,92%). Los resultados mostran que el motivo principal para iniciar el curso de graduación em EDF difere del principal motivo de permanencia, sugiriendo que las experiencias e contenidos de los cursos acercan los estudiantes de uma compreensión más adecuada de la realidade concreta de la pratica profesional el este campo, así como de las habilidades requeridas para tal fin. Estos resultados permitén una mejor comprensión de realidad brasileña cuanto al processo de formasión professional en la EDF.

O objetivo desta pesquisa é investigar os motivos de ingresso, interrupção e permanência em cursos de graduação em EDF em Porto Velho, comparando estudantes de LIC e BAC. Participaram 238 estudantes de duas instituições de ensino superior, uma pública, com curso de LIC e outra privada, com curso de BAC. Trata-se de uma pesquisa descritiva de caráter quanti-qualitativo. Para coleta de questionário foi solicitado com as perguntas abertas e empregada análise de conteúdo, com categorização das respostas, seguida pela apresentação em frequência e percentuais.Os principais motivos de ingresso foram: identificação pessoal (48,31%) e profissional (16,39%). A maioria dos estudantes nunca interrompeu uma graduação (63, 44%) e nunca proposta em desistir (69,48%). Sobre a interrupção, fatores extrínsecos ao sujeito, relacionado à necessidade de prover o sustento próprio ou da família (47,82%) ou problemas de saúde (23,95%) foram os motivos mais frequentes. Os principais motivos de permanência foram gosto / identificação com a EDF (28,14%) e oportunidade / necessidade de concluir o curso superior (25,92%).Destaca-se que o principal motivo de ingresso em cursos de graduação em EDF difere do principal motivo de permanência, sugerindo que as vivências e conteúdos dos cursos aproximam os acadêmicos de um conhecimento mais adequado sobre a realidade concreta da atuação profissional, bem como das especificações requeridas para tanto. Tais resultados possibilitam uma melhor compreensão sobre os processos que afetam a formação profissional em EDF no Brasil.

Resumo As fraturas de Tillaux são fraturas da margem lateral da tíbia distal, geralmente relatadas em crianças entre 12 e 14 anos. Como fraturas intra-articulares, requerem redução e fixação anatômica para evitar complicações pós-traumáticas. Como o mecanismo de lesão é a rotação externa do pé na perna, essas lesões são comumente associadas a outras fraturas ou lesões ligamentares. Atualmente, a artroscopia está sendo cada vez mais utilizada para auxiliar e melhorar o tratamento cirúrgico das fraturas do tornozelo. Os autores descrevem um acompanhamento de 12 meses de um caso raro de uma fratura não percebida de Tillaux associada a lesão por sindesmose em um paciente de politrauma com 76 anos de idade, tratado com sucesso por redução e fixação interna assistida por artroscopia.

Abstract Tillaux fractures are fractures of the lateral margin of the distal tibia, usually reported in children between 12 and 14 years old. As intraarticular fractures, they require anatomic reduction and fixation to avoid posttraumatic complications. Since the injury mechanism is external rotation of the foot on the leg, these injuries are commonly associated with other fractures or ligamentous lesions. Currently, arthroscopy is being increasingly used to assist and improve surgical treatment of ankle fractures. The authors describe a 12-month follow-up of a rare case of a missed Tillaux fracture associated with syndesmosis injury in a 76-year-old polytrauma patient, successfully treated by arthroscopically-assisted reduction and internal fixation.

Resumo Introdução: A administração de surfactante por técnica minimamente invasiva (MIST) é um procedimento que visa reduzir o número de intubações e riscos associados. O objetivo deste estudo foi comparar a técnica MIST com a técnica INtubation-SURfactant-Extubation (INSURE). Material e métodos: Análise retrospetiva (de janeiro 2015 a junho 2019) de recém-nascidos (RN) pré-termo sob ventilação não invasiva com pressão positiva contínua nas vias aéreas (nCPAP) tratados com surfactante. Resultados: Foram incluídos 54 RN, que foram divididos em dois grupos: MIST (n=34) e INSURE (n=20). Não foram observadas diferenças significativas entre grupos relativamente à idade gestacional (p=0.480), peso ao nascer (p=0.299), fração inspirada de O2 (FiO2) prévia ao surfactante (p=0.220), duração da oxigenoterapia (p=0.306), progressão para intubação (p=0.712) ou tempo de permanência na unidade de cuidados intensivos neonatais (p=0.778). A variação de FiO2 antes e após a administração de surfactante foi maior no grupo MIST (14% vs 9%, p=0.078). Nenhuma das técnicas registou complicações relevantes. Conclusões: MIST é uma técnica segura em RN pré-termo em nCPAP. Este estudo evidencia resultados semelhantes entre as técnicas MIST e INSURE, com uma maior diminuição das necessidades de FiO2 com a técnica MIST. Pode concluir-se que a técnica MIST é menos invasiva e tão eficaz como a INSURE.

Abstract Introduction: Minimally invasive surfactant therapy (MIST) is a surfactant administration procedure that intends to reduce intubations and associated risks. The aim of this study was to compare MIST with INtubation-SURfactant-Extubation (INSURE) technique. Material and methods: Retrospective analysis (from January 2015 to June 2019) of preterm infants on nasal continuous positive airway pressure (nCPAP) treated with surfactant. Results: Fifty-four preterm infants were included and divided in two groups: MIST (n=34) and INSURE (n=20). No significant differences were found between groups regarding gestational age (p=0.480), birth weight (p=0.299), fraction of inspired oxygen (FiO2) prior to surfactant (p=0.220), oxygen therapy duration (p=0.306), progression to intubation (p=0.712), or length of Neonatal Intensive Care Unit stay (p=0.778). FiO2 variation before and after surfactant administration was higher in MIST group (14% vs 9%, p=0.078). No significant complications were reported with either technique. Conclusions: MIST is a safe technique in preterm infants on nCPAP. This study shows similar outcomes with MIST and INSURE procedures, with a greater reduction in FiO2 requirements with MIST. Overall, MIST is less invasive and as effective as INSURE in preterm infants.

Resumo Objetivo: analisar as evidências científicas relacionadas aos riscos ocupacionais e às intervenções que promovem segurança no trabalho para a equipe de enfermagem oncológica. Métodos: revisão de literatura realizada nas bases de dados MEDLINE, Web of Science, Scopus, LILACS, IBECS e BDENF. Resultados: a busca identificou 17 estudos primários, publicados em inglês, no período de 2008 a 2017. Dentre os fatores que configuram riscos ocupacionais, destacam-se: o déficit no conhecimento; a pouca disponibilidade de equipamentos de proteção individual para manuseio dos medicamentos antineoplásicos; o estresse; e a ansiedade. Quanto às intervenções que promovem a segurança, evidenciam-se: a manutenção do ambiente laboral favorável ao desempenho da assistência; a disponibilidade de insumos; o bom relacionamento interpessoal; a organização do trabalho; o dimensionamento profissional adequado; o conhecimento, a disponibilidade e o incentivo ao uso dos equipamentos de proteção; e a compreensão dos riscos ocupacionais aos quais os profissionais estão expostos. Conclusão: o estudo possibilitou a identificação de fatores associados à atividade laboral que comprometem a saúde da equipe de enfermagem oncológica, demonstrando a necessidade de intervenções voltadas para a melhoria das relações interprofissionais, a capacitação dos profissionais e o oferecimento de um ambiente de trabalho seguro e condições organizacionais que promovam a saúde dos trabalhadores.

Abstract Objective: to analyze scientific evidence on occupational risks and interventions aimed at promoting workplace safety for oncology nurses. Methods: literature review carried out in MEDLINE, Web of Science, Scopus, LILACS, IBECS, and BDENF databases. Results: the search returned 17 primary studies, published in English from 2008 to 2017. The most common factors identified as associated with occupational risks were knowledge deficit, insuficient personal protective equipment (PPE) available for handling anticancer drugs, stress, and anxiety. The interventions identified as promoting occupational safety were: keeping the work environment suitable for care performance; supplies availability; good interpersonal relationship; work organization; suitable professional dimensioning; availability, awareness and encouragement on the use of PPE; and the understanding of the occupational risks to which workers are exposed. Conclusion: the results enable the identification of work-related factors that endanger the oncology nursing teams’ health, evincing the need for interventions aimed at improving interprofessional relationships, qualifying professionals, and offering a safe working environment and organizational conditions that will promote workers’ health.

OBJECTIVES: Co-infection with hepatitis A or B viruses may aggravate liver injury in patients infected with hepatitis C virus (HCV). However, few studies have assessed co-infection with hepatitis E virus (HEV) and HCV. Therefore, this study aimed to assess the prevalence and impact of HEV infection among Brazilian patients with chronic HCV infection. METHODS: This observational study included adult patients with chronic HCV infection who were naive to antiviral therapy from January 2013 to March 2016. A total of 181 patients were enrolled, and HEV serology and PCR were performed for all patients. RESULTS: Seropositivity for anti-HEV IgG was detected in 22 (12.0%) patients and anti-HEV immunoglobulin M in 3 (1.6%). HEV RNA showed inconclusive results in nine (4.9%) patients and was undetectable in the remaining patients. HEV serology positive patients had more severe liver disease, characterized by liver fibrosis ≥3 versus ≤2 (p<0.001), Aspartate Aminotransferase-to-Platelet Ratio Index of ≥1.45 (p=0.003), and Fibrosis-4 score of ≥3.25 (p=0.001). Additionally, the odds of HEV-positive patients developing diabetes mellitus were 3.65 (95% CI 1.40-9.52) times the corresponding odds of HEV-negative patients. A case-control-based histological analysis (n=11 HEV-HCV-positive patients and n=22 HCV-positive patients) showed no significant differences between the groups. CONCLUSIONS: This prevalence is higher than that reported in previous studies of the general population in Brazil. Thus, HEV infection may influence the severity of liver disease and may represent an additional risk of developing diabetes mellitus in patients with HCV infection.
