Resumo Foram analisadas tendências da mortalidade prematura por doenças crônicas não transmissíveis (DCNT) entre 1990 e 2019, as projeções até 2030 e os fatores de risco atribuíveis a estas doenças na Comunidade dos Países de Língua Portuguesa (CPLP). Utilizou-se estimativas do estudo Carga Global de Doenças e análise da carga de mortalidade prematura por DCNT para nove países da CPLP, utilizando taxas padronizadas por idade, usando-se RStudio. Portugal, Brasil, Guiné Equatorial, Angola e Guiné Bissau apresentam taxas de mortalidade prematura por DCNT em declínio e; Timor Leste, Cabo Verde, São Tomé e Príncipe e Moçambique apresentaram aumento das taxas. As projeções indicam que nenhum dos países deverá atingir as metas de redução em um terço da mortalidade prematura por DCNT até 2030. A carga de doença atribuível mostrou que os fatores de riscos mais importantes em 2019 foram: pressão arterial sistólica elevada, tabaco, riscos dietéticos, índice de massa corporal elevado e poluição do ar. Conclui-se pelas profundas diferenças na carga de DCNT entre os países, com melhores resultados em Portugal e Brasil e que nenhum país do CPLP deverá atingir a meta de redução das DCNT até 2030.

Abstract The present study analyzed trends in premature mortality from Noncommunicable diseases (NCDs) between 1990 and 2019, the projections up to 2030, and the risk factors (RFs) attributable to these diseases in the Community of Portuguese Language Countries (CPLP). Estimates from the Global Burden of Disease (GBD) study and the analysis of the burden of premature mortality due to NCDs were used for nine CPLP countries, applying age-standardized rates, using RStudio. Portugal, Brazil, Equatorial Guinea, Angola, and Guinea Bissau showed declining premature mortality rates caused by NCDs, while East Timor, Cape Verde, São Tomé and Príncipe, and Mozambique showed an increase in rates. Projections indicate that none of the countries is expected to achieve the goals of reducing premature mortality due to NCDs by one third by 2030. The attributable burden of disease showed that the most important RFs in 2019 were: high systolic blood pressure (SBP), tobacco, dietary risks, high body mass index (BMI), and air pollution. It can therefore be concluded that there are profound differences in the burden of NCDs among the countries, with better results in Portugal and Brazil, and that no CPLP country is likely to reach the NCD reduction target by 2030.

ABSTRACT Numerous tests employed to predict cardiac and functional status are expensive and not widely accessible for a considerable number of patients, particularly those diagnosed with Chagas disease (CD) residing in remote and endemic regions. To date, there is no knowledge of studies that have validated instruments that address functionality in an expanded way, including the biopsychosocial factors in patients with CD. This study aims to evaluate the psychometric properties of the World Health Organization Disability Assessment Schedule (WHODAS 2.0), in its 12-item shortened version (WHODAS-12) when applied to patients with CD. This is a cross-sectional study of a prospective cohort that follows individuals with CD (SaMi-Trop). Data collection took place between October 2019 and March 2020. In the interviews, sociodemographic information, life habits, clinical information, and indicators of disability measured by WHODAS-12 were collected. Descriptive analysis, internal consistency and construct validity of the instrument were performed. A total of 628 patients with CD were interviewed, most were women (69.5%), their mean age was of 57 years, and most declared an average self-perception of health (43.4%). The 12 items of WHODAS-12 were distributed into three factors, which together account for 61% of the variance. The Kaiser–Meyer–Olkin (KMO) index was 0.90, indicating adequacy of the sample for factor analysis. The internal consistency of the global scale showed alpha = 0.87. The percentage of incapacity was 16.05%, indicating mild incapacity for the evaluated patients. WHODAS-12 is a valid and reliable measure to assess the disability of the Brazilian population with CD.

Resumo Mudanças legislativas em relação à adoção vêm trazendo importantes repercussões para a compreensão do instituto. Neste artigo, temos como objetivo discutir especificidades da entrega voluntária de uma criança para adoção, no contexto da Justiça, e as motivações de demanda posterior da genitora para a viabilização de um reencontro. Problematizamos a amplitude do direito de acesso às origens, assegurado em lei aos adotados, a partir do entrelaçamento das temáticas entrega e reencontro, procurando compreender essas experiências pela perspectiva da genitora. Este trabalho parte de um caso paradigmático, atendido em uma Vara da Infância, Juventude e Idoso no estado do Rio de Janeiro, que culminou com o contato, mediado pelo Poder Judiciário, entre a adotada e sua genitora, por iniciativa desta. Trata-se de um estudo qualitativo, no qual foi realizada uma entrevista semiestruturada com a genitora, quatro anos após o acolhimento de seu pedido à Justiça. Os dados obtidos na entrevista foram analisados por meio do método de análise de conteúdo, em sua vertente categorial, resultando em duas categorias: entrega em adoção e segredos; reencontro: motivações e trajetórias. Constatamos a ausência de publicações brasileiras sobre a temática do reencontro, apontando que o assunto ainda é um tabu. Identificamos que, após o reencontro com a filha, foi possível à genitora uma transformação de si mesma, favorecendo o rompimento do segredo da entrega e de parte de sua história. Resumen Los cambios legislativos respecto a la adopción han tenido importantes repercusiones en la comprensión de la materia. Este artículo pretende discutir los detalles de la entrega espontánea de un niño para adopción, en el contexto de la Justicia, y las motivaciones de la posterior demanda de la madre biológica para hacer factible un reencuentro. Se problematiza la amplitud del derecho de acceso a los orígenes, garantizado por la ley a los adoptados, a partir del entrelazamiento de los temas entrega y reencuentro, analizando estas experiencias desde la perspectiva de la madre biológica. Este trabajo parte de un caso paradigmático que se llevó a cabo en un Juzgado de la Infancia, Juventud y Persona Mayor del Estado de Río de Janeiro y que culminó en el contacto entre la adoptada y su madre, por iniciativa de esta última, mediado por el Poder Judicial. Este estudio cualitativo realizó una entrevista semiestructurada con la madre biológica cuatro años después de su solicitud a la Justicia. A los datos obtenidos en la entrevista se aplicaron el método de análisis de contenido en su vertiente categórica, en el cual surgieron dos categorías: entrega en adopción y secretos; reencuentro: motivaciones y trayectorias. Se encontró que la falta de estudios brasileños sobre reencuentro apunta a que el concepto del sujeto todavía es un tabú. Se constató que luego del encuentro la madre biológica pasó por una autotransformación, lo que favoreció la ruptura del secreto sobre la entrega y parte de su historia. Abstract Legislative changes related to adoption have brought important repercussions for understanding its regulations. In this article, we aim to discuss the peculiarities of a voluntary relinquishment of a child for adoption, in the context of justice, and the motivations of subsequent demand from the birth mother to set a reunion. We problematize the dimension of the right to access origins, guaranteed by law to adoptees, based on the intertwining of the themes voluntary relinquishment and reunion, seeking to understand these experiences from the perspective of the biological mother. This work is based on a paradigmatic case, attended at a Juvenile Court in the State of Rio de Janeiro, that culminated on the reunion of the adopted and her birth mother, at the initiative of the latter, mediated by the Judiciary. This is a qualitative study, in which we interviewed the biological mother, four years after her legal requirement. The data obtained in the interview were analyzed using the content analysis method, in its categorical aspect, resulting in two categories: voluntary relinquishment in adoption and secrets; reunion: motivations and trajectories. We concluded the absence of Brazilian studies about the theme of reunion, pointing out that the subject still as a taboo. We identified that, after the reunion with the daughter, it was possible for the biological mother to modify herself, favoring the breaking of the secret about the relinquishment and of part of her story. Abstract INTRODUCTION: Non-Communicable Diseases (NCDs) have become the main cause of disease burden in Brazil. Our objective was to describe trends (1990 to 2019) in prevalence and attributable burden of five modifiable risk factors and related metabolic risk factors in Brazil and its states. METHODS: In Global Burden of Disease 2019 analyses, we described trends in prevalence of modifiable risk factors and their metabolic mediators as percentage change in Summary Exposure Value (SEV). We estimated deaths and disability-adjusted life years (DALYs) attributable to the risk factors. RESULTS: Age-adjusted exposures to alcohol [41.0%, Uncertainty Interval (UI): 24.2 - 63.4], red meat (61.2%, UI: 42.4-92.3), low physical activity (3.9%, UI: -5-17.5) and ambient particulate matter pollution (3.3%, UI: -48.9-128.0) have worsened. Those for smoking (-51.4%, UI: -54.7- - 47.8), diet low in fruits (-28.1%, UI: -39.1- -18.7) and vegetables (-19.6%, UI: -32.7 - -8.7), and household air pollution (-85.3%, UI: -92.9- -74.3) have improved. All mediating metabolic risk factors, except high blood pressure (0.7%, UI: -6.9-8.3), have worsened: BMI (110.2%, UI: 78.6-161.7), hyperglycemia (15.1%, UI: 9.3-21.2), kidney dysfunction (12.0%, UI: 8.4-17.2), and high LDL-c (11.8%, UI: 6.9-17.2). CONCLUSIONS: A variable pattern of progress and failure in controlling modifiable risk factors has been accompanied by major worsening in most metabolic risk factors. The mixed success in public health measures to control modifiable risk factors for NCDs, when gauged by the related trends in metabolic risk factors, alert to the need for stronger actions to control NCDs in the future.

Abstract INTRODUCTION: Monitoring trends in risk factors (RFs) and the burden of diseases attributable to exposure to RFs is an important measure to identify public health advances and current inadequate efforts. Objective: Analyze the global burden of disease attributable to exposure RFs in Brazil, and its changes from 1990 to 2019, according to the sex and age group. METHODS: This study used data from the Global Burden of Disease study. The Summary Exposure Value, which represents weighted prevalence by risk, was used to estimate exposure to RFs. The mortality and DALYs (Disability Adjusted Life Years) measurements were used to estimate the burden of diseases. For comparisons by year and between Brazilian states, age-standardized rates were used. RESULTS: Arterial hypertension was the factor responsible for most deaths in both sexes. For DALYs, the most important RF was the high body mass index (BMI) for women and alcohol consumption for men. Smoking had a substantial reduction in the attributable burden of deaths in the period. An important reduction was identified in the exposure to RFs related to socioeconomic development, such as unsafe water, lack of sanitation, and child malnutrition. Metabolic RFs, such as high BMI, hypertension, and alcohol consumption showed an increase in the attributable burden. CONCLUSIONS: Our findings point to an increase in metabolic RFs, which are the main RFs for mortality and DALYs. These results can help to consolidate and strengthen public policies that promote healthy lifestyles, thus reducing disease and death.

ABSTRACT This study aims to assess COVID-19 and other respiratory viruses in pediatric patients. Between April 17 and September 30, 2020, we collected 1,566 respiratory samples from 1,044 symptomatic patients who were younger than 18 years old to assess SARS-CoV-2 infection. Of these, 919 were analyzed for other respiratory pathogens (ORP). Patients with laboratory-confirmed COVID-19 or ORP were included. We evaluated 76 pediatric COVID-19 infections and 157 other respiratory virus infections. Rhinovirus occurred in 132/157 (84%). COVID-19 patients who were significantly older, had more fevers, headaches and pneumonia than those with ORP. The median white blood cell count was lower in patients with SARS-CoV-2 than in those with ORP (6,470 versus 8,170; p=0.02). COVID-19 patients had significantly worse symptoms than those with ORP.

Objective: to estimate the prevalence and investigate the sociodemographic, health and lifestyle factors associated with the self-reported diagnosis of Cardiovascular Disease (CVD) in the Brazilian adult population. Methods: Data from the National Health Survey (PNS 2019).  The presence of CVD was self-reported through the question: "Has any doctor ever given you a diagnosis of heart disease?". Sociodemographic factors, health conditions and lifestyle were evaluated. For data analysis, Poisson Regression with robust variance was used. Results: 5.3% (95% CI: 5.04 -5.57) of Brazilian adults reported CVD, and of these, 29.08% (95% CI: 27.04-31.21) underwent bypass surgery or angioplasty and 8.26% (95% CI: 7.09-9.6) reported severe limitation in usual activities due to CD. The factors associated with CVD were advanced age; be male; race/ color white; have complete elementary and high school education; have a health plan; self-assess health as regular or bad/very bad; self-report hypertension, high cholesterol and diabetes; be a former smoker, consume fruits and vegetables as recommended, do not abuse alcohol abusively and do not practice leisure-time physical activity. Conclusions: CVD is associated with sociodemographic, health and lifestyle factors. It is important to support public policies, programs and goals for the reduction of cardiovascular diseases in Brazil, especially in the most vulnerable groups.

Objetivo: estimar a prevalência e investigar os fatores sociodemográficos, de saúde e estilo de vida associados ao diagnóstico autorreferido de doença cardiovascular (DCV)  na população adulta brasileira. Métodos: Foram analisados dados da Pesquisa Nacional de Saúde (PNS 2019). A presença de DCV foi autorreferido por meio da pergunta: “Algum médico já lhe deu o diagnóstico de uma doença do coração?”. Foi avaliado os fatores sociodemográficos, condições de saúde e estilo de vida. Para análise dos dados utilizou-se a Regressão de Poisson com variância robusta. Resultados: 5,3% (IC95%: 5,04 -5,57) dos adultos brasileiros referiram DCV, e destes, 29,08% (IC95%: 27,04-31,21) realizaram cirurgia revascularização miocárdica ou angioplastia coronariana e 8,26% (IC95%: 7,09-9,6) relataram limitação intensa nas atividades habituais devido a DCV. Os fatores associados a DCV foram a idade avançada; ser do sexo masculino; raça/cor branca; ter ensino fundamental completo e médio incompleto; possuir plano de saúde; autoavaliar a saúde como regular ou ruim/muito ruim; autorrelatar hipertensão, colesterol alto e diabetes; ser ex-fumante e consumir frutas e hortaliças conforme o recomendado, não consumir álcool de forma abusiva e não praticar atividade física no lazer. Conclusões: A DCV está associada à fatores sociodemográficos, de saúde e estilos de vida. Torna-se importante apoiar políticas públicas, programas e metas para a redução das doenças cardiovasculares no Brasil, especialmente nos grupos mais vulneráveis.

Resumo Objetivos: determinar a prevalência e fatores associados à sibilância recorrente no primeiro ano de vida entre recém-nascidos prematuros egressos de Unidades de Terapia Intensiva Neonatais, na cidade de Montes Claros, norte de Minas Gerais. Métodos: estudo transversal, com coleta de dados em prontuários de ambulatório de seguimento, entrevistas com mães e, eventualmente, consultas aos prontuários hospitalares. Foram realizadas análises bivariadas entre as características sociodemográficas e clínicas e a sibilância recorrente. As variáveis associadas ao desfecho até um nível de significância de p ≤20% foram analisadas por regressão logística binária e as associações definidas pelas Odds Ratios e respectivos intervalos de confiança de 95%. Somente variáveis associadas a um nível de significância de 5% foram mantidas no modelo final de regressão logística. Resultados: entre 277 crianças estudadas, cerca de um quinto (21,3%) eram prematuros de extremo baixo peso e mais da metade (60,7%) tinha peso de nascimento abaixo de 1500 gramas. A prevalência de sibilância recorrente foi de 14,4% (IC95%=10,3-18,4). Ventilação mecânica (OR=2,12; IC95%= 1,09-4,76; p=0,030) e tempo de oxigenioterapia ≥15 dias (OR=2,49; IC95%=1,12-5,00; p=0,010) foram os fatores de risco para o evento. Conclusão: existe uma elevada prevalência de sibilância recorrente no grupo avaliado e as variáveis associadas reiteram o risco do uso prolongado de oxigenioterapia e da ventilação mecânica para recém-nascidos prematuros.

Abstract Objectives: to determine the prevalence and factors associated with recurrent wheezing in the first year of life among premature newborns from Neonatal Intensive Care Units, in the city of Montes Claros, northern Minas Gerais. Methods: cross-sectional study, with data collection from medical records of a follow-up clinic, interviews with mothers and, eventually, search in hospital records. Bivariate analyzes were carried out between sociodemographic and clinical characteristics and recurrent wheezing. Variables associated up to the level of 20% were analyzed by binary logistic regression, and associations defined by the Odds Ratio and respective 95% confidence intervals. Only variables associated with a 5% significance level were maintained in the final model of logistic regression. Results: among 277 infants studied, about one fifth (21.3%) were extremely low birth weight preterm and more than half (60.7%) had birth weight below 1500 grams. The prevalence of recurrent wheezing was 14.4% (CI95% = 10.3-18.4). Mechanical ventilation (OR = 2.12; CI95% = 1.09-4.76; p = 0.030) and oxygen therapy time ≥ 15 days (OR = 2.49; CI95%= 1.12-5.00; p = 0.010) were the risk factors for the event. Conclusions: there is a high prevalence of recurrent wheezing in the evaluated group and the associated variables reiterate the risk of prolonged oxygen therapy and mechanical ventilation for premature newborns.

RESUMO O consumo excessivo de sal é um fator de risco para o desenvolvimento de doenças crónicas não transmissíveis como a hipertensão arterial, doenças cardiovasculares e alguns tipos de cancro. Atualmente é frequente a realização de pelo menos uma refeição diária fora de casa, em cantinas ou restaurantes, não havendo controlo da confeção destas refeições por parte do consumidor. O objetivo desta revisão sistemática foi analisar estudos de intervenção para a redução de sal em cantinas públicas ou privadas, nomeadamente avaliar quais as metodologias utilizadas. Foi realizada uma pesquisa nas plataformas PubMed e Web Of Knowledge (entre 23 e 28, setembro 2020), tendo sido incluídos sete artigos. As estratégias adotadas agrupam-se em três áreas principais: ementas e ambiente alimentar, consumidores e cozinheiros/manipuladores de alimentos. As intervenções que resultaram numa redução do consumo de sal foram as alterações das ementas, a formação dos cozinheiros (por cozinheiro ou nutricionista), a substituição do sal de adição por especiarias, a realização de sessões de educação alimentar para os consumidores e a redução do número de refeições pré-embaladas com teor de sal acima de 200 mg/porção fornecidas pelas cantinas. Futuramente será importante realizar estudos com maior uniformização dos procedimentos de avaliação do impacto das estratégias, com um maior período de intervenção e com um grupo controlo, de modo a obter resultados que efetivamente permitam a elaboração de recomendações nesta área.

ABSTRACT The excessive consumption of salt is a risk factor for the development of non-transmissible chronic diseases like hypertension, cardiovascular diseases, and cancer. Nowadays it is frequent to have at least one daily meal out of the house, in canteens or restaurants, where there isn’t any control, by the customer, with the meals produced. This paper is a systematic review of the existing intervention studies on salt reduction in public or private canteens and evaluate the used methodologies. The search was made on the platforms PubMed and Web of Knowledge (n=72), where, after the proper screening, seven studies fitted the criteria. The methodologies used in the studies that had significant results when it came to reducing salt consumption were changes in the menu, training cookers (with the help of a cooker or nutritionist), substituting added salt for spices, food education sessions for the consumers, and reducing the number of pre-packaged meals with more than 200 mg/portion of salt supplied in canteens. In the future, it would be important to perform studies with greater uniformity of the evaluation procedures of the strategies’ impact, with a larger intervention period and a control group, to obtain accurate results, to elaborate recommendations in this area.

RESUMO: Objetivo: Estimar a prevalência e investigar os fatores sociodemográficos, de saúde e estilo de vida associados ao diagnóstico autorreferido de doença cardiovascular na população adulta brasileira. Métodos: Analisaram-se dados da Pesquisa Nacional de Saúde (2019). A presença de doença cardiovascular foi autorreferida por meio da pergunta: “Algum médico já lhe deu o diagnóstico de uma doença do coração?”. Avaliaram-se fatores sociodemográficos, condições de saúde e estilo de vida. Para analisar os dados, utilizou-se a regressão de Poisson com variância robusta. Resultados: 5,3% (IC95% 5,04–5,57) dos adultos brasileiros referiram doença cardiovascular, destes, 29,08% (IC95% 27,04–31,21) realizaram cirurgia de revascularização miocárdica ou angioplastia coronariana e 8,26% (IC95% 7,09–9,60) relataram limitação intensa nas atividades habituais pela doença cardiovascular. Os fatores associados à doença cardiovascular foram idade avançada; ser do sexo masculino; raça/cor branca; ter ensino fundamental completo e médio incompleto; possuir plano de saúde; autoavaliar a saúde como regular ou ruim/muito ruim; autorrelatar hipertensão, colesterol alto e diabetes; ser ex-fumante; consumir frutas e hortaliças conforme o recomendado; não consumir álcool de forma abusiva; não praticar atividade física no lazer. Conclusões: A doença cardiovascular está associada a fatores sociodemográficos, de saúde e estilos de vida. Torna-se importante apoiar políticas públicas, programas e metas para reduzir doenças cardiovasculares no Brasil, especialmente nos grupos mais vulneráveis.

ABSTRACT Objective: to estimate the prevalence and investigate the sociodemographic, health, and lifestyle factors associated with the self-reported diagnosis of Cardiovascular Disease (CVD) in the adult Brazilian population. Methods: Data from the National Health Survey (PNS 2019) were analyzed. The presence of CVD was self-reported through the question: “Has any doctor ever given you a diagnosis of heart disease?”. Sociodemographic factors, health conditions, and lifestyle were evaluated. For data analysis, Poisson Regression with robust variance was used. Results: 5.3% (95%CI 5.04–5.57) of Brazilian adults reported CVD, of which, 29.08% (95%CI 27.04–31.21) underwent coronary artery bypass surgery or angioplasty and 8.26% (95%CI 7.09–9.6) reported severe limitation in usual activities due to CVD. The factors associated with CVD were advanced age; being male; white race/color; complete middle school and incomplete high school education; have health insurance; self-assessing health as regular or bad/very bad; self-reported hypertension, high cholesterol, and diabetes; being a former smoker; consuming fruits and vegetables as recommended; not consuming alcohol in excess; and not practicing leisure-time physical activity. Conclusions: CVD is associated with sociodemographic, health, and lifestyle factors. It is important to support public policies, programs, and goals for the reduction of cardiovascular diseases in Brazil, especially in the most vulnerable groups.

OBJECTIVES: Co-infection with hepatitis A or B viruses may aggravate liver injury in patients infected with hepatitis C virus (HCV). However, few studies have assessed co-infection with hepatitis E virus (HEV) and HCV. Therefore, this study aimed to assess the prevalence and impact of HEV infection among Brazilian patients with chronic HCV infection. METHODS: This observational study included adult patients with chronic HCV infection who were naive to antiviral therapy from January 2013 to March 2016. A total of 181 patients were enrolled, and HEV serology and PCR were performed for all patients. RESULTS: Seropositivity for anti-HEV IgG was detected in 22 (12.0%) patients and anti-HEV immunoglobulin M in 3 (1.6%). HEV RNA showed inconclusive results in nine (4.9%) patients and was undetectable in the remaining patients. HEV serology positive patients had more severe liver disease, characterized by liver fibrosis ≥3 versus ≤2 (p<0.001), Aspartate Aminotransferase-to-Platelet Ratio Index of ≥1.45 (p=0.003), and Fibrosis-4 score of ≥3.25 (p=0.001). Additionally, the odds of HEV-positive patients developing diabetes mellitus were 3.65 (95% CI 1.40-9.52) times the corresponding odds of HEV-negative patients. A case-control-based histological analysis (n=11 HEV-HCV-positive patients and n=22 HCV-positive patients) showed no significant differences between the groups. CONCLUSIONS: This prevalence is higher than that reported in previous studies of the general population in Brazil. Thus, HEV infection may influence the severity of liver disease and may represent an additional risk of developing diabetes mellitus in patients with HCV infection.
