ResumoIntrodução:Apesar de rara, a fístula átrio-esofágica é uma das complicações mais temidas na ablação por radiofrequência da fibrilação atrial pelo alto risco de mortalidade.Objetivo:Este é um estudo prospectivo controlado, realizado durante a ablação por radiofrequência da fibrilação atrial regular, para testar se o deslocamento do esôfago ao manipular o transdutor de ecocardiografia transesofágica poderia ser usado para a proteção de esôfago.Métodos:Setecentos e quatro pacientes (158 mulheres e 546 homens [22,4%/77,6%]; 52,8±14 [17-84] anos), com EF média igual a 0,66±0,8 e com fibrilação atrial refratária ao tratamento medicamentoso, foram submetidos à terapia híbrida com ablação por radiofrequência (isolamento convencional das veias pulmonares e ninhos de fibrilação atrial e ablação de taquicardia de background) com deslocamento do esôfago o mais longe possível do alvo da radiofrequência por manuseio do transdutor de ecocardiografia transesofágica. A temperatura luminal esofágica foi monitorada com e sem deslocamento em 25 pacientes.Resultados:O deslocamento esofágico significativo foi de 4 a 9,1 centímetros (5,9±0,8 cm). Em 680 dos 704 pacientes (96,6%), isso foi o suficiente para permitir a entrega completa e segura de radiofrequência (30W/40ºC/cateter irrigado ou 50W/60ºC/cateter de 8 milímetros) sem sobreposição do esôfago. As alterações médias de temperatura luminal esofágica com e sem deslocamento de esôfago foram de 0,11±0,13ºC versus 1,1±0,4ºC, respectivamente, P<0,01. A radiofrequência teve que ser interrompida em 68% dos pacientes sem deslocamento de esôfago devido ao aumento da temperatura luminal esofágica. Não houve nenhum caso, suspeito ou confirmado, de fístula átrio-esofágica. Foram observados apenas dois sangramentos superficiais causados por inserção do transdutor de ecocardiografia transesofágica.Conclusão:O deslocamento mecânico do esôfago pelo transdutor de ecocardiografia transesofágico durante a ablação com radiofrequência foi capaz de impedir o aumento da temperatura luminal esofágica, ajudando a evitar lesão térmica. Na maioria dos casos, o deslocamento esofágico foi suficiente para permitir a aplicação segura de radiofrequência sem sobreposição do esôfago, sendo uma alternativa conveniente para reduzir o risco de fístula átrio-esofágica.

AbstractIntroduction:Although rare, the atrioesophageal fistula is one of the most feared complications in radiofrequency catheter ablation of atrial fibrillation due to the high risk of mortality.Objective:This is a prospective controlled study, performed during regular radiofrequency catheter ablation of atrial fibrillation, to test whether esophageal displacement by handling the transesophageal echocardiography transducer could be used for esophageal protection.Methods:Seven hundred and four patients (158 F/546M [22.4%/77.6%]; 52.8±14 [17-84] years old), with mean EF of 0.66±0.8 and drug-refractory atrial fibrillation were submitted to hybrid radiofrequency catheter ablation (conventional pulmonary vein isolation plus AF-Nests and background tachycardia ablation) with displacement of the esophagus as far as possible from the radiofrequency target by transesophageal echocardiography transducer handling. The esophageal luminal temperature was monitored without and with displacement in 25 patients.Results:The mean esophageal displacement was 4 to 9.1cm (5.9±0.8 cm). In 680 of the 704 patients (96.6%), it was enough to allow complete and safe radiofrequency delivery (30W/40ºC/irrigated catheter or 50W/60ºC/8 mm catheter) without esophagus overlapping. The mean esophageal luminal temperature changes with versus without esophageal displacement were 0.11±0.13ºC versus 1.1±0.4ºC respectively, P<0.01. The radiofrequency had to be halted in 68% of the patients without esophageal displacement because of esophageal luminal temperature increase. There was no incidence of atrioesophageal fistula suspected or confirmed. Only two superficial bleeding caused by transesophageal echocardiography transducer insertion were observed.Conclusion:Mechanical esophageal displacement by transesophageal echocardiography transducer during radiofrequency catheter ablation was able to prevent a rise in esophageal luminal temperature, helping to avoid esophageal thermal lesion. In most cases, the esophageal displacement was sufficient to allow safe radiofrequency application without esophagus overlapping, being a convenient alternative in reducing the risk of atrioesophageal fistula.

FUNDAMENTO: A estimulação endocárdica convencional do ventrículo direito (VD) ocasiona alargamento do QRS e dessincronização do miocárdio, comprometendo a função ventricular. Com a necessidade de estimulação menos deletéria, a estimulação septal do VD tem sido mais utilizada. Eventualmente têm sido relatados limiares mais altos e ondas R menores na estimulação septal. OBJETIVO: Comparar os parâmetros das estimulações apical e septal, intrapaciente, para verificar se existem diferenças que possam interferir na escolha do ponto de estimulação. MÉTODOS: Estudo prospectivo controlado. Foram incluídos 25 pacientes, com 67,2 ± 9 anos, 10 (40%) mulheres, com indicações de marca-passo por bradiarritmias. Etiologias foram degenerativa em nove (36%), coronariopatia em oito (32%), doença de Chagas em sete (28%), e valvopatia em um (4%) pacientes. Foram utilizados eletrodos de fixação ativa e avaliados os limiares de comando, impedância e onda R uni e bipolares no implante e após seis meses. RESULTADOS: A média aguda dos limiares de comando, ondas R e impedâncias unipolares/bipolares septais x apicais foram, respectivamente, 0,73x0,74V e 0,73x0,78V; 10x9,9 mV e 12,3x12,4 mV; 579x621 Ω e 611x629 Ω. Comparações entre parâmetros septais e apicais com teste t-pareado bicaudal demonstraram um P > 0,1. Após seis meses, a média dos limiares de comando, ondas R e Impedâncias unipolares/ bipolares septais x apicais foram, respectivamente, 0,5 x 0,72 V e 0,71 x 0,87 V; 11,4x9,5 mV e 12x11,2 mV; 423x426Ω e 578x550Ω, com P > 0,05, exceto comparando-se limiar de estimulação unipolar septal com apical unipolar p de 0,02. CONCLUSÃO: Utilizando comparações intrapaciente, não existem diferenças expressivas entre parâmetros eletrofisiológicos de estimulação septal e apical sendo que não há restrições para a escolha da estimulação septal em ventrículo direito.

BACKGROUND: The conventional right ventricle (RV) endocardial pacing leads QRS widening and desynchronization myocardial compromising ventricular function. With the need for stimulation less deleterious, RV septal pacing has been used more. Eventually have been reported higher thresholds and smaller R waves in the septal stimulation. OBJECTIVE: To compare the parameters of the septal and apical stimulation, intra-patient, if there are any differences that may affect the choice of the point of stimulation. METHODS: A prospective controlled study. We included 25 patients, 67.2±9 years, 10 (40%) women with indications for pacemaker for bradyarrhythmias. Etiologies were degenerative in nine (36%), Coronary disease in eight (32%), Chagas disease in seven (28%), and valve disease in one (4%) patient. Electrodes were active fixation and assessed the thresholds of command, impedance and R wave in uniand bipolar implant and after six months. RESULTS: The average acute threshold command, R wave and impedance unipolar / bipolar septais x apicais were respectively 0.73 x 0.73V and 0,74V x 0,78V; 10 x 9,9mV and 12,3 x 12,4mV; 579 x 621Ω and 611 x 629Ω. Comparisons between parameters with septal and apical two-tailed paired t-test showed a P > 0.1. After six months, the mean control thresholds, R wave impedances and unipolar/bipolar septais x apicais were respectively 0.5V x 0 72V and 0.71V x 0,87V; 11.4 x 9,5mV and 12x11,2mV; 423x426 Ω and 578x550 Ω, with P > 0.05, except compared to unipolar pacing threshold septal apical unipolar P 0.02. CONCLUSION: Using intra-patient comparisons, no significant differences between electrophysiological parameters septal and apical pacing and there are no restrictions for choosing the right ventricular septal pacing.

FUNDAMENTO: Através de mapeamento espectral-(ME) endocárdico em ritmo sinusal, observam-se dois tipos de miocárdio atrial: o compacto de espectro liso e o fibrilar de espectro segmentado ("Ninho de FA" [NFA]). Durante a FA o compacto tem ativação organizada e baixa freqüência (passivo) enquanto o fibrilar apresenta ativação bastante desorganizada e alta freqüência (ativo/ressonante) sendo ambos ativados por uma taquicardia protegida de alta freqüência, taquicardia de background (TB). OBJETIVO: Descrever o tratamento da FA pela ablação dos NFA e da TB. MÉTODOS: 1) Ablação por cateter-RF [4/8mm-60º/30-40J/30s] dos NFA guiada por ME em ritmo sinusal, fora das veias pulmonares; 2) Estimulação atrial-300ppm; 3) Ablação adicional de NFA se induzida FA; 4) Ablação focal se induzida TB e/ou Flutter; 5) Seguimento clínico+ECG+Holter. RESULTADOS: Foram tratados 50±18 NFA/paciente. Após 11,3±8m 81p (88%) estavam sem FA (28,3% com antiarrítmico). Após a ablação dos NFA: a FA não foi reinduzida em 61p(71%); TB foi induzida e tratada em 24p(26%). Ocorreram 2 sangramentos pericárdicos (1 tratado clinicamente e 1 cirurgicamente) ocasionados por bainhas não mais utilizadas. CONCLUSÃO: O ME em ritmo sinusal ablaciona os NFA. Durante a FA os NFA apresentam um padrão reativo-ressonante e o miocárdio compacto apresenta-se passivo, estimulados pela alta freqüência da TB. Após a ablação dos NFA e da TB não foi possível reinduzir FA sustentada. A ablação dos NFA fora das VP se mostrou segura e altamente eficiente para a cura e/ou o controle clínico da FA.

BACKGROUND: Two types of myocardia can be observed through the endocardial spectral mapping (SM) in sinus rhythm: the compact type with a smooth spectrum and the fibrillar type with a segmented spectrum (atrial fibrillation nests). During the atrial fibrillation (AF), the compact type has an organized activation and low frequency (passive), whereas the fibrillar type has a rather disorganized activation and high frequency (active/resonant), with both being activated by high-frequency sustained tachycardia - the background tachycardia (BT). OBJECTIVE: To describe the treatment of AF by the ablation of the AF nests and BT. METHODS: 1) Catheter ablation of the AF nests with RF [4/8mm-60º/30-40J/30s] guided by SM in sinus rhythm, outside the pulmonary vein; 2) atrial stimulation -300ppm; 3) Additional ablation of the AF nests if AF is induced; 4) Focal ablation if BT and/or Flutter is induced; 5)Clinical follow-up+ ECG+ Holter. RESULTS: A total of 50±18 AF nests/patient were treated. After 11.3±8m, 81 patients (88%) did not present AF (28.3% with antiarrhythmic drugs). After the ablation of the AF nests, AF was not reinduced in 61 patients (71%) and BT was induced and treated in 24 patients (26%). There were two episodes of pericardial bleeding (1 treated clinically and 1 surgically), caused by sheaths that are no longer used CONCLUSION: The SM in sinus rhythm can be used in the ablation of AF nests. During the AF, the AF nests present a reactive-resonant pattern and the compact myocardium is passive, stimulated by the high frequency of the BT. After the ablation of the AF nests and the BT, it was not possible to reinduce the sustained AF. The Ablation of AF nests outside the pulmonary veins showed to be safe and highly effective in the cure and/or clinical control of the AF.

INTRODUÇÃO: O alargamento do QRS pela estimulação ventricular artificial (EVa) convencional prejudica a contratilidade cardíaca e a função mitral. OBJETIVO: Propôr um modo alternativo de EVa com QRS mais estreito, com 2 eletrodos em ventrículo direito(VD) e testá-lo frente à EVa convencional. CASUÍSTICA E MÉTODOS: Cinco pacientes com indicação para implantação de marcapasso definitivo(MP), 4M/1F, idade média de 52,2 anos, portadores de ICC (III/IV) por miocardiopatia dilatada (Chagas 4, Indeterminada 1), AC 3+/4+, FA crônica com BAV, foram submetidos a implante de MP endocárdico com 2 eletrodos em VD, o primeiro na área do feixe de His (septal) e o segundo, convencional, na ponta do VD. O gerador foi Biotronik Dromos DR (único com AV mínimo de 15ms). O eletrodo septal foi conectado à saída atrial e o de ponta à saída ventricular. Após 2 semanas, os pacientes foram avaliados ecocardiograficamente, na mesma sessão, em 3 modos de estimulação na mesma freqüência: modo "AAI" (septal), modo VVI (convencional) e modo "DDT" com intervalo AV=15ms (estimulação bifocal no VD quase simultânea). RESULTADOS: As médias dos parâmetros estudados estão na tabela abaixo: <img src="http:/img/fbpe/rbccv/v15n1/n1a07t0a.gif" alt="n1a07t0a.gif (7233 bytes)"> CONCLUSÃO: Sob o ponto de vista ecocardiográfico, a comparação imediata no mesmo paciente dos 3 modos de estimulação revelou: 1-O modo convencional foi o de pior rendimento hemodinâmico; 2-O modo bifocal mostrou o melhor resultado com aumento médio da FE em 6,8% e do DC em 0,6 l/min, redução média do AE em 7,5 cm2, redução média da área de refluxo mitral em 7,6 cm2 e redução média na duração do QRS em 61 ms, (p < 0,03), sendo a melhor EVa na miocardiopatia dilatada com insuficiência cardíaca, insuficiência mitral funcional e indicação de MP.

BACKGROUND: Widening of QRS by ventricular conventional pacing (CP) impairs ventricular contractility and mitral performance. PURPOSE: To study an alternative mode of cardiac stimulation with narrower QRS, using 2 leads in the right ventricle and to test it, compared to CP. MATERIAL AND METHODS: 5 dilated cardiomyopathy patients (3 Chagas´ disease, 2 unknown) with cardiac failure (NYHA III/4) and pacemaker (PM) indication, 4 male/1 female, mean age 52.2. years, cardiac shape 3+/4+, chronic AF with AVB, were submitted to endocardial PM implants with 2 right ventricular leads. The first in the His bundle area (septal) and the second, conventional, in the right ventricular apex. The generator was Biotronik Dromos DR (the short programmable AV = 15ms). The septal lead was connected to the atrial outflow and the one of the apex to the ventricular exit. After 2 weeks, they were studied by echocardiogram, in 3 stimulation modes in the same session with same cardiac rate: mode "AAI" (septal), mode VVI (conventional) and mode "DDT" with AV interval=15ms (almost simultaneous bifocal stimulation). RESULTS: The averages of the parameters studied were: <img src="http:/img/fbpe/rbccv/v15n1/n1a07t0b.gif" alt="n1a07t0b.gif (9767 bytes)"> CONCLUSION: The echocardiographic evaluation for the immediate comparison in the same patient with 3 stimulation modes, revealed: 1. The conventional mode showed the worse hemodynamic performance; 2. the bifocal mode showed the best result with mean increase of the EF of 6.8% and of CO in 0.6 l/min, mean reduction of LA in 7.5 cm2, mean reduction of the area of mitral regurgitation in 7.6 cm2 and mean reduction in the QRS duration of 61ms, (p < 0.03). It was the best stimulation in cardiomyopathy with heart failure, functional mitral regurgitation and pacemaker indication.
