Intraocular anterior chamber implants for myopic correction in phakic eyes

RESUMO

Objetivo:

O presente estudo avalia a segurança e a eficácia dos implantes de lentes de câmara anterior em olhos fácicos para a correção da miopia.

Métodos:

Foram estudadas, retrospectivamente, a acuidade visual com e sem correção, a refratometria, a contagem endotelial e a incidência de complicações intra e pós-operatórias, em 15 olhos de 12 pacientes seguidos por um mínimo de 6 meses (média 25,8 meses) após a cirurgia.

Resultados:

Não houve complicações nesta série. A média do equivalente esférico diminuiu de -15,15 DE para -0,77 DE após a cirurgia (p<0,001). A acuidade visual sem correção pós-operatória média foi de 20/34, e estava melhor ou igual a 20/40 em 11 dos 13 olhos (85%) avaliados para este fim. Nenhum paciente apresentou diminuição da acuidade visual corrigida por óculos, e houve melhora desta em 8 dos 15 olhos (53%). Não houve diminuição da densidade endotelial central pré-operatória (2719,4 células/mm²) em relação à pós-operatória (2774 células/mm²; p>0,05).

Conclusões:

Nesta pequena amostra, com a metodologia utilizada, o procedimento se mostrou seguro e eficaz. No entanto, estudos com uma população e seguimento maiores são necessários para a definitiva comprovação clínica deste procedimento.

SUMMARY

Purpose:

To assess the safety and the effectiveness of intraocular anterior chamber implants in phakic eyes for myopic correction.

Methods:

Retrospective assessment of spectacle corrected and uncorrected visual acuity, refraction, central endothelium density and incidence of postoperative complications. Fifteen eyes of 12 patients were fitted into the criteria of inclusiveness and were followed for a minimum of 6 months (average 25.8 months) after surgery.

Results:

There were no complications in this sample. The average spherical equivalent decreased from -l5.l5D to -0.77D after the surgery (p<0.001). Average postoperative uncorrected visual acuity was 20/34, and was at least 20/4 0 in 11 out of 13 eyes (85%) examined. No patient presented decrease in spectacle corrected visual acuity which improved in 8 of the 15 eyes (53%). No endothelium cell loss was detected. Endothelium density averaged 2719.4 cells/mm² and 2774 cells/mm² before and after the surgery, respectively (p>0.05).

Conclusions:

While using the methodology this procedure proved to be safe and effective in this sample. Certainly, larger segments of the population should be evaluated to determine the long lasting clinical value of the procedure.
