Vitrectomia via "pars plana" na prevenção das lesões causadas pela injeção experimental de gentamicina na câmara anterior

RESUMO

Este estudo foi realizado com o objetivo de avaliar, em coelhos, a eficácia da vitrectomia via "pars plana" imediata na prevenção ou redução das lesões da retina que habitualmente se seguem à injeção de doses supraterapêuticas de gentamicina em câmara anterior. Foram injetadas duas doses distintas de gentamicina (4 e 20mg) na câmara anterior, sendo observado que todos os olhos do experimento, independentemente da realização ou não da vitrectomia, apresentaram alterações retinianas no exame histopatológico, porém de grau muito menor no grupo submetido à vitrectomia. As lesões retinianas foram doses-dependentes, sendo mais intensas nos olhos de coelhos que receberam 20mg, que é a dose usual para injeção subconjuntival em seres humanos. A presença de cápsula anterior íntegra foi também um fator adicional de proteção da retina. Este trabalho mostra que, pelo menos experimentalmente, a vitrectomia via "pars plana" atenua as lesões decorrentes da injeção de doses supraterapêuticas de gentamicina em câmara anterior e talvez possa ter um papel importante, em humanos, nos casos de injeção acidental.

SUMMARY

The present experimental and histopathologic study examines the effect of immediate pars plana vitrectomy with posterior segment lavage in rabbits submited to two different dosage of gentamicin (4 and 20mg) in the anterior chamber, in arder to verify the effectiveness to prevent or reduce the lesions. The results indicate that despite receiving an immediate pars plana vitrectomy and posterior segment lavage following intraocular injection of gentamicin, all rabbit eyes exhibited varying degrees of retinal toxicity. The lesions were less severe for those animais receiving an immediate pars plana vitrectomy. The Retinal toxicity was dose dependent, with those rabbit eyes receiving 20mg of gentamicin more severely damaged than those eyes receiving four mg of gentamicin. The results of this study is highly suggestive that the presence of an intact tens capsule may afford additional protection to the retina. The results of this study showed that, at least experimental, "pars plana" vitrectomy play a role in the management of intraocular injection of gentamicin in high dosage in anterior chamber, and may be can be effective in human cases of inadvertent intraocular injection of gentamicin.
