Addition of dipivefrin for users of pilocarpine and timolol maleate

RESUMO

Objetivo:

Avaliar a ação hipotensora do cloridrato de dipivefrin 0,1% em olhos de pacientes usuarios de pilocarpina 2% e maleato de timolol 0,5%.

Métodos:

Em um estudo prospectivo, 26 pacientes com glaucoma primário de ângulo aberto usuários de pilocarpina 2% de 4/4 h e maleato de timolol 0,5% de 12/12 h foram submetidos a 5 medidas de PIO às 8,10,12,14 e 16 h. Duas semanas após a introdução de dipivalil epinefrina 0,1% de 12/12 h em um dos olhos, novas medidas de PIO foram realizadas nos mesmos horários.

Resultados:

Após a introdução do dipivefrin observamos redução significativa das pressões intra-oculares medidas em todos os horários (1,49 ± 2,36 mmHg, p = 0,003). Não observamos diferença estatisticamente significante quando comparamos a variabilidade, o pico pressórico e o número de indivíduos com PIO > 21 mmHg pré e pós tratamento com dipivefrin. Apenas oito pacientes apresentaram redução de PIO superior a 15%.

Conclusões:

Apesar das reduções da PIO serem estatisticamente significantes, o efeito adicional do dipivefrin em pacientes utilizando pilocarpina e maleato de timolol é clinicamente relevante em apenas um terço dos pacientes.

SUMMARY

Purpose:

To determine the hypotensive effect of dipivefrin hydrochloride 0.1% to eyes previously using pilocarpine 2% and timolol maleate 0.5%.

Methods:

In a prospective study, 26 patients with primary open angle glaucoma using pilocarpine 2% four times daily and timolol maleate 0.5% twice daily underwent 5 lOP measurements at 8 and 10 am and 12, 2 and 4pm. Two weeks after the introduction of dipivefrin twice daily in one eye, new measurements of lOP were made at the same intervals.

Results:

Following the addition of dipivefrin we noticed a significant decrease in the mean lOP (all intervals) (1.49 ± 2.36 mmHg, p = 0.003). There was no statistically significant difference regarding the variability, the lOP spikes and the number of patients with lOP higher than 21 mmHg before and after treatment with dipivefrin. Only 8 patients showed lOP reduction more than 15%.

Conclusions:

In spite of the statistically significant lOP reduction, the additional effect of dipivefrin in patients using pilocarpine and timolol maleate is clinically relevant in only one third of the patients.
