  • Uma abordagem dos sistemas dinâmicos: restrições para a reabilitação Review Article

    CONTEXTUALIZAÇÃO: Sistemas de classificação (Nagi e Classificação International de Funcionalidade (CIF)) têm se tornado populares para categorização do nível de habilidade (CIF) ou de incapacidade (Nagi) associado com distúrbios do movimento. No entanto, essas classificações não exploram as formas pelas quais um nível pode influenciar outros níveis; por exemplo, como a fraqueza e a rigidez observadas em alguns casos de paralisia cerebral podem resultar no padrão estereotipado de marcha equina. Neste artigo, descreve-se uma abordagem denominada sistemas dinâmicos/restrições (DS/C) para compreender as relações entre níveis e como ela pode ser utilizada para racionalizar um novo processo que norteie a avaliação e a intervenção de distúrbios do movimento. OBJETIVOS: Este artigo tem três objetivos específicos: apresentar uma abordagem geral sistêmica para compreender o comportamento em diferentes níveis de análise; apresentar ferramentas e resultados de estudos empíricos que utilizaram a abordagem DS/C e, por fim, discutir as implicações clínicas e os resultados de intervenções motivadas pela análise DS/C voltadas para crianças com paralisia cerebral e indivíduos com Doença de Parkinson.

    BACKGROUND: Classification systems (Nagi, International Classification for Function [ICF]) have become popular for categorizing the level of ability (ICF) or disability (Nagi) associated with movement disorders. Nevertheless, these classifications do not explore the ways in which one level may influence other levels. For example, how might the weakness and stiffness associated with some cases of cerebral palsy result in a stereotypical toe-gait? In this overview we describe a dynamic systems/constraints (DS/C) approach to understand relationships between levels, and how the approach can be used to rationalize a novel process for the evaluation and treatment of movement disorders. OBJECTIVES: There are three specific aims in this paper: first to present a general systems approach to understanding behavior at different levels; second to present tools of, and the results of empirical work using the DS/C approach; third to discuss the clinical implications and results of clinical interventions motivated by DS/C analysis for children with cerebral palsy, and individuals with Parkinson disease.
  • Relação entre cifose dorsal, densidade mineral óssea e controle postural em idosas Artigos Originais

    OBJETIVO: Verificar a relação entre medida angular da cifose dorsal, densidade mineral óssea (DMO) e controle postural em mulheres idosas. MÉTODOS: Por meio de um estudo transversal, 95 idosas foram divididas em quatro grupos segundo as medidas angulares da cifose dorsal (obtidas pelo método flexicurva) e os resultados de densitometria óssea. Na plataforma de força e por meio de teste dinâmico, foram obtidos os dados estabilométricos. Para fins estatísticos, analisou-se apenas o desempenho, na plataforma de força, de cada grupo por meio de testes não paramétricos, um grupo em relação ao outro (Mann-Whitney), e segundo a condição dos olhos - abertos ou fechados (Signed Rank). RESULTADOS: Na plataforma de força, houve diferença estatisticamente significativa apenas entre os desempenhos dos grupos 1 (com perda de massa óssea e com aumento da cifose dorsal) e 3 (sem perda de massa óssea e sem aumento da cifose dorsal) na direção ântero-posterior (AP) (p=0,0124). Com exceção do grupo 3 (p=0,4263), todos os demais grupos apresentaram diferença no desempenho entre as tentativas de olhos abertos (OAs) e de olhos fechados (OFs) na direção médio-lateral (ML), enquanto que, na direção AP, nenhum grupo apresentou diferença entre as tentativas. CONCLUSÃO: Os resultados da pesquisa sugerem que houve influência da medida angular da cifose dorsal e da DMO no controle postural na direção AP e na posição ortostática na população estudada.

    OBJECTIVES: To verify the relationship between the angle of thoracic kyphosis, bone mineral density, and postural control in elderly women. METHODS: Through a cross-sectional study, 95 elderly participants were subdivided into four groups according to the thoracic kyphosis angle (obtained by the flexicurve method) and to bone densitometry results. On the force platform and through the dynamic test, stabilometric data were obtained. For statistical analysis, we assessed the performance of each group on the force platform by non-parametric tests: between group comparison (Mann-Whitney) and within group comparison according to the condition of the eyes - open or closed (Signed Rank). RESULTS: On the force platform, the only statistically significant difference was found between groups 1 (loss of bone mass and increased thoracic kyphosis) and 3 (no loss of bone mass or increase in thoracic kyphosis) in the anteroposterior direction (p=0.0124). All groups presented different performances with the eyes open and closed in the mediolateral direction, except for group 3 (p=0.4263), whereas in the anteroposterior direction, we did not observe differences. CONCLUSION: The results suggest an influence of the angle of thoracic kyphosis and bone mineral density on the postural control of our sample in the anteroposterior direction and in the standing position.
  • Relação entre força de tosse e nível funcional em um grupo de idosos Artigos Originais

    CONTEXTUALIZAÇÃO: A força muscular respiratória é influenciada pelo envelhecimento. OBJETIVOS: Avaliar, em uma população de idosos saudáveis, a influência da atividade física e do nível funcional sobre parâmetros da função pulmonar, sobre a força da musculatura respiratória e sobre a tosse. MÉTODOS: Estudo observacional em que foram incluídos 61 idosos com idade igual ou superior a 60 anos (72,3±7,2 anos), com espirometria normal. Foram avaliados: pressão inspiratória máxima (PImax), pressão expiratória máxima (PEmax), pico de fluxo expiratório (PFE), pico de fluxo da tosse (PFT) e perfil de atividade humana (PAH). Os idosos foram divididos em dois grupos (ativo e moderadamente ativo) classificados conforme o PAH. Utilizou-se o teste t de Student nas análises univariadas e foram desenvolvidos modelos de regressão linear nas análises multivariadas. RESULTADOS: Os idosos ativos apresentaram, em média, 13,5 cmH2O a mais no valor da PEmax (88±21,4 cmH2O) e 16,2 cmH2O na PImax (76±17,7 cmH2O), ocorrendo queda, em média, de 1 cmH2O a cada ano nessas variáveis. O PFE e o PFT foram maiores nos idosos do sexo masculino (p<0,001 para ambos) e nos ativos (p=0,046 e p=0,004; respectivamente). Observou-se correlação positiva entre PAH e as seguintes variáveis: PEmax, PImax e PFT (r=0,527, p<0,001; r=0,498, p<0,001 e r=0,365, p=0,004 respectivamente). CONCLUSÕES: O estilo de vida mais ativo pode influenciar de forma positiva, relacionando-se com maior força da musculatura respiratória e valores do PFT. O aumento da idade está relacionado com a redução da força muscular inspiratória e expiratória. As mulheres apresentaram menor PEmax.

    BACKGROUND: Respiratory muscle strength is influenced by aging. OBJECTIVES: To assess, in a healthy elderly population, the influence of physical activity and functional performance on the pulmonary function parameters, on respiratory muscle strength and on coughing. METHODS: Observation study that sixty one elderly with age equal or more than 60 years (72.3±7.2 years), with normal spirometry were included. Maximal Inspiratory Pressure (MIP), Maximal Expiratory Pressure (MEP), Peak Expiratory Flow (PEF), Peak Cough Flow (PCF) and Human Activity Profile (HAP) were evaluated. The elderly were separated in two groups (active and moderately active) according to HAP. Student`s test was used for univariate analysis and regression analysis models was used for multivariate analysis. RESULTS: Elderly participants who were classified as active presented on average 13.5 cmH2O higher MEP (88±21.4 cmH2O), and 16.2 cmH2O higher MIP (76±17.7 cmH2O). Maximal respiratory pressure decreases 1 cmH2O per year. The PEF and PCF were higher for male (p<0.001) and active elderly (p=0.046 e p=0.004 respectively). A positive correlation was observed between HAP and the following variables: MEP MIP and PCF (r=0.527, p<0.001; r=0.498, p<0.001 and r=0.365, p=0.004, respectively). CONCLUSIONS: The active life style can positively affect the respiratory muscle strength and PCF values. Aging is associated to respiratory muscle strength reduction (both inspiratory and expiratory). Women showed lower PEmax.
  • Efeitos da eletroestimulação do músculo vasto medial oblíquo em portadores de síndrome da dor patelofemoral: uma análise eletromiográfica Artigos Originais

    CONTEXTUALIZAÇÃO: O uso da eletromiografia de superfície (EMG-S) tem sido considerado como instrumento de avaliação quantitativa na síndrome da dor patelofemoral (SDPF). Tratamentos conservadores objetivam melhorar o alinhamento patelar, e a estimulação elétrica do músculo vasto medial oblíquo (VMO) tem sido considerada por ser seletiva e não causar irritação articular. OBJETIVO: Verificar o efeito de um programa de fortalecimento muscular com estimulação elétrica do VMO na SDPF por meio da capacidade de avaliação da EMG-S. MÉTODOS: Participaram deste estudo 10 mulheres jovens (idade: 23,1±4,9 anos; massa corporal: 66,8±14,0 kg; estatura: 1,63±6,9 cm; IMC: 25,1±5,6 kg/m²) com SDPF unilateral, as quais realizaram o teste funcional de subir degrau para captação da atividade eletromiográfica dos músculos VMO e vasto lateral (VL), antes e após um programa de estimulação elétrica do VMO. A eletroestimulação foi realizada três vezes por semana, durante seis semanas. Foram consideradas, para análise entre VMO e VL, as variáveis razão do tempo do início até o pico de ativação, razão da integral do sinal (teste t para amostras dependentes) e diferença de início de ativação (teste de Wilcoxon), com nível de significância de p<0,05. RESULTADOS: Os resultados mostraram que ocorreu alteração somente no comportamento eletromiográfico relativo à razão da integral do sinal, mostrando que, após o treinamento muscular, ocorreram mudanças na capacidade de geração da força. CONCLUSÃO: O uso da eletroestimulação deve ser considerado no sentido de complementar a abordagem terapêutica conservadora em portadores da SDFP e a análise da razão da integral do sinal de EMG-S, como instrumento de avaliação.     BACKGROUND: The use of surface electromyography (SEMG) has been considered a tool for quantitative assessment of patellofemoral pain syndrome (PFPS). Conservative treatments aim to improve patellar alignment, and electrical stimulation of the vastus medialis obliquus (VMO) muscle has been considered effective because it is selective and does not cause joint irritation. OBJECTIVE: This study aims to investigate the efficiency of a muscle strengthening program with electrical stimulation of the VMO muscle in PFPS by SEMG. METHODS: A group of ten young women (age: 23.1±4.9 years; body mass: 66.8±14.0 kg; height: 1.63±6.9 cm; BMI: 25.1±5.6 kg/m²) with unilateral PFPS participated in the study. They performed the functional test of stair stepping to capture the electromyographic (EMG) activity of the VMO and vastus lateralis (VL) muscles, before and after a program of electrical stimulation of the VMO muscle. The electrical stimulation was performed three times per week for six weeks. For analysis between the VMO and VL muscles, we considered the variables: ratio of time of onset to peak of activation, ratio of the integrals of the signals (t-test for dependent samples), and difference between onsets of activation (Wilcoxon test), with significance level of p<0.05. RESULTS: The results only showed change in behavior in the EMG signal for the ratio of the integrals of the signals, indicating that changes occurred in the force-generating capacity of the muscle after the training. CONCLUSION: The use of electrical stimulation should be considered to complement the conservative therapeutic approach in patients with PFPS, and the analysis of the ratio of the integrals of the SEMG signals should be considered as an instrument of evaluation.   • Velocidade ideal da esteira para estimular passadas do andar em bebês Artigos Originais

    CONTEXTUALIZAÇÃO: A esteira motorizada desencadeia passadas em bebês similares ao andar. Entretanto, os efeitos da velocidade da esteira nas características das passadas desencadeadas por ela ainda são desconhecidos. OBJETIVO: Examinar o efeito da velocidade da esteira em passadas desencadeadas em bebês. MÉTODOS: Seis bebês com desenvolvimento típico, com idades entre 11 e 13 meses, iniciando o andar independente, foram filmados andando na esteira em quatro velocidades (0,10; 0,16; 0,22 e 0,28 m/s), com marcas passivas posicionadas em suas principais articulações (ombro, quadril, joelho e tornozelo). As imagens foram digitalizadas e analisadas com os softwares Ariel Performance Analysis System, Matlab e SPSS (Statistical Package for Social Sciences, v.10). As variáveis analisadas foram as características descritivas, temporais, coordenativas e angulares das passadas desencadeadas pela esteira nas quatro velocidades. RESULTADOS: Em 0,22 e 0,28 m/s, ocorre aumento da velocidade e duração da passada, além de uma menor duração da fase de apoio (FA) na velocidade de 0,22 m/s. Nas velocidades de 0,22 e 0,28 m/s,observa-se aumento da amplitude articular do joelho. CONCLUSÕES: As velocidades moderadas e rápidas parecem ser as mais indicadas para desencadear passadas em bebês que estão iniciando o andar independente, sendo ainda que a velocidade de 0,22 m/s parece ser a mais apropriada.     BACKGROUND: The motorized treadmill elicits stepping in infants, however the effects of belt speed on treadmill-elicited stepping patterns are still unknown. OBJECTIVE: To examine the effect of belt speed on treadmill-elicited infant stepping. METHODS: Six normally developing infants, aged between 11 and 13 months and acquiring independent walking, were videotaped while stepping on a treadmill at four belt speeds (0.1, 0.16, 0.22, and 0.28 m/s), with passive markers affixed to the major joints (shoulder, hip, knee, and ankle). The images were digitized and analyzed using the software programs Ariel Performance Analysis System, Matlab, and SPSS (Statistical Package for Social Sciences, v.10). The analyzed variables were the descriptive, temporal, coordinative, and angular characteristics of the treadmill-elicited steps at the four speeds. RESULTS: At 0.22 and 0.28 m/s, stride velocity and duration increased, and the duration of the stance phase decreased at 0.22 m/s. Moreover, at 0.22 and 0.28 m/s, the knee joint range of motion increased. CONCLUSIONS: The moderate and high speeds seem to be more appropriate to elicit stepping in infants who are acquiring independent walking, with 0.22 m/s being the speed that seemed the most appropriate.   • Mobilidade funcional e função executiva em idosos diabéticos e não diabéticos Artigos Originais

    CONTEXTUALIZAÇÃO: Idosos diabéticos tendem a ter declínio da funcionalidade motora e a apresentar déficits cognitivos relacionados a processos mais complexos, como a função executiva, o que pode levar a um maior risco de quedas. OBJETIVOS: Comparar idosos com e sem diabetes tipo 2 quanto à mobilidade funcional, ao risco de quedas e à função executiva e verificar a correlação entre essas variáveis. MÉTODOS: Participaram do estudo 40 idosos da comunidade, divididos em dois grupos: G1, idosos com diabetes tipo 2 e G2, idosos sem diabetes, sendo os grupos semelhantes quanto ao gênero, idade e índice de massa corporal. A mobilidade funcional e o risco de quedas foram avaliados pelo Timed Up and Go (TUG e TUGcognitivo), e a função executiva, pelo teste de fluência verbal (categoria animal). RESULTADOS: Os diabéticos apresentaram pior desempenho no teste de fluência verbal (G1:14,9±4,5; G2:17,7±5,6; p=0,031). Uma diferença estatisticamente significativa foi observada entre os grupos em relação à mobilidade funcional, sendo que o G1 apresentou pior desempenho no TUG (G1:10,5±1,8s; G2:8,9±1,9s; p=0,01) e noTUGcognitivo (G1:13,9±3,2s; G2:10,9±2,3s; p=0,004). Uma correlação siginificativa foi observada entre o TUGcognitivo e o teste de fluência verbal apenas no G1 (G1: Spearman's rho = -0,535; p=0,015; G2: Spearman's rho = -0,250; p=0,288). CONCLUSÕES: Os diabéticos apresentaram um pior desempenho nos testes de mobilidade funcional e de fluência verbal que os idosos sem a doença, sugerindo um maior risco de quedas para idosos diabéticos. A inclusão desses parâmetros de avaliação para diabéticos na prática clinica da fisioterapia é fundamental para preservar a funcionalidade e evitar quedas.

    BACKGROUND: Elderly diabetics tend to show cognitive deficits related to more complex processes such as the executive function, which can lead to a greater risk of falls. OBJECTIVES: The aims of this study were to compare the functional mobility, the risk of falls and the executive function among elderly with and without type 2 diabetes, and to check the correlation between these variables. METHODS: Forty community elderly participated in the study and were divided into two groups: G1 elderly with type 2 diabetes and G2 elderly without diabetes, being the variables age, body mass index and gender similar between the groups. The functional mobility and the risk of falls were assessed by the "Timed Up and Go" test (TUG and cognitive TUG) and the executive function was assessed by the Verbal Fluency Test (VFT) (animal category). RESULTS: Elderly with diabetes showed worse performance in the verbal fluency test (G1:14.9±4.5; G2:17.7±5.6; p=0.031). A statistically between-group difference was observed regarding the functional mobility; being the G1 presenting worse performance in TUG (G1:10.5±1.8sec; G2:8.9±1.9sec; p=0.01) and cognitive TUG (G1:13.9±3.2sec; G2:10.9±2.3sec; p=0.004) tests. A significant correlation was observed between the cognitive TUG and VFT only in G1 (G1: Spearman's rho = -0.535; p=0.015; G2: Spearman's rho =-0.250; p=0.288). CONCLUSIONS: Diabetics presented worse performance in the functional mobility and in the verbal fluency test than non-diabetics elderly that suggests a greater risk of falls for the elderly with diabetes. The inclusion of these evaluation parameters for diabetics on the physical therapy clinical practice is crucial in order to maintain the functionality and to prevent falls.
  • Impacto do alongamento estático no ganho de força muscular dos extensores de joelho em idosas da comunidade após um programa de treinamento Artigos Originais

    CONTEXTUALIZAÇÃO: A diminuição da força muscular em idosos tem impacto na funcionalidade. Programas de reforço muscular são propostos, porém existem controvérsias quanto ao efeito de alongamentos prévios no ganho de força muscular. OBJETIVO: Verificar o impacto do alongamento estático no ganho de força dos músculos extensores de joelho em idosas da comunidade após programa de treinamento. MÉTODOS:Trata-se de um estudo quase-experimental, em que se realizou um programa de exercícios de fortalecimento muscular dos extensores de joelho durante 10 semanas, com avaliação final de 12 idosas da comunidade divididas em dois grupos: AE - exercícios com carga e alongamentos prévios em todas as sessões, média de idade de 73,8 (±5,36) anos e E - mesmos exercícios com carga do grupo acima, sem a realização de alongamentos prévios, média de idade de 72,14 (±5,43) anos. Para avaliar o ganho de força muscular dos extensores de joelho, utilizou-se o dinamômetro isocinético Biodex System 3 Pro, no modo concêntrico, na velocidade de movimento angular de 60 e 180º/segundos (s) e, para a análise, a diferença das médias da pré e da pós-intervenção da variável trabalho, normalizada pelo peso corporal. RESULTADOS: Os dados apresentaram-se normais pelo teste de Shapiro-Wilk (p>0,266). Quando comparadas as médias da diferença da pré e da pós-intervenção pelo teste t para amostras independentes, não houve diferença significativa em nenhum dos membros e nas velocidades avaliadas (p>0,383). CONCLUSÃO: O alongamento estático prévio não interferiu no ganho de força muscular após um programa de fortalecimento de 10 semanas na população estudada.     BACKGROUND: The decrease in muscle strength in older adults has an impact on functionality. Muscle strengthening programs have been proposed, however there is some controversy surrounding the effects of stretching prior to strengthening exercises on muscle strength gain. OBJECTIVE: To verify the impact of static stretching on the gain in knee-extensor strength of community-dwelling elderly women after a training program. METHODS: This was a quasi-experimental study that included a ten-week knee-extensor strengthening program and a final assessment of 12 community-dwelling elderly women divided into two groups: group SE - strengthening exercises with prior static stretching in all sessions, mean age 73.8 (±5.36) years; and group E - same strengthening exercises as SE without prior stretching, mean age 72.14 (±5.43) years. To measure knee-extensor strength gains, we used the isokinetic dynamometer Biodex System 3 Pro, in concentric mode at angular velocities of 60º and 180º/s, and for statistical analysis we used the difference between pre- and post-intervention means of work adjusted by body weight. RESULTS: The data showed normality in the Shapiro-Wilk test (p>0.266). When comparing the differences between the pre- and post-intervention means using the t test for independent samples, there was no significant difference in any of the limbs at the velocities evaluated (p>0.383). CONCLUSION: Previous static stretching did not interfere in muscle strength gain following a ten-week muscle strengthening program in the population studied.   • Comparação do índice de respiração rápida e superficial (IRRS) calculado de forma direta e indireta no pós-operatório de cirurgia cardíaca Artigos Originais

    OBJETIVOS: Comparar e analisar a correlação entre o índice de respiração rápida e superficial (IRRS) determinado com os valores fornecidos pelo software do ventilador mecânico Raphael® e pelo ventilômetro digital. MÉTODOS: Participaram do estudo 22 indivíduos adultos (17 homens e 5 mulheres), intubados, no pós-operatório de cirurgia cardíaca. Antes da coleta de dados, cada indivíduo foi avaliado, recebeu atendimento fisioterapêutico a fim de promover higiene brônquica e reexpansão pulmonar e foi posicionado em decúbito dorsal elevado em 45º, e depois foram registrados os valores de volume minuto (VM) e frequência respiratória (FR) obtidos pelo ventilador e pelo ventilômetro. O IRRS foi calculado pela relação FR/volume corrente (VC). Aplicou-se o teste t-pareado para comparação das variáveis relacionadas. Utilizou-se o coeficiente de correlação intraclasse (CCI) para mensuração da replicabilidade dos escores. RESULTADOS: Observou-se diferença significativa entre o IRRS obtido pelo ventilador mecânico e o obtido pelo ventilômetro (p-valor=0,011) e concordância moderada para VM (CCI=0,74), alta concordância para FR (CCI=0,80), VC (CCI=0,79) e IRRS (CCI=0,86). Para todas as variáveis, o p-valor foi menor que 0,05. CONCLUSÕES: Houve concordância estatisticamente significativa entre o IRRS calculado pelos valores registrados no ventilador mecânico e pela ventilometria Artigo registrado no Australian New Zealand Clinical Trials Registry (ANZCTR) sob o número ACTRN12610000756022.

    OBJECTIVES: To compare and to analyze whether the values of rapid shallow breathing index (RSBI) determined by a ventilator display and a digital ventilometer were correlated. METHODS: Twenty-two adult patients (17 males and 5 females) in the postoperative period of cardiac surgery and in mechanical ventilation were studied. Prior to the data collection, each patient was evaluated, received physical therapy, in order to promote bronchial hygiene and pulmonary reexpansion, and was positioned in elevated dorsal recumbent at 45º. After these procedures, minute ventilation (MV) and respiratory rate (RR) obtained from mechanical ventilator display and digital ventilometer were recorded. The RSBI was calculated by the ratio of RR and tidal volume (VT). Paired t-test was used to compare related variables. The intra-class correlation coefficients (ICCs) were used to measure the reproducibility of the scores. RESULTS: A significant difference was found between the RSBI obtained from the ventilator and by the digital ventilometer (p=0.011). A high agreement for the RSBI (ICC=0.86), for the RR (ICC=0.80) for the VT (ICC=0.79) and a moderate agreement for the MV (ICC=0.74) were observed. The p-value was <0.05 for all variables. CONCLUSIONS: There were a significant agreement between the RSBI obtained from the ventilator display and the digital ventilometer.     CONTEXTUALIZAÇÃO: Apesar do amplo uso do Pilates na Fisioterapia, há poucos estudos que avaliaram a ativação elétrica dos músculos nos exercícios. OBJETIVO: Verificar a influência de diferentes regulagens de mola e posições do indivíduo sobre a ativação elétrica dos multífidos (MU) e oblíquos externos (OE) durante a flexoextensão do quadril (FEQ) no Cadillac. MÉTODOS: Oito mulheres praticantes de Pilates por seis meses realizaram 10 repetições de FEQ nas situações: mola baixa (MB), mola fixada a 30 cm do nível em que estava o indivíduo; mola alta (MA), mola fixada a 90 cm do nível em que estava o indivíduo; posição próxima (PP), distância de 10 cm da fixação da mola; posição distante (PD), distância de 30 cm da fixação da mola. Dados cinemáticos e de eletromiografia (EMG) foram coletados sincronizadamente, e os músculos monitorados bilateralmente foram os OE e os MU. Cada movimento de FEQ foi recortado em duas fases (extensão e flexão). O sinal de EMG foi calculado e normalizado usando a contração voluntária máxima (CVM). O Wilcoxon test foi usado para investigar diferenças entre as situações (p<0,05). RESULTADO: Os músculos MU apresentaram valores de ativação muscular de 10 a 20% da CVM, sendo os maiores valores observados na MA e na PD. Para os OE, valores de ativação de 20 a 45% da CVM foram encontrados, com os maiores valores obtidos na MB e na PP. CONCLUSÃO: Os músculos OE e MU apresentaram uma ativação elétrica distinta durante as diferentes regulagens de mola e posições dos indivíduos avaliados.

    BACKGROUND: Despite of the widepread use of Pilates in Physical Therapy, there are few studies that have assessed the muscle electrical activation during Pilates exercises. OBJECTIVE: Verify the influence of different spring adjustments and individual positions on the electrical activation of multifidus (MU) and oblique external (OE) muscles during hip flexion-extension (HFE) exercise on the Cadillac. METHODS: Eight women practicing Pilates exercises for at least six months performed 10 repetitions of HFE in the following situations: Lower Spring, spring fixed at 30 cm in relation to level which the individuals were positioned. Higher Spring, spring fixed at 90 cm in relation to level which the individuals were positioned. Near Position, distance of 10 cm from the fixed spring. Distant Position, distance of 30 cm from the fixed spring. Kinematic and eletromyographic data (EMG) were collected simultaneously and the MU and OE muscles were monitored. Each movement of HFE was splitted in two phases (extension and flexion). The EMG signal was calculated and normalized using the maximal voluntary contraction (MVC). The Wilcoxon test was used to investigate differences between the situations (p<0.05). RESULTS: MU muscle presented muscle activation values ranging from 10 to 20 % MVC, and the highest muscle activation in the lower spring and in the near position. OE muscles presented muscle activation values ranging from 20 to 45% MVC, and the highest values in the higher spring and in the distant position. CONCLUSION: MU and OE muscles presented a distinct electrical activation during different available spring adjustments and individual positions.
  • Elaboração e confiabilidade da escala funcional do subir e do descer escada para Distrofia Muscular de Duchenne Artigo Metodológico

    CONTEXTUALIZAÇÃO: Os instrumentos de avaliação funcional utilizados para pacientes com distrofia muscular de Duchenne (DMD), citados na literatura, são limitados e escassos, dificultando a tomada de decisão clínica fisioterapêutica. OBJETIVOS: Descrever o processo de criação de uma escala de avaliação funcional do subir e do descer escadas, específica para crianças com diagnóstico de DMD, e examinar sua confiabilidade inter e intraexaminadores. MÉTODOS: A construção da escala seguiu cinco etapas, a saber, elaboração de um roteiro para observação dirigida com base na literatura; observação do subir e do descer em 120 registros filmados de 30 crianças com DMD (5 a 11 anos); elaboração da escala, considerando o grau crescente de dificuldade de execução dos movimentos; criação do manual de preenchimento e submissão da escala e do manual a 10 examinadores, seguida de reajustes para criação da versão final. A confiabilidade foi testada pelo pesquisador (repetibilidade) e dois examinadores independentes (reprodutibilidade). Utilizou-se o Índice de Correlação Intra-Classe (ICC) e a Correlação de Kappa Ponderado. RESULTADOS: A escala elaborada abrange cinco fases para o subir e quatro fases para o descer escadas. Encontrou-se excelente confiabilidade intra/interexaminadores, com valores da Correlação de Kappa Ponderado > 0,78 em todas as fases e ICCs > 0,89, com p<0,05 entre todos os escores. CONCLUSÃO: A escala proposta mostrou excelente repetibilidade e reprodutibilidade, sendo necessária a continuidade do estudo com os objetivos de avaliar sua acurácia e validade e de construir uma ferramenta digital para otimizar a coleta de dados.

    BACKGROUND: Instruments of functional evaluation for patients with Duchenne Muscular Dystrophy (DMD), available from the literature, are limited and scarce, making clinical decision on Physiotherapy difficulty. OBJECTIVES: To describe the process of creating a functional assessment scale in patients with DMD while going up and down the stairs, as well as to analyse the intra and inter-rater reliability of this scale. METHODS: The scale development consisted of five stages: 1) to elaborate a script for directed observation based upon literature, 2) to describe a sequence of movements assessing 120 video recordings from 30 children (from 5 to 11 years) with DMD while going up and down stairs, 3) to elaborate a scale considering the degree of difficulty to execute the movements, 4) to create handbook, and 5) to submit both the scale and the handbook to be assess by 10 examiners, and review to create the final version. Both repeatability (researcher) and reproducibility (two independent examiners) were tested by using the IntraClass Correlation Coefficient (ICC) and Weighted Kappa respectively. RESULTS: The scale encloses five phases for going up and four phases for going downstairs. Our results showed both excellent intra and intertester reliability, with values of Weighted Kappa > 0.78 in all phases and ICCs > 0.89, with p<0.05 for all scores. CONCLUSION: The proposed scale showed excellent repeatability and reproducibility, requiring continuing the studies to assess its accuracy, validity and to create digital tools to improve data collection.
  • Versão Brasileira do Shoulder Pain and Disability Index: tradução, adaptação cultural e confiabilidade Artigo Metodológico

    OBJETIVOS: Traduzir e adaptar culturalmente o questionário Shoulder Pain and Disability Index (SPADI) para a Língua Portuguesa do Brasil e avaliar sua confiabilidade. MÉTODOS: A primeira etapa consistiu na tradução, síntese, retro-tradução, revisão pelo Comitê, pré-teste e avaliação dos documentos pelo comitê e pelo autor do SPADI. A versão revisada pelo Comitê foi aplicada a 90 indivíduos com disfunção no ombro, com faixa etária acima de 18 anos e níveis educacional e sociocultural variados. Os itens não compreendidos por 20% ou mais dos pacientes foram reformulados e reaplicados até alcançarem valores menores que esse. A segunda etapa consistiu de duas aplicações do SPADI a 32 pacientes com disfunção de ombro, no intervalo de 2 a 7 dias. Os dados de tradução foram analisados descritivamente, a confiabilidade teste-reteste, pelo Coeficiente de Correlação Intraclasse (ICC) e a consistência interna, pelo Alpha de Cronbach. RESULTADOS: Algumas expressões foram adaptadas à população brasileira, e os itens da escala de dor e incapacidade foram alterados para maior facilidade de leitura na Língua Portuguesa. O pré-teste revelou a necessidade de se alterar apenas um item de dor e de se aplicar o questionário por entrevista, pois ele se repetiu três vezes e, nas duas primeiras aplicações, com o questionário autoaplicado, os pacientes não realizaram o relato referente à "semana passada" e utilizaram inadequadamente o item "Não se aplica". A confiabilidade teste-reteste variou de 0,90 a 0,94, e a consistência interna de 0,87 a 0,89. CONCLUSÃO: Após a tradução e adaptação cultural, foi obtida uma versão confiável do SPADI-Brasil.

    OBJECTIVES: To translate and culturally adapt the Shoulder Pain and Disability Index (SPADI) into Brazilian-Portuguese and to assess its reliability. METHODS: The first step was the translation, synthesis, back-translation, revision by the committee, pre-testing and evaluation of documents by the committee and the author of the SPADI. The revised version by the committee was applied to 90 subjects with shoulder dysfunction, aged over 18 years from different education and sociocultural levels. The items misunderstood by 20% or more of patients were reformulated and reapplied until they reach values lower than 20%. The second stage consisted of two applications of SPADI to 32 patients with shoulder dysfunction in a interval ranging from 2 to 7 days. The data from the translation were analyzed descriptively, the test-retest reliability by Intraclass Correlation Coefficient (ICC) and the internal consistency by Cronbach's Alpha. RESULTS: Some expressions have been adapted to the Brazilian population and the items in the pain and disability subscales were changed for an easier reading in Portuguese Language. The pre-test revealed a need to change only one item of the pain domain and to administer the questionnaire by interview, since it was repeated three times and in the first two applications with self-reported questionnaire the patients had not been reporting their symptoms with regards to the past week and also they have not been properly used the item "Not Applicable". The test-retest reliability ranged from 0.90 to 0.94 and the internal consistency ranged from 0.87 to 0.89. CONCLUSION: After the translation and cultural adaptation, it was obtained a reliable version of SPADI-Brazil.
  • Adaptação transcultural para o Brasil do Sistema de Classificação da Função Motora Grossa (GMFCS) Artigo Metodológico

    CONTEXTUALIZAÇÃO: Em função da complexidade das manifestações clínicas da paralisia cerebral (PC) e das dificuldades na sua classificação baseada apenas nos tipos motores e topografia de distribuição corporal, estudiosos canadenses propuseram o Gross Motor Function Classification System (GMFCS). Apesar de esse sistema de classificação estar sendo bastante utilizado no Brasil, ele ainda não havia sido adapatado transculturalmente para tal. OBJETIVOS: Realizar a adaptação transcultural para o Brasil do Sistema de Classificação da Função Motora Grossa (GMFCS) para a PC e verificar a confiabilidade entre observadores do instrumento adaptado com crianças brasileiras. MÉTODOS: Este estudo consistiu em duas etapas, sendo a primeira relacionada com o processo de adaptação transcultural, e a segunda referente à testagem do instrumento. A adaptação transcultural do instrumento foi feita por meio da tradução, retrotradução, análise semântica, análise de conteúdo, retrotradução da versão final e aprovação dos autores do instrumento. A testagem do instrumento ocorreu junto a 40 crianças com PC, as quais foram avaliadas por dois examinadores para verificar a confiabilidade entre observadores. RESULTADOS: Os resultados demonstram que as etapas de tradução e retrotradução não apresentaram dificuldades, e a equivalência semântica e a conceitual foram obtidas. A confiabilidade entre examinadores demonstrou que as avaliações quase não diferiam e que havia excelente correlação e consistência interna do constructo, com coeficiente de correlação intraclasse (ICC) de 0,945 (com intervalo de 0,861 a 0,979) e a de Cronbach de 0,972. CONCLUSÕES: A versão final do GMFCS mostrou bom potencial de aplicabilidade por graduandos e profissionais da área de neuropediatria.

    BACKGROUND: Due to the complexity of clinical symptoms of cerebral palsy and the difficulties in classifying it based upon the motor types and the topography of the body distribution only, Canadian researchers have proposed the Gross Motor Function Classification System (GMFCS). Although this classification system has been largely used in Brazil, it has not been cross culturally adapted yet. OBJECTIVES: To perform the cross adaptation of the Gross Motor Function Classification System for the Cerebral Palsy (GMFCS) into Brazilian-Portuguese and to verify the reliability among observers of the adapted instrument in Brazilian children. METHODS: This study was performed in two stages; the first stage was related to the process of cross-cultural adaptation and the second stage tested the instrument. Translation, back-translation, semantic and content analysis, back-translation of the final version and the approval of the authors were used for the cross-cultural adaptation. The test of the instrument was performed in 40 children with cerebral palsy, who were evaluated by two raters to verify the reliability among the observers. RESULTS: The results showed that the stages of translation and back-translation did not present any difficulties and the semantic and conceptual equivalence was achieved. The reliability among the observers showed that the evaluations do not differ and that there is an excellent correlation and internal consistency of the construct with an ICC of 0.945 (95% CI 0.861 to 0.979) and a Cronbach a of 0.972. CONCLUSIONS: The final version of the GMFCS showed good potential of applicability for undergraduate students and professionals of the neuropediatric area.
