    A presença de dentes na cavidade nasal é rara e inclui alguns sinais e sintomas característicos, como obstrução nasal, rinite casual e odor fétido, entre outros, que podem ser confundidos com outras patologias. Por esta razão, o diagnóstico diferencial deve ser bem executado, por meio dos exames clínico e radiográfico da região. O tratamento, geralmente, é cirúrgico e pode prevenir complicações secundárias. O presente trabalho apresenta um caso de dente intranasal e seu respectivo tratamento cirúrgico para remoção.

    The presence of teeth at the nasal cavity is rare and involves some characteristic signs and symptoms, such as nasal obstruction, eventual rhinitis and fetid smell, and also others that may be confused with other pathologies. Therefore, differential diagnosis should be carefully accomplished by means of clinical and radiographic examinations of the area. Treatment is usually surgical and may prevent secondary complications. The present paper presents a case of intranasal tooth and the respective surgical treatment for its removal.
  • Splint maxilar modificado como auxiliar na correção da má oclusão de Classe II, Divisão 2, sem crescimento: relato clínico Artigos Inéditos

    Neste artigo é descrito o caso clínico de um paciente adulto, sem crescimento, com má oclusão de Classe II, divisão 2, tratado com um splint maxilar modificado. Foi exercida força extra-bucal com direção de tração parietal com força de 400 gramas, com uso diário de 14 horas durante 1 ano. Com este aparelho removível corrigiu-se completamente a relação de Classe II dos molares e pré-molares, levando estes dentes à oclusão normal, não havendo extrusão dentária, mantendo constante o plano mandibular. Após a correção dos dentes posteriores com o splint maxilar modificado, utilizou-se aparelhagem fixa convencional para corrigir as inclinações dos dentes anteriores e finalizar o caso.

    In this article is reported a Class II division 2 clinical case of an adult without growth treated with a modified maxillary splint. It was used extra-buccal force with parietal traction with 400 grams of force 14 hours a day. With this removable device it was corrected the molars and bicuspids Class II relation without extrusion and then controlling the opening of the mandibular plane. After the correction of the posterior teeth with the modified maxillary splint, it was utilized conventional fixed orthodontic therapy to correct the inclinations of the anterior teeth and finish the case.

    Controvérsias na leitura cefalométrica da AFAi (Altura Facial Anterior inferior) e no diagnóstico clínico baseado na análise facial em casos de más oclusões do padrão II por deficiência mandíbular, motivaram este artigo. Compusemos uma amostra formada por 26 pacientes portadores desta má oclusão, sem distinção de gênero e idade, tratados no curso de especialização da PROFIS do HRAC - USP, Bauru. A amostra foi caracterizada utilizando-se um conjunto de medidas cefalométricas clássicas (SNA, SNB, ANB, NAP, AFAi, SNPO, SNGoGn, CoA, CoGn) e testamos uma forma alternativa para medir a AFAi, onde os pontos espinha nasal anterior (ENA) e mentoniano (Me) foram projetados perpendicularmente a um plano frontal que passa por násio, perpendicular ao plano de Frankfurt (Nperp A) e a distância entre eles medida linearmente. Os resultados foram confrontados com os valores obtidos da medição da AFAi como preconizado por McNamara, em 1984. Encontrou-se uma relação inversa entre a AFAi convencional e a AFAi perpendicular. Esta diferença foi estatisticamente significante para toda a amostra e apresentou alta correlação. A AFAi perpendicular foi sempre menor em toda amostra e exibiu uma tendência de ser tanto menor quanto maior fosse o valor de SNGoGn. Estes achados levaram a crer que a AFAi perpendicular, medida como preconizada neste artigo, torna o triângulo de McNamara mais equilibrado. Isso confere à avaliação cefalométrica a capacidade de identificar a diminuição da AFAi, característica facial típica do portador de deficiência mandibular, além de permitir um entendimento mais claro dos efeitos do tratamento nessa dimensão.

    Controversies between the cephalometrics analysis of the ALFH (anterior lower facial height) and the clinical diagnosis based on the facial examination on Class II with mandibular deficiency has motivated this article. A sample composed of 26 Class II division 1 with mandibular deficiency individuals without distinction of gender or age already treated were selectedThis sample were characterized with a whole of classical cephalometrics measurements (SNA, SNB, ANB, NAP, ALFH, SNOP, SNGoGn, CoA, CoGn) and a new alternative way of measuring the ALFH were tested. The anterior nasal spine (ANS) and menton (Me) points were projected perpendicularly to a frontal plane that goes through nasion, perpendicular to Frankfourt plane (NperpA, McNamara 1984) and the distance between them measured in a lineal way. The results were compared with the obtained values from ALFH measurements as dictated by McNamara, 1984. We found a reverse relationship between the different measurements (conventional ALFH and ALFH perpendicular). This reverse relationship were statistically significant to whole sample and showed a high correlation. The ALFH perpendicular was always minor than conventional ALFH and exhibited a tendency to be even minor, as higher as were the value obtained to SNGoGn angle. These findings leaded to believe that the ALFH perpendicular, used as preconized in this article, turns the McNamara triangle more balanced and consequently its obtained diagnostic in more concordance with the orthodontic therapy adopted to correct the Class II division 1 with mandibular deficiency.
  • O efeito da exodontia dos primeiros pré-molares sobre a AFAI Artigos Inéditos

    A extração dos pré-molares ocorre convencionalmente como parte da terapia ortodôntica, no intuito de resolver uma série de problemas. Todavia sua utilização para o controle da dimensão vertical gera muitas controvérsias. Esse estudo retrospectivo avaliou cefalometricamente as telerradiografias, tomadas em norma lateral, de 30 jovens do gênero feminino, com má oclusão do tipo Classe II, divisão 1, dolicofaciais, com idade média de 12,3 anos, variando entre 9 e 17 anos, tratadas ortodonticamente com aparelho fixo tipo edgewise e uso de extrabucal com tração alta, das quais 15 submeteram-se à exodontia dos primeiros pré-molares e as outras 15 não realizaram exodontias. As medidas cefalométricas N-Me, N-ENA, ENA-Me, S-Go, Ar-Go, 6-PP, 6-ENAperp, 6-GoGn, 6-Pogperp, SN.GoGn, SN.PP, PP.GoGn, ENA-Me/N-Me, S-Go/N-Me e Go-Ar/S-Go foram estudadas. Os resultados revelaram: 1) que a exodontia dos primeiros pré-molares, associada à extrusão dos primeiros molares e ao crescimento, não permitiu uma diferença nas alterações da AFAI entre os grupos, 2) ocorreu uma mesialização dos molares superiores e inferiores no grupo com exodontias, 3) a AFAT aumentou similarmente nos dois grupos, 4) o aumento da AFAT ocorreu de forma significativamente maior no grupo sem exodontias, devido ao maior aumento da AFAS, indicando que o crescimento possui um papel preponderante no controle da dimensão vertical durante o tratamento ortodôntico, 5) observamos a manutenção dos valores angulares durante o tratamento devido principalmente ao crescimento vertical posterior que ocorreu na mesma proporção do crescimento da região anterior da face em ambos os grupos durante o tratamento ortodôntico.

    Premolar extractions are often utilized as part of orthodontic treatment to solve a wide range of problems. However, its use to control vertical dimension is still controversial. This retrospective study investigated the lateral cephalometric measurements of 30 dolichocephalic girls, with Class II division 1 malocclusion. These patients were treated with edgewise appliances and high traction headgear. From the whole group 15 had four first bicuspids extractions and 15 had no extractions. The mean age was 12.3 years, ranging from 9 to 17 years. The cephalometric measurements N-Me, N-ANS, ANS-Me, S-Go, Ar-Go, 6-PP, 6-ANSperp, 6-GoGn, 6-Pogperp, SN.GoGn, SN.PP, PP.GoGn, ANS-Me/N-Me, S-Go/N-Me, and Go-Ar/S-Go were used in the study. The study revealed that: 1) the extraction of the first premolar associated with extrusion of the first molars and growth did not cause any difference in the lower anterior facial height between the two groups; 2) both upper and lower first molars moved mesially in the extraction group; 3) the increase in the lower anterior facial height was similar for both groups; 4) the total anterior facial height was larger in the non extraction group due to a higher increase in the upper anterior facial height; and 5)- the angular measurements did not change significantly in both groups during treatment, most likely because growth between the posterior and the anterior vertical dimension were balanced.
  • Análise in vitro da resistência ao cisalhamento de braquetes metálicos colados em incisivos bovinos e humanos Artigos Inéditos

    O objetivo deste trabalho foi comparar a resistência ao cisalhamento de braquetes metálicos fixados em esmalte bovino e humano e observar o índice de remanescente de adesivo (IRA). A amostra consistiu de 30 dentes divididos em três grupos, formados por incisivos inferiores permanentes humanos, incisivos inferiores permanentes bovinos e incisivos inferiores decíduos bovinos. Foi realizada profilaxia com pedra-pomes e água, seguida de condicionamento do esmalte com ácido fosfórico a 37%, em todos os dentes. Foram colados braquetes metálicos com compósito Concise Ortodôntico de acordo com instruções do fabricante e os mesmos foram submetidos ao ensaio de cisalhamento com velocidade de 0,5 mm por minuto. O valor médio de resistência à colagem em incisivos inferiores humanos foi de x= 12,96 MPa, em incisivos permanentes bovinos de x= 15,05 MPa e em incisivos decíduos bovinos de x= 17,03 MPa. Não foram encontradas diferenças estatísticas significantes entre os grupos, tanto nos valores de resistência ao cisalhamento quanto na avaliação do IRA.

    The purpose of this study was to compare shear bond strength of steel brackets in bovine and human teeth and observe adhesive remnant index (ARI). The sample consisted of thirty teeth, divided into three groups: human permanent lower incisors, bovine permanent lower incisors and bovine deciduous lower incisors. Prophylaxis has been made with pumice and water, followed by enamel acid etching with phosphoric acid at 37% on the surface of the teeth. Steel brackets were bonded with Concise composite according to the manufacturer instructions, and they were submitted to a shear bond strength test for a 0,5 mm / minute speed. The medium values of bond strength in human permanent lower incisors was of x= 12,96 MPa, in bovine permanent lower incisors of x= 15,05 MPa, and bovine deciduous lower incisors of x= 17,03 MPa. There were not significant estatistic differences among the groups, not for strength shear values nor for the evaluation of the ARI.
  • Avaliação do coeficiente de atrito de braquetes metálicos e estéticos com fios de aço inoxidável e beta-titânio Artigos Inéditos

    Um fator importante que define a eficácia dos aparelhos ortodônticos fixos é o atrito existente entre as superfícies de fios e braquetes. Assim, este estudo teve como objetivo investigar o coeficiente de atrito estático entre fios de aço inoxidável e beta-titânio (TP Orthodontics) e braquetes de aço inoxidável (Dynalock® - Unitek), braquetes estéticos com slot de aço inoxidável (Clarity® - Unitek) e estéticos convencionais (Allure® - GAC). Para tanto, construiu-se um equipamento no Departamento de Engenharia Mecânica e Mecatrônica da PUCRS. Antes de serem iniciados os testes, foi quantificado o erro de método e constatou-se que não houve interferência significante (p>0,05) do fator operador nas medições. Então, pôde-se calcular o valor do coeficiente de atrito, obtido pela divisão da força de atrito pela carga normal. O método estatístico utilizado neste estudo foi Análise de Variância (ANOVA) e teste de Comparações Múltiplas (Tukey). Constatou-se que: 1) a combinação com menor coeficiente de atrito foi composta pelo fio de aço inoxidável e braquete Dynalock® e a que apresentou maior coeficiente foi a do braquete Allure® com o fio de beta-titânio; 2) o fio de beta-titânio apresentou coeficiente de atrito significativamente maior do que o fio de aço inoxidável; 3) o braquete Dynalock® não apresentou diferenças significativas em relação ao coeficiente de atrito do braquete Clarity® quando o fio utilizado foi de beta-titânio. No entanto, quando o fio testado foi de aço inoxidável, apresentou coeficiente de atrito significativamente menor. O braquete Clarity® apresentou coeficiente de atrito significativamente menor do que o braquete Allure®.

    An important factor that defines the effectiveness of the appliances is the friction between the surfaces of wires and brackets. Thus, that study was developed in order to investigate the static friction coefficient between stainless steel and beta-titanium wires (TP Orthodontics) and the stainless steel brackets (Dynalock® - Unitek), esthetic brackets with a stainless steel slot (Clarity® - Unitek) and conventional esthetic brackets (Allure® - GAC). For this purpose, an equipment was built in the Department of Mechanical Engineering and Mechatronics at PUCRS. It should be stressed that before the tests began, the Method Error was quantified and it was found that there was no significant interference (p>0,05) by the factor operating in the measurements. From then, the friction coefficient could be calculated, obtained by dividing friction force by the normal load. The statistic method used in these study was Analysis of Variance (ANOVA) and Multiple Comparison test (Tukey). It was found that: 1) the combination with the lowest friction coefficient was constituted by the stainless steel wire against the Dynalock® bracket, and the one with the highest friction coefficient was that of the Allure® bracket with the beta-titanium wire; 2) the beta-titanium wire presented a friction coefficient that was significantly higher than the stainless steel wire; 3) the Dynalock® bracket did not present any significant differences in relation to the friction coefficient of the Clarity® bracket when the wire used was of beta-titanium. However, when the wire tested was of stainless steel, the Dynalock® bracket presented a significantly lower friction coefficient. The Clarity® bracket presented a significantly lower friction coefficient than the Allure® bracket.
  • O paradigma da relação entre oclusão, ortodontia e disfunção têmporo-mandibular Artigos Inéditos

    O relacionamento entre oclusão, Ortodontia e disfunção têmporo-mandibular tem sido objeto de discussões e controvérsias. No passado recente, a literatura científica mostrava que os problemas de disfunção têmporo-mandibular eram ocasionados pela má oclusão e, para sua correção, um tratamento ortodôntico deveria ser realizado. Estas teorias, aparentemente, tornaram-se obsoletas e, devido aos atuais artigos científicos mais consistentes, o relacionamento destas três entidades tem sido questionado. Esta revisão de literatura procura mostrar uma possível mudança de paradigma quanto ao relacionamento destas três entidades.

    The relationship between occlusion, Orthodontics and temporomandibular disorders have been subject of discussions and controversies. In the past, the scientific literature stated that the temporomandibular disorders occurred due to the malocclusion, and, for its correction, an orthodontic treatment should be performed. Apparently, those theories have become obsoletes, and according to more consistent scientific articles, the relationship among these entities have been questioned. This literature review shows a possible change in the paradigm of the relationship among the three entities.
  • Responsabilidade civil e ética do ortodontista Artigos Inéditos

    O que se observa atualmente é um aumento no processo de desenvolvimento social que abrange, praticamente, todas as áreas da ciência. Essa conscientização global assegura direitos bem definidos para toda a sociedade, bem como deveres para os profissionais que detêm o conhecimento científico. Em virtude disso, surge o aumento de conflitos entre profissionais de todas as áreas com seus clientes, assim como de dentistas para com seus pacientes. A preocupação desse estudo é apresentar aspectos relacionados à natureza legal e ética a que os ortodontistas estão submetidos, diariamente, em suas clínicas, mencionando aspectos pilares da responsabilidade civil do ordenamento jurídico atual, como também a postura ética que o ortodontista deverá ter com seu assistido.

    Considering the social development process, an increase that practically embraces all the areas of science is observed nowadays. This global consciousness properly assures defined rights to the society as a whole, as well as duties to the professionals who detain the scientific knowledge. Therefore, the increase of conflicts among professionals from all fields and their clients, and among the odontologists and their patients arouses. The purpose of this study is to show aspects related to the legal and ethical nature to which the orthodontists are daily submitted to, in their offices, mentioning the main aspects of the civil responsibility of the current juridical ordering, as well as the ethical posture that the orthodontist must have in relation to his patient.
  • A utilização da estatística na Ortodontia Artigos Inéditos

    A estatística exerce um papel fundamental no método científico, a qual se preocupa em organizar, descrever, analisar e interpretar os dados obtidos a partir de uma observação ou de um experimento. Todavia, ela continua sendo pouco acessível na interpretação, seja por parte do leitor ou por parte daquele que a utiliza. Às vezes, a estatística é mal aplicada ou não compreendida devido aos termos, com significado próprio. Além disso, existem poucos artigos científicos da área de Ortodontia relacionados com este assunto. Assim, o presente artigo propõe realizar um levantamento sobre as freqüências do uso da estatística, fazer considerações sobre a seleção dos seus métodos e explanar sobre as possíveis interpretações dos resultados.

    Statistics plays a fundamental role in scientific method, which aims to organize, to describe, to analyze and to interpret the data obtained from an observation or an experiment. However, it continues unavailable, in interpretation, by the readers as well as by those who employ it. Sometimes, statistics is mistakenly applied or comprehended due to some terms, which, statistically, present their own meanings. Additionally, there are few articles in Orthodontics that describe this subject. Thus, the present article aims to perform a survey about the frequencies of the use of statistics, to make considerations about the selection of its methods and to explain about the possible interpretations of the data.
  • Caminhos e descaminhos da Ortodontia no Brasil Artigos Inéditos

    Devido à constatação do aumento do número de ortodontistas e de Cursos de Ortodontia, delineou-se este estudo cujo objetivo foi sondar o futuro da especialidade. Avaliaram-se formulários respondidos por 413 alunos e 130 professores de 42 Cursos de Especialização dos 73 relacionados no Catálogo da Associação Brasileira de Ortodontia e Ortopedia Facial (ABOR) 1999/2000. Obteve-se uma taxa de resposta de 57,53%. A amostra foi dividida em: alunos e professores e nas regiões: Sul + Sudeste e Centro-Oeste + Nordeste. A mediana de idade do pós-graduando foi 30,8 e a do professor 42,8 anos. O índice preconizado pela Organização Mundial de Saúde é de 1 CD para 1.500 habitantes. O Brasil com uma população de 175 milhões e 175.637 dentistas, apresenta 1 CD para 996 habitantes. O Reino Unido apresenta 1/2.000, a França 1/1.519, e a Itália 1/1.333 (1998). Com população 40% e renda 82% menor do que os EUA, o Brasil possui quase o triplo de cursos de Ortodontia (n=124, 2001), graduando 344 novos ortodontistas/ ano. Considerando-se os 175.637 dentistas, o número de especialistas ainda é pequeno no Brasil. Para a maioria dos entrevistados, o mercado de trabalho foi considerado pessimista/saturado (58,7%) devido aos cursos não oficiais (85,1%) e à concorrência por clínicos-gerais (63,4%). Medidas proibitivtas de cursos não regulamentados foram desejáveis pela maior parte (88,3%). Os honorários foram considerados insuficientes para 44,8% e satisfatórios para 47,3%. O incremento da renda é feito com a clínica-geral (58,2%) pelos alunos e com as atividades docentes (73,3%) pelos professores. A maioria não estimularia seu filho a estudar Odontologia hoje (66,9%) e a principal fonte de indicação provém dos pacientes (89,3%). Outro notável achado foi que as documentações intermediárias e finais não estão sendo realizadas por 82,8% dos ortodontistas.

    Due to the increase in the number of orthodontists and Graduate Programs, this study was outlined aiming to the future of our Speciality. Questionaires were sent to 413 students and 130 teachers in 42 Graduate Programs. Respondents achieved 57.53%. The sample included respondents from several areas in Brazil. The average age of the students was 30.8 and the teachers' 42,8. The average index suggested by the Health World Organization is 1 dentist for each 1.500 habitants. Brazil has a population of 175 millions people and has 175 thousand dentists, one for each 996 inhabitants. This proportion in England is 1/2.000, in France 1/1.519, in Italy 1/1.333. With a populacion 40% smaller and income 82% smaller than the USA, Brazil holds almost three times the amount of Orthodontic Courses (124, 2001), graduating 344 ortodontists/ year. Considering the 175.637 dentists, the specialists number still is little in Brazil. Among orthodontists, 58.5% consider the professional market satureted due to non official courses (85,1%) and to the competition with general practioners (63,4%). Besides, 88,3% of the orthodontists would favor rules to inhibit irregular courses. The increase of their incomes comes from the general practice by the students (58,2%) while 73,3% of the teachers raise their income through teaching activities. The fees are considered insuficient by 44.85% of the orthodontists and satisfactory by 47.3% of them. Nowadays, 66.9% of these specialists would not encourage their children to study Dentristy. The main referral source comes from patients (89.3%). Another rmarkable issue is that intermediate and final records have not being taken by 82.8% of the orthodontists.
  • Princípios biomecânicos do aparelho extrabucal Artigos Inéditos

    A utilização do aparelho extrabucal no tratamento da má oclusão de Classe II de Angle, exige que os princípios biomecânicos sejam entendidos e aplicados para que o caso clínico seja conduzido de maneira que os efeitos colaterais sejam minimizados e os benéficos maximizados. Para tanto, o ortodontista deve ser capaz de prever os efeitos da inclinação da linha de ação de força e sua relação com os centros de resistência da maxila e dos dentes. Quaisquer movimentos de rotação, inclinação ou translação dos dentes, bem como rotação horária ou anti-horária da maxila e da mandíbula, podem ser previstos e planejados quando o ortodontista conhece os mecanismos que os produzem, e também os efeitos que os diferentes tipos de tração produzem em indivíduos dolicofacial, mesofacial ou braquifacial. Assim, o intuito desse trabalho é apresentar fatores preponderantes e imprescindíveis para a utilização do aparelho extrabucal no tratamento da má oclusão de Classe II de Angle.

    The use of the headgear appliance for the treatment of Angle Class II malocclusion demands that the biomechanic principles are understood and applied so that the clinic case is conducted in a way that the side effects are minimized and the benefits maximized. Therefore, the orthodontist must be able to predict the effects of the inclination of the line of traction and its relation to the teeth and maxillary centers of resistance. Any rotation movement, inclination or translation of the teeth and maxillary and mandibular rotation clockwise or counter clockwise can be predict and planned when the orthodontist knows the mechanism that cause them, as well as the effects that the different types of traction produce in subjects with dolichofacial, mesofacial or brachyfacial types.
