  • Fatores de risco para as infecções relacionadas ao caracter venoso central em terapia intensiva pediátrica Clinical Sciences

    OBJETIVOS: Identificar fatores de risco para as infecções relacionadas a cateter venoso central de curta permanência, inserido por punção, em crianças e avaliar a eficiência de um escore de mortalidade pediátrica em prever o risco de infecção. MÉTODOS: Revisão dos casos de infecção relacionada a cateter ocorridos na unidade de terapia intensiva pediátrica de um hospital universitário seguida de estudo caso-controle com 51 pares. Foram analisadas variáveis relacionadas aos pacientes e à inserção e utilização dos cateteres, sendo definidos fatores de risco por análise de regressão logística. A eficiência de Pediatric Risk of Mortality em discriminar o risco de infecção foi testada pela curva receiver operating characteristic. RESULTADOS: Foram variáveis associadas à infecção: insuficiência respiratória, duração da internação, tempo de intubação, inserção do cateter na unidade de terapia intensiva e nutrição parentérica. O sítio de inserção foi indiferente quando comparadas as veias jugular interna e femoral. Foram fatores de risco: inserção de mais de um cateter (p=0,014) e tempo de permanência do cateter (p=0,0013). Foram fatores de proteção: uso concomitante de antibióticos (p=0,0005) e infusão intermitente seguida de heparinização quando comparada à infusão contínua sem heparinização (p=0,0002). Pediatric Risk of Mortality não discriminou o risco de infecção. CONCLUSÕES: Deve-se suspender a nutrição parentérica e retirar o cateter venoso central assim que possível. A cateterização da veia femoral implica em risco de infecção semelhante ao da veia jugular interna. O escore Pediatric Risk of Mortality não deve ser utilizado para estimar o risco de infecção relacionada ao cateter venoso central.

    OBJECTIVES: To identify risk factors for short-term percutaneously inserted central venous catheter-related infections in children and to evaluate the accuracy of a mortality score in predicting the risk of infection. METHOD: After reviewing the charts of patients who developed catheter-related infection in a university hospital's pediatric intensive care unit, we conducted a case-controlled study with 51 pairs. Variables related to patients and to catheter insertion and use were analyzed. Risk factors were defined by logistic regression analysis. The accuracy of the Pediatric Risk of Mortality score to discriminate the risk for infection was tested using the Receiver Operating Characteristic curve. RESULTS: Infection was associated with respiratory failure, patient's length of stay, duration of tracheal intubation, insertion of catheter in the intensive care unit and parenteral nutrition. Insertion site (femoral or internal jugular) was unimportant. Multivariate logistic regression analysis identified the following variables. Risk factors included more than one catheter placement (p=0.014) and duration of catheter use (p=0.0013), and protective factors included concomitant antibiotic use (p=0.0005) and an intermittent infusion regimen followed by heparin filling compared to continuous infusion without heparin (p=0.0002). Pediatric Risk of Mortality did not discriminate the risk of infection. CONCLUSIONS: Central parenteral nutrition and central venous catheters should be withdrawn as soon as possible. Femoral vein catheterization carries a risk of infection similar to internal jugular catheterization. The Pediatric Risk of Mortality score should not be used to predict the risk of central catheter-related infections.
  • Impacto do delirium sobre mortalidade e desempenhos cognitivo e funcional em idosos com fratura de fêmur Clinical Sciences

    OBJETIVO: Avaliar efeito do delirium na mortalidade e na evolução dos desempenhos cognitivo e funcional em idosos com fratura de fêmur, 4 anos após a alta hospitalar. ESTUDO: coorte, prospectivo. LOCAL: Instituto de Ortopedia e Traumatologia do HC-FMUSP; Enfermaria de Ortopedia Geriátrica. PACIENTES: 103 pacientes com 65 anos ou mais, consecutivos, internados em 2001-2002, na enfermaria de ortopedia geriátrica, por fratura de fêmur. 30 idosos (29,1%) apresentaram delirium durante a internação e foram comparados com os 73 que evoluíram sem delirium (30 casos x 73 controles). Houve 6 óbitos, 97 receberam alta hospitalar. O estudo atual mostra reavaliação de 85 desses pacientes. MÉTODOS: Dentre os 85 pacientes, temos 43 óbitos e 42 sobreviventes. Foram obtidos dados de atividades básicas de vida diária (ADL), atividades instrumentais de vida diária ( IADL), desempenho cognitivo (Blessed), referentes aos sobreviventes, através de entrevista telefônica com os mesmos cuidadores que forneceram as informações durante a internação . Comparamos esses dados com aqueles de 4 anos atrás (42 pacientes). Dos pacientes que foram a óbito, obtivemos data e causa de óbito. Dados referentes aos 85 pacientes entraram na análise de mortalidade. RESULTADOS: Não foram observadas relações entre delirium e mortalidade, delirium e perda cognitiva, delirium e perda funcional. Déficit cognitivo inicial foi preditor de mortalidade (RR= 2,54 ; p=0,016), perda funcional (OR=1,80; p=0,027) e perda cognitiva(OR=1,53; p=0,024). CONCLUSÕES: Delirium não teve impacto sobre mortalidade e perdas funcional e cognitiva na evolução tardia de idosos com fratura de fêmur. O déficit cognitivo inicial pode identificar pacientes em risco para mortalidade, perda funcional e perda cognitiva futuras em idosos com fratura de fêmur. A fragilidade e heterogeneidade da nossa amostra pode ter atenuado o poder preditor de mau prognóstico do delirium.

    OBJECTIVE: To evaluate the evolution of cognitive and functional performance and mortality among elderly patients who were delirious during hospitalization due to femoral fracture. STUDY TYPE: Prospective cohort. LOCATION: Orthopedics and Traumatology Institute of HC-FMUSP; geriatric orthopedic ward. PATIENTS: 103 patients, aged 60 years or over, who were hospitalized in the geriatric orthopedics ward with femoral fracture in 2001-2002. Thirty of them (29.1%) presented with delirium during their hospital stay and were compared with another 73 (70.9%) who did not present with delirium. There were six deaths, and 97 patients were discharged from the hospital. We obtained information on 85 of these patients four years after discharge; 42 patients were still alive and 43 had died at the time of the evaluation. METHODS: Data on vital status was obtained for 85 patients. For the 42 survivors, we acquired information on their basic activities of daily living (ADL), instrumental activities of daily living (IADL), and cognitive performance (BDRS) by means of telephone interview with the same caregivers who had provided information at the time of the hospitalization. We compared this data with that obtained during their hospitalizations four years prior. For the 43 patients who died, we obtained information regarding their deaths and used this data in the analysis of mortality. RESULTS: No relationships were observed between delirium and mortality, delirium and cognitive loss, or delirium and functional loss, after four years from discharge of elderly patients with hip fractures. An initial cognitive deficit was a predictor for mortality (RR = 2.54; p = 0.016), functional loss (OR = 1.80; p = 0.027) and cognitive loss (OR = 1.53; p = 0.024). Cognitive loss was also related to age. CONCLUSIONS: Delirium had no impact on mortality or functional or cognitive losses in long term evolution (2 years) among elderly patients with femoral fractures. An initial cognitive impairment may identify patients at risk of mortality, functional and cognitive losses over the long term evolution.
  • Efeitos da terapia de reposição hormonal estroprogestativa sobre o sistema de coagulação e de fibrinólise em mulheres na pós-menopausa Clinical Sciences

    OBJETIVO: Avaliar os marcadores antitrombina III (AT), fragmento 1 + 2 da trombina (F1+2) e complexo trombina-antitrombina (TAT), bem como outros parâmetros da coagulação, como tempo de pró-trombina, tempo parcial de tromboplastina ativado, tempo de trombina, fibrinogênio e tempo de lise da euglobulina em mulheres na pós-menopausa após terapia hormonal. DESENHO DO ESTUDO: Foram incluídas 45 voluntárias que receberam estrogênios conjugados eqüinos (ECE) 0,625 mg/dia, isoladamente ou associado ao acetato de medroxiprogesterona (AMP) ou usaram o 17beta-estradiol (50 µg/dia) transdérmico com AMP. Os exames foram realizados antes do tratamento (T0) e após três (T3), seis (T6) e doze (T12) meses após o início do tratamento. AT foi avaliada pelo método amidolítico, enquanto que o F1+2 e o complexo TAT por ELISA. RESULTADOS: Houve redução significante nos níveis de AT em pacientes que receberam ECE associado ao AMP no T3. Não houve redução na AT em mulheres que usaram ECE isoladamente ou aquelas com 17beta-estradiol transdérmico e AMP. O F1+2 aumentou em todos os grupos, mas apenas o grupo com 17beta-estradiol transdérmico e AMP apresentou diferença significante durante o T3. CONCLUSÕES: A associação de ECE e AMP pode reduzir os níveis de AT, enquanto ECE isoladamente ou 17beta-estradiol transdérmico com AMP não modificam-o acentuadamente. Essas alterações poderiam ser mais relevantes clinicamente na análise populacional. Todavia, a terapia de reposição hormonal aumentaria o risco de trombose em mulheres com trombofilia prévia congênita ou adquirida.

    OBJECTIVE: To evaluate antithrombin III (AT), thrombin (Fragment 1+2 [F1+2] and thrombin-antithrombin [TAT]) generation markers, as well as other coagulation parameters, such as prothrombin time, partial activated thromboplastin time, thrombin time, fibrinogen, euglobulin lysis time, and platelet count, in postmenopausal women after hormonal therapy. STUDY DESIGN: Forty-five patients who received either 0.625 mg/day unopposed oral conjugated equine estrogen (CEE), 0.625 mg/day oral CEE plus medroxyprogesterone acetate (MP), or 50 µg/day transdermal 17beta-estradiol plus MP, were included. Tests were performed before (T0) and after 3 (T3), 6 (T6) and 12 (T12) months of treatment. AT was determined by an amidolytic method, whereas F1+2 and TAT complex were measured by ELISA. RESULTS: There was a significant reduction in the AT level of patients who received oral CEE plus MP at T3. There was no AT reduction in patients taking either oral CEE alone or transdermal 17beta-estradiol plus MP. F1+2 increased in all patients, but it reached statistical significance only in patients receiving transdermal 17beta-estradiol MP at T3. CONCLUSIONS: The CEE associated with MP treatment may reduce AT levels, whereas unopposed CEE or transdermal 17beta-estradiol plus MP does not change AT. These changes might not be clinically relevant in the general population; however, hormonal replacement therapy may increase the risk of thrombosis in women with congenital or acquired thrombophilia.
  • Queixas de ardência bucal: características clínicas de amostra brasileira Clinical Sciences

    OBJETIVO: Avaliar as características clínicas de pacientes brasileiros com queixas de ardência buccal atendidos em um hospital escola. MÉTODO: 66 pacientes com queixas de ardência bucal foram avaliados através de exame padronizado para esse tipo de queixa. RESULTADOS: 56 mulheres e 10 homens foram examinados consecutivamente. As idades variaram de 35-83 anos. A localização das queixas foi principalmente na língua e 36 pacientes relataram algum evento precipitante. A Escala Visual Analógica (EVA) a intensidade da ardência (dor) foi: mulheres 7.5 (média) e homens 6.11 (média). Os níveis de estradiol foram baixos (<13 pg/ml); 80% dos pacientes relataram doença crônica associada, 55% usavam dentadura; 54% relataram xerostomia subjetiva; 48% distúrbios subjetivos do sono e 66% gosto fantasma. Não houve diferença da intensidade da EVA (p=0.139) ou dor pelo questionário McGill NWC (p=0.259) and PRI (p=0.276), entre os grupos com e sem eventos precipitantes. CONCLUSÕES: A existência de doenças crônicas associadas, o auto-relato de distúrbios do sono e as alterações de paladar indicam necessidade de avaliação sistêmica cuidadosa nesses pacientes; não houve diferenças entre os grupos com e sem evento precipitante.

    OBJECTIVE: Evaluation of the clinical characteristics of burning mouth complaints (BMC) in a series of Brazilian patients referred to a large teaching hospital. MATERIALS AND METHODS: 66 patients with burning mouth complaints were evaluated through a standardized protocol. RESULTS: 56 women and 10 men were examined, ranging in age from 35-83 years. The primary location of the complaints was reported to be the tongue. Thirty-six patients reported a precipitating event. The mean VAS pain levels were 7.5 in women and 6.11 in men. The average estradiol levels in women were low (<13 pg/ml); 80% of all patients reported a concomitant chronic disease, 55% of all patients wore total dentures, 54% of all patients reported subjective xerostomia, 48% of all patients reported sleep disturbances and 66% reported phantom taste. No statistical differences were found between groups with or without a precipitating event in VAS: (p=0.139), in the Number of Words Chosen (NWC) (p=0.259) and Pain Rating Index (PRI) (p=0.276) sections of the McGill Pain Questionnaire (MPQ). CONCLUSION: The existence of systemic comorbidities, self-reported sleep disturbances and taste alterations indicates possible correlations and the need for a careful systemic evaluation of each patient; there were no differences between patients with and without precipitating events.
  • Avaliação da atuação dos farmacêuticos na prestação da assistência a saúde em um hospital universitário Clinical Sciences

    OBEJTIVO: O propósito deste estudo foi avaliar as práticas dos farmacêuticos em um hospital, na realização da Assistência Hospitalar. MÉTODO: foram entrevistados 20 farmacêuticos da Divisão de Farmácia, sendo aplicado um instrumento estruturado, em setembro de 2005. Esse instrumento abordou aspectos relativos às principais atividades da Farmácia Hospitalar, que foram avaliadas segundo indicadores organizados em cinco áreas: gerenciamento das unidades, farmacotécnica hospitalar, atividades em comissões, informação e seguimento Farmacoterapêutico, e, atividades de ensino e pesquisa. RESULTADOS: A Divisão de Farmácia contemplou todos os aspectos estruturais considerados como essenciais para o bom desenvolvimento e aplicação de seus serviços. Verificou-se a ausência de alguns serviços essenciais como o Serviço de Informação de Medicamentos e de Seguimento Farmacoterapêutico. Os profissionais farmacêuticos encontravam-se insatisfeitos quanto ao dimensionamento dos recursos humanos e à estrutura física, e se demonstram pouco atuantes em relação à Assistência Farmacêutica. CONCLUSÃO: Os resultados indicaram uma atenção ainda centralizada no medicamento, com poucas atividades clínicas. Sugerem-se reformulações no gerenciamento dos serviços com ênfase na administração dos farmacêuticos.

    OBJETIVE: This study aimed to assess the practices of pharmacists in Hospital Care. Method - we interviewed 20 pharmacists from the Pharmacy Division by applying a structured instrument, in September 2005. This instrument addressed aspects related to the main activities at the Hospital Pharmacy, which were assessed according to indicators organized into five areas: sector management, hospital pharmacotechniques, committee activities, information and pharmacotherapeutic follow-up, as well as teaching and research activities. RESULTS: The Pharmacy Division considered all structural aspects under analysis as essential for the good development and application of its services. We found that some essential services, such as the Medication Information Service and Pharmacotherapeutic Follow-up, were absent. Pharmacist professionals were dissatisfied about human resource and physical structure dimensioning, and they presented as not very active in terms of Pharmaceutical Care. CONCLUSION: Results indicate that care is still centered on the drug, with few clinical activities. We suggest reformulations in service management, particularly in the management of pharmacists.
  • Qualidade de vida de pacientes com degeneração macular relacionada à idade com cegueira legal monocular e binocular Clinical Sciences

    OBJETIVO: Avaliar a qualidade de vida de portadores de degeneração macular relacionada à idade com cegueira legal monocular e binocular. MÉTODOS: Foi realizado estudo transversal analítico por meio do questionário National Eye Institute Visual Functioning Questionnaire (NEI VFQ-25). Os critérios de inclusão foram: indivíduos de ambos os sexos, idade maior que 50 anos, ausência de catarata, diagnóstico de degeneração macular relacionada à idade avançada em pelo menos um dos olhos, sem outras maculopatias. O Grupo Controle foi pareado por sexo, idade e sem doença ocular. RESULTADOS: O Grupo 1 (cegueira monocular) foi composto por 54 pacientes (72,22% de mulheres e 27,78% de homens, idade entre 51 e 87 anos, média de 74,61 anos ± 7,27 anos); o Grupo 2 (cegueira binocular), por 54 (46,30% de mulheres e 53,70% de homens, idade entre 54 e 87 anos, média de 75,61 anos ± 6,34 anos). O Grupo Controle foi composto por 40 pacientes (40% de mulheres e 60% de homens, idade entre 50 e 81 anos, média de 65,65 anos ± 7,56 anos). A maioria dos escores foi significativamente maior no Grupo 1 e no Controle em relação ao Grupo 2 e maior no Controle comparado ao Grupo 1. CONCLUSÕES: Evidenciou-se que a qualidade de vida de indivíduos com cegueira binocular foi mais limitada em relação aos portadores de cegueira monocular. Quando comparados com indivíduos normais, ambos os grupos tiveram prejuízo na qualidade de vida foi significativo.

    OBJECTIVE: To evaluate the quality of life for persons affected by age-related macular degeneration that results in monocular or binocular legal blindness. METHODS: An analytic transversal study using the National Eye Institute Visual Functioning Questionnaire (NEI VFQ-25) was performed. Inclusion criteria were persons of both genders, aged more than 50 years old, absence of cataracts, diagnosis of age-related monocular degeneration in at least one eye and the absence of other macular diseases. The control group was paired by sex, age and no ocular disease. RESULTS: Group 1 (monocular legal blindness) was composed of 54 patients (72.22% females and 27.78% males, aged 51 to 87 years old, medium age 74.61 ± 7.27 years); group 2 (binocular legal blindness ) was composed of 54 patients (46.30% females and 53.70% males aged 54 to 87 years old, medium age 75.61 ± 6.34 years). The control group was composed of 40 patients (40% females and 60% males, aged 50 to 81 years old, medium age 65.65 ± 7.56 years). The majority of the scores were statistically significantly higher in group 1 and the control group in relation to group 2 and higher in the control group when compared to group 1. CONCLUSIONS: It was evident that the quality of life of persons with binocular blindness was more limited in relation to persons with monocular blindness. Both groups showed significant impairment in quality of life when compared to normal persons.
  • Atraso na introdução de terapia antiretroviral em pacientes infectados pelo HIV no Brasil, 2003-2006 Clinical Sciences

    OBJETIVO: Caracterizar a população de indivíduos infectados pelo HIV que inicia tardiamente a terapia com anti-retrovirais (TARV) no Brasil, utilizando informações do Sistema de Controle de Exames Laboratoriais. MÉTODOS: Foram analisados todos os indivíduos de 15 anos ou mais de idade que realizaram exame inicial para contagem de linfócitos T CD4+ para avaliação de indicação de tratamento entre os anos de 2003 e 2006, cuja data de início da terapia foi posterior ou igual à data de solicitação da contagem de células T CD4+ (84694 pacientes). Esses pacientes foram considerados como virgens de tratamento com anti-retrovirais. A distribuição da contagem inicial de linfócitos T CD4+ foi analisada por sexo, idade, Grande Região e ano de realização do exame. RESULTADOS: A maioria dos pacientes tinha entre 15 e 49 anos de idade (91%); 56% eram do sexo masculino; 76% assintomáticos; 50% residiam na Região Sudeste e 20% na Região Sul. A proporção de indivíduos cujo resultado do primeiro exame para contagem de linfócitos TCD4+ era inferior a 200 cel/mm³ foi de 33%. Somando-se a esses os indivíduos sintomáticos, o valor atinge 41% para a totalidade; 47% para os homens e 53% para os pacientes com mais de 50 anos de idade. CONCLUSÕES: Em que pese o acesso universal à TARV, no Brasil, os resultados mostram que uma alta proporção de pacientes inicia o tratamento em um estádio avançado da doença, apontando para a necessidade do estabelecimento de estratégias de diagnóstico precoce da infecção pelo HIV.

    PURPOSE: To characterize the population of HIV+ Brazilian patients with late introduction of antiretroviral therapy (ARVT), using information from the Laboratory Exam Control System. METHODS: The study analyzed 84,694 patients, representing all individuals in Brazil age 15 or over with an initial CD4+ T lymphocyte count requested between 2003 and 2006, and whose ARVT start date was later than their initial CD4+ T cell count. These patients were considered antiretroviral treatment naive. The initial CD4+ T cell distribution was analyzed according to sex, age, region and year. RESULTS: Most of the patients were between 15 and 49 years of age (91%); 56% were males; 76% were asymptomatic; 50% lived in the Southeastern region of the country, with an additional 20% in the South. Initial CD4+ counts for one-third of the patients were less than 200 cells/mm³. When combined with the number of symptomatic individuals, 41% of the total group was in need of immediate ARVT. This group included 47% of the men and 53% of the patients aged 50 years and over. CONCLUSIONS: Despite universal access to ARVT in Brazil, results show that a high proportion of patients initiate ARVT at an advanced stage of disease, indicating the need to develop strategies to promote early diagnosis of HIV infection nationwide.
  • Parâmetros clínicos e laboratoriais no diagnóstico diferencial de efusões pleurais secundárias à tuberculose ou ao cancer Clinical Sciences

    OBJETIVO: Avaliar as características clínicas e laboratoriais de derrames pleurais secundários à tuberculose ou câncer. MÉTODOS: Um total de 326 pacientes com derrame pleural por tuberculose (n=182) ou câncer (n=144) foi avaliado. Os seguintes parâmetros foram analisados: sexo e idade dos pacientes e características do líquido pleural (tamanho, localização, aspecto macroscópico, concentração de proteínas, atividade da desidrogenase lática (DHL) e da adenosina deaminase (ADA) e contagem de células nucleadas). RESULTADOS: A tuberculose pleural predominou nos pacientes mais jovens e do sexo masculino. Em ambos os grupos, os derrames pleurais foram de tamanho moderado e unilaterais. Derrames com aspecto amarelo-citrino com níveis mais elevados de proteínas predominaram na tuberculose (5,3 ± 0,8 g/dL), quando comparados aos neoplásicos (4,2 ± 1,0 g/dL), enquanto que níveis mais elevados de DHL foram observados nos derrames neoplásicos (1.177 ± 675 x 1.030 ± 788 UI; p = 0,003). Conforme esperado, a atividade da ADA foi maior na tuberculose que no câncer (107,6 ± 44,2 x 30,6 ± 57,5 U/L; p < 0,001). Ambos os derrames apresentaram alta celularidade, embora mais pronunciada no grupo neoplásico (p < 0,001). Os derrames de etiologia tuberculosa se caracterizaram por apresentar uma maior percentagem de leucócitos e de linfócitos (p < 0,001) e um pequeno número de células mesoteliais (p = 0,005). Linfócitos e macrófagos foram as células nucleadas que predominaram nos derrames pleurais malignos. CONCLUSÃO: Nossos resultados demonstram que em exsudatos pleurais linfocíticos de pacientes com evidências clínicas e radiológicas de tuberculose, os níveis de proteína e de ADA foram os parâmetros que melhor caracterizaram esses derrames. Da mesma maneira, quando a suspeita clínica é câncer, um líquido serohemorrágico e linfocítico deve ser submetido à citologia oncótica, uma vez que este exame laboratorial de fácil realização e baixo custo apresenta alto desempenho diagnóstico (<FONT FACE=Symbol>@</FONT> 80%). Neste contexto, sugerimos que a toracocentese, com exames bioquímicos e citológicos do líquido pleural, seja a primeira abordagem diagnóstica do paciente.

    PURPOSE: To evaluate the clinical and laboratory characteristics of pleural effusions secondary to tuberculosis (TB) or cancer (CA). METHODS: A total of 326 patients with pleural effusion due to TB (n=182) or CA (n=144) were studied. The following parameters were analyzed: patient gender, age and pleural effusion characteristics (size, location, macroscopic fluid aspect, protein concentration, lactate dehydrogenase (DHL) and adenosine deaminase activity (ADA) and nucleated cell counts). RESULTS: Young male patients predominated in the tuberculosis group. The effusions were generally moderate in size and unilateral in both groups. Yellow-citrine fluid with higher protein (p < 0.001) levels predominated in effusions from the tuberculosis group (5.3 + 0.8 g/dL) when compared to the CA group (4.2 ± 1.0 g/dL), whereas DHL levels were more elevated in CA (1,177 ± 675 x 1,030 ± 788 IU; p = 0.003) than in TB. As expected, ADA activity was higher in the TB group (107.6 ± 44.2 x 30.6 ± 57.5 U/L; p < 0.001). Both types of effusions presented with high nucleated cell counts, which were more pronounced in the malignant group (p < 0.001). TB effusion was characterized by a larger percentage of leukocytes and lymphocytes (p < 0.001) and a smaller number of mesothelial cells (p = 0.005). Lymphocytes and macrophages were the predominant nucleated cell in neoplastic effusions. CONCLUSION: Our results demonstrate that in lymphocytic pleural exudate obtained from patients with clinical and radiological evidence of tuberculosis, protein and ADA were the parameters that better characterize these effusions. In the same way, when the clinical suspicion is malignancy, serous-hemorrhagic lymphocytic fluid should be submitted to oncotic cytology once this easy and inexpensive exam reaches a high diagnostic performance (<FONT FACE=Symbol>@</FONT> 80%). In this context, we suggest thoracocentesis with fluid biochemical and cytological examination as the first diagnostic approach for these patients.
  • O "problema oligoanalgesia" no cuidado da emergência Clinical Sciences

    INTRODUÇÃO: A dor é um evento comum em vítimas de trauma com efeitos nocivos ao organismo, no entanto, há uma lacuna na literatura sobre essa problemática ainda sub-avaliada e sub-tratada em nosso meio, sobretudo na utilização de opióides. OBJETIVOS: Aferir a intensidade dolorosa e o uso da analgesia em vítimas de acidentes de transportes. MATERIAL E MÉTODO: Estudo prospectivo, envolvendo 100 vítimas de causas externas (acidentes de transporte), que foram entrevistadas em dois momentos distintos pós-trauma em um hospital de referência no Município de São Paulo.. Foram anotadas todas as medicações em uso para essas vítimas. Todos os pacientes tinham Escala de Coma de Gasglow (ECGl) = 15, parâmetros hemodinâmicos estáveis e vindos diretamente da cena do evento. RESULTADOS: A dor foi um fenômeno presente em 90% dos casos, sendo as dores de intensidade moderada e forte as mais encontradas. Após um período de três horas, uma significativa parte da população(48%) permanecia sem analgesia, mesmo na vigência de dor e o uso de opióides foi baixo. CONCLUSÃO: A dor é um fenômeno comum associada ao trauma com sub-tratamento e sub-avaliação em nosso meio e o uso de opióides para dores reconhecidamente muito intensas ainda é um recurso pouco utilizado no Serviço de Emergência, mesmo em pacientes hemodinamicamente estáveis e com Escala de Coma de Gasglow = 15.

    INTRODUCTION: Pain is a common occurrence in trauma victims that provokes harmful effects on the body. However, there is a gap in the literature about this problem, which is still underevaluated and undertreated in Brazil, especially concerning the use of opioids. OBJECTIVES: To estimate pain intensity and the use of analgesia in traffic accident victims. MATERIALS AND METHODS: A prospective study, involving 100 accident victims (traffic accidents), who were interviewed at 2 separate posttraumatic moments, in a reference hospital of the city of São Paulo. All the medications used for these victims were recorded. All patients displayed a Glasgow Coma Scale (ECGl) of 15, had stable hemodynamic parameters, and were brought directly from the scene of the accident. RESULTS: Pain of moderate and severe intensity (in 90% of cases) was the most noted. After a 3-hour period, a significant number of patients with pain (48%) continued without analgesia, and few opioids were used. CONCLUSION: Pain is a common event associated with trauma. It is still undertreated and underevaluated in Brazil, and the use of opioids for admittedly very severe pain is not frequently employed in the Emergency Service even in hemodynamically stable patients and with a Glasgow Coma Scale of 15.
  • Índice antropométrico para Pectus excavatum Clinical Sciences

    INTRODUÇÃO: O pectus excavatum (PEX) caracteriza-se por depressão do esterno em relação ao gradeado costal. Medidas clínicas e objetivas para classificar esse defeito são raras e de difícil aplicação. Este trabalho tem por objetivo criar um índice antropométrico para PEX (IA) como método diagnóstico e de avaliação pré e pós-operatória, comparando-o ao índice de Haller (IH) e ao índice vertebral inferior (IV). MÉTODOS: No período de dezembro de 2001 a fevereiro de 2004 foram estudados dois grupos de pacientes no Serviço de Cirurgia Torácica do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo (HC-FMUSP): a) 30 pacientes com a caixa torácica com configuração normal ao exame físico; b) 20 pacientes portadores de PEX. Estes últimos foram operados pela técnica de Ravitch modificada por Robicsek e pelo Serviço de Cirurgia Torácica do HC-FMUSP, sendo estudados no pós-operatório. Todos os pacientes foram avaliados a partir do IA (medidas clínicas), do IH (medidas tomográficas) e do IV (medidas radiográficas) no nível da maior deformidade (MD), no caso dos PEX e no terço distal do esterno (TD), nos normais. Os pacientes operados foram novamente medidos entre o 60º e o 80º dia do pós-operatório. RESULTADOS: Houve elevada correlação entre o IA e o IH (80% p< 0,001) e entre o IA e o IV (79% p< 0,001). A acurácia dos três índices foi similar, sendo que se estabeleceram os seguintes pontos de corte: IA= 0,12; IH= 3,10; e IV= 0,25. Ao ser analisado o pré-operatório, foi verificado para os três índices, que mais de 75% dos portadores de PEX encontravam-se acima dos pontos de corte e foram confirmados como portadores do defeito. No pós-operatório os valores dos índices encontrados abaixo do ponto de corte foram considerados dentro do padrão normal e isso ocorreu em 100% para o IA, em mais de 50% para o IH, e em 50% para o IV. CONCLUSÕES: O IA permitiu mensurar adequadamente o defeito, mantendo: a) alta correlação com o IH e o IV e elevada acurácia, semelhante à desses índices já consagrados; b) eficaz comparação entre o pré e pós-operatório.

    BACKGROUND: Pectus excavatum (PEX) is a depression of the sternum in relation to the costal cartilages. Clinical and objective measures for classifying the defect are rare and difficult to apply. The present study aimed to create an anthropometric index (AI) for PEX as a method for diagnosis and for preoperative and postoperative assessment by comparing it to the Haller index (HI) and to the lower vertebral index (LVI). METHODS: From December 2001 to February 2004, 2 groups of patients were studied at our institution: a) 30 patients with normal configuration of the thoracic cage, upon physical examination; b) 20 patients with PEX. The latter underwent surgery according to the Ravitch technique modified by Robicsek, and they were evaluated in the postoperative period. All patients were assessed by means of the AI (clinical), HI (tomographic), and LVI (radiographic) measures at the level of deepest deformity in the case of the PEX patients, and in the distal third of the sternum in the normal patients. The patients who had undergone surgery were once again measured between the 60th and the 80th postoperative days. RESULTS: There was a high correlation between the AI and the HI (80% P < .001) and between the AI and the LVI (79% P < .001). The accuracy of the 3 indices was similar, in that the following cut points were established: AI = 0.12, HI = 3.10, and LVI = 0.25. Upon analyzing the preoperative results, we verified that for the 3 indices, over 75% of the patients with pectus excavatum were above the cut points and were confirmed as having the defect. In the postoperative results, the value of the indices found below the cut point was considered within the normal standard, and this occurred in 100% for the AI, in over 50% for the HI, and in 50% for the LVI. CONCLUSIONS: The AI allowed adequate measurement of the defect, maintaining a) a high correlation with the HI and the LVI and a high accuracy, similar to the already acknowledged and published indices and b) an efficient comparison between the preoperative measurement and the postoperative results.
  • Efeitos da terapia de reposição hormonal estroprogestativa sobre o sistema de coagulação e de fibrinólise em mulheres na pós-menopausa Basic Research

    OBJETIVO: O alvo deste estudo foi analisar o efeito do sulfato de glicosamina no disco epifisário da tíbia em ratas ooforectomizadas. MÉTODOS: Após a ooforectomia (OVx), 28 ratas foram aleatoriamente divididas em 4 grupos experimentais de 7 animais cada, tratados da seguinte maneira: OVx 21 - veículo (0,5ml de NaCl 0.9% ip uma vez ao dia) por 21 dias; OVx GS21 230 - mg/kg peso corporal por dia de sulfato de glicosamina, 21 dias; OVx 45 - tratados com NaCl 0.9% igual ao grupo OVx 21, por 45 dias; e OVx GS45 - 230 mg/kg peso corporal por dia com sulfato de glicosamina, 45 dias. Sete animais intactos, na fase de proestro, foram usados como controle (CG). Ao completar o tratamento, os animais foram sacrificados e a articulação do joelho esquerdo foi dissecada e preparada para análise histológica. RESULTADOS: A porcentagem de cartilagem remanescente no novo osso do CG foi a menor. Os achados no grupo OVx GS45 foi significantemente menor do que no grupo OVx 21, OVx GS21 e OVx 45. A porcentagem de osso trabecular nos animais em pró-estro foi a maior. O grupo OVx GS45 mostrou valores maiores em relação aos outros grupos ooforectomizados. Esses resultados foram correspondentes aos achados em relação às células da zona proliferativa, desde que o CG teve os maiores valores e os valores do grupo OVx GS45 foram superiores aos dos grupos OVx 21, OVx GS21 e OVx 45. CONCLUSÃO: Nossos estudos sugerem que a glicosamina pode estimular o crescimento da cartilagem e do osso tibial após a ooforectomia em ratas.

    OBJECTIVE: The aim of this study was to analyze the effect of glucosamine sulfate on the tibial epiphyseal disk of the ovariectomized rats. METHODS: After ovariectomy (OVx), 28 female rats were randomly divided into 4 experimental groups with 7 animals each, treated as follows: OVx 21 - vehicle (NaCl 0.9%) 0.5 mL/day) for 21 days; OVx GS21 - 230 mg/kg/day glucosamine sulphate for 21 days; OVx 45 - treated with NaCl 0.9% as above for 45 days; and OVx - GS45230 mg/kg/day glucosamine sulphate for 45 days. Seven intact animals in the proestrous phase were used as controls (CG). Upon treatment completion, the animals were sacrificed and the left knee joint was dissected and prepared for histological analysis. RESULTS: The percentage of remaining cartilage in new bone of the CG; that found in THE OVx GS45 group was significantly less than that of the OVx 21, OVx GS21, and OVx 45 groups. The percentage of trabecular bone in proestrous animals was the highest. The OVx GS45 group showed higher values compared with the other ovariectomized groups. These results were paralleled by the findings regarding the cells of the proliferative zone, since the CG had the highest values, and the values of the OVx GS45 group were greater than those of the OVx 21, OVx GS21, and OVx 45 groups. CONCLUSION: Our studies suggested that glucosamine may stimulate tibial cartilage and bone growth after ovariectomy in rats.
  • Posicionamento do túnel femoral na reconstrução artroscópica do LCA: estudo comparativo de duas técnicas Basic Research

    OBJETIVO: Comparar a acurácia do posicionamento do túnel femoral na reconstrução do ligamento cruzado anterior através de duas vias: túnel tibial e portal ântero - medial. MÉTODO: Foram perfurados túneis femorais em vinte joelhos de cadáveres humanos por via artroscópica. Grupo I: túnel femoral por acesso trans túnel tibial. Grupo II: via portal ântero-medial. Quatro variáveis foram estudadas: A) espessura da parede posterior; B) posicionamento do túnel no intercôndilo; C) angulação do túnel em relação ao eixo do fêmur; D) distância entre a saída do fio guia e o epicôndilo lateral. RESULTADO: A) grupo I: 2,23 mm, grupo II: 2,36 mm (p=0,543); B) grupo I: 25,5º, grupo II: 30º (p=0,226); C) grupo I: 23,9º, grupo II: 32º (p= 0,014); D) grupo I: 7,8 cm, grupo II 3,9 cm (p<0,001). CONCLUSÃO: As duas técnicas obteveram o posicionamento desejado da entrada do túnel femoral e espessura adequada da cortical posterior. A perfuração via portal ântero-medial pode propiciar maior proteção contra rotura da parede posterior.

    OBJECTIVE: To compare the accuracy of positioning of the femoral tunnel in reconstructing the anterior cruciate ligament by means of 2 techniques: tibial tunnel and anteromedial portal. METHOD: Femoral tunnels were drilled in 20 knees from human cadavers by means of arthroscopy. Group I had the femoral tunnel drilled via a trans-tibial tunnel, and Group II via the anteromedial portal. Four variables were measured: A) posterior wall thickness; B) tunnel positioning at the notch; C) tunnel inclination in relation to the femoral axis; and D) distance between the wire guide exit and the lateral epicondyle. RESULTS: As above, respectively, A) 2.23 mm for group I and 2.36 mm for group II (P =.54); B) 25.5° for group I and 30.0° for group II (P =.23); C) 23.9° for group I and 32.0° for group II (P =.02); D) 7.8 cm for group I and 3.9 cm for group II (P <.001). CONCLUSIONS: Both techniques achieved the desired positioning for the femoral tunnel entrance and satisfactory thickness for the posterior cortex. Drilling via the anteromedial portal may provide greater protection against rupture of the posterior wall.
  • Avaliação experimental do comportamento cinemático tridimensional dos ligamentos cruzados Basic Research

    OBJETIVO: Este trabalho sugere uma técnica de baixo custo e de fácil reprodutibilidade para o estudo biomecânico em cadáveres. Neste tipo de estudo os efeitos das cargas naturais na articulação não são estudados. O objetivo é descrever a deformação plástica dos ligamentos no espaço tridimensional. MÉTODO: 18 joelhos provenientes de cadáveres humanos tiveram seus ligamentos cruzados divididos em três feixes de fibras cada e suas respectivas fixações tibiais e femorais marcadas, quando foram submetidos a duas exposições radiográficas perpendiculares para podermos obter uma projeção dos ligamentos nos três planos anatômicos (frontal, sagital e transversal). Através das medidas feitas nas radiografias nós obtivemos a média da distância entre os pontos de fixação na tíbia e no fêmur em quatro diferentes graus de flexão. RESULTADOS: Analisando os dados nós observamos que a distância entre os pontos de fixação dos feixes medial e lateral de ambos os ligamentos cruzados não alterou de forma significativa durante o movimento. Contudo, houve variação significativa (p>0.05) na distância entre os pontos de fixação do feixe posterior do ligamento cruzado anterior e do feixe anterior do ligamento cruzado posterior. CONCLUSÕES: A técnica foi eficiente ao demonstrar a deformação plástica dos ligamentos cruzados. Os resultados provenientes deste tipo de estudo podem auxiliar no planejamento de programas de reabilitação física.

    PURPOSE: The purpose of this study was to evaluate a low-cost and easily reproducible technique for biomechanical studies in cadavers. In this kind of study, the natural effect of loading of the joint and shear forces are not taken into account. The objective is to describe the plastic deformation of the ligaments into 3-dimensional space. METHOD: For 18 intact human cadaver knees, the cruciate ligaments were divided into 3 fiber bundles, the tibial or femoral fixation points were marked, and 2 perpendicular different x-ray exposures were performed, thus obtaining radiographs of spatial projections of the bundle in 3 anatomic planes (frontal, sagittal, and transversal). From the measurements made on the x-ray films, we obtained the average distance between the 2 fixation points of the cruciate ligaments on the tibia and the femur at 4 different flexion angles. RESULTS: The distance between the fixation points of the medial and lateral fiber bundles of the cruciate ligaments did not change significantly during movement. There were, however, significant variations (P < .05) in the distance between the fixation points of the posterior fiber bundles of the anterior cruciate ligament and the anterior fiber bundles of the posterior cruciate ligament. CONCLUSIONS: This technique was efficient for demonstrating the plastic deformability of the cruciate ligaments. The results proceeding from this type of study can assist in the planning of physical rehabilitation programs.
  • Repercussões clínicas e inflamatórias de pleurodese induzida pela administração intrapleural de talco Review

    Apesar dos relatos sobre pleurodese remontarem ao início do século XX, ainda hoje se busca o agente esclerosante ideal. Diversos agentes foram estudados e utilizados, mas o talco é considerado o mais popular. No entanto, efeitos sistêmicos potencialmente tóxicos tem sido associados à pleurodese pelo talco. Neste artigo discutimos os prováveis mecanismos de inflamação pleural e pleurodese, com ênfase na resposta sistêmica produzida pela instilação intrapleural de talco.

    Although reports on pleurodesis date back to the beginning of the 20th century, the search for the ideal sclerosing agent is ongoing. Several agents have been studied and used, but talc continues to be the most popular. However, potentially harmful systemic side effects have been associated with talc pleurodesis. In this article we discuss the likely mechanisms of pleural inflammation and pleurodesis with emphasis on the systemic response due to the instillation of talc into the pleural space.
