Open-access The expenses rationalization in assistance planning: a discursive analysis of the ordinances MPAS 3,046/82 and GM/MS 1,101/02

A discursive analysis of the Ordinances 3.046/82 and 1.101/02 of the Ministry of Health is presented in this article. The discourse of cost rationalization and its effects — the emphasis on the principles of decentralization, focus and privatization that drives the neoliberal way of health systems organization and the efficient use of financial resources — were the findings. In addition, it was also found that the main condition for production/reproduction of this discourse is given by the economic policy of fiscal adjustment. Finally, one tension was identified: the costs rationalization is directly related to the residual and meritocratic models of social protection, and the same documents convey the discourse of health that relates to the institutional-redistributive model of social policies.
