Abstract in Portuguese:

RESUMO As políticas públicas de Estado e Governo na saúde são abordadas sob a percepção de como o poder Executivo vem assumindo sua responsabilidade constitucional de formular e realizar estratégias implementadoras do Sistema Único de Saúde desde 1990: as congruentes e as incongruentes com os princípios e as diretrizes constitucionais da seguridade social e da saúde. A prática cotidiana desse contraditório e sua resultante sugere a possibilidade real da vigência de outra política pública de saúde, ‘compensatória’ aos 78% da população, não-afiliada aos planos e seguros privados de saúde e complementar aos 22% afiliados, com acessibilidade estratificada segundo os vários segmentos sociais. São também reconhecidos insuficiências e equívocos na implementação das estratégias por parte dos entes federados (gestores e controle social), do movimento da Reforma Sanitária e das próprias entidades e movimentos sociais quanto à politização. Ao final, são apresentadas recomendações com base na ampliação do debate político, envolvendo os poderes Executivo, Legislativo e Judiciário, o Ministério Público e as entidades e movimentos da sociedade civil. Este texto objetiva consolidar textos anteriores de contribuição ao II Simpósio Nacional do Centro Brasileiro de Estudos de Saúde, à Agenda Estratégica para a Saúde no Brasil e à preparação da 14° Conferência Nacional de Saúde.

ABSTRACT The State and Government health public policies are approached under the perception of how the Executive has been assuming its constitutional responsibility of forming and carrying out strategies that implement the Brazilian National Health System since 1990: the consistent and inconsistent ones with the constitutional principles and guidelines of social security and of health. The daily practice of this contradictory and its result suggest a real possibility of force of another health public policy, which is ‘equalized’ to the 78% of people that are not affiliated in health insurance private plans and to complement the 22% that are affiliated, with a stratified accessibility, according to several social segments. Insufficiencies and mistakes are also recognized in the implementation of strategies from federated states (managers and social control), from the Sanitary Reform movement and from entities and social movements as to make politics. Finally, recommendations based on enlargement of the political discussion have also been presented, involving Executive, Legislative, and Legal, the Public Prosecution Service, and civil society bodies and movements. This text aims at improving previous texts to contribute for the II National Symposium of CEBES, to the Strategic Health Agenda for Brazil and for the elaboration of the 14th Health National Conference.

RESUMO São examinados aspectos normativos dos espaços participativos do Sistema Único de Saúde (SUS), discutindo os sentidos da deliberação política. O objetivo é investigar a natureza política da atuação deliberativa das instâncias participativas da política de saúde. Foram identificadas fontes normativas da constituição e operacionalização desses espaços, as quais foram analisadas com referência em três autores da corrente deliberacionista: Jürgen Habermas, Joshua Cohen e James Bohman. Divergências nos sentidos de deliberação coletiva entre as normas da constituição e atuação dos espaços participativos do SUS foram identificadas, prevalecendo a perspectiva habermasiana.

ABSTRACT This paper examines normative aspects of participatory spaces of the National Health System, discussing the meanings of political deliberation. The objective was to analyze the political nature of the performance of deliberative participatory models of health policy. Normative sources of the constitution and operation of these facilities were identified, which were analyzed with reference to three authors of the current of deliberative democracy: Jürgen Habermas, Joshua Cohen and James Bohman. Divergences in the senses of collective deliberation among the rules of the constitution and operation of participatory spaces in the National Health System were been identified, prevailing the habermasian perspective.

RESUMO O artigo analisa se o discurso de responsabilização por competição administrada se materializou no sistema sanitário brasileiro a partir de 1990. Para tanto, discute a responsabilização por competição administrada; as reformas no sistema sanitário; a responsabilização com base nas categorias de análise - contratualização, pluralidade de provedores, estrutura de regulação da burocracia e participação dos usuários na avaliação dos serviços. Finalmente conclui que a responsabilização ainda é um processo em construção no Sistema Único de Saúde (SUS). Há uma preferência para os aspectos que permitem uma abertura ao mercado em detrimento dos aspectos que se referem ao cidadão e à estrutura de regulação.

ABSTRACT The article analyzes if the accountability by managed competition has materialized itself in the brazilian sanitary system since 1990. For that, it approaches the accountability by managed competition; the reforms in the sanitary system; the accountability by managed competition based on the analysis categories – contractualization, plurality of providers, bureaucracy regulation structure, and user participation in the service rendering evaluation. Finally, it concludes that the accountability is still an ongoing process at the National Health System. There is a preference for the aspects that allow an opening to the market in detriment to the aspects that refer to the citizen and to the regulation structure.

RESUMO Com este ensaio, objetivou-se descrever e analisar a evolução do sistema público de saúde no Ceará, nas décadas de 1980 e 1990, organizando fatos históricos, conforme tipologia proposta por Andrade, em 2007, em eixos político, discursivo e paradigmático. Foram revisadas publicações de autores centrais na discussão da Reforma Sanitária e nos documentos oficiais. O Ceará é referência na implantação do Sistema Único de Saúde, com destaque para descentralização dos serviços e Atenção Primária. Concluiu-se que a via institucional foi uma experiência que gerou transformações positivas nos saberes e nas práticas de saúde, contrastando com o cenário natural e político-administrativo adverso.

ABSTRACT This essay aimed at describing and analyzing the evolution of the public health system in Ceará in the 1980s and 1990s, organizing historical facts in political, discursive and paradigmatic axes how it was proposed by Andrade, in 2007. We reviewed publications of the main authors that discussed health reform and official documents. Ceará is a reference in the implementation of the National Health System, especially by its health services decentralization and primary health care. We conclude that the institutional path was an experience that led to positive changes in knowledge and health practices, in contrast to the natural and political-administrative adversities.

RESUMO Trata-se de um estudo descritivo, qualitativo, com a finalidade de identificar e analisar as concepções que norteiam as práticas dos profissionais de uma equipe de Saúde da Família, em João Pessoa (PB), acerca da violência de gênero. O material empírico foi produzido por meio de entrevistas com profissionais, cujos discursos foram analisados segundo a proposta de Fiorin. Evidenciou-se a invisibilidade da violência como um problema de saúde no serviço, o que pode contribuir para o desenvolvimento de práticas profissionais sem potencial para a emancipação de gênero. Ressaltamos a necessidade de mudanças e reorientação dos processos de trabalho na atenção à mulher em situação de violência.

ABSTRACT This is a descriptive qualitative study in order to identify and analyze the concepts that guide the practices of a team of Family Health, in João Pessoa (PB), about gender violence. The empirical material was produced through interviews with professionals, whose speeches were analyzed according to the proposed by Fiorin. It was evidenced the invisibility of violence as a health problem in the service, which may contribute to the development of professional practices with no potential for gender emancipation. We emphasize the need for change and reorientation of work processes in attention to women in situations of violence.

RESUMO O acúmulo de responsabilidades femininas representa uma facilidade ao tardio cuidado com a saúde e favorece o surgimento de hipertensão e complicações associadas. Diante disso, objetivou-se caracterizar as mulheres hipertensas e com complicações associadas quanto aos aspectos sociodemográficos e clínico-epidemiológicos. Trata-se de uma pesquisa de natureza quantitativa, documental e transversal, realizada com 814 fichas de cadastro do Hiperdia. Percebeu-se frequência significativa de fatores de risco modificáveis. Sugere-se que esses fatores sejam mais bem abordados e com mais frequência pela Estratégia de Saúde da Família no sentido de prevenir novos agravos e complicações.

ABSTRACT Women’s responsibility accumulation makes easy the late health care and collaborates to the emergence of hypertension and associated complications. Given this, it was aimed to characterize the hypertensive women and with complications associated with regard to their socio-demographic, clinical and epidemiological aspects. This is a quantitative research, documentary and cross, made with 814 sheets of Hiperdia register. We noticed a significant incidence of modifiable risk factors. It is suggested that these factors are more and better addressed by the Family Health Strategy to prevent new injuries and complications.

RESUMO O objetivo é captar a percepção de diferentes atores sociais (cirurgiões-dentistas, usuários e gestores) sobre a introdução da Equipe de Saúde Bucal (ESB) na Estratégia de Saúde da Família (ESF) em um município do Rio Grande do Sul (RS). Trata-se de um estudo de hermenêutica-dialética sob abordagem qualitativa. Utilizou-se a entrevista semiestruturada para a coleta de dados. Os resultados mostram que as mudanças mais visíveis são a melhora da estrutura física e o aumento de atendimentos. Percebe-se a compreensão limitada sobre a ESF por parte dos usuários. Este estudo sugere que o modelo tradicional, voltado essencialmente para ações curativas, não foi superado com a implantação da ESF.

ABSTRACT The objective of the study is to capture the perception of different social actors (dentists, users and managers) on the introduction of oral health team in a Family Health Strategy (FHS) in a city of Rio Grande do Sul (RS). It is a hermeneutic-dialectic study with qualitative approach. It was used a semi-structured interview for data collection. The results show that the most noticeable changes are the improvement of the physical structure and the increased provision of clinical care. The users seem not to understand the FHS. This study suggests that the traditional model, which aims primarily at curative, was not overcome with the implementation of FHS.

RESUMO Este artigo teve por objetivo identificar as propostas de inclusão dos trabalhadores informais e desempregados nas políticas públicas, mediante análise dos relatórios finais das Conferências Nacionais de Saúde do Trabalhador. Após coleta e organização do material, analisamos as resoluções identificando aquelas que apresentam palavras ou termos que remetem a esses trabalhadores. Das 807 resoluções encontradas, 24 versavam sobre a garantia de acesso aos serviços de saúde, controle social, direitos trabalhistas e previdenciários aos trabalhadores informais e desempregados. Apesar de conquistas alcançadas, esforços ainda são necessários para efetivação das propostas e resoluções apresentadas.

ABSTRACT This article aims to identify the proposals for inclusion of informal workers and unemployed people in public policy through examination of the final reports of the National Conferences on Occupational Health. After collecting and organizing the documents, we analyzed the resolutions identifying those that have words or terms that refer to these workers. From 807 resolutions found, 24 were about ensuring access to health services, social control, rights and social security to informal workers and unemployed. Despite its achievements, efforts are still needed for execution of the proposals and resolutions presented.

RESUMO O texto objetiva refletir sobre o desafio de expressão do cuidado de Enfermagem no campo da saúde pública, buscando apresentar uma aproximação das práticas de Enfermagem neste campo de atuação e um breve histórico da construção do cuidado na área da Enfermagem. Sendo o cuidado o elemento estrutural na prática de Enfermagem, torna-se necessário desenvolver estudos que objetivem identificar, conhecer e analisar as práticas de cuidados que são realizadas pelos enfermeiros no campo da saúde pública, já que estudos publicados sobre os cuidados de Enfermagem são mais direcionados para o cuidado na perspectiva hospitalar.

ABSTRACT The text seeks to reflect on the challenge of expressing the care of nursing in the field of public healthcare, aiming at present proximity between Nursing practices in this field of operation and a brief summary of the development of care in the area of Nursing. As care is a structural element in the Nursing practice, it is necessary to develop studies that seek to identify, acknowledge and analyze the care practices that are being carried out by nurses in the field of public healthcare since the studies published on Nursing care are more focused on care in the hospital perspective.

RESUMO A subjetividade produzida no cotidiano no Centro de Atenção Psicossocial (CAPS) é o objeto em estudo. O estudo apresenta uma dimensão crítica e reflexiva dentro de uma abordagem qualitativa. Os participantes foram dez sujeitos, os quais eram usuários, familiares e trabalhadores de um CAPS de Fortaleza. Foram utilizadas para coleta de dados a entrevista semiestruturada e a observação sistemática. A análise dos dados teve como referência as categorias empíricas resultantes do cruzamento entre as diferentes informações e a articulação com o referencial teórico. Conforme os resultados evidenciam, coexistem diversos modos de atenção operando no mesmo espaço de produção do cuidado em saúde mental.

ABSTRACT The subjectivity produced everyday in CAPS is the object of the study, which presents a reflective and critical view within a qualitative approach. Participants were ten subjects, including users, relatives and employees of a CAPS of Fortaleza. In order to collect data, it was used: a semi-structured interviews and systematic observation. The data analysis had as reference the empirical categories resulting from crosses between the several information and links with the theoretical referential. As the results show, there are many coexisting modes of attention operating in the same area of production of mental health care.

RESUMO Os objetivos foram identificar os motivos que levam os profissionais de segurança pública usuários de álcool a não aderir integralmente ao tratamento e analisar as repercussões na vida desse profissional. Foi feito um estudo descritivo, qualitativo, com técnica de análise de conteúdo, sendo realizada entrevista semiestruturada, em hospital militar com 31 militares. Os motivos da não adesão foram: complicações físicas; depressão; relações de estresse no trabalho associadas a problemas de saúde; negação; estigma, preconceito e discriminação; influência da família; violência urbana; condições financeiras. Concluiu-se que a não adesão integral ao tratamento resulta de um quadro complexo de fatores pessoais, financeiros e relacionados ao trabalho.

ABSTRACT The aim was to identify the reasons why public safety officers who used alcohol do not fully adhere to treatment and to analyze the impact of this in the professional life. A qualitative descriptive study was done with technique of content analysis. Semi-structured interview was carried out in a military hospital with 31 soldiers. The reasons for noncompliance were: physical illnesses; depression; relationships of work stress associated with health problems; denial, stigma, prejudice and discrimination; family influence; urban violence; financial conditions. It was concluded that non-adherence to full treatment results from a complex framework of personal, financial and work-related problems.

RESUMO Este artigo objetivou analisar o significado de ser obeso e da cirurgia bariátrica na perspectiva dos ex-obesos mórbidos. Foi realizado por meio do Discurso do Sujeito Coletivo, com as seguintes categorias: ser obeso e cirurgia bariátrica. Ser obeso representa um corpo feio e limitado, doente, frágil e fracassado pelas frustrações dos diversos tratamentos. A cirurgia bariátrica representa um renascimento, retorno ao convívio social e não ser diferente das outras pessoas. Ainda há um longo caminho a ser percorrido para atingir a satisfação, porém a cirurgia foi um dos grandes passos para o alcance de uma mudança que representa um passado próximo e um futuro não muito distante.

ABSTRACT This article aims at analyzing the meaning of being obese and of the bariatric surgery from the perspective of morbidly ex-obese ones. It was accomplished through the discourse of the collective subject, with the following categories: being obese and bariatric surgery. Being obese represents an ugly and limited body, sick, fragile and failed by the frustrations of the various treatments. Bariatric surgery represents a rebirth, returning to social life, and not to be different from the others people. There is still a long way to go in order to achieve satisfaction, but it’s important to emphasize that the surgery was a major step to achieve a change that represents a near past and a not too far future.

RESUMO Esta pesquisa surgiu da necessidade do Grupo de Trabalho de Humanização em planejar melhor suas ações a partir de avaliação do grau de satisfação no trabalho dos profissionais de saúde do ambulatório do Pronto-Socorro Cardiológico Universitário de Pernambuco. Os dados deste estudo de caso foram coletados a partir de entrevista estruturada contendo os parâmetros de avaliação da satisfação propostos pela Política Nacional de Humanização. A análise foi realizada pelo programa Statistical Package for the Social Sciences (SPSS). Quanto aos principais resultados, identificaram-se falhas nos canais de comunicação, pouca oferta de Educação Permanente e críticas quanto à fragmentação do processo de trabalho, em que as ações não são conjuntas.

ABSTRACT This research arose from the need of the Humanization Working Group to better plan their actions, from assessing the degree of job satisfaction of health professionals at the ambulatory of University Cardiac Emergency of Pernambuco. Data were collected from a structured interview containing the parameters for the assessment of satisfaction offered by the National Policy of Humanization. The analysis was performed using the Statistical Package for the Social Sciences (SPSS) program. Concerning the main results, we identified gaps in communication channels, low provision of continuing education and criticism regarding the fragmentation of the working process, in which joint actions are not together.

RESUMO Objetivou-se descrever as interfaces da atenção à saúde bucal dos portadores de insuficiência renal crônica (IRC) no Sistema Único de Saúde com a rede de atenção à saúde. Trata-se de um estudo qualitativo, descritivo, tipo estudo de caso. A coleta de dados foi realizada em uma instituição pública hospitalar catarinense, que presta atendimento aos portadores de IRC e possui serviço odontológico ambulatorial. O fluxo de informações se restringe ao ambiente clínico-hospitalar, não havendo mecanismo de referência e contrarreferência com a atenção básica. A atenção e assistência à saúde bucal dos portadores de IRC são prestadas de modo desarticulado, fragmentado e necessitam ampliar suas interfaces com a rede.

ABSTRACT The objective was to describe the interface of oral health care for patients with chronic renal failure (CRF) with the network of health care, in the Brazilian Health System. This is a qualitative, descriptive case study. Data collection was performed within a public hospital, in a municipality of Santa Catarina State, that provides assistance to CRF patients and offers dental services. The information flow is restricted to the clinical setting, without the referral and counter-referral with primary health care services. The attention to oral health of CRF patients are also provided in a disjointed and fragmented way and still need to broaden their interfaces with the network of health care.
