Efficacy of fluoride gel in arresting active non-cavitated caries lesions: a randomized clinical trial

Abstract: This study evaluated the efficacy of fluoride gel in arresting active non-cavitated caries lesions in permanent teeth. This randomized, triple-blind, placebo-controlled clinical trial randomized 100 schoolchildren aged 10.7 ± 2.2 years to test treatment (1.23% acidulated phosphate fluoride [APF] gel) or control treatment (placebo gel) for 4–6 applications at weekly intervals. Data collection included the visible plaque index, gingival bleeding index, visible plaque accumulation on the occlusal surfaces, eruption stage, and dental caries. The association between group and lesion arrestment was assessed using logistic regression, and estimates were adjusted for plaque accumulation over the lesion at baseline, surface type, and tooth type. Models were fitted using generalized estimating equations for accounting for the clustering of data (i.e., the same individual contributed > 1 lesion). Ninety-eight children completed the study (48 fluoride and 50 placebo). When all dental surfaces were analyzed, the likelihood of lesion arrestment was similar between both groups (p > 0.05). A secondary analysis including only the occlusal lesions in molars showed that for teeth under eruption, lesions receiving the 1.23% APF gel were about 3-fold more likely to become arrested than lesions receiving the placebo gel (OR = 2.85; 95%CI = 1.23–6.61; p = 0.01). No significant difference was detected for molars with complete eruption (p > 0.05). The benefit of fluoride gel for arresting non-cavitated caries lesions could not be identified by clinical assessment in this short-term trial. Notwithstanding, when the cariogenic challenge was greater (as on the occlusal surfaces of erupting molars), 1.23% APF gel treatment was an important tool for caries control.

Abstract Controlled release systems can modify the release rate of drugs and direct them to specific sites of action, making them more effective and/or reducing the adverse effects. The objective of this study was investigated, poly(β-hydroxybutyrate) (PHB) and poly(ε-caprolactone) (PCL) nanospheres to improve the delivery of Simvastatin (SIM). Nanospheres were prepared by the emulsion/evaporation technique of the solvent, varying the amount of SIM added. The SIM quantification was performed using a validated high-performance liquid chromatography method. The average diameter and PDI of formulations without SIM were lower 250 nm and 0.3, respectively. Nanospheres containing 30% of SIM showed values of 265 nm and 0.09, respectively. The average zeta potential was -31.8 mV, suggesting the predominance of repulsive forces that prevent aggregation. In vitro release suggest transport occurs by diffusion. Morphological analysis demonstrated spherical particles and rough surfaces. In conclusion, data suggest that PHB/PCL nanospheres are promising delivery systems to SIM.

Resumo Um plano instável para o desenvolvimento de currículos e questões de apoio na educação física (PE) criou confusão entre os profissionais. O objetivo desta pesquisa é investigar os fatores percebidos como importantes no desenvolvimento da educação física de qualidade (QPE) por profissionais de países da América Latina (AL). Um questionário composto por 24 itens com base no QPE foi respondido por 468 profissionais coletados em 6 cidades da América Latina. Uma análise fatorial exploratória dos 24 itens usando extração ML e rotação obliminar direta foram aplicados, e os 17 itens retidos foram agrupados em uma solução de três fatores denominada Elementos de Desenvolvimento e Suporte para QPE na Escola (DSFQPE) (α = 0,935), Valor essencial do QPE (CVPE) (α = 0,890) e Arranjo Curricular das Atividades Físicas (CAPA) (α = 0,850). Os itens retidos indicaram propriedades excelentes e o referencial básico percebido pelos profissionais de EF em países da América Latina como importante na investigação do PEQ.
Abstract An unsteady plan for curriculum development and supportive issues in physical education (PE) has created confusion among professionals. The purpose of this research is to investigate the factors perceived as important in the development of quality physical education (QPE) by professionals in Latin American (LA) countries. A questionnaire consisting of 24 items based on QPE was responded by 468 professionals collected from 6 LA cities. An exploratory factor analysis (EFA) of the 24 items using ML extraction and direct oblimin rotation were applied, and the retained 17 items were clustered in a three factor solution referred to as, Development and Supportive Elements for QPE in School (DSFQPE) (α = .935), Core Value of QPEtabl (CVPE) (α = .890), and Curriculum Arrangement of Physical Activities (CAPA) (α = .850). The retained items indicated excellent properties and the basic framework as perceived by professionals from PE in LA countries as important in the investigation of QPE.
Resumen Un plan inestable para el desarrollo curricular y aspectos de apoyo para la educación física (EF) han creado confusión entre los profesionales. El propósito de esta investigación es investigar los factores percibidos como importantes en el desarrollo de la educación física de calidad (EFC) por profesionales en países de América Latina (AL). Un cuestionario que consta de 24 ítems basados en EFC fue respondido por 468 profesionales recolectados en 6 ciudades de LA. Se aplicó un Análisis factorial exploratorio de los 24 ítems que utilizan extracción ML y rotación oblicua directa, y los 17 ítems retenidos se agruparon en una solución de tres factores denominada Elementos de desarrollo y de apoyo para EFC en la escuela (EDAEFC) (α = 0,935); Valor principal de EFC (CVPE) (α = 0,890) y Arreglo del plan de actividades físicas (CAPA) (α = 0,850). Los ítems retenidos indicaron excelentes propiedades y el marco básico es percibido por los profesionales de EF en los países de AL como importante en la investigación de EFC.

Resumo Objetivo É uma tarefa particularmente difícil transmitir más notícias em perinatologia. Habilidades de comunicação podem ser aprendidas, ensinadas e praticadas. O presente estudo avalia se uma sessão de treinamento estruturado para comunicar más notícias ampliaria as habilidades dos residentes de perinatologia. Métodos Estudo de intervenção controlado e aleatorizado com residentes do 1° ao 4° ano do curso de perinatologia de uma faculdade de ciências médicas no ano letivo de 2014/15. Um total de 61 dos 100 residentes elegíveis (61%) voluntariaram-se para um programa de treinamento envolvendo comunicar uma perda perinatal para uma paciente simulada no papel da mãe, seguido do feedback imediato da atriz, ambos filmados. Posteriormente, os residentes foram aleatoriamente designados para um grupo de treinamento em más notícias baseado na estratégia SPIKES e revisão dos vídeos (intervenção) ou para um grupo-controle, sem treinamento. Todos os residentes retornaram numa segunda simulação análoga à primeira, com a mesma paciente simulada cega à intervenção. Avaliou-se as habilidades dos residentes segundo um checklist preenchido pela atriz. A análise estatística incluiu análise de covariância para medidas repetidas (ANCOVA-MR). Os residentes avaliaram a atividade de simulação com feedback. Resultados O programa foi completado por 58 residentes. As simulações duraram em média 12 minutos, o feedback 5 minutos, e o treinamento SPIKES entre 1h e 2,5h. Não houve diferença significativa nas atuações dos residentes segundo a paciente simulada (p = 0.55). Os residentes avaliaram a simulação com feedback positivamente. Essas atividades podem ter reduzido o impacto do treinamento SPIKES. Conclusão O treinamento SPIKES não teve impacto significativo na atuação dos residentes. Os residentes consideraram as simulações com feedback úteis. Mais pesquisas são necessárias para determinar qual modalidade é mais eficaz.
Abstract Objective Breaking bad news (BBN) is particularly difficult in perinatology. Previous research has shown that BBN skills can be learned and improved when taught and practiced. This project evaluated whether a structured training session would enhance perinatology residents’ skills in BBN. Methods This was a randomized controlled intervention study with year 1 to 4 Perinatology residents from a medical school in Brazil, during the 2014/15 school year. A total of 61 out of 100 (61%) eligible residents volunteered to a structured training program involving communicating a perinatal loss to a simulated patient (SP) portraying the mother followed by the SP’s immediatefeedback,bothvideo recorded. Later, residents were randomly assigned to BBN training based on a setting, perception, invitation, knowledge, emotion and summary (SPIKES) strategy with video reviews (intervention) or no training (control group). All residents returned for a second simulation with the same SP blinded to the intervention and portraying a similar case. Residents’ performances were then evaluated by the SP with a checklist. The statistical analysis included a repeated measures analysis of covariance (RM-ANCOVA). Complementarily, the residents provided their perceptions about the simulation with feedback activities. Results Fifty-eight residents completed the program. The simulations lasted on average 12 minutes, feedback 5 minutes and SPIKES training between 1h and 2h30m. There was no significant difference in the residents’ performances according to the SPs’ evaluations (p = 0.55). The participants rated the simulation with feedback exercises highly. These educational activities might have offset SPIKES training impact. Conclusion The SPIKES training did not significantly impact the residents’ performance. The residents endorsed the simulation with feedback as a useful training modality. Further research is needed to determine which modality is more effective.

RESUMO Racional: A proteína C reativa (PCR) é um dos marcadores inflamatórios com maior acurácia na apendicite aguda (AA). A obesidade leva a um estado pró-inflamatório com PCR aumentada, o que pode interferir na interpretação deste teste de laboratório na AA. Objetivo: Avaliar a sensibilidade, especificidade, valor preditivo positivo (VPP) e valor preditivo negativo (VPN) da PCR em pacientes com AA e sua correlação com o índice de massa corporal (IMC) e a composição da gordura corporal. Métodos: Este é um estudo retrospectivo baseado em registros clínicos e estudos de imagem de 191 indivíduos histopatologicamente confirmados com AA em comparação com 249 controles que foram submetidos à tomografia computadorizada abdominal (TC). Dados clínicos e epidemiológicos, valores de IMC e PCR foram extraídos de registros médicos. As TC foram avaliadas para achados de AA e medidas de composição corporal. Resultados: Os valores da PCR aumentaram de acordo com o IMC dos pacientes, com sensibilidade variável de 79,78% em indivíduos com IMC normal ou magro, 87,87% em excesso de peso e 93,5% em indivíduos com obesidade. Um padrão semelhante foi observado para o VPN: um aumento com o aumento do IMC, 69,3% em indivíduos com IMC normal ou magro, 84,3% em excesso de peso e 91,3% em indivíduos com obesidade. Houve uma correlação positiva entre a PCR e a área de gordura visceral em pacientes com AA. Conclusão: Existem variações quanto à sensibilidade, especificidade, VPP e valores VPN da PCR em pacientes com AA, estratificados pelo IMC. Um aumento na área de gordura visceral está associado à PCR elevada em todo o espectro do IMC.
ABSTRACT Background: The C reactive protein (CRP) is one of the most accurate inflammatory markers in acute appendicitis (AA). Obesity leads to a pro-inflammatory state with increased CRP, which may interfere with the interpretation of this laboratory test in AA. Aim: To assess sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of CRP in patients with AA and their correlation to body mass index (BMI) and body fat composition. Method: This is a retrospective study based on clinical records and imaging studies of 191 subjects with histopathologically confirmed AA compared to 249 controls who underwent abdominal computed tomography (CT). Clinical and epidemiological data, BMI, and CRP values were extracted from medical records. CT scans were assessed for AA findings and body composition measurements. Results: CRP values increased according to patients’ BMI, with varying sensitivity from 79.78% in subjects with normal or lean BMI, 87.87% in overweight, and 93.5% in individuals with obesity. A similar pattern was observed for NPV: an increase with increasing BMI, 69.3% in individuals with normal or lean BMI, 84.3% in overweight, and 91.3% in individuals with obesity. There was a positive correlation between CRP and visceral fat area in patients with AA. Conclusions: Variations exist for sensitivity, specificity, PPV, and NPV values of CRP in patients with AA, stratified by BMI. An increase in visceral fat area is associated with elevated CRP across the BMI spectrum.

Impact of a medication therapy management service offered to patients in treatment of breast cancer

ABSTRACT Cancer has high morbidity and mortality rates related to medication use and produce a costly impact in health care. Thus, patients require constant monitoring and proper coordination of care between different professionals. This study aimed to evaluate the impact generated by a Medication Therapy Management service (MTM) offered to patients with breast cancer in use of polypharmacy. Observational, exploratory, descriptive and retrospective study of a MTM service that included 93 patients. Sociodemographic and clinical data related to pharmacotherapy and the processes associated with the systematization of the service were collected and analyzed. Patients were followed-up by the MTM service on average for 18 months (±4.31) and 185 drug-related problems (DRP) were identified, an average of two DRP per patient. Of these DRP, 48.11% were resolved and 49.73% were in the resolution process. The most common DRP were in the categories of Indication (37.84%), followed by Safety (23.78%). The safety category showed the highest resolution rate (59.09%). The study revealed an increased risk of DRP for patients with three or more comorbidities and using 5 or more medications. The process of systematization of a MTM service in oncology was associated with positive outcomes.

Resumen Introducción: Se han propuesto técnicas ecocardiográficas avanzadas como el strain longitudinal global por ecocardiografía bidimensional speckle tracking para la detección de alteraciones precoces de la función sistólica del ventrículo izquierdo. La evaluación de la reproducibilidad del strain longitudinal global es fundamental para su aplicación clínica en diferentes escenarios. Objetivo: Estimar la reproducibilidad del strain longitudinal global del ventrículo izquierdo en individuos de un estudio de cohorte del Brasil. Métodos: La reproducibilidad del strain longitudinal global del ventrículo izquierdo fue evaluada mediante lectura y análisis de imágenes de ecocardiografía de una muestra aleatoria de 50 participantes de la línea de base del Estudio Longitudinal de Salud del Adulto (ELSA-Brasil). Resultados: Los participantes tenían una edad promedio de 49,7 ± 7,3 años, 54% eran mujeres y la media del strain longitudinal global fue -19,5% ± 1,9%. La reproducibilidad interobservador de la medida del strain longitudinal global mostró un coeficiente de variación de 7,4% y un coeficiente de correlación intraclase de 0,76 (IC 95%: 0,61, 0,86). El análisis del acuerdo interobservador de las medidas del strain longitudinal global mediante el método de Bland & Altman evidenció un promedio de diferencias de 0,1% ± 1,4% y unos límites de acuerdo superior de 2,9 e inferior de -2,7. Conclusiones: Hubo una reproducibilidad adecuada de las medidas del strain longitudinal global del ventrículo izquierdo en participantes del ELSA-Brasil y los valores fueron similares a los reportados en otros estudios epidemiológicos longitudinales. Los hallazgos refuerzan la utilidad del strain longitudinal global como un índice clínico de deformación miocárdica, capaz de detectar alteraciones subclínicas de la contractilidad miocárdica.
Abstract Introduction: Advanced echocardiography techniques, such as the global longitudinal strain using two-dimensional ultrasound speckle tracking, have been proposed for the detection of early changes in the left ventricular systolic function. The evaluation of the reproducibility of left ventricular global longitudinal strain is essential for its clinical application in different scenarios. Objective: To determine the reproducibility of left ventricular global longitudinal strain in individuals from a Brazil cohort study. Methods: The reproducibility of left ventricular global longitudinal strain was evaluated by the reading and analysis of echocardiograph images of a random sample of 50 participants of the baseline Longitudinal Study of Adult Health (ELSA-Brasil). Results: The mean age of the participants was 49.7 ± 7.3 years, of whom 54% were women, and the mean global longitudinal strain was -19.5% ± 1.9%. The inter-observer reproducibility of the mean global longitudinal strain, had a coefficient of variation of 7.4%, and an intra-class correlation of 0.76 (95% CI: 0.61 - 0.86). The analysis of the inter-observer agreement of the global longitudinal strain measurements using the Bland and Altman method showed a mean differences of 0.1% ± 1.4%, and upper and lower limits of agreement of 2.9 and -2.7, respectively. Conclusions: There was adequate reproducibility of the left ventricular global longitudinal strain measurements in participants of the ELSA-Brasil study, and the values were similar to those reported in other longitudinal epidemiological studies. The findings reinforce the use of the global longitudinal strain as a clinical marker of myocardial deformation, capable of detecting subclinical changes in myocardial contractility.

Resumo Objetivo Descrever a frequência, características, localização, intensidade da dor em pacientes com câncer de mama em uso do quimioterápico Docetaxel. Métodos Estudo longitudinal realizado com 17 mulheres com câncer de mama em tratamento com Docetaxel. As pacientes foram avaliadas durante três ciclos da quimioterapia quanto à dor, utilizando-se os instrumentos Questionário McGill de Dor (Br-MPQ) e Brief Pain Inventory (BPI). Utilizou-se a correlação de Spearman e o teste de Mann-Whitney. Resultados Houve aumento na média da dor em todas as variáveis do BPI. Quando comparados os valores do Pain Rating Index (PRI) total foram verificados respectivamente 0,20; 0,33 e 0,24 na primeira, segunda e terceira avaliações, sendo encontrada correlação entre a intensidade da dor e a interferência em todas as atividades do cotidiano no BPI na segunda avaliação. Conclusão Houve aumento na ocorrência da dor, comprometendo as atividades diárias de vida das mulheres participantes.

<p>The phytochemical investigation and evaluation of the phytotoxic effect of the extract and fractions obtained from the leaves of <italic>Annona nutans</italic>(R. E. Fr.) R. E. Fr. were performed. Phytotoxic activity was assessed on radicle and hypocotyl of <italic>Allium cepa</italic> and <italic>Lactuca sativa</italic>, where chloroform and ethyl acetate fractions proved active. Phytochemical investigation of the chloroform fraction was allowed identification of polyketides derivatives: triacontanal, 16-hentriacontane, octacosanol and triacontanol, using the <sup>1</sup>H NMR technique associated with data from GC/MS. Using ethyl acetate fraction, with low activity, flavonoids 3-<italic>O</italic>-β-D-galactopyranosyl-isorhamnetin, 3-<italic>O</italic>-β-D-galactopyranosyl-quercetin and 3-<italic>O</italic>-β-D-apiofuranosyl-(1→2)-galactopyranosyl-quercetin were identified, determined by spectrometric techniques one and two-dimensional NMR, combined with mass spectral data. All substances are being reported for the first time in <italic>Annona nutans</italic>. The phytotoxic activity of chloroform fraction may be related to the presence of triacontanol and similar substances. Triacontanol stimulates growth at very low concentrations, but can have an inhibitory effect at higher concentrations, such as those reported for auxin analogs. The toxicity assay using <italic>Artemia salina</italic> (BST) was also performed, with the chloroform fraction showing a negligible lethal dose, LD<sub>50</sub> = 500 mg mL<sup>-1</sup>, while the other fractions and extracts showed no activity. Thus, the presence of acetogenins was ruled out.</p>

Probit regression to estimate the physiological potential of hybrid maize seed

Com o interesse de estudar o potencial fisiológico de lotes de sementes de milho envelhecidas artificialmente, este trabalho teve por objetivo ajustar uma equação simplificada de Andreolli e apresentar uma metodologia utilizando a análise de regressão probito, dada pela equação . Foram utilizadas sementes de três lotes de milho do híbrido OC 705, submetidas ao teste de envelhecimento acelerado, à temperatura de 43 ºC a cada 24 horas. A equação simplificada não proporcionou um bom ajuste dos dados, com r2 de no máximo 92%. Os testes do Chi-quadrado de Pearson e do Chi-quadrado da razão de log-verossimilhança indicaram que a regressão probito proporcionou um bom ajuste aos dados, fornecendo valores estimados com boa precisão. Observou-se que o lote três manteve o vigor mais elevado ao longo do período de armazenamento.
This work was carried out to study the physiological potential of artificially aged seed lots of maize. The specific aim of this study was to fit a simplified equation from Andreoli, , and present a methodology using probit regression analysis, given by the equation . We used seeds from three lots of the maize hybrid OC 705 which were submitted to the accelerated aging test, at the temperature of 43 ºC, every 24 hours. The simplified equation did not provide a good fit to the data, with r2 of at most 92%. Pearson's Chi-square test and the log-likelihood ratio Chi-square test indicated that probit regression had a good fit to the data, providing estimated values with high accuracy. It was observed that lot three maintained the highest vigor throughout the storage period.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Miocardite fulminante associada ao vírus influenza H1N1: relato de caso e revisão de literatura

Caso de miocardite fulminante associada ao vírus influenza H1N1, em que foi descrita a evolução clínica do paciente e enfatizada a importância do ecocardiograma à beira do leito como auxílio no diagnóstico precoce e manejo de crianças com disfunção miocárdica grave, além de terem sido discutidos aspectos relevantes relacionados à terapêutica e ao prognóstico da miocardite fulminante. Trata-se de paciente do sexo feminino, 4 anos e 8 meses, previamente hígida, com história de quadro gripal há 2 semanas. Admitida no pronto-socorro com sinais de instabilidade hemodinâmica, necessitando de suporte ventilatório e drogas vasoativas. Exames laboratoriais, radiografia de tórax e ecocardiograma sugestivos de miocardite. Pesquisa positiva para H1N1 em secreção de nasofaringe. Evoluiu com choque cardiogênico refratário a despeito das medidas clínicas, indo a óbito em 48 horas após admissão na unidade de terapia intensiva. O vírus influenza H1N1 é agente etiológico associado a quadros de miocardite aguda, porém poucos são os casos relatados de miocardite fulminante pelo vírus H1N1. A identificação de sinais e sintomas sugestivos de evolução fulminante deve ser imediata e o ecocardiograma à beira do leito é uma ferramenta útil para detecção precoce de disfunção miocárdica e orientação terapêutica. O uso de terapia imunossupressora, em casos de miocardite fulminante de etiologia viral, é controverso, bem como o de terapia antiviral, de tal forma que o tratamento baseia-se em suporte hemodinâmico e ventilatório. O uso de suporte hemodinâmico, por meio de oxigenação por membrana extracorpórea, aparece como terapia promissora.

Contexto: pacientes de cirurgia cardíaca submetidos a revascularização do miocárdio com circulação extracorpórea. Objetivo: avaliar o efeito hemodinâmico da adição de sufentanil intratecal para anestesia geral. Desenho: estudo prospectivo, randômico e aberto, após aprovação do Comitê de Ética em Pesquisa. Cenário: estudo monocêntrico feito no Instituto Dante Pazzanese de Cardiologia, São Paulo, Brasil. Pacientes: foram submetidos à revascularização eletiva 40 pacientes de ambos os sexos que assinaram o termo de consentimento informado. Critérios de exclusão: doença renal crônica, procedimentos de emergência, reoperações, contraindicação para raquianestesia, fração de ejeção do ventrículo esquerdo inferior a 40%, índice de massa corporal acima de 32 kg/m2 e uso de nitroglicerina. Intervenções: os pacientes foram randomizados para receber (ou não) 1μg/kg de sufentanil intratecal. Anestesia foi induzida e mantida com infusão contínua de sevoflurano e remifentanil. Principais medidas de desfecho: variáveis hemodinâmicas, níveis sanguíneos de troponina I cardíaca, peptídeo natriurético do tipo B, interleucina-6 e fator de necrose tumoral alfa durante e após a cirurgia. Resultados: os pacientes do grupo sufentanil precisaram de menos suporte inotrópico com dopamina, comparados aos do grupo controle (9,5% vs 58%, p = 0,001), e menos aumentos de doses de remifentanil (62% vs 100%, p = 0,004). Os dados hemodinâmicos em oito intervalos de tempo diferentes e os dados bioquímicos não apresentaram diferenças entre os grupos. Conclusões: os pacientes que receberam sufentanil intratecal apresentaram uma estabilidade hemodinâmica maior, como sugerido pelo suporte inotrópico reduzido, e menos ajustes nas doses intravenosas de opiáceos.
Context: Cardiac surgery patients undergoing coronary artery bypass grafting with cardiopulmonary bypass. Objective: Evaluate the effect of adding intrathecal sufentanil to general anesthesia on hemodynamics. Design: Prospective, randomized, not blinded study, after approval by local ethics in Research Committee. Setting: Monocentric study performed at Dante Pazzanese Institute of Cardiology, Sao Paulo, Brazil. Patients: 40 consenting patients undergoing elective coronary artery bypass, both genders. Exclusion criteria: Chronic kidney disease; emergency procedures; reoperations; contraindication to spinal block; left ventricular ejection fraction less than 40%; body mass index above 32 kg/m2 and use of nitroglycerin. Interventions: Patients were randomly assigned to receive intrathecal sufentanil 1 μg/kg or not. Anesthesia induced and maintained with sevoflurane and continuous infusion of remifentanil. Main outcome measures: Hemodynamic variables, blood levels of cardiac troponin I, B-type natriuretic peptide, interleukin-6 and tumor necrosis factor alfa during and after surgery. Results: Patients in sufentanil group required less inotropic support with dopamine when compared to control group (9.5% vs 58%, p = 0.001) and less increases in remifentanil doses (62% vs 100%, p = 0.004). Hemodynamic data at eight different time points and biochemical data showed no differences between groups. Conclusions: Patients receiving intrathecal sufentanil have more hemodynamical stability, as suggested by the reduced inotropic support and fewer adjustments in intravenous opioid doses.

Objetivo: evaluar el efecto hemodinámico de la adición de sufentanilo intratecal para anestesia general. Proyecto: estudio prospectivo, aleatorizado y abierto, con posterior aprobación del Comité de Ética en Investigación. Escenario: estudio monocéntrico realizado en el Instituto Dante Pazzanese de Cardiología, São Paulo, Brasil. Pacientes: cuarenta pacientes de ambos sexos que firmaron el consentimiento informado fueron sometidos a revascularización electiva. Criterios de exclusión: enfermedad renal crónica, procedimientos de urgencia, reintervenciones, contraindicación para la raquianestesia, fracción de eyección del ventrículo izquierdo inferior al 40%, índice de masa corporal por encima de 32 kg/m2 y uso de nitroglicerina. Intervenciones: los pacientes fueron aleatorizados para recibir (o no), 1 µg/kg de sufentanilo intratecal. La anestesia se indujo y se mantuvo con infusión continua de sevoflurano y remifentanilo. Principales medidas de resultados: variables hemodinámicas, niveles sanguíneos de troponina i cardíaca, péptido natriurético del tipo B, interleucina 6 y factor de necrosis tumoral alfa durante y después de la cirugía. Resultados: los pacientes del grupo sufentanilo necesitaron menos soporte inotrópico con dopamina, comparado con el grupo control (9,5 vs. 58%, p = 0,001) y menos aumentos de dosis de remifentanilo (62 vs. 100%, p = 0,004). Los datos hemodinámicos en 8 intervalos de tiempo diferentes y los datos bioquímicos no presentaron diferencias entre los grupos. Conclusiones: los pacientes que recibieron sufentanilo intratecal tuvieron una estabilidad hemodinámica mayor, evidenciada por el soporte inotrópico reducido y menos ajustes en las dosis intravenosas de opiáceos.
