ABSTRACT BACKGROUND: Considering the ability of the health and self-management in diabetes questionnaire (HASMID-10) to verify the impact of self-management on diabetes, we highlight its relevance to scientific research and clinical applicability. However, to date, no study has been conducted to scientifically support its use in other languages. OBJECTIVE: To translate, cross-culturally adapt, and validate the HASMID-10 into the Brazilian Portuguese. DESIGN AND SETTING: A translation, cross-cultural adaptation, and validation study conducted at Ceuma University. METHODS: Study was conducted in accordance with the Guidelines for the Process of Cross-Cultural Adaptation of Self-Report Measures and Consensus-based Standards for the Selection of Health Measurement Instruments. We included participants of both sexes diagnosed with diabetes, aged between 18 and 64 years, and without cognitive deficits or any other limitations that would prevent them from answering the questionnaire. We assessed participants using the problem areas in diabetes (PAID) scale and HASMID-10. We assessed reliability using a test-retest model with a 7-day interval between assessments. We used intraclass correlation coefficient (ICC), 95% confidence interval (CI), standard error of measurement (SEM), minimum detectable difference (MDD), Spearman correlation coefficient, and floor and ceiling effects. RESULTS: Sample comprised 116 participants, most of whom were women, overweight, non-practitioners of physical activity, and nonsmokers. We observed significant correlations (P = 0.006; rho = −0.256) between the HASMID-10 and PAID, adequate reliability (ICC = 0.780) and internal consistency (Cronbach's alpha = 0.796). No ceiling or floor effects were observed. CONCLUSION: HASMID-10 has adequate measurement properties and may be used for Brazilians.

RESUMO OBJETIVO Identificar características relacionadas ao estilo de vida, sociodemográficas e saúde mental de pessoas com sintomas de insônia e pessoas sem insônia durante a pandemia. MÉTODOS A partir de dados coletados por amostragem em bola de neve, por meio de um questionário online foi realizado um estudo caso-controle. Durante o período de novembro de 2020 a abril de 2021, 6.360 pessoas com idade média de 43,5 anos (DP = 14,3) participaram da pesquisa. No presente estudo, foram considerados 158 casos de transtorno de insônia e 476 controles (três controles por caso) selecionados aleatoriamente dentre os participantes sem problemas de sono. RESULTADOS Os resultados da análise comparativa entre casos e controles mostraram que dormir menos de seis horas diárias (OR = 3,89; IC95% 2,50–6,05), sentir tristeza frequentemente (OR = 2,95; IC95% 1,69–5,17), residir em metrópoles (OR = 1,71; IC95% 1,04–2,84), estar com 40 anos ou mais (OR = 1,93; IC95% 1,22–3,06) e a interação entre ocupação e escolaridade mais precária (OR = 2,12; IC95% 1,22–3,69) foram fatores preditores para sintomas de transtorno de insônia durante a pandemia. CONCLUSÕES Além da confirmação da hipótese de que problemas de saúde mental estão associados a sintomas de insônia, os resultados apontam para a insônia como um desfecho importante para estudos sobre efeitos do desemprego, vulnerabilidade e baixa escolaridade da população, sobretudo nas grandes metrópoles, ressaltando que os efeitos da crise sobre a saúde e a economia são distribuídos de forma extremamente desiguais.

ABSTRACT This article highlights an intersection between hermeneutics and education by assuming as its ground the philosophy of Hans-Georg Gadamer. Its objective is to think, in a purposeful manner, how philosophical hermeneutics can, as “practical knowledge”, contribute for discussing the formative-educational experience in its dialogical and transforming feature. Starting from hermeneutics critique over technical-scientific rationality and its expansion through social life, some conceptual-hermeneutical cores (comprehension, experience, tradition and dialogue), which are relevant for the research on teaching-learning process and the educational phenomenon. The thesis “teaching as teaching oneself” is developed as a dialogical experience with the world, effectual fusion of horizons and in the perspective of rethinking formative and methodological dynamics in educational experiences, properly contextualizedes.

RESUMO O presente artigo privilegia uma interface entre hermenêutica e educação considerando os fundamentos da filosofia de Hans Georg Gadamer. Seu objetivo é pensar, de modo propositivo, como a hermenêutica filosófica enquanto “sabedoria prática” pode contribuir para a discussão da experiência formativo-educacional em seu caráter dialógico e transformador. Tendo em vista a crítica hermenêutica dirigida à racionalidade técnico-científica e sua expansão na vida sociocultural, aborda-se alguns núcleos hermenêutico-conceituais (compreensão, experiência, tradição e diálogo) considerados fundamentos relevantes para pesquisas sobre o ensino-aprendizagem e sobre o fenômeno educacional. Problematiza-se a tese do “educar como educar-se” enquanto uma experiência dialógica com o mundo, fusão de horizontes atualizada e na perspectiva de repensar dinâmicas formativas e metodológicas nas experiências educacionais, devidamente contextualizadas.

RESUMO Antecedentes: O Doppler transcraniano (DTC) é uma técnica não invasiva para a avaliação da hemodinâmica cerebral, porém existem controvérsias na literatura sobre sua aplicabilidade preditiva em situações de elevada pressão intracraniana (PIC). Objetivo: Investigar o fluxo sanguíneo cerebral pelo DTC e correlacioná-lo com as variações da PIC na fase aguda da hipertensão intracraniana em modelo animal. Métodos: Dois grupos de animais (suínos) foram submetidos a hipertensão intracraniana secundária à indução de diferentes volumes de hematoma, por meio da insuflação de balão intracraniano controlado com 4 e 7 mL de solução salina fisiológica (grupos A e B, respectivamente). Em seguida, administrou-se infusão venosa de solução salina hipertônica (SSH 3%). Foram coletados os valores dos parâmetros de PIC e DTC (velocidade sistólica [FVs], diastólica [FVd] e média [FVm] do fluxo sanguíneo cerebral), bem como o índice de pulsatilidade (IP). Comparações entre os parâmetros do DTC e o monitoramento da PIC foram realizadas. Resultados: Vinte porcos foram estudados, dez no grupo A e dez no grupo B. Correlação significativa entre IP e PIC foi obtida, principalmente logo após a elevação abrupta da PIC. Não houve correlação entre PIC e FVs, FVd ou FVm separadamente. Também não houve alteração significativa na PIC após a infusão de SSH. Conclusões: Esses resultados demonstram o potencial do IP como um bom parâmetro para a avaliação de pacientes com suspeita de elevação da PIC.

ABSTRACT Background: Transcranial Doppler has been tested in the evaluation of cerebral hemodynamics as a non-invasive assessment of intracranial pressure (ICP), but there is controversy in the literature about its actual benefit and usefulness in this situation. Objective: To investigate cerebral blood flow assessed by Doppler technique and correlate with the variations of the ICP in the acute phase of intracranial hypertension in an animal model. Methods: An experimental animal model of intracranial hypertension was used. The experiment consisted of two groups of animals in which intracranial balloons were implanted and inflated with 4 mL (A) and 7 mL (B) for controlled simulation of different volumes of hematoma. The values of ICP and Doppler parameters (systolic [FVs], diastolic [FVd], and mean [FVm] cerebral blood flow velocities and pulsatility index [PI]) were collected during the entire procedure (before and during hematoma simulations and venous hypertonic saline infusion intervention). Comparisons between Doppler parameters and ICP monitoring were performed. Results: Twenty pigs were studied, 10 in group A and 10 in group B. A significant correlation between PI and ICP was obtained, especially shortly after abrupt elevation of ICP. There was no correlation between ICP and FVs, FVd or FVm separately. There was also no significant change in ICP after intravenous infusion of hypertonic saline solution. Conclusions: These results demonstrate the potential of PI as a parameter for the evaluation of patients with suspected ICP elevation.

Abstract: Cerrado is the second largest biome in Brazil and majorly contributes to the country's grain production. Previous studies on soil metagenomics from the Cerrado revealed an outstanding microbial diversity. In this study, the abundance of pathogenic fungi was analyzed using metagenomic sequences of the Cerrado soils under native vegetation, and under agriculture with no-tillage and conventional tillage. In total, 128,627 sequences of fungi were identified, with 43,439 representing pathogenic fungi and were distributed as follows: native 17,301 (40%), no-tillage 13,780 (32%), and conventional tillage 12,358 (28%). We identified 41 pathogenic fungal species associated with human and animal infections. The data analysis revealed that the native soils had a higher relative abundance of fungal sequences, similar to pathogenic species sequences, in relation to the total eukaryotic sequences, than the conventional tillage and no-tillage treatments, which observed a reduction in fungal abundance because of anthropogenic activities.

SUMMARY OBJECTIVE: Decompressive craniectomy may be a life-saving measure in ischemic stroke patients, who still have several associated complications. The objective of this study is to evaluate a novel decompressive surgery technique for severe hemispheric ischemic stroke. METHODS: For the hinge decompressive craniectomy (HDC), linear durotomies were performed. Vertical (one or two frontal and two parietal), and two horizontal (temporal), with approximately 5 cm long, linear durotomies were carried out. Duroplasty was performed using an autologous subgaleal tissue graft fixed with separate sutures to avoid CSF leak and direct contact of the cortex with the bone flap. The bone flap was fixed in three parietal locations. We compared 10 patients who underwent our modified HDC with 9 patients submitted to classical decompressive craniectomy (CDC). The primary outcome of this study was mortality. RESULTS: Nineteen patients were included, with a mean age of 52.3 years (±8.2). Four (44%) patients from the HDC group had to be reoperated to remove the bone flap because of brain swelling worsening, but none of them died. The average time of HDC was 90 minutes. Overall 14-days mortality was 21.1% (n=4), and cumulative six-months mortality was 42.1% (n=8). Five (50%) patients submitted to CDC died, while 3 (33.3%) submitted to HDC died (χ2=0.07, p=0.79). The mean length of stay was 46.7 days (±32.1) for HDC and 38.7 (±27.1) for CDC (p=0.60). CONCLUSIONS: We present a modified technique of hinge craniectomy with linear vertical and horizontal durotomies, which seems to have reduced operative time and mortality compared to classical decompressive craniotomy, although the difference was not statistically significant.

Resumo A gravidez e a lactação são estados de intensa variação hormonal e mudanças estruturais e secretórias no parênquima mamário. Essas alterações se traduzem em características importantes nos aspectos de imagem da mama, bem como propiciam o surgimento de lesões benignas e malignas específicas. A presente revisão de literatura tem por objetivo discutir a segurança da utilização dos métodos imagem da mama (mamografia, ultrassonografia e ressonância magnética) durante o ciclo gravídico-lactacional e apresentar as alterações fisiológicas esperadas e os aspectos de imagem das principais doenças mamárias que podem ocorrer nesse período, como galactocele, adenoma lactacional, fibroadenoma, mastites puerperais e câncer de mama relacionado à gestação.

Abstract Pregnancy and lactation constitute states of intense hormonal variation with secretory and structural changes in the breast parenchyma. These changes translate into important features on breast imaging, as well as the emergence of specific benign and malignant lesions. This literature review aims to discuss the safety of the use of breast imaging methods (mammography, ultrasound, and magnetic resonance imaging) during the pregnancy-lactation cycle, and to present the expected physiological changes and imaging appearance of the main breast diseases that may occur in this period, such as galactocele, lactating adenoma, fibroadenoma, puerperal mastitis, and pregnancy-associated breast cancer.

RESUMO O presente estudo buscou investigar a utilização de aplicativos móveis em smartphones, especificamente o aplicativo Hidromapp, como ferramenta técnica de suporte para a coleta de marcas de cheia, incluindo também a aplicação dos dados coletados no mapeamento de áreas de inundação. Para tal, no município de Jaqueira-Pernambuco, foram coletadas em campo informações sobre a inundação ocorrida em maio de 2017, utilizando smartphones e o aplicativo Hiromapp. As informações foram posteriormente utilizadas como dados de entrada para a modelagem e reconstituição da mancha de inundação, por meio de técnicas de geoprocessamento e Modelo Digital de Terreno de alta resolução (Pernambuco Tridimensional – PE3D). Para a avaliação dos resultados, a mancha de inundação reconstituída foi comparada com resultados de modelagem hidrodinâmica, a partir de indicadores estatísticos (RMSE e Índice de Sucesso Crítico - C). Além disso, foram observados aspectos relacionados ao desempenho do aplicativo Hidromapp enquanto ferramenta de suporte para a coleta em campo de marcas de cheia. Como resultados, o aplicativo demonstrou bom desempenho para a coleta de dados em campo, e o método aplicado possibilitou uma estimativa parcialmente adequada do evento reconstituído, em que a área (extensão) da inundação apresentou alto grau de similaridade com o modelo comparado, porém com ressalvas para as profundidades da lâmina d’água, em que foram detectadas discrepâncias consideráveis em alguns pontos da área de estudo.

ABSTRACT The present study aimed to investigate the use of mobile applications on smartphones, specifically the Hidromapp application, as a technical support tool for flood data collection, as well as its use in flood mapping. To this end, information about a flood event of May 2017 was collected in the municipality of Jaqueira-Pernambuco, using smartphones and the Hidromapp application. Information was later used as input data for modeling and floodplain reconstitution. The study used geoprocessing techniques and a high-resolution Digital Terrain Model (Pernambuco Tridimensional - PE3D). For result evaluation, the reconstituted floodplain was compared with hydrodynamic modeling results using statistical indicators (RMSE and Critical Success Index - C). The app presented a good performance as a support tool for field collection of floodmark data. The applied method allowed a partially adequate estimation of the reconstituted event, in which the flooded area (extension) showed a high degree of similarity with the compared model, but with caveats for water surface elevations, where considerable discrepancies were detected in some points of the study area.

Resumo: Esta pesquisa objetiva analisar aspectos da morfologia urbana da cidade de Pau dos Ferros, localizada no interior semiárido do Rio Grande do Norte, tomando por base a atuação de agentes produtores do espaço. Enquanto procedimentos metodológicos, foram desenvolvidas: revisão bibliográfica, visitas in loco e análise documental. Concluiu-se que a lógica de acumulação capital pauta a produção desses espaços, contribuindo para a valorização imobiliária e para a segregação socioespacial na cidade.

Abstract: This research aims to analyze aspects of the urban morphology of the city of Pau dos Ferros, located in the semi - arid interior of Rio Grande do Norte, based on the performance of agents that produce space. Methodological procedures were developed: bibliographic review, on-site visits and documentary analysis. It was concluded that the logic of capital accumulation guides the production of these spaces, contributing to the real estate valuation and the socio-spatial segregation in the city.

Resumen: Esta investigación objetiva analizar aspectos de la morfología urbana de la ciudad de Pau dos Ferros, ubicada en el interior semiárido de Rio Grande do Norte, tomando como base la actuación de agentes productores del espacio. En cuanto procedimientos metodológicos, fueron desarrolladas: revisión bibliográfica, visitas in situ y análisis documental. Se concluyó que la lógica de acumulación capital pauta la producción de esos espacios, contribuyendo para la valorización inmobiliaria y para la segregación socioespacial en la ciudad.

ABSTRACT The objective of this study was to determine prediction equations to estimate the nitrogen-corrected apparent metabolizable energy (AMEn) values of animal meals used in broiler diets through meta-analysis. A bibliographic review was undertaken with studies conducted in Brazil from 2000 to 2016 to catalogue information on AMEn values and the following chemical elements in the composition of the feedstuffs: crude protein (CP), ether extract (EE), gross energy (GE), mineral matter (MM), calcium (Ca), and phosphorus (P). Groups were also catalogued and formed according to sex and age of birds. Chemical correlations were analyzed, and a multiple linear regression model with the stepwise procedure was used to examine the association between the variables, which were included in the equation as a function of their importance. High and significant correlation coefficients between the independent (GE, MM, CP, EE, Ca, and P) and dependent variable (AMEn) contribute to the understanding of variations in the energy values of these feedstuffs. According to the coefficients of determination, the best equations to estimate AMEn of poultry offal meal and meat and bone meal are AMEn = 6139 − 45.5 CP + 0.356 GE − 123.5 MM ( R 2 = 0.8302 ) and AMEn = 2267 + 19.9 CP + 67.9 EE − 44.4 MM ( R 2 = 0.9021 ) , respectively.

Resumo Objetivo: Avaliar o papel da ressonância magnética (RM) no planejamento terapêutico de pacientes com câncer de mama. Materiais e Métodos: Foram avaliadas 160 mulheres com diagnóstico de câncer de mama submetidas a RM para estadiamento préoperatório. O tamanho do tumor principal avaliado pela RM e pelos exames convencionais (mamografia e ultrassonografia) foi comparado com o exame anatomopatológico (padrão ouro), utilizando o coeficiente de correlação de Pearson (r). Foi avaliada, ainda, a presença de lesões adicionais não identificadas nos exames prévios e sua influência no planejamento terapêutico. Resultados: A idade média das pacientes foi 52,2 anos (variação: 30-81 anos) e o tipo histológico mais comum foi o carcinoma ductal invasivo (60,6%). A medida do tumor na RM teve melhor correlação com o tamanho no exame anatomopatológico, quando comparado com a mamografia (r: 0,872 × 0,710) e com a ultrassonografia (r: 0,836 × 0,704). A RM identificou lesões adicionais em 53 pacientes (33,1%), sendo 20 malignas (12,5%), e modificou o planejamento terapêutico em 23 pacientes (14,4%). Conclusão: A RM das mamas demonstrou ser mais acurada que os exames convencionais na avaliação das dimensões do tumor principal e foi capaz de identificar lesões adicionais não identificadas pelos outros métodos, que alteraram o planejamento terapêutico em um percentual importante dos casos.

Abstract Objective: To assess the role of magnetic resonance imaging (MRI) in the planning of breast cancer treatment strategies. Materials and Methods: The study included 160 women diagnosed with breast cancer, who underwent breast MRI for preoperative staging. Using Pearson's correlation coefficient (r), we compared the size of the primary tumor, as determined by MRI, by conventional imaging (mammography and ultrasound), and in the pathological examination (gold standard). The identification of lesions not identified in previous examinations was also evaluated, as was its influence on treatment planning. Results: The mean age of the patients was 52.2 years (range, 30-81 years), and the most common histological type was invasive ductal carcinoma (in 60.6% of the patients). In terms of the tumor size determined, MRI correlated better with the pathological examination than did mammography (r = 0.872 vs. 0.710) or ultrasound (r = 0.836 vs. 0.704). MRI identified additional lesions in 53 patients (33.1%), including malignant lesions in 20 (12.5%), which led to change in the therapeutic planning in 23 patients (14.4%). Conclusion: Breast MRI proved to be more accurate than conventional imaging in determining the dimensions of the main tumor and was able to identify lesions not identified by other methods evaluated, which altered the therapeutic planning in a significant proportion of cases.

Resumo Abordagem Na prática clínica é extremamente comum a admissão hospitalar de pacientes com nível de consciência alterado, sonolentos ou em estado não responsivo. Essa condição clínica demanda uma investigação eficaz e um tratamento precoce. É fundamental a realização de uma avaliação focada e objetiva, com a realização de anamnese e exame físico de qualidade para localizar a lesão e definir condutas. Exames de imagem e laboratoriais têm desempenhado papéis cada vez mais relevantes no suporte à investigação clínica. Nesta revisão, são discutidos os principais tipos de alterações de consciência e os pontos imprescindíveis que devem ser avaliados na abordagem clínica desses pacientes.

Summary In clinical practice, hospital admission of patients with altered level of consciousness, sleepy or in a non-responsive state is extremely common. This clinical condition requires an effective investigation and early treatment. Performing a focused and objective evaluation is critical, with quality history taking and physical examination capable to locate the lesion and define conducts. Imaging and laboratory exams have played an increasingly important role in supporting clinical research. In this review, the main types of changes in consciousness are discussed as well as the essential points that should be evaluated in the clinical management of these patients.
