RESUMO
Objetivo:
estimar a prevalência de marcadores sorológicos para hepatite B e C em pacientes com diabetes mellitus e analisar potenciais fatores associados.
Método:
estudo transversal com 255 pacientes com diabetes mellitus. Elegeram-se variáveis demográficas, clínicas e comportamentos de risco para hepatite B e C. Investigou-se os marcadores HBsAg, Anti-HBc IgG, Anti-HBc IgM, Anti-HBs e Anti-HCV. Utilizou-se um questionário e coleta de sangue venoso e análise por estatística inferencial.
Resultados:
16,8% pacientes apresentaram marcador Anti-HBc total reagente, 8,2% Anti-HBs isolado e 75% foram não reagentes para todos os marcadores de hepatite B. Nenhum caso de HBsAg reagente foi encontrado, 3,3% dos pacientes apresentaram marcador anti-HCV reagente. A prevalência de infecção pregressa pelo vírus da hepatite B mostrou-se diretamente associado ao tempo de diabetes mellitus, e a prevalência de infecção pelo vírus da hepatite C não teve associação com as variáveis investigadas. A prevalência de infecção por hepatite B e C em pacientes com diabetes mellitus foi superior a nacional, 16,8% e 3,3% respectivamente.
Conclusão:
os resultados sugerem que pacientes com diabetes sejam uma população de maior vulnerabilidade às hepatites B e C, ensejando à adoção de medidas preventivas de sua ocorrência.

Objective:
to estimate the prevalence of serological markers for hepatitis B and C in patients with diabetes mellitus and analyze potential associated factors.

The markers HBsAg, Anti-HBc IgG, Anti-HBc IgM, Anti-HBs, and Anti-HCV were investigated. A questionnaire and venous blood collection and inferential statistical analysis were used.
Results:
16.8% of the patients had a total reactive Anti-HBc marker, 8.2% an isolated Anti-HBs, and 75% were non-reactive for all hepatitis B markers. No case of reactive HBsAg was found and 3.3% of the patients had a reactive anti-HCV marker. The prevalence of prior hepatitis B virus infection was directly associated with the time of diabetes mellitus and the prevalence of hepatitis C virus infection was not associated with the investigated variables. The prevalence of hepatitis B and C infection in patients with diabetes mellitus was higher when compared to the national, with values of 16.8% and 3.3%, respectively.
Conclusion:
the results suggest that patients with diabetes are a population of higher vulnerability to hepatitis B and C, leading to the adoption of preventive measures of their occurrence.

La prevalencia de infección previa por el virus de la hepatitis B se mostró directamente asociado al tiempo de diabetes mellitus, y la prevalencia de infección por el virus de la hepatitis C no tuvo asociación con las variables investigadas. La prevalencia de infección por hepatitis B y C en pacientes con diabetes mellitus fue superior al nacional, 16,8% y 3,3% respectivamente.
Conclusión:
los resultados sugieren que pacientes con diabetes son una población de mayor vulnerabilidad a las hepatitis B y C, lo que da lugar a la adopción de medidas preventivas de su ocurrencia.

Introdução
A literatura internacional mostra surtos de infecção pelo vírus da hepatite B (HBV) e vírus da hepatite C (HCV) em instituições hospitalares, ambulatoriais e de longa permanência. Estes surtos acontecem quando as normas de controle de infecção durante o monitoramento da glicemia capilar são negligenciadas, tais como, o compartilhamento de canetas lancetadoras, lancetas e de glicosímetros sem o adequado processo de desinfecção, devido à transmissão de microrganismos pelo sangue. . No entanto, desconhece-se o comportamento da doença em indivíduos com DM e fatores de risco relacionados à infecção nesta população.
Diante do exposto, ao considerar o crescimento expressivo da prevalência de DM no município de Ribeirão Preto-SP, que passou de 12,1% em 1997 para 15,1% em 2006, o impacto da infecção pelo HBV e HCV na morbidade e mortalidade, agravados pelo DM; que os pacientes com DM constituem população de risco aumentado para hepatite B e C, este estudo teve como objetivo estimar a prevalência de marcadores sorológicos para hepatite B e C em pacientes com DM e, analisar potenciais fatores de risco relacionados. Acredita-se que o estudo proposto possa fornecer subsídios para conhecer a magnitude do problema e, avançar na produção do conhecimento sobre hepatite B e C e DM. Esse estudo pode representar o surgimento de uma nova temática de investigação que poderá conduzir outros estudos, traduzindo-se em qualidade das informações em saúde e, por conseguinte, melhoria da rede de atenção à saúde.
Método
Estudo transversal, realizado em uma unidade de saúde de nível secundário um município do estado de São Paulo, Brasil. A população de estudo foi constituída por 314 pacientes com DM tipo 1 e 2, que compareceram em pelo menos uma consulta no período de julho a dezembro de 2014. Foram considerados elegíveis todos os paciente com diagnóstico de DM tipo 1 e 2, registrados em prontuário de saúde, de ambos os sexos, com idade igual ou superior a 18 anos e que compareceram à consulta médica no período de julho a dezembro de 2014. Foram excluídos sete pacientes por limitações auditivas ou cognitivas que impossibilitaram responder às questões do instrumento e 17, devido à dificuldade de estabelecer contato pelo pesquisador. Desta forma 290 pacientes com DM foram convidados a participar do estudo, sendo que 35 recusaram. Os principais motivos referidos para a recusa foram à falta de tempo para responder o questionário, falta de interesse em participar do estudo devido já ter participado de outros projetos de pesquisa e indisponibilidade para a coleta de sangue. A amostra de conveniência foi constituída por 255 pacientes com DM que compareceram à consulta médica no período de coleta de dados e atenderam aos critérios de inclusão. Esse valor (n=255) representa 88% dos pacientes convidados a participar do estudo, 81% da população de estudo e 39% dos pacientes com DM atendidos, em 2014, na referida unidade. As variáveis explanatórias foram: demográficas (sexo, idade e escolaridade) e clínicas (tempo de DM, uso de insulina, monitoramento da glicemia capilar, intervenções médicas, cirúrgicas, diagnósticas e terapêuticas, situações e comportamentos de risco para hepatite B e C) e os desfechos foram infecção pelo HBV e pelo HCV.

. O questionário foi composto por 96 questões subdivididas em cinco partes: Identificação (11 questões); Variáveis demográficas (quatro questões); Variáveis clínicas (51 questões); Variáveis comportamentais (24 questões); Resultados de exames de sorologia para hepatite B e C (seis questões).
O instrumento de coleta de dados foi submetido a pré-teste com dez pacientes a fim de identificar possíveis ajustes na sequência das questões, testar a abordagem ao paciente, bem como estimar o tempo de aplicação do questionário. Para a coleta de dados a pesquisadora contou com a colaboração de uma aluna de Iniciação Cientifica previamente capacitada a fim de padronizar a coleta de dados. Após a aplicação do pré-teste o questionário foi mantido, sem necessidade de ajustes quanto à forma e conteúdo. Os dez pacientes foram incluídos na amostra final do estudo. A coleta de dados foi realizada no período de julho a dezembro de 2014.
Dos 255 pacientes, 226 compareceram à unidade para a coleta de sangue, 19 realizaram a coleta no domicílio, 10 pacientes recusaram realizar a coleta de sangue. Desta forma 245 pacientes realizaram a coleta de sangue. Os principais motivos para a recusa foram à falta de tempo e a desistência em participar desta fase da coleta de dados.
A análise estatística dos dados foi realizada utilizando o programa STATA 11.0 (Statacorp LP, College Station, Estados Unidos). A descrição dos dados demográficos e clínicos foram apresentados por meio de estatística descritiva, considerando todos os pacientes que participaram do estudo (n=255). A análise sorológica para hepatite B, dos paciente que realizaram a coleta de sangue (n=245) permitiu avaliar a presença dos marcadores HBsAg, anti-HBc IgG, anti-HBc IgM e anti-HBs. Devido os marcadores HBsAg e anti-HBc IgM por terem se apresentado não reagentes para todos os pacientes para efeitos de análise e apresentação dos dados, considerou-se o marcador Anti-HBc total como equivalente ao Anti-HBc IgG. A análise sorológica para hepatite C permitiu avaliar a presença do marcador anti-HCV. Resultado reagente para este marcador foi considerada infecção pelo HCV. A análise univariada das possíveis associações entre as variáveis demográficas e clínicas e a infecção pelo HBV e HCV foi determinada pelos testes de Qui-quadrado corrigido por Pearson ou Teste exato de Fisher bicaudal e Wilcoxon. Resultados
As características demográficas e clínicas dos 255 (100%) pacientes investigados estão descritas na Tabela 1.
Dos 245 (100%) que compareceram para a coleta de sangue, 41 (16,8%) apresentaram marcador correspondente à infecção pregressa com cura espontânea, 20 (8,2%) soroconversão por vacinação e 184 (75%) susceptibilidade à infecção. Nenhum caso de hepatite B aguda ou crônica foi encontrado. Portanto, a prevalência de infecção pregressa pelo HBV encontrada em pacientes com DM foi de 16,8%.
A Tabela 2 mostra os resultados obtidos da análise univariada de infecção pregressa pelo HBV segundo as variáveis demográficas e clínicas. Infecção pregressa apresentou associação direta com a idade (p=0,014) e tempo de DM (p=0,043). Não se obteve associação significativa para as outras variáveis.
A Tabela 3 apresenta à análise univariada de infecção pregressa segundo as variáveis relacionadas à história de intervenções médicas, cirúrgicas, diagnósticas e terapêuticas e situações e comportamentos de risco para hepatite B. Os resultados mostram associação entre infecção pregressa e relato de contato domiciliar com caso de hepatite B (p=0,001), trabalho como policial (p=0,016) e maior número de parceiros sexuais ao longo da vida (p=0,004).
As variáveis explanatórias incluídas na análise de regressão logística foram aquelas que exibiram possível associação com o desfecho (p≤0,20). Dentre as variáveis incluídas no modelo, tempo de doença permaneceu diretamente associado à infecção pregressa após a análise multivariada, sendo que o tempo de DM aumenta o risco de hepatite B em aproximadamente 4% a cada ano de diagnóstico da doença. Também, o trabalho como policial se mostrou associado à infecção (Tabela 4).
Dos 245 pacientes investigados, oito (3,3%) apresentaram marcador anti-HCV reagente. Portanto, a prevalência de infecção pelo HCV encontrada em pacientes com DM foi de 3,3%. Não houve associação significativa entre as variáveis demográficas e clínicas investigadas e infecção pelo HCV.
Discussão
Ao comparar os resultados obtidos aos do inquérito de base populacional realizado, no Brasil, constata-se que a prevalência de exposição ao HBV encontrada (16,8%) foi superior à nacional (11,6%) na população geral de 20 a 69 anos. Em síntese, ao considerar, a maior prevalência de exposição ao HBV encontrada e sua relação com tempo de DM, sugere-se o aprofundamento de novas investigações relacionadas no manejo do diabetes que possam contribuir para a infecção pelo HBV. A ausência de associação de infecção pelo HCV e as variáveis estudadas pode ser atribuída ao número relativamente baixo de infectados. Essa investigação é pioneira no Brasil, e oferece subsídios para comparações com futuros estudos e avanço no conhecimento da temática.
Este estudo oferece subsídios para conhecer a magnitude do problema e avançar na produção do conhecimento sobre hepatite B e C e DM. O estudo pode gerar novas temáticas de investigação, traduzindo-se em qualidade das informações em saúde e, por conseguinte, qualificação do cuidado de enfermagem.
Conclusão
A prevalência de infecção pelo HBV em pacientes com DM foi de 16,8%, superior à nacional e mostrou-se diretamente associado ao tempo de DM. Nenhum caso de hepatite B aguda ou crônica foi encontrado. A prevalência de infecção pelo HCV foi de 3,3%, superior à nacional e não teve associação com as variáveis demográficas e clínicas investigadas. Outros estudos precisam ser desenvolvidos a fim de investigar essas questões e aprofundar o conhecimento da relação entre hepatite C e DM na população nacional, visando à adoção oportuna de medidas preventivas.
