Abstract Introduction: IgA nephropathy (IgAN) is the most common glomerular disease globally, and its susceptibility and the risk for the development of end-stage kidney disease are related to genetic and environmental factors. IgAN recurrence after kidney transplantation is relatively common, impacting graft function and survival. This study evaluated the risk factors and the clinical, laboratory, and histological characteristics of post-transplant IgAN recurrence based on the Oxford classification. Material and methods: Retrospective single-center cohort study including kidney transplant recipients with biopsy-proven pre-transplantation IgAN, with analysis of risk factors and clinical, laboratory, and histological characteristics of the IgAN recurrence cases. Results: 53 patients fulfilled the inclusion criteria and were included in the study. The majority was male, white, eutrophic, with a mean age of 27 ± 9 years at IgAN diagnosis. Systemic arterial hypertension and proteinuria were frequent in the pretransplant period. Four recipients (7.5%) presented IgAN recurrence in a period of 6 to 122 months post-transplant. According to the Oxford classification, they had high scores of mesangial hypercellularity and segmental glomerulosclerosis in the native kidney biopsies and there was mesangial hypercellularity in all analyzed graft biopsies. None of these patients had received induction immunosuppression and all of them presented graft failure in the follow-up. Conclusions: In this series, there was a high prevalence of mesangial hypercellularity and segmental glomerulosclerosis on native kidney biopsies, and mesangial hypercellularity occurred in all IgAN recurrence graft biopsies. Despite the lower incidence of recurrence of IgAN post-transplant compared to previous reports, progression to graft loss was of 100%.

Resumo Introdução: Nefropatia por IgA (NIgA) é a doença glomerular mais comum mundialmente. Sua suscetibilidade e risco para desenvolvimento de doença renal em fase terminal estão relacionados a fatores genéticos e ambientais. A recidiva de NIgA pós-transplante é relativamente comum, impactando na função e sobrevida do enxerto. Este estudo avaliou fatores de risco e características clínicas, laboratoriais e histológicas da recidiva de NIgA pós-transplante, com base na classificação de Oxford. Material e métodos: Estudo de coorte retrospectivo de centro único, incluindo receptores de transplante renal com NIgA pré-transplante comprovada por biópsia, com análise dos fatores de risco e características clínicas, laboratoriais e histológicas dos casos de recidiva de NIgA. Resultados: 53 pacientes preencheram critérios de inclusão e foram incluídos no estudo. A maioria era homem, branco, eutrófico, com idade média de 27 ± 9 anos no diagnóstico de NIgA. Hipertensão arterial sistêmica e proteinúria foram frequentes no período pré-transplante. Quatro receptores (7,5%) apresentaram recidiva de NIgA entre 6-122 meses pós-transplante. Segundo a classificação de Oxford, eles apresentaram altos escores de hipercelularidade mesangial e glomeruloesclerose segmentar nas biópsias de rins nativos. Houve hipercelularidade mesangial em todas as biópsias de enxerto analisadas. Nenhum destes pacientes recebeu imunossupressão de indução. Todos apresentaram falência do enxerto no acompanhamento. Conclusões: Nesta série, houve alta prevalência de hipercelularidade mesangial e glomeruloesclerose segmentar em biópsias de rins nativos, e hipercelularidade mesangial ocorreu em todas as biópsias do enxerto de recidiva da NIgA. Apesar da menor incidência de recidiva de NIgA pós-transplante comparada a relatos anteriores, a progressão para perda do enxerto foi de 100%.

ABSTRACT: The present study proposed to describe, through a literature review, the use of new therapeutic management which allows for offering a better quality of life to individuals affected by these pathologies. A bibliographic search was conducted in the main health databases PUBMED (www.pubmed.gov) and Scholar Google (www.scholar.google.com.br), in which studies published from 1987 to 2023 were collected. In the first stage, the list of retrieved articles was examined by reading the titles and abstracts. In the second stage, the studies were selected by reading the full contents. Two authors (JDMM and DAQ) performed stages 1 and 2. Experimental, clinical, case-control, randomized controlled, and laboratory cohort studies, case reports, systematic reviews, and literature reviews, which were developed in living individuals, were included. Therefore, articles that did not deal with the subject in question, letters to the editor, opinion articles, duplicated literature in databases, and literature that did not address the variables under study, we re excluded. Contemporary dentistry uses alternative treatments capable of improving the patient's condition since a cure is not always possible. Therefore, the possibility of improving the quality of life becomes an important point to be reached. Evidence-based healthcare has made great advances in recent decades, especially in the areas of orofacial pain, TMD, and occlusion, especially related to orthodontic, prosthetic, and restorative care.

RESUMEN: En el presente estudio se propuso describir, a través de una revisión bibliográfica, el uso de nuevos manejos terapéuticos que permitan brindar una mejor calidad de vida a los individuos afectados por estas patologías. Se realizó una búsqueda bibliográfica en las principales bases de datos de salud PUBMED (www.pubmed.gov) y Scholar Google (www.scholar.google.com.br), en las que se recopilaron estudios publicados entre 1987 y 2023. En la primera etapa, se examinó la lista de artículos recuperados mediante la lectura de los títulos y resúmenes. En la segunda etapa, los estudios fueron seleccionados mediante la lectura del contenido completo. Dos autores (JDMM y DAQ) realizaron las etapas 1 y 2. Se incluyeron estudios de cohortes experimentales, clínicos, de casos y controles, controlados aleatorios y de laboratorio, informes de casos, revisiones sistemáticas y revisiones de la literatura, que se desarrollaron en individuos vivos. Por lo tanto, se excluyeron artículos que no trataran el tema en cuestión, cartas al editor, artículos de opinión, literatura duplicada en bases de datos y literatura que no abordara las variables en estudio. La odontología contemporánea utiliza tratamientos alternativos capaces de mejorar el estado del paciente, ya que no siempre es posible la curación. Por lo tanto, la posibilidad de mejorar la calidad de vida se convierte en un objetivo importante. La atención médica basada en la evidencia ha logrado grandes avances en las últimas décadas, especialmente en las áreas de dolor orofacial, TMD y oclusión, especialmente en relación con la atención de ortodoncia, prótesis y restauración.

Abstract The present study evaluated the actual rotational speed of three different endodontic motors compared to the values provided by the manufacturers. A total of three endodontic motors (X-Smart Plus, VDW.Silver, and iRoot) were tested at 400 rpm and 800 rpm and 2 N/cm2 torque. The kinematics of the devices was recorded by using a custom angle-measuring disc with a 50-mm diameter attached to the handpiece provided by the manufacturer, whereas their movement was captured by a high-speed camera at 2,400 frames per second, 800 x 800 pixel-resolution and distance of 0.3 m from the target object. Statistical analysis was performed at a significance level of 5%. At 400 rpm, the iRoot motor had a value of 17.94 rpm above that indicated by the manufacturer, which was significantly different from those of X-Smart Plus (5.20 rpm below that indicated by the manufacturer) and VDW.Silver (0.62 rpm above that indicated by the manufacturer) motors (P < 0.05). At 800 rpm, the iRoot motor had a value of 51.34 rpm below that indicated by the manufacturer, whereas the X-Smart Plus motor had a value of 13.00 rpm below that indicated by the manufacturer (P > 0.05). The VDW.Silver motor statistically differed from the iRoot and X-Smart Plus ones, showing a value of 1.68 rpm above that indicated by the manufacturer. In conclusion, the X-Smart Plus, VDW.Silver, and iRoot motors showed lower variations in the rotational speed values compared to those reported by their manufacturers. The endodontic motors presented different behaviors between them, with the VDW.Silver motor presents the most accurate values and the iRoot presents the most divergent values.

Resumo O presente estudo avaliou a real velocidade rotacional de três diferentes motores endodônticos comparados aos valores fornecidos pelos fabricantes. Um total de três motores endodônticos (X-Smart Plus, VDW.Silver e iRoot) foram testados a 400 rpm e 800 rpm e torque de 2 N/cm2. A cinemática dos dispositivos foi verificada utilizando-se um disco customizado de 50 mm de diâmetro acoplado ao contra ângulo fornecido pelo fabricante e o movimento foi capturado através de uma câmera ajustada em 2400 quadros por segundo com resolução de 800 x 800 pixels distante 0,3 m do objeto alvo. A análise estatística foi realizada com nível de significância de 5%. A 400 rpm, o motor iRoot apresentou 17,94 rpm acima do indicado pelo fabricante, sendo o valor significativamente diferente do X-Smart Plus (5,20 rpm abaixo do indicado pelo fabricante) e VDW.Silver (0.62 rpm acima do indicado pelo fabricante) (P > 0,05). A 800 rpm, iRoot apresentou 51.34 rpm abaixo do indicado pelo fabricante e X-Smart Plus 13,00 rpm abaixo do indicado pelo fabricante (P > 0,05). VDW.Silver foi estatisticamente diferente de iRoot e X-Smart Plus, apresentando 1,68 rpm acima do indicado pelo fabricante. Concluiu-se que os motores X-Smart Plus, VDW.Silver e iRoot apresentaram baixa variação na velocidade rotacional quando comparada ao fornecido pelo fabricante. Os motores endodônticos apresentaram diferentes comportamentos entre eles, sendo que o motor VDW.Silver apresentou valores mais acurados enquanto o iRoot foi o mais divergente.

The aim of this work was to identify and quantify the metabolites present in the roots of Combretum lanceolatum inoculated with its endophytic fungi. The metabolomics was accomplished using the 1H nuclear magnetic resonance (NMR) spectral data and evaluated via rNMR software and Madison Metabolomics Consortium Database (MMCD). The principal component analysis (PCA) and partial least squares-discriminant analysis (PLS-DA) showed that plants inoculated with Trichoderma spirale (Ts) present differentiation and discrimination over the time compared to control. Seven days after Ts fungal inoculation, 15 metabolites were identified at different concentrations comparing to the control plants. The plants inoculated with Ts fungus present the metabolites spermidine and pantothenate in higher concentrations and 3-hydroxybutyric acid and b-alanine in lower concentrations compared to control plants, indicating any response to biotic stress. These metabolites are involved in various plant processes, including secondary metabolites biosynthesis, energy metabolism and self-defense. Therefore, this work demonstrates the diversification of primary metabolites composition influenced by endophytes inoculation.

Objectives: To re-evaluate a sample of older adults enrolled in a randomized controlled trial of lithium for amnestic mild cognitive impairment (MCI) after 11 to 15 years, re-assessing their current (or last available) global cognitive and functional state. Methods: We recalled all former participants of the Lithium-MCI trial conducted by our group between 2009 and 2012 to perform a single-blinded, cross-sectional evaluation of their global clinical state to compare the long-term outcome of those who received lithium vs. those who received placebo. Results: Of the original sample (n=61), we were able to reach 36 participants (59% of retention), of whom 22 had previously received lithium (61% of the recall sample) and 14 (39%) had received placebo. Since 30.5% of the recalled sample was deceased, psychometric data were collected only for 69.5% of the participants. We found statistically significant differences in current mean Mini Mental State Examination score according to previous treatment group (25.5 [SD, 5.3] vs. 18.3 [SD, 10.9], p = 0.04). The lithium group also had better performance in the phonemic Verbal Fluency Test than the control group (34.4 [SD, 14.4] vs. 11.6 [SD, 10.10], p < 0.001). Differences in these measures also had large effect sizes, as shown by Cohen’s d values of 0.92 and 1.78, respectively. Conclusion: This data set suggests that older adults with amnestic MCI who had been treated with lithium during a previous randomized controlled trial had a better long-term global cognitive outcome than those from a matched sample who did not receive the intervention.

RESUMO: Este estudo teve como objetivo avaliar a diversidade genética e a suscetibilidade a antimicrobianos de cepas de Staphylococcus aureus isoladas de vacas leiteiras em Minas Gerais, Brasil. Trinta e sete cepas provenientes de cinco municípios (oito rebanhos) foram genotipadas usando a técnica multi-locus sequence typing (MLST) e a suscetibilidade a 12 antimicrobianos foi avaliada pelo método de difusão em disco. Foram detectadas altas taxas de resistência para penicilina [75,68% (28/37)], ampicilina [70,27% (26/37)] e tetraciclina [70,27% (26/37)] entre os isolados. A multirresistência foi observada em sete [18,92% (7/37)] isolados e dois foram classificados como sugestivos de Staphylococcus aureus resistente à meticilina (MRSA). Entre os 37 isolados, 33 novos sequence types (ST) e dois STs conhecidos (ST126 e ST746) foram identificados pelo MLST. Os complexos clonais mais frequentemente observados foram: CC97 [78,38%; (29/37)], CC1 [8,11%; (3/37)] e CC5 [5,40%; (2/37)]. Foi construída uma minimum‐spanning tree (MSTs) com todos isolados estudados e esta não mostrou padrão de agrupamento quando comparada com dados epidemiológicos como municípios e rebanho, do qual foi isolado, e perfis de resistência a antimicrobianos. Uma segunda MST foi construída comparando os isolados deste estudo e todas as cepas de S. aureus depositadas no banco de dados PubMLST provenientes do Brasil, que mostrou associação entre o genótipo, STs e a origem da cepa. Foi possível observar entre os isolados do estudo que, aqueles que pertenciam ao CC97, eram geneticamente mais próximos das cepas depositadas no PubMLST isoladas de leite e de produtos lácteos, enquanto aqueles que pertenciam aos CC1 e CC5 estavam mais próximos a cepas isoladas de humanos ou do ambiente de fazendas e indústrias de laticínios. Em conclusão, nossos resultados mostraram um alto índice de resistência às penicilinas e tetraciclinas e grande diversidade genética entre as cepas de S. aureus isoladas de casos de mastite bovina.

ABSTRACT: This study evaluated the genetic diversity and antimicrobial susceptibility of Staphylococcus aureus isolates from dairy cows in Minas Gerais, Brazil. Thirty-seven isolates from five municipalities (8 herds) were genotyped using multi-locus sequence typing (MLST) and susceptibility to 12 antimicrobial agents was tested using the disk diffusion method. High resistance rates for penicillin [75.68% (28/37)], ampicillin [70.27% (26/37)], and tetracycline [70.27% (26/37)] were detected. Multidrug resistance was observed in seven [18.92% (7/37)] isolates, and two were suggestive of Methicillin-resistant Staphylococcus aureus (MRSA). Among the 37 isolates, 33 novel sequence types (ST) and two known STs (ST126 and ST746) were identified in MLST. The clonal complexes more frequently observed were: CC97 [78.38%; (29/37)], CC1 [8.11%; (3/37)] and CC5 [5.40%; (2/37)]. Minimum‐spanning tree (MST) analysis according to data from municipalities, herds, and resistance patterns for all isolates did not show any clustering pattern. However, the MST comparing all Brazilian S. aureus isolates deposited in the PubMLST database and from this study depicted an association between the genotype and strain origin (clinical sample). Isolates from this study that belong to CC97 were close to database isolates from milk and dairy products, while those that belong to CC1 and CC5 were close to database isolates from human sources and the environment of dairy farms or industries. In conclusion, our results showed a high rate of resistance to penicillins and tetracyclines and great genetic diversity among the S. aureus isolates from bovine mastitis genotyped in the present study.

Resumen El objetivo de este trabajo es realizar un análisis empírico de los efectos macroeconómicos de la política macroprudencial y la influencia del régimen monetario sobre esta política en los principales países de América Latina desde 2007 hasta 2019. Los países analizados son Argentina, Bolivia, Brasil, Costa Rica, Chile, Colombia, Ecuador, México, Perú y Uruguay, divididos en países con Régimen de Metas de Inflación (RMI) y los que siguen otra estructura monetaria. Para ello se utiliza el método PVAR. En los países con RMI, la política macroprudencial alcanza su objetivo de reducir el crédito y las demás variables macroeconómicas tienen el comportamiento esperado. Por otro lado, la política macroprudencial parece tener impactos contradictorios en los países que no son RMI.

Abstract The objective of this study was to perform an empirical analysis of the macroeconomic effects of macroprudential policies and the influence of monetary regimes on these policies in the main countries of Latin America from 2007 to 2019. The countries analyzed were Argentina, Bolivia, Brazil, Costa Rica, Chile, Colombia, Ecuador, Mexico, Peru, and Uruguay. They were divided into countries that adopt the Inflation Targeting Regime (ITR) and those that have another monetary structure. For this end, the panel vector autoregression (PVAR) method was used. This paper points out that, in ITR countries, macroprudential policies achieve their goal of reducing credit. Moreover, other macroeconomic variables have the expected behavior. On the other hand, these policies seem to have contradictory impacts on non-ITR countries.

ABSTRACT: Agriculture is central in Brazil’s political and socioeconomic history; agricultural exploitation is mainly conducted by families, creating a type of agriculture developed in small rural properties named family farming. In Brazil, family farming accounts for a very diverse group of businesses, of which 91% of all the establishments are family establishments that build different production strategies according to different degrees of potentiality and precariousness. This study evaluated the role of rural credit policies in developing family farming activities with agricultural income generation in northeastern Brazil on various family-based farms. For this, the authors conducted a case study in Pernambuco, analyzing the spatial relationship between the distribution of family farms and the access to credit resources from the National Program to Strengthen Family Farming from 2006 to 2017 and the change probabilities in types of farms considering the next 10 years, given the maintenance of conditionsreported in the analyzed period. Results indicatedthe weakening of productive potentiality of farming activities in family farms in Pernambuco, with a significant change in the distribution of family farm types, a reduction in the participation of Specialized Farms and an expressive growth of Farms with Rural Residents. The findings also indicated the relevance of access to rural credit in the supply of production needs of different family farm types and development of farming activities with income generation.

RESUMO: A agricultura é central na história política e socioeconômica do Brasil, a exploração agrícola é realizada principalmente por famílias, criando um tipo de agricultura desenvolvida em pequenas propriedades rurais denominada agricultura familiar que abrange 91% dos estabelecimentos rurais no país. O presente trabalho tem como objetivo avaliar o papel das políticas de crédito rural no desenvolvimento das atividades agrícolas com geração de renda em diferentes tipos de estabelecimentos familiares do Nordeste Brasileiro. Para isso, foi realizado estudo de caso no estado de Pernambuco onde foi analisada a relação espacial entre a evolução da distribuição dos tipos de estabelecimentos familiares e o acesso aos recursos de crédito do Programa Nacional de Fortalecimento da Agricultura Familiar no período de 2006 a 2017 e as probabilidades de mudança nos tipos de fazendas, considerando os próximos 10 anos, dada a manutenção das condições encontradas no período de análise. Os resultados indicam o enfraquecimento da potencialidade produtiva das atividades agropecuárias pela agricultura familiar em Pernambuco, com mudança significativa na distribuição dos tipos de estabelecimentos familiares, redução da participação dos estabelecimentos especializados e crescimento expressivo dos estabelecimentos com residentes rurais. Os resultados indicaram, também, a relevância do acesso ao crédito rural no suprimento das necessidades produtivas nos diferentes tipos de estabelecimentos familiares e em viabilizar o desenvolvimento de atividades agropecuárias com geração de renda.

ABSTRACT Plant species have different adaptation mechanisms regarding seed survival to desiccation, preventing cellular destruction during the water loss. The knowledge of these mechanisms is of great importance for the understanding of how the seed formation/maturation processes and the processes involved in germination occur. Thus, this study aimed to determine physiological and enzymatic changes during the maturation process of melon seeds of the cultivar BRS Anton, obtained from fruits at different maturation stages and subjected to post-harvest storage, by the action of total proteins and the enzymes catalase, peroxidase, and superoxide dismutase. Melon fruits were grown in a greenhouse at Embrapa Vegetables and harvested at five different times: 30, 45, 60, 75, and 90 days after anthesis (DAA). Thirty fruits were harvested in each season and 15 fruits had their seeds extracted immediately after harvest and the other 15 fruits were stored for 15 days. After processing and drying, the seeds were submitted to the following analyses: germination, first count, protein profile analysis, and activity of antioxidant enzymes (total proteins, peroxidase, catalase, and superoxide dismutase). The activity of the antioxidant enzymes from 60 DAA under storage showed a more orderly behavior, reaffirming the results of physiological tests. It shows that the seeds at this point of physiological maturation are well-formed and drying caused no damage to their cell membranes.

RESUMO As espécies vegetais apresentam diferentes mecanismos de adaptação no que diz respeito à sobrevivência das sementes à dessecação, prevenindo a destruição celular durante a perda de água. O conhecimento desses mecanismos é de grande importância para o entendimento de como ocorre os processos de formação/maturação de sementes e dos processos envolvidos na germinação. Dessa forma, o objetivo deste trabalho foi determinar as alterações fisiológicas e enzimáticas durante o processo de maturação das sementes de melão da cultivar “BRS Anton”, obtidas de frutos em diferentes estádios de maturação e submetidas ao armazenamento pós-colheita, por meio da ação das proteínas totais e das enzimas catalase, peroxidase e superóxido dismutase. Os frutos de melão foram cultivados em casa de vegetação na Embrapa Hortaliças e colhidos em cinco épocas distintas: aos 30, 45, 60, 75 e 90 dias após a antese (DAA). Em cada época, foram colhidos 30 frutos, sendo que 15 frutos tiveram suas sementes extraídas imediatamente após a colheita e 15 frutos foram armazenados por 15 dias. Após o beneficiamento e secagem, as sementes foram submetidas às seguintes análises: germinação, primeira contagem, análise do perfil de proteínas e atividade de enzimas antioxidantes (proteínas totais, peroxidase, catalase e superóxido dismutase). Observou-se que as atividades das enzimas antioxidativas a partir dos 60 DAA sob armazenamento, apresentou um comportamento mais ordenado reafirmando os resultados dos testes fisiológicos. Demostrando que as sementes neste ponto de maturação fisiológica, encontra-se bem formadas e que a secagem não provocou danos em suas membranas celulares.

Abstract The present study aims to evaluate the applicability of the Surface Water Supply Index (SWSI) and the Drought Recovery Index (RDI) as monitoring instruments, to identify the frequency, intensity and duration of hydrological drought in the basins of the Banabuiú reservoirs., Castanhão and Orós, located in Ceará, Brazil. The period of analysis was from 2002 to 2020 and for both indices the annual scale was used. The evaluation of the results showed that the SWSI and the RDI behave similarly and present a good modeling of drought or water abundance events. The correlation between them was above 70% for the three studied reservoirs. However, the RDI is lagged in identifying drought, with a tendency to point to more extreme droughts. Thus, the use of indices can be considered a proactive strategy for the management of water resources, as they provide important information for decision-making by managers. In addition, the application of the SWSI in drought monitoring and early warning systems is more advantageous compared to the RDI, as it presents a simpler calculation methodology, requiring less computational effort and fewer input variables.

Resumo O presente trabalho tem como objetivo avaliar a aplicabilidade do índice de Abastecimento de água Superficial (SWSI) e do índice de Recuperação de Seca (RDI) como instrumentos de monitoramento, para identificar a frequência, intensidade e duração da seca hidrológica nas bacias dos reservatórios Banabuiú, Castanhão e Orós, localizados no Ceará, Brasil. O período de análise foi de 2002 a 2020 e para ambos os índices se utilizou a escala anual. A avaliação dos resultados mostrou que o SWSI e o RDI comportam-se semelhantemente e apresentam uma boa modelagem dos eventos de seca ou de abundância hídrica. A correlação entre eles ficou acima de 70% para os três reservatórios estudados. Entretanto, o RDI mostra-se defasado na identificação da seca, com tendência a apontar secas mais extremas. Desse modo, a utilização dos índices pode ser considerada uma estratégia proativa para a gestão dos recursos hídricos, pois eles fornecem informações importantes para a tomada de decisão por parte dos gestores. Além disso, a aplicação do SWSI em sistemas de monitoramento e alerta precoce das secas é mais vantajosa se comparada ao RDI, pois ele apresenta uma metodologia de cálculo mais simples, requerendo um menor esforço computacional e menos variáveis de entrada.

Abstract This study evaluates the knowledge about palliative care of medical students in their final years at universities that adopt problem-based learning methodology in Northeast Brazil. It was observed that 78% of the students did not have sufficient information on the care of terminally ill patients, and only 55% knew the World Health Organization’s definition of palliative care. Regarding specific knowledge, students showed an unsatisfactory level of achievement in the domains of dyspnea, psychiatric and gastrointestinal problems; acceptable in the pain domain and excellent in philosophy of palliative care and communication skills. Knowledge regarding pain management, dyspnea and psychiatric problems improved during the internship. Therefore, it was found that students come into contact with the subject but have a significant deficit in issues related to the management of common symptoms in palliative care.

Resumen Este estudio evalúa el conocimiento de cuidados paliativos por los estudiantes de medicina de los últimos años en universidades que adoptan la metodología de aprendizaje basado en problemas en el Noreste de Brasil. El 78% de los estudiantes no tenía suficiente información sobre la atención a pacientes terminales, y solo el 55% conocía la definición de cuidados paliativos de la Organización Mundial de la Salud. Respecto a los conocimientos específicos, los estudiantes demostraron un insatisfactorio nivel de precisión en los dominios disnea, problemas psiquiátricos y gastrointestinales, aceptable en el dominio dolor y excelente en la filosofía de cuidados paliativos y habilidades de comunicación. En el internado adquirieron conocimientos sobre el manejo del dolor, la disnea y los problemas psiquiátricos. Se constató que los estudiantes tienen contacto con el tema, pero presentan un déficit significativo en temas relacionados con el manejo de los síntomas comunes en los cuidados paliativos.

Resumo O presente estudo avalia o conhecimento sobre cuidados paliativos de estudantes de medicina dos últimos anos nas universidades que adotam a metodologia de aprendizagem baseada em problemas no Nordeste do Brasil. Observou-se que 78% dos estudantes não tiveram informações suficientes sobre cuidados de pacientes em situação terminal, e apenas 55% conheciam a definição de cuidados paliativos da Organização Mundial da Saúde. Quanto aos conhecimentos específicos, os estudantes demonstraram um nível de acerto insatisfatório nos domínios dispneia, problemas psiquiátricos e gastrintestinais; aceitável no domínio dor e excelente em filosofia dos cuidados paliativos e habilidade de comunicação. Durante o internato houve ganho de conhecimento quanto a manejo da dor, dispneia e problemas psiquiátricos. Portanto, verificou-se que os estudantes entram em contato com o tema, porém apresentam déficit significativo nas questões relacionadas ao manejo de sintomas comuns em cuidados paliativos.

Abstract Manual voting processes have two main problems, the reliability of the result and the delay in counting the votes. To defraud the election, that is, change the amount of votes of the candidates, an attacker can replace ballots with votes for other candidates with ballots with votes for the candidate to be benefited. In this scenario, the security of the electoral process strongly depends on the control of physical access to the ballot boxes. Abstract The objective of the study was to evaluate the growth of B. vulgaris seedlings by incorporating five doses of sewage sludge (0, 10, 30, 50, and 70%) into the substrate, with three types of propagules. The design used was completely randomized (DIC), with 30 replicates per treatment. At 30 and 90 days, the parameters were measured: height, number of shoots, and the dry mass of the roots and aerial part. The means were compared by the Scott-Knott test. It was found that the incorporation of sewage sludge was not detrimental to the growth and development of the seedlings. Concerning the three types of propagules, the highlight was P1, which was more promising in the seedling production via cuttings. The most indicated sewage sludge dosage is the incorporation of 30% since it showed higher growth of seedlings. With dry mass, from the aerial part, the highlight was the propagulum P2, followed by P1. Root dry mass values were low in all treatments evaluated, suggesting the need for a longer evaluation time in the development and establishment of bamboo seedlings in a nursery.

Resumo O objetivo do trabalho foi avaliar o crescimento de mudas de B. vulgaris através da incorporação cinco doses de lodo de esgoto (0, 10, 30, 50 e 70%) ao substrato, com três tipos de propágulos. O delineamento utilizado foi inteiramente casualizado (DIC), com 30 repetições por tratamento. Aos 30 e 90 dias foram mensurados os parámetros: altura, número de brotos, e a massa seca das raízes e parte aérea. As médias foram comparadas pelo teste Scott-Knott. Verificou-se que a incorporação de lodo de esgoto não foi prejudicial ao crescimento e desenvolvimento das mudas. Em relação aos três tipos de propágulos o destaque foi P1, que se mostrou mais promissor à produção de mudas via estaquia. Para a dosagem de lodo de esgoto mais indicada é a incorporação de 30%, uma vez que apresentou maior crescimento das mudas. Em relação a massa seca, da parte aérea o destaque foi o propágulo P2, seguido de P1. Já os valores da massa seca de raiz foram baixos em todos os tratamentos avaliados, o que sugere a necessidade de um maior tempo de avaliação no desenvolvimento e estabelecimento das mudas de bambu em viveiro.

RESUMO Contexto A doença hepática crônica associa-se com osteoporose, osteopenia ou osteomalácia. A osteoporose e as fraturas por fragilidade óssea têm altas prevalências e são mais frequentes em pacientes com cirrose hepática do que na população geral. A busca por osteopenia e osteoporose nesta população pode permitir a intervenção precoce e modificar os desfechos desfavoráveis. Objetivo Conhecer a prevalência de osteopenia ou osteoporose e de fraturas por fragilidade óssea em portadores de cirrose hepática, fatores de risco associados e seu comprometimento na qualidade de vida. Métodos Estudo observacional e transversal realizado com 71 pacientes portadores de cirrose hepática do Serviço de Hepatologia do Hospital de Base do Distrito Federal, Brasília, DF, Brasil, no período de julho de 2017 a dezembro de 2018. Os pacientes foram submetidos à densitometria óssea de coluna lombar e colo de fêmur, raio-x de coluna lombo sacra e ao questionário Chronic Liver Disease Questionnaire (CLDQ, na sigla em inglês) para avaliação de qualidade de vida. Foi calculado o escore de Fracture Risk Assessment Tool “FRAX Maior” nos pacientes >50 anos. As análises foram realizadas para a avaliação dos fatores de risco associados à fratura de coluna lombo sacra. Resultados Dos 71 pacientes avaliados, a maioria (62%) foi diagnosticada com osteoporose ou osteopenia à densitometria. Dos 44 portadores de osteopenia ou osteoporose, 52,3% eram do sexo feminino, com idade média de 62,6±9,51 anos, sendo a maioria (72,7%) Child A, cirróticos de etiologia alcoólica (36,4%) e com qualidade de vida intermediária ao CLDQ (3,3). Dos pacientes com fratura de coluna lombo sacra, a média de idade foi de 61,6±11,1 anos, 60% eram do sexo feminino, a maioria Child A (66,7%), de etiologia alcoólica (46,7%), e apresentaram qualidade de vida intermediária ao CLDQ (3,5). A presença de osteopenia e/ou osteoporose esteve associada à fratura lombo sacra (P<0,001), sem correlação com as demais variáveis analisadas: idade, índice de massa corporal (IMC), gênero, presença e ausência de ascite, classificação de Child-Pugh, concentrações séricas de vitamina D, cálcio e fósforo, etiologia da cirrose e “FRAX maior”. Conclusão A prevalência de osteodistrofia hepática foi elevada, e a ocorrência de fratura de coluna lombo sacra esteve mais associada à osteoporose e/ou osteopenia entre cirróticos estudados. A qualidade de vida se mostrou intermediária e sem diferença entre cirróticos com e sem fratura.

ABSTRACT Background Chronic hepatic disease is associated with osteoporosis, osteopenia or osteomalacia. Osteoporosis and fractures due to bone fragility present high prevalences and are more frequent in patients with liver cirrhosis than in the general population. The search for a diagnosis of osteopenia and osteoporosis in this population may allow early intervention and modify unfavorable outcomes. Objective To know the prevalence of osteopenia or osteoporosis and of fracture due to bone fragility in individuals with liver cirrhosis, the associated risk factors, and its compromise in their quality of life (QoL). Methods Observational, transversal study performed with 71 liver cirrhosis patients of the Hepatology Service of the Hospital de Base do Distrito Federal, Brasília, DF, Brazil, between July 2017 and December 2018. The patients were submitted to bone densitometry (DXA) of the lumbar spine and of the femoral neck, to x-ray of the lumbosacral spine and to the Chronic Liver Disease Questionnaire (CLDQ) for the evaluation of quality of life (QoL). The Fracture Risk Assessment (FRAX) major was calculated for patients >50 years old. The analyses were performed for the evaluation of the risk factors associated with lumbosacral spine fracture. Results The majority (62%) of the 71 evaluated patients was diagnosed with osteoporosis or osteopenia on DXA. Of the 44 patients with osteopenia or osteoporosis, 52.3% were female, with a mean age of 62.6±9.51 years old, with the majority (72.7%) being Child A, cirrhotics of alcoholic etiology (36.4%), and with an intermediate QoL according to the CLDQ (3.3). Regarding the patients with lumbosacral spine fracture, the mean age was 61.6±11.1 years old, 60% were female, most of them Child A (66.7%), of alcoholic etiology (46.7%), and with an intermediary QoL according to the CLDQ (3.5). The presence of osteopenia and/or osteoporosis was associated with lumbosacral fracture (P<0.001), without correlation with the other analyzed variables: age, body mass index, gender, presence and absence of ascites, Child-Pugh classification, vitamin D, calcium, and phosphorus serum concentration, cirrhosis etiology and FRAX major. Conclusion The prevalence of hepatic osteodystrophy was high, and the occurrence of lumbosacral spine fracture was more associated with osteoporosis and/or osteopenia among the cirrhotic patients studied. The QoL was intermediate and with no differences between cirrhotics with and without fracture.
